Validation of a laser visual feedback and core stability exercise programme to improve lateropulsion reduction and pushing intensity in stroke patients with pusher syndrome: A Delphi study
Background
The patient with pusher syndrome (PS) is characterized by showing postural control alterations due to a lack of perception of his own body in the space. It appears when the patient actively pushes with his unaffected limbs towards the injured side, reacting with resistance to passive straightening towards the midline. Between 10% and 50% of strokes present PS. Nowadays, there is no clearly defined treatment for PS.
Objective
To design and validate an exercise program using visual feedback and specific core stability exercises (FeViCoS) for the treatment of patients with PS.
Methods
Validation was conducted by expert consensus using the Delphi method. Thirteen neurorehabilitation experts participated in the process. An online questionnaire with 18 Likert-type questions was used to evaluate the designed program. Consensus was considered reached if there was convergence between the quartile 1 and 3 values (RIQ = Q1-Q3) or if the relative interquartile range (RIR) was less than 20%. The degree of agreement between experts was measured by calculating the Fleiss' kappa coefficient.
Results
A total of 2 rounds were required to achieve 97.44% consensus with 100% participation. The RIR was less than or equal to 20% for all questions. The Fleiss' kappa index (0.831) showed that the degree of agreement between experts was excellent.
Conclusion
Neurorehabilitation experts considered FeViCoS valid for the therapeutic approach to patients with PS. Expert consensus suggests a novel strategy in physical therapy clinical practice to improve balance and postural orientation in patients with subacute stroke and PS
Airway clearance management in people with bronchiectasis: data from the European Bronchiectasis Registry (EMBARC)
Background: International guidelines recommend airway clearance management as
one of the important pillars of bronchiectasis treatment. However, the extent to
which airway clearance is used for people with bronchiectasis in Europe is unclear.
The aim of the study was to identify the use of airway clearance management in
patients with bronchiectasis across different countries and factors influencing
airway clearance use.
Methods: This was a prospective observational study using data from the European
Multicentre Bronchiectasis Audit and Research Collaboration (EMBARC) Registry
between January 2015 and April 2022. Prespecified options for airway clearance
management were recorded, including airway clearance techniques, devices and
use of mucoactive drugs.
Results: 16 723 people with bronchiectasis from 28 countries were included in the
study. The mean age was 67 years (interquartile range 57-74 years, range 18-100
years) and 61% were female. 72% of the participants reported daily sputum
expectoration and 52% (95% CI 51-53%) of all participants reported using regular
airway clearance management. Active cycle of breathing technique was used by
28% of the participants and airway clearance devices by 16% of participants. The
frequency of airway clearance management and techniques used varied
significantly between different countries. Participants who used airway clearance
management had greater disease severity and worse symptoms, including a higher
daily sputum volume, compared to those who did not use it regularly. Mucoactive
drugs were also more likely to be used in participants with more severe disease.
Access to specialist respiratory physiotherapy was low throughout Europe, but
particularly low in Eastern Europe.
Conclusions: Only a half of people with bronchiectasis in Europe use airway
clearance management. Use of and access to devices, mucoactive drugs and
specialist chest physiotherapy appears to be limited in many European countriesEMBARC3 is funded by the European Respiratory Society through the EMBARC3 clinical

Investigación y aplicación de tecnologías 3D y otras herramientas para facilitar la creación de concept art en videojuegos indies
el crecimiento de la industria de videojuegos indie es común ver como en muchas empresas
existen perfiles híbridos, personas capaces de desarrollar tareas de diferentes departamentos
gracias a su amplio conocimiento sobre varias materias. Por otra parte, en la fase de creación
concept art de un videojuego tradicionalmente se hacía uso de herramientas y metodologías 2D,
pero con la aparición de estos perfiles cada vez es más común el uso de tecnologías 3D y otras
herramientas para que el proceso o pipeline sea óptimo y eficaz. Este tipo de software es usado
por el artista creando una base 3D con la que lograr mejores resultados o resultados más rápidos,
teniendo siempre presentes los art fundamentals.
Para lograr los objetivos de este proyecto, primero se investigó sobre la profesión, su importancia
en el desarrollo de videojuegos y las fases que definen la creación de concept art. Seguidamente
se ahondó en la industria de videojuegos indie, sus características principales, la importancia del
uso de los art fundamentals y el uso de estos para desarrollar un estilo diferenciador y original.
Finalmente, se analizaron múltiples software y herramientas 3D usadas en la industria con el
objetivo de encontrar para qué contextos son más optimas, y más tarde este análisis se puso a
prueba con un ejemplo de desarrollo real.
Por último, tras el proceso de implementación, se desarrolló una dirección de arte clara y hojas
de concept art de diferentes elementos de un videojuego indie, usando para cada uno una de las
herramientas previamente analizadas. Con la información recopilada durante la producción, se
realizó una comparativa de tiempos y resultados para facilitar la elección de herramientas y
metodología dependiendo del contexto. Finalmente, estas hojas de concept brindan una idea
completa del juego final y actúan como un libro de arte real del propio proyecto.With the growth of the indie video game industry, it is common to see how in many companies
there are hybrid profiles, people capable of developing tasks of different departments thanks to
their extensive knowledge of various subjects. On the other hand, in the concept art creation
phase of a video game, traditionally 2D tools and methodologies were used, but with the
emergence of these profiles it is increasingly common to use 3D technologies and other tools for
the process or pipeline to be optimal and effective. This type of software is used by the artist
creating a 3D base with which to achieve better results or faster results, always keeping in mind
the art fundamentals.
To achieve the objectives of this project, first we investigated the profession, its importance in
the development of video games and the phases that define the creation of concept art. Then we
delved into the indie video game industry, its main characteristics, the importance of the use of
art fundamentals and the use of these to develop a differentiating and original style. Finally,
multiple software and 3D tools used in the industry were analyzed in order to find for which
contexts they are more optimal, and later this analysis was tested with an example of real
development.
Finally, after the implementation process, a clear art direction and concept art sheets of different
elements of an indie video game were developed, using for each one of the previously analyzed
tools. With the information gathered during production, a comparison of times and results was
made to facilitate the choice of tools and methodology depending on the context. Finally, these
concept sheets provide a complete idea of the final game and act as a real art book of the project
itself
Sarcopenia screening based on the assessment of gait with inertial measurement units: a systematic review
Background Gait variables assessed by inertial measurement units (IMUs) show promise as screening tools for agingrelated diseases like sarcopenia. The main aims of this systematic review were to analyze and synthesize the scientific
evidence for screening sarcopenia based on gait variables assessed by IMUs, and also to review articles that investigated which gait variables assessed by IMUs were related to sarcopenia. Methods Six electronic databases (PubMed, SportDiscus, Web of Science, Cochrane Library, Scopus and IEEE Xplore) were searched for journal articles related to gait, IMUs and sarcopenia. The search was conducted until December 5, 2023. Titles, abstracts and full-length texts for studies were screened to be included. Results A total of seven articles were finally included in this review. Despite some methodological variability among the included studies, IMUs demonstrated potential as effective tools for detecting sarcopenia when coupled with artificial intelligence (AI) models, which outperformed traditional statistical methods in classification accuracy. The findings suggest that gait variables related to the stance phase such as stance duration, double support time, and variations between feet, are key indicators of sarcopenia. Conclusions IMUs could be useful tools for sarcopenia screening based on gait analysis, specifically when artificial intelligence is used to process the recorded data. Competência cultural de estudantes de graduação em enfermagem: um estudo multicêntrico
Objetivo: avaliar o nível de competência cultural de uma população de estudantes de graduação em enfermagem de quatro instituições europeias de ensino superior. Método: um total de 168 estudantes de enfermagem de quatro países diferentes foram incluídos em nosso estudo. A metodologia do estudo envolveu uma avaliação transversal da competência cultural entre estudantes de graduação em enfermagem de quatro universidades europeias. A coleta de dados incluiu variáveis sociodemográficas, bem como as seguintes ferramentas validadas: Escala de Sensibilidade Intercultural, Ferramenta de Avaliação de Competência Cultural (versão do aluno) e Escala de Consciência Cultural. Resultados: nossos resultados indicaram que os alunos demonstraram um alto nível de sensibilidade intercultural, mas um nível moderado de competência e consciência cultural. Houve variações entre os alunos de diferentes países, o que sugere possíveis diferenças nas abordagens educacionais. Apesar das expectativas de que os alunos de nível superior apresentariam maior competência cultural, não foram encontradas diferenças significativas por ano de estudo, indicando uma falta de integração eficaz da competência cultural nos currículos de enfermagem. Conclusão: os educadores de enfermagem devem considerar a competência cultural dos alunos antes de elaborar programas de estudo relacionados. The Management and Prevention of Delirium in Elderly Patients Hospitalised in Intensive Care Units: A Systematic Review
Background: Delirium or an acute confusional state (ACS) is characterised as being a frequent and complex hospital complication in older adult patients, which can affect their level of independence and increase patient morbidity and mortality. Critically ill patients in the intensive care unit (ICU) frequently develop ICU delirium, leading to longer hospital and ICU stays, increased mortality and long-term impairment. Objectives: This review aims to assess existing evidence of interventions that can be considered effective for the management and prevention of delirium in ICUs, reducing short-term morbidity and mortality, ICU and hospital admission times and the occurrence of other long-term complications. Methodology: For this systematic review, we searched Medline, PubMed, Cochrane Library, CINHAL, LILACS, SciELO and Dialnet from January 2018 to August 2024, in English, Spanish and French. MeSH descriptors were adjusted to search the different databases. We also checked Prospero for ongoing systematic reviews. Main results: The electronic search yielded a total of 2656 studies, of which 14 trials met the eligibility criteria, with a total of 14,711 participants. We included eight randomised clinical trial (RCTs), four cohort analyses, one systematic review and one observational trial, including participants over 65 years admitted to the ICU. Ten of these studies were based on pharmacological interventions, three of them examined non-pharmacological interventions and the remaining study examined mixed (pharmacological and non-pharmacological) interventions. Six placebo RCTs were included, plus four reported comparisons between different drugs. Regarding non-pharmacological interventions, nursing programmes focused on optimising modifiable risk factors or the use of therapies such as bright light are emerging. Regarding mixed interventions, we found the combination of invasive techniques and with sedoanalgesia. Conclusions: Due to its satisfactory level of sedation, dexmedetomidine is presented as a viable option because, although olanzapine offers safer results, postoperative administration angiotensin inhibitor systems significantly reduced the incidence of delirium. As for propofol, no significant differences were found. Among the non-pharmacological and mixed therapies, bright light therapy was able to reduce the incidence of delirium, and the combination of epidural/general anaesthesia was effective in all subtypes of delirium. Concerning the remaining interventions, the scientific evidence is still insufficient to provide a definitive recommendation
Effects of a neuromuscular training program on the performance and inter-limb asymmetries in highly trained junior male tennis players
Background: The objective of this study was to assess the effectiveness of neuromuscular training on the performance of highly trainer junior tennis players. Methods: Twelve male tennis players (age: 13.4 ± 0.36 years; weight: 50.2 ± 6.29 kg; height: 163 ± 4.41 cm) participated and were randomly divided into two groups. The experimental group
(EG) performed neuromuscular training that included exercises for speed, strength, throws, agility, jumps and coordination twice a week for a duration of 10 weeks. Performance was evaluated using various variables, including bilateral and unilateral countermovement jump, 30 cm drop jump and horizontal jump, 505 change of direction test conducted with both the right and left legs, 20-m sprint, and overhead 3 kg medicine ball throw. Asymmetries were also evaluated during the unilateral tests. The impact of the training was assessed through the utilization of ANCOVA tests and effect size measurements. Results: The results indicated a significant enhancement in the EG, specifically in bilateral vertical jump and horizontal jump, as well as explosive strength and speed. Conversely, the control group (CG) did not display similar advancements. Furthermore, there was no increase in asymmetries. Conclusion: This suggests that the implementation of a neuromuscular training program could prove to be an effective approach in enhancing explosive power in the lower limbs among young competitive tennis players. Migration and Sustainable Development Goals in the Spanish press (2014-2021): topics, sources, and frames
One of the biggest global challenges is the international movement of people. For this reason, the 2030 Agenda recognises the importance of migration for development within its Sustainable Development Goals (SDGs) and their respective targets. The objective of our analysis is to investigate the new situation
introduced by the 2030 Agenda in relation to migration processes in the media, in this case based on eight leading Spanish newspapers between 2014 and 2021. This study uses the quantitative content analysis methodology and the framing approach. As frames, it uses the five themes proposed by the 2030
Agenda: People, Prosperity, Planet, Peace, and Partnerships. It is interesting to note that seven years after the 2030 Agenda was launched, the SDGs are not often cited by the Spanish press to analyse and discuss the migration phenomenon from a sustainability perspective. Moreover, it is observed that discourses on migration in terms of sustainability present a clear political approach that emphasises the framework of international partnerships and agreements in economic terms, a fact that can contribute to a change in audiences’ perception of the migration phenomenon. On the other hand, it should be noted that the frames chosen by newspapers to talk about the migration phenomenon coincide with each other, although slight differences can be observed in the themes and use of sources
