    Fonoaudiología en cuidados paliativos para adultos y adultos mayores: fundamentos, roles, abordajes y desafíos

    The Chilean population is aging rapidly, generating a sustained increase in the number of people living with a chronic disease that limits their life, condition that frequently requires specialized teams in palliative care. In these, the inclusion of the speech-language pathologist is recent in most of the countries, even more so in our country. Therefore, there are few local guidelines that facilitate the active participation of this professional under a model focused on the care, well-being and accompaniment of the person, their family and/or significant others. In this context, In this context, the objective of this narrative review is to delve into the foundations that support the inclusion of the speech-language pathologist in palliative carea teams. For this, speech-language pathology action areas, roles and responsibilities are described. In this regard, the available information shows two major fields of action. related to communication and swallowing, which must be addressed in orden to promote the participation and autonomy of the person in making decisions about their care, as well as respect for their advance directives, when necessary. Under this paradigm, the findings are clear in stating that the speech-language pathologist should move away from the traditional rehabilitation model, to migrate towards a group of actions that aloe them to address -in the best possible way- those conditions that generate suffering, pain or discomfort to the person. These actions are addressed in this review, with sufficient detail to allow six major challenges to be raised and proposed for those professionals who work (or want to) in the area.La población chilena envejece rápidamente, lo cual se traduce en un incremento en el número de personas que vive con alguna enfermedad crónica que limita su vida, condición que requiere frecuentemente de equipos especializados en cuidados paliativos. En ellos, la inclusión del/la fonoaudiólogo/a a nivel internacional es reciente, aún más en nuestro país. Por lo anterior, son escasas las orientaciones locales que faciliten la activa participación de este profesional bajo un modelo centrado en el cuidado, bienestar y acompañamiento de la persona, su familia y/o personas significativas. En este contexto, es objetivo de esta revisión narrativa el profundizar sobre los fundamentos que soportan la inclusión del/a fonoaudiólogo/a en equipos de cuidados paliativos. Para lo anterior se describen las áreas de acción fonoaudiológica, sus roles y responsabilidades. Al respecto, la información disponible da cuenta de dos grandes campos de acción, vinculados a la comunicación y alimentación, los que deben ser abordados con el propósito de fomentar la participación y autonomía de la persona en la toma de decisiones sobre sus cuidados y el respeto de sus voluntades anticipadas, cuando sea necesario. Bajo este paradigma, los hallazgos son claros en plantear que el/la fonoaudiólogo/a debe alejarse del modelo rehabilitador tradicional, para migrar hacia un grupo de acciones que le permitan abordar –de la mejor manera posible– aquellas condiciones que generan sufrimiento, dolor o molestia a la persona. Dichas acciones son abordadas en esta revisión, con el detalle suficiente para permitir enunciar y proponer seis grandes desafíos para aquello/as profesionales que se desempeñan (o desean hacerlo) en el área

    Estudio de la validez y fiabilidad del Test de Repetición Fonético-Fonológica (TREFF) como instrumento para evaluar el desempeño fonético-fonológico en niños chilenos

    At present, a greater repertoire of valid and reliable tests is required to evaluate the phonetic-phonological performance in Chilean children. The objective of this study is to determine the validity and reliability of the Phonetic-Phonological Repetition Test (TREFF). Validation was carried out by expert judgment and reliability was determined by statistical analysis of temporal stability and intra and inter-judge agreement. For this process, the instrument was applied in a medium-high sociocultural level child population, with a phonetic-phonological development according to what was expected for their age. The analysis of the responses was carried out by three evaluating judges, who determined if the boy or girl achieved to produce the elements evaluated. In terms of validation, the instrument was modified considering the experts' suggestions. Concerning to reliability, the results obtained demonstrated temporal stability and a high degree of concordance at the intra-judge and inter-judge levels. The study allows us to conclude that the TREFF test meets the validity and reliability criteria.En la actualidad, se requiere un mayor repertorio de pruebas válidas y confiables que evalúen el desempeño fonético-fonológico en niños chilenos. El objetivo de este estudio es determinar la validez y fiabilidad del Test de Repetición Fonético-Fonológica (TREFF). La validación fue llevada a cabo mediante un juicio de expertos y la fiabilidad fue determinada mediante el análisis estadístico de la estabilidad temporal y la concordancia intra- e interjueces. Para este proceso, se aplicó el instrumento en una población infantil perteneciente al nivel sociocultural medio alto, con un desarrollo fonético-fonológico acorde a lo esperado para su edad. El análisis de las respuestas fue realizado por tres jueces evaluadores, quienes determinaron si el niño o la niña lograba producir los elementos evaluados. Respecto a la validación, el instrumento fue modificado considerando las sugerencias de los expertos. En cuanto a la fiabilidad, los resultados obtenidos demostraron estabilidad temporal y un alto grado de concordancia a nivel intra- e interjueces.     Manejo de la presión del cuff en usuarios adultos con vía aérea artificial por profesionales de salud en Chile

    There are different international recommendations on the cuff pressure range for the management of adult patients with an artificial airway. Some Chilean health institutions have developed their own airway management protocols based on these recommendations due to the lack of a national guide. A literature review was undertaken at the University of Southampton, U.K., to determine the appropriate cuff pressure range. Results found that this is between 20-30 cmH2O. Afterwards, a valid and reliable online questionnaire was developed in Spanish at the University of Southampton for application in Chile. The aim of this study is to analyse the cuff pressure management of adult patients with an artificial airway performed by non-medical health professionals in Chile. An analytical, observational and cross-sectional study was performed. A database which contained the responses of nurses, physiotherapists and speech therapists with experience attending adult patients with an artificial airway in Chile was analysed. Results show that 58% of the participants used exclusively the objective technique whilst 7% only used subjective techniques. The minimum and maximum pressures used were 25 and 34 cmH2O respectively, which were higher than the recommended range. 38% of the participants declared that there was an existing protocol at their workplace. There was a significant difference between the maximum pressures used by the clinicians and the pressures recommended on the protocols (p = .029). In conclusion, the participants showed a poor manage of the cuff pressure which could create risks linked with over-inflated or under-inflated cuffs. It is suggested to develop a national guide.Existen diferentes recomendaciones internacionales sobre el rango de presión del cuff para utilizar en usuarios adultos con vía aérea artificial. Algunas instituciones de salud chilenas han creado sus protocolos de vía aérea basadas en dichas recomendaciones, ya que no existe una guía nacional. Se desarrolló una revisión bibliográfica en la Universidad de Southampton, Reino Unido, para determinar el rango adecuado de presión del cuff siendo entre 20-30 cmH2O. Posteriormente, se creó un cuestionario online válido y confiable en español en la Universidad de Southampton, Reino Unido, para ser aplicado en Chile. El objetivo de este estudio es analizar las respuestas emanadas de los profesionales de salud en Chile sobre el manejo de la presión del cuff en pacientes adultos con vía aérea artificial. Es un estudio analítico, observacional y transversal. Consistió en analizar las respuestas de enfermeros, kinesiólogos y fonoaudiólogos con experiencia en pacientes con vía aérea artificial tras aplicar el cuestionario. Los resultados muestran que la técnica objetiva fue ampliamente utilizada de forma aislada (58%) en comparación al uso exclusivo de subjetivas (7%). La presión mínima fue de 25 cmH2O mientras que la máxima fue de 34 cmH2O, rango mayor a lo reportado por la evidencia. Solo un 38% declaró la existencia de protocolo en su trabajo. Hubo diferencia estadísticamente significativa entre las presiones máximas usadas y las recomendadas en dichos protocolos (p=0,029). Se concluye que los participantes en Chile mostraron un manejo poco seguro de la presión del cuff en pacientes adultos con vía aérea artificial que puede generar riesgos asociados a cuffs sobre o sub insuflados.     Fonoaudiología en la Atención Primaria de Salud en Chile desde la perspectiva de usuarios/as, fonoaudiólogos/as y otros/as profesionales de la salud de la ciudad de Santiago

    The health system in Chile is based on the Comprehensive Care Model with a family and community approach, whose structural and articulating core is primary care. Insertion of speech and language professionals at this level is incipient and studies on the subject are scarce. This research sought to describe the practices and experiences of speech therapy in Primary Health Care (PHC) from the perspective of users, speech therapists and other professionals in the health area, who belonged to the city of Santiago during the year 2017. Thus, a qualitative exploratory study was carried out, based on the hermeneutic-interpretive paradigm, in which three focus groups were carried out and the information was analyzed through content analysis. The most relevant categories that emerged from the analysis show that the three groups agree with the growing need for speech therapy in PHC to respond to situations associated with human communication and oral feeding. Also, the three groups agree that this need is linked with a diversification of the professional actions of the speech therapists who work in PHC, in addition to the need for training in the family and community health model. Therefore, it is concluded that the conceptual-theoretical framework on which the professionals are based should be reconfigured and training should be reoriented towards PHC.El sistema de salud en Chile se basa en el Modelo de Atención Integral con enfoque familiar y comunitario, cuyo núcleo estructural y articulador es la atención primaria. La inserción de profesionales fonoaudiólogas/os en este nivel es incipiente y los estudios sobre el tema son escasos. Con esta investigación se buscó describir las prácticas y experiencias del trabajo fonoaudiológico en la Atención Primaria desde la perspectiva de usuarios, fonoaudiólogos/as y otros profesionales del área de la salud pertenecientes a la ciudad de Santiago durante el año 2017. Para cumplir este objetivo, se realizó un estudio de tipo cualitativo exploratorio, basado en el paradigma hermenéutico-interpretativo, en el cual se realizaron tres grupos focales y se analizó la información mediante un análisis de contenido. Las categorías más relevantes que surgen del análisis realizado muestran que los tres grupos focales concuerdan con la creciente necesidad de atención fonoaudiológica en APS para dar respuesta a situaciones asociadas a la comunicación humana y la alimentación oral. También, los grupos concuerdan que aquello va de la mano con una diversificación de las actuaciones profesionales de los/las fonoaudiólogos/as que trabajan en APS, además de una clara necesidad de formación en el modelo de salud familiar y comunitario. Por esto, se concluye que es necesario reconfigurar los marco-teórico conceptuales en los que se basan los profesionales y reorientar la formación hacia la APS

    Autopercepción de la voz en Mujeres Trans de la Región Metropolitana: primer estudio chileno

    At the international level, it is recognized that transgender women, due to their voice, experience psychosocial consequences on a daily basis. In Chile there is no background on this issue, so it is necessary to collect information that allows verifying the background obtained in the international literature. Therefore, the objective of the work is to describe the self-perception of the voice of a group of transgender women from the Metropolitan Region, according to the Trans Woman Voice Questionnaire (TWVQ) instrument. For this, an observational, descriptive correlational cross-sectional study with a quantitative approach was carried out, which was approved by the Ethics Committee of the Faculty of Medicine - Clínica Alemana Universidad del Desarrollo (GP-70/2018). Thirty transgender women participated, responding first to a medical history questionnaire and then to the TWVQ. The latter was analyzed based on the total score and its three items (anxiety and avoidance, vocal function and vocal identity). The results show that voice self-perception is affected first by the vocal function item, then by the anxiety and avoidance item, and finally by the vocal identity item, according to perceived difficulty. The data also provide preliminary evidence that self-perception of the voice could be associated with attendance at the voice specialist and with the length of time on hormonal therapy. Finally, the study provides an empirical way to analyze the TVWQ in detail, based on the proposal of three items.A nivel internacional se reconoce que las mujeres transgéneros, debido a su voz, experimentan diariamente consecuencias psicosociales. En Chile no se cuenta con antecedentes sobre esta temática, por lo que se requiere recabar información que permita constatar los antecedentes obtenidos en la literatura internacional. Por ello, el objetivo del trabajo es describir la autopercepción de la voz de un grupo de mujeres transgénero de la Región Metropolitana, según el instrumento Trans Woman Voice Questionnaire (TWVQ). Para ello, se realizó un estudio observacional, descriptivo correlacional de corte transversal con enfoque cuantitativo, el cual fue aprobado por el Comité de Ética de la Facultad de Medicina – Clínica Alemana Universidad del Desarrollo (GP-70/2018). Participaron 30 mujeres transgénero que respondieron en primer lugar un cuestionario de antecedentes clínicos y luego el TWVQ. Este último se analizó en base al puntaje total y por sus tres ítems (ansiedad y evitación, función vocal e identidad vocal). Los resultados muestran que la autopercepción de la voz se ve afectada en primer lugar por el ítem de función vocal, posteriormente por el ítem de ansiedad y evitación y por último por el de identidad vocal, según la dificultad percibida. Los datos también proporcionan evidencia preliminar de que la autopercepción de la voz podría asociarse con la asistencia al especialista de la voz y con el tiempo de terapia hormonal cursado. Finalmente, el estudio proporciona una forma empírica de analizar detalladamente el TVWQ, en base a la propuesta de tres ítems

    Evaluación perceptual de la insuficiencia velofaríngea en personas con fisura del paladar: una revisión integradora de literatura

    Secondary Velopharyngeal insufficiency (VPI) to cleft palate corresponds to the incomplete closure of the velopharyngeal mechanism during speech, due to lack of tissue in the soft palate or the walls of the pharynx, which generates a hypernasal resonance and nasal air emission in oral sounds. In this regard, there are various proposals in the literature for the perceptual evaluation of VPI. For this reason, the objective of the present study is to describe the auditory perceptual evaluation of velopharyngeal insufficiency, through an integrative literature review. To this end, in May 2020 the electronic databases PUBMED, LILACS, SciELO and Cochrane were consulted using the key words in English: “Velopharyngeal Sphincter”, “Velopharyngeal Insufficiency”, “Cleft Palate”, “Speech Intelligibility”, “Speech Production Measurement”, “Speech Articulation Tests” and “Speech-Language Pathology”, and their equivalent properties in Portuguese and Spanish. Original articles related to the topic were selected, and a specific protocol for data extraction was created. In total, 2,385 articles were found. Of these, 2,354 were excluded due to the title, 13 due to the abstract and 3 after reading the full text. Finally, based on the methodology developed, 33 articles were used in this review. From the review carried out, it is concluded that the parameters most used in the evaluation are hypernasality, nasal emission and the compensatory joint associated with IVF. These parameters are evaluated mainly in sentences, spontaneous speech and words, by an expert speech therapist, live and through audio recordings.La insuficiencia velofaríngea (IVF) secundaria de fisura del paladar corresponde al cierre incompleto del mecanismo velofaríngeo durante el habla, debido a una falta de tejido en el paladar blando o las paredes de la faringe, lo cual genera una resonancia hipernasal y una emisión nasal de aire en los sonidos orales. Al respecto, en la literatura existen diversas propuestas para la evaluación perceptual de la IVF. Por esto, el objetivo del presente estudio es describir la evaluación perceptiva auditiva de la insuficiencia velofaríngea, mediante una revisión integradora de literatura. Para ello, en mayo de 2020 las bases de datos electrónicas PUBMED, LILACS, SciELO y Cochrane, fueron consultadas utilizando las palabras claves en inglés: “Velopharyngeal Sphincter”, “Velopharyngeal Insufficiency”, “Cleft Palate”, “Speech Intelligibility”, “Speech Production Measurement”, “Speech Articulation Tests” y “Speech-Language Pathology” y sus respectivos equivalentes en portugués y español. Se seleccionaron artículos originales relacionados al tema, y se creó un protocolo específico para la extracción de los datos. En total se encontraron 2.385 artículos. De ellos, 2.354 fueron excluidos por el título, 13 por el resumen y 3 luego de la lectura del texto completo. Finalmente, a partir de la metodología desarrollada, en esta revisión fueron utilizados 33 artículos. A partir de la revisión realizada se concluye que los parámetros más utilizados en la evaluación son la hipernasalidad, la emisión nasal y la articulación compensatoria asociada a IVF. Estos parámetros son evaluados principalmente en oraciones, habla espontánea y palabras, por un fonoaudiólogo experto, en vivo y mediante grabaciones de audio

    Disfluencias típicas del habla en niños fluentes de Santiago de Chile

    The study of fluency of speech, at different stages of the life cycle, is relevant in that it contributes both to the understanding of typical speech development and to the understanding of pathological speech, which is useful for diagnosis and treatment. The objectives of this work are a) to determine the presence and distribution of the disfluencies present in different age groups and b) to determine if there is variation in the distribution of disfluencies in relation to the gender of the informants. The sample consisted of 60 children and adolescents between 4.01 years old and 15.0 years old distributed in three groups. The results indicate that vowel prolongation was the most frequent disfluency, while syllable repetition was the least present in the groups analyzed. In the group of younger women, there was a significantly high amount of word repetition when comparing the presence of this disfluency in the other two groups. There were no more differences reported in relation to the gender of the informants. Finally, the results and some considerations related to the methodology in this type of research are discussed.El estudio de la fluidez del habla, en distintas etapas del ciclo vital, es relevante porque contribuye tanto a la comprensión del desarrollo típico del habla como a la comprensión del habla patológica, lo que aporta al diagnóstico y al tratamiento. Los objetivos del trabajo son: a) determinar la presencia y distribución de las disfluencias en distintos grupos etarios y b) determinar si existe variación en la distribución de las disfluencias en relación con el género de los informantes. La muestra estuvo compuesta por 60 niños y adolescentes entre los 4.01 años y 15.0 años distribuidos en tres grupos. Los resultados indican que la prolongación de vocal fue la disfluencia más frecuente, mientras que la repetición de sílaba fue la menos presente en los grupos analizados. En el grupo de mujeres de menor edad hubo una cantidad significativamente alta de repetición de palabras. No se encontraron otras diferencias en relación con el género de los informantes. Finalmente, se discuten los resultados y algunas consideraciones relacionadas con la metodología en este tipo de investigaciones

    Dotación fonoaudiológica para la atención de pacientes adultos en hospitales de alta complejidad en Chile

    International institutions have suggested one Speech and Language Therapist per ten critical beds, and possibly even more for high-complexity institutions. However, there are no similar recommendations in Chile, although the Ministry of Health has developed recommendations to calculate the number of professionals required according to the number and type of clinical bed. The aim of this study is to describe the number of Speech and Language Therapists along with the number of theorical working hours per week required for the attention of hospitalised adult patients in high-complexity hospitals in Chile, for which a descriptive, observational and transversal study was performed. High-complexity hospitals were included, excluding psychiatric and paediatric institutions. Microsoft Excel, the “Modelo de Gestión Red de Rehabilitación” guideline from the Ministry of Health, and the updated database of the clinical network beds were also used. This study was previously approved by a Research Ethics Committee from Hospital San Juan de Dios, and a total of 59 out of 188 hospitals were included. According to the analysis based on features of the clinical beds, 269 Speech and Language Therapists are required for 44 hours per week to attend adult patients in high-complexity hospitals across the country. Of the 269 professionals, 104 are needed in the Metropolitan region, approximately 31 in the Bío-Bío region and circa 24 in the Valparaíso region. According to these results, it is suggested the Ministry review its guidelines and undertake a further project in order to identify the current professional gap in Chile.A nivel internacional se propone un mínimo de un profesional fonoaudiólogo por cada diez camas críticas o más, dependiendo de la complejidad de cada institución. En Chile no existen lineamientos similares, no obstante, el Ministerio de Salud ha generado recomendaciones para el cálculo de la brecha profesional según número y complejidad de las camas hospitalarias. El objetivo de este estudio es estimar el número de fonoaudiólogos y horas de contratación teóricas requeridas para la atención de pacientes adultos hospitalizados en instituciones públicas de alta complejidad en Chile. Para ello se realizó un estudio descriptivo, observacional, transversal. Se incluyeron todos los hospitales públicos de alta complejidad del país, excluyendo aquellos pediátricos y psiquiátricos. Como instrumentos se utilizaron el documento “Modelo de Gestión Red de Rehabilitación” del Ministerio de Salud, la base de datos actualizada del total de camas de la red hospitalaria y el software Microsoft Excel. El estudio fue aprobado por Comité Ético Científico del Hospital San Juan de Dios. En total 59 de 188 hospitales existentes en Chile fueron incluidos. De acuerdo con el análisis realizado basado en el número de camas en Chile, se requieren aproximadamente 269 fonoaudiólogos contratados 44 horas semanales para la atención de pacientes adultos en hospitales de alta complejidad. De ellos 104 se ubican en la Región Metropolitana; alrededor de 31 en la Región del BíoBío y cerca de 24 en la Región de Valparaíso. Se sugiere la revisión de las orientaciones ministeriales y la realización de un segundo estudio sobre la brecha profesional existente en Chile con los datos obtenidos

    Recomendaciones para la atención fonoaudiológica en las áreas de audiología y otoneurología en contexto COVID-19: Una revisión narrativa

    The COVID-19 pandemic caused by the SARS-CoV-2 virus has rapidly spread worldwide. Public health authorities have recommended limiting non-essential healthcare, in search of a balance between the risk of delaying treatment and the potential exposure to the virus. Most services in the audiology and otoneurology areas can easily exceed 45 minutes and it is not possible to keep the recommended physical distancing. Both factors increase the risk of professionals being infected. Therefore, the purpose of this review is to describe the current evidence about recommendations in audiology and otoneurology care in the context of COVID-19. In addition to the search of scientific articles in various databases, recommendations issued by the main associations in the area were consulted. The information was gathered considering four relevant clinical aspects: health care services, procedures, personal protective equipment and the cleaning of equipment and supplies. Scientific evidence and suggestions made by associations are being constantly updated, and therefore there is limited content on some topics. However, the recommendations agree on prioritizing remote health care, increasing the use of personal protective equipment, implementing protocols for hand hygiene and for the cleaning of equipment and supplies. The incorporation communication strategies other than oral language to interact with people with hearing loss to facilitate speech discrimination is also recommended.La pandemia por el virus SARS-CoV-2 causante de la enfermedad COVID-19 se ha expandido rápidamente a nivel mundial. Las autoridades de salud pública han recomendado limitar las prestaciones de salud no esenciales, buscando el equilibrio entre el riesgo del retraso en el tratamiento y la potencial exposición al virus. En la mayoría de las prestaciones en las áreas de audiología y otoneurología no es posible cumplir con el distanciamiento físico recomendado y la duración de la atención puede superar los 45 minutos, factores que aumentan el riesgo de contagio para el profesional. El objetivo de la presente revisión es describir la evidencia actual sobre las recomendaciones de atención para las áreas de audiología y otoneurología en contexto COVID-19. Además de la búsqueda de artículos científicos en diversas bases de datos, se revisaron las recomendaciones emitidas por las principales asociaciones en el área. La información obtenida se organizó considerando cuatro aspectos clínicos relevantes: prestaciones, procedimientos, elementos de protección personal y limpieza de equipamiento e insumos. La evidencia científica y las sugerencias de las asociaciones están en constante actualización y algunos temas se han desarrollado escasamente. Sin embargo, las recomendaciones coinciden en priorizar la atención a distancia, extremar el uso de elementos de protección personal, implementar protocolos de higiene de manos y limpieza de equipos e insumos. También, se sugiere incorporar estrategias de comunicación alternativas al lenguaje oral con las personas con hipoacusia para facilitar la discriminación de la palabra
