The study of the Jewish quarters, which behave as living organisms in the concert of the towns and cities where they are embedded and interact, cannot be limited to their spatial dimension because religious, anthropological, and cultural components concur. Thus, we reflect on the semantics of space and its mental and symbolic universe in the kingdom of Aragon, for which purpose we use the heuristic term "Jewishness", which contemplates the sociological reality ad intra and not so much from the perspective of the majority society. The Management and Prevention of Delirium in Elderly Patients Hospitalised in Intensive Care Units: A Systematic Review
Background: Delirium or an acute confusional state (ACS) is characterised as being a frequent and complex hospital complication in older adult patients, which can affect their level of independence and increase patient morbidity and mortality. Critically ill patients in the intensive care unit (ICU) frequently develop ICU delirium, leading to longer hospital and ICU stays, increased mortality and long-term impairment. Objectives: This review aims to assess existing evidence of interventions that can be considered effective for the management and prevention of delirium in ICUs, reducing short-term morbidity and mortality, ICU and hospital admission times and the occurrence of other long-term complications. Methodology: For this systematic review, we searched Medline, PubMed, Cochrane Library, CINHAL, LILACS, SciELO and Dialnet from January 2018 to August 2024, in English, Spanish and French. MeSH descriptors were adjusted to search the different databases. We also checked Prospero for ongoing systematic reviews. Main results: The electronic search yielded a total of 2656 studies, of which 14 trials met the eligibility criteria, with a total of 14,711 participants. We included eight randomised clinical trial (RCTs), four cohort analyses, one systematic review and one observational trial, including participants over 65 years admitted to the ICU. Ten of these studies were based on pharmacological interventions, three of them examined non-pharmacological interventions and the remaining study examined mixed (pharmacological and non-pharmacological) interventions. Six placebo RCTs were included, plus four reported comparisons between different drugs. Regarding non-pharmacological interventions, nursing programmes focused on optimising modifiable risk factors or the use of therapies such as bright light are emerging. Regarding mixed interventions, we found the combination of invasive techniques and with sedoanalgesia. Conclusions: Due to its satisfactory level of sedation, dexmedetomidine is presented as a viable option because, although olanzapine offers safer results, postoperative administration angiotensin inhibitor systems significantly reduced the incidence of delirium. As for propofol, no significant differences were found. Among the non-pharmacological and mixed therapies, bright light therapy was able to reduce the incidence of delirium, and the combination of epidural/general anaesthesia was effective in all subtypes of delirium. Concerning the remaining interventions, the scientific evidence is still insufficient to provide a definitive recommendation
Eficacia de las estrategias de automanejo en pacientes con dolor de rodilla: Revisión bibliográfica
Introducción: Los TME relacionados con el dolor de rodilla son una queja común y un motivo frecuente de consulta médica que afecta significativamente la calidad de vida de los pacientes y representa una
importante carga económica para los sistemas de salud. El automanejo se presenta como una estrategia efectiva para abordar los TME y evitar el colapso sanitario. Sin embargo, la evidencia en patología de rodilla sigue siendo controvertida. El objetivo de esta revisión bibliográfica es sintetizar la evidencia sobre la eficacia del automanejo en la disminución del dolor y la mejora de la función y calidad de vida en pacientes con TME de rodilla.
Metodología: Se realizó una búsqueda de ensayos clínicos aleatorizados en las bases de datos electrónicas Pubmed, Web Of Science y Scopus. Se incluyeron intervenciones dirigidas a promover el automanejo en adultos con TME relacionados con dolor de rodilla. La calidad metodológica fue evaluada mediante la escala PEDro.
Resultados: De un total de 923 artículos, se eligieron 11 ECAs publicados en los últimos 5 años, incluyendo un total de 1062 participantes con KOA (1014) o PFP (48). Todos los estudios reportaron mejoras en las
variables de dolor, función de la rodilla y calidad de vida tras la intervención, mostrando diferencias significativas en favor del automanejo, excepto cuando se comparó con ejercicio supervisado. La calidad
metodológica de los estudios osciló entre “moderada” (5/10) y “alta” (8/10).
Conclusión: Los resultados sugieren que las estrategias de automanejo tienen efectos positivos sobre el dolor, la función de la rodilla y la calidad de vida en pacientes con KOA y PFP. Aunque las limitaciones de los estudios impiden sacar conclusiones definitivas, dado que es una intervención conservadora, segura y de bajo coste, es una opción a considerar tanto en las consultas médicas de atención primaria como por los fisioterapeutas.Introduction: Knee-related musculoskeletal disorders (MSDs) are a frequent complaint and a common reason for seeking health care that significantly affect patients' quality of life. MSDs represent a substantial economic burden for healthcare systems. Self-management is shown as a potential effective strategy to address MSDs and prevent healthcare collapse. However, evidence of its effectiveness in knee-related conditions remains controversial. The aim of this systematic review is to synthesize the evidence on the effectiveness of self-management strategies in reducing pain and improving knee function and quality of life in patients with knee-related MSDs.
Method: A systematic review of randomized clinical trials was conducted in the electronic databases of PubMed, Web of Science, and Scopus. Interventions aimed at promoting self-management in adults with knee-related MSDs were included. Methodological quality was evaluated using the Spanish version of PEDro scale.
Results: From a total of 923 studies, 11 randomized clinical trials published within the last 5 years were selected, including a total of 1062 participants diagnosed with KOA (1014) or PFP (48). All studies reported improvements in pain, knee function, and quality of life at the end of the intervention, showing significant differences in favor of self-management in all of them, except when compared with supervised exercise.
The methodological quality of the included studies varied between "moderate" (5/10) to "high" (8/10).
Conclusions:
The results suggest that self-management strategies have positive effects on pain, knee function, and quality of life in patients with KOA and PFP. Although the limitations of the studies make it difficult to draw definitive conclusions, given that it is a conservative, safe, and low-cost intervention, self-management is an option to be considered both by general practitioners and physiotherapists
La inmersión: un estudio de la psicología de los jugadores en los videojuegos
Cada vez el mundo de los videojuegos apunta más hacia crear experiencias únicas e inmersivas para los jugadores y no hacia creaciones vacías sin ninguna intención más allá de la de entretener al jugador. Para ello, se ha trabajado mucho en los últimos años para lograr que los videojuegos causen una inmersión mayor en los jugadores.
Este efecto hace que los jugadores tengan experiencias que les permiten no solo abstraerse de la realidad, sino también poder experimentar situaciones que les generen emociones semejantes a las de la vida real.
El objetivo de este trabajo es analizar si ciertos procesos que ocurren en la mente del jugador están relacionados con la inmersión, y estará respaldado por entrevistas en profundidad a jugadores que hayan podido experimentar esta inmersión.The world of videogames is developing towards the creating of more unique and immersive experiences for the players instead of empty creations without any value outside the entertainment of the users. For that, this lasts years, immersion has been a very studied matter for the achievement of more immersive videogames.
This effect, immersion, causes the players to have experiences that allows them to not only escape from reality but also to live situations that causes them to feel emotions similar to those in real life.
The objective of this work is to analyse if those processes that occur in the player’s mind, are related with immersion and it’ll be supported by in-depth interviews with player who have experience this effect
El fotolibro: más que un álbum de fotos
Los medios de creación y difusión del arte han cambiado a lo largo de la Historia. Desde los soportes más básicos como las paredes de las cuevas, hasta las instalaciones y perfomances el arte ha ampliado sus fórmulas creativas y comunicativas en una atomización infinita de combinaciones y permutaciones. En este estado de cosas un formato que bebe de los códigos clásicos (literarios) ha tenido un importante renacimiento y mantiene una revitalización que le están permitiendo sobresalir entre otras formas creativas visuales, tal y como afirman las últimas investigaciones como las de Marta Martín (2022) o Vega (2020). Se trata del fotolibro, que es a la vez medio o técnica creativa y soporte de difusión de la propia obra. Es decir, el fotógrafo que usa el lenguaje de la imagen trabaja con el lenguaje específico del libro creando una pieza visual y material a la vez, en la que combina las especificidades propias de lo visual (composición, color o escala de grises, género, relación entre las partes, ritmo, etc.) con las del material editorial (papel, tamaño del libro, formato de lectura, texto, etc.). Validity and stability of the International Physical Activity Questionnaire Short-Form for stroke survivors with preserved walking ability
Background: Stroke survivors usually present sedentary lifestyles and fail to comply with the World Health Organization physical activity recommendations. Reliable, low-cost, and fast
tools are needed to monitor physical activity levels in this population.
Objectives: This study aimed to evaluate the content and face validity, construct validity, and test-retest stability of the International Physical Activity Questionnaire Short-Form (IPAQ-SF)
in stroke survivors.
Methods: 120 stroke survivors able to walk independently and preserved comprehension and communication abilities (61±12 years, 35% female) were involved in this psychometric study.
Participants completed the interview form of the IPAQ-SF via standardised videoconference twice, one week apart, under identical conditions, to evaluate test-retest stability. Correlations
between IPAQ-SF and the caloric expenditure during the minutes of activity registered with the Fitbit Inspire 2 activity tracker wristband and 6-Metre Timed Walk (6MTW) were explored to
assess construct validity.
Results: The IPAQ-SF showed good content and face validity. ‘Moderate’ to ‘strong’ correlations were found with the Fitbit Inspire 2 (rho: 0.40 to 0.63), while ‘weak’ to ‘moderate’
correlations were found with the 6MTW (rho: 0.35 to 0.50). Test-retest stability was ‘moderate’ to ‘excellent’ (κ: 0.844 to 0.881; ICC: 0.533 to 0.917).
Conclusions: The IPAQ-SF demonstrated satisfactory content and construct validity, and stability in stroke survivors, supporting its clinical and research utility when the data collection
is conducted by trained evaluators using a standardized interview protocol in large samples
XI Buenas Prácticas de Innovación Docente en el Espacio Europeo de Educación Superior
The call to promote educational innovation and to increase the quality in the teaching-learning process takes special relevance in the European Higher Education Area. The challenge lies in clarifying what innovation means and how to implement it. The application of new tools, methodologies, and practices to sustain more active and meaningful learning also requires a deep reflection on the goals pursued, as well as on the methods teachers use to help achieve these goals.La llamada a promover la innovación educativa y la calidad en el proceso de enseñanza-aprendizaje adquiere una relevancia especial en la implantación del Espacio Europeo de Educación Superior. ¿Por qué estudiar? ¿Para qué estudiar? ¿Hasta cuándo estudiar? Una experiencia para promover la motivación del estudiante universitario
La formación es un proceso largo que requiere de una motivación constante y persistente. Es por ello por lo que se ha llevado a cabo una actividad con la que se ha pretendido la reflexión por los alumnos sobre la importancia de buscar permanentemente la consecución de un fin académico. La actividad, desarrollada con los alumnos del 2º curso del Grado de Fisioterapia de la USJ, se planteó mediante la presentación a los alumnos de una serie de preguntas sobre las que debían reflexionar en grupo. Los resultados muestran que esta experiencia ha resultado positiva al permitirles reflexionar de manera individual y consensuar con otros las respuestas a preguntas de índole personal. Al hacerles recapacitar sobre la oportunidad que significa llevar a cabo unos estudios universitarios. A los docentes les ha permitido conocer la impresión de los alumnos sobre aspectos que pueden ayudarles en la consecución de sus objetivos.Training is a long process that requires constant and persistent motivation. That is why an activity has been carried out with the aim of encouraging students to reflect on the importance of constantly seeking to achieve an academic goal. The activity, developed with the second year students of the Physiotherapy Degree at USJ, was proposed by presenting a series of questions to the students on which they had to reflect in groups. The results show that this experience has been positive by allowing them to reflect individually and reach a consensus with others on personal questions. By making them think about the opportunity that university studies represent, it has allowed teachers to know the impression of the students on aspects that can help them in achieving their objectives
Efecto de un protocolo de movilidad de tobillo en deportistas de crossfit con restricción de la flexión dorsal de tobillo en la cinemática tridimensional del hombro durante un movimiento de “squat snatch”: Protocolo de un ensayo clínico aleatorizado Autor
Introducción: CrossFit fusiona la gimnasia balística y el levantamiento de pesas olímpico con ejercicios convencionales, sometiendo al complejo del hombro a una presión significativa durante el entrenamiento con pesas. La ejecución de ejercicios típicos, como el “squat snatch”, a menudo coloca al hombro en posiciones desfavorables, generando desequilibrios articulares y musculares propensos a lesiones. La búsqueda de niveles más altos de rendimiento por parte del levantador, junto con la posición desfavorable del manguito rotador y la realización de movimientos en series con repeticiones extensas y cargas pesadas, puede resultar en fatiga muscular, deterioro técnico y alteración en la alineación del hombro. En la sentadilla con barra, gesto deportivo, en cuanto a extremidades inferiores, similar al “squat snatch”, mantener el centro de gravedad vertical para garantizar el equilibrio es esencial, requiriendo la aplicación de principios de antropometría y biomecánica. La movilidad del tobillo, relacionada con el ángulo del tronco, se presenta como un factor clave en esta técnica. Objetivos: En primer lugar, evaluar la influencia que tiene una limitación en la flexión dorsal del tobillo en la cinemática del hombro durante el movimiento de crossfit/halterofilia “squat snatch” y por otro lado, en aquellos deportistas de crossfit con una limitación en la flexión dorsal de tobillo, evaluar el efecto de un programa de movilidad de tobillo en la cinemática de tobillo y hombro durante la ejecución de un “squat snatch”. Material y métodos: Protocolo de un ensayo clínico aleatorizado prospectivo doble ciego en el que la muestra se reclutará de diferentes box de crossfit en Navarra y será distribuida en dos grupos: grupo control (Programa educación técnica “squat snatch” + 4 entrenamientos de crossfit/semana) y grupo intervención (Programa de automovilización de tobillo + 4 entrenamientos de crossfit/semana). Los pacientes serán evaluados antes de comenzar el estudio, una vez finalizada la intervención y al mes y a los 3 meses tras finalizar la intervención. Se analizarán los cambios pre y post intervención en el ROM de flexión dorsal de tobillo y flexión/ABD/RE de hombro en el punto más bajo de la ejecución del movimiento “squat snatch”. Resultados esperados: Se espera encontrar una mejora significativa en el ROM de flexión dorsal del tobillo tras la intervención, la cual podría repercutir en menores compensaciones a nivel proximal, en última instancia en la articulación del hombro, durante la ejecución del movimiento “squat snatch” Conclusión: La aplicación de un programa de automovilización de tobillo, podría generar un mayor ROM de flexión dorsal del tobillo, el cual evitaría posibles compensaciones de las articulaciones proximales, para así mantener el equilibrio durante la ejecución de un “squat snatch”ntroduction: CrossFit fuses ballistic gymnastics and Olympic weightlifting with conventional exercises, putting significant stress on the shoulder complex during weight training. Performing typical exercises, such as the squat snatch, often places the shoulder in unfavourable positions, creating joint and muscle imbalances that are prone to injury. The lifter's pursuit of higher levels of performance, coupled with unfavourable rotator cuff positioning and the performance of movements in sets with extensive repetitions and heavy loads, can result in muscle fatigue, technical deterioration and altered shoulder alignment. In the barbell squat, a lower limb sporting gesture similar to the squat snatch, maintaining a vertical centre of gravity to ensure balance is essential, requiring the application of anthropometric and biomechanical principles. The mobility of the ankle, related to the angle of the trunk, is a key factor in this technique. Objectives: Firstly, to evaluate the influence that a limitation in dorsal flexion of the ankle has on the kinematics of the shoulder during the crossfit/halterophilia movement "squat snatch" and secondly, in those crossfit athletes with a limitation in dorsal flexion of the ankle, to evaluate the effect of an ankle mobility programme on the kinematics of the ankle and shoulder during the execution of a squat snatch. Material and methods: Protocol of a prospective double-blind randomised clinical trial in which the sample will be recruited from different crossfit boxes in Pamplona and will be distributed into two groups: control group (squat snatch technical education programme + 4 crossfit training sessions/week) and intervention group (ankle self-mobilisation programme + 4 crossfit training sessions/week). Patients will be evaluated before starting the study, once the intervention is finished and at one month and 3 months after finishing the intervention. Pre- and post-intervention changes in ankle dorsiflexion ROM and shoulder flexion/ABD/RE at the lowest point of the squat snatch movement will be analysed. Expected results: We expect to find a significant improvement in dorsiflexion ROM after the intervention, which could result in less compensation at the proximal level, ultimately at the shoulder joint, during the execution of the squat snatch movement. Conclusion: The application of an ankle self-mobilisation programme could generate a greater ROM of dorsal ankle flexion, which would avoid possible compensations of the proximal joints, in order to maintain balance during the execution of a squat snatch
