    Klinički značaj cirkulišućih miR-21, miR-142, miR-143 i miR-146a u pacijenata sa karcinomom prostate

    Background: Prostate cancer (PCa) is the most common type of solid tissue cancer among men in western countries. In this study, we determined the levels of circulating miR-21, miR-142, miR-143, miR-146a, and RNU 44 levels as controls for early diagnosis of PCa. Methods: The circulating miRNA levels in peripheral blood samples from 43 localized PCa patients, 12 metastatic PCa (MET) patients, and a control group of, 42 benign prostate hyperplasia (BPH) patients with a total of 97 volunteers were determined the by PCR method. Results: No differences in the Delta CT values were found among the groups. In PCa and PCaMet groups the expression of miR21 and miR142 were higher compared to the BHP group. No other differences were observed among the other groups. miR21 expression in the PCa group was 6.29 folds upregulated whereas in the PCaMet group 10.84 folds up-regulated. When the total expression of miR142 is evaluated, it showed a positive correlation with mir21 and mir 146 (both p<0.001). Also, the expression of miR146 shows a positive correlation with both miR21 and miR143 (both p<0.001). Expression of miRNAs was found to be an independent diagnostic factor in patients with Gleason score, PSA, and free PSA levels. Conclusions: Our study showed that co-expression of miR21, miR-142, miR-143, and miR-146a and the upregulation of miR-21 resulted in increased prostate carcinoma cell growth. In the PCaMet group, miR21 is the most upregulated of all miRNAs. These markers may provide a novel diagnostic tool to help diagnose PCa with aggressive behavior.Uvod: Karcinom prostate (PCa) je naj~e{}i tip raka ~vrstog tkiva me|u mu{karcima u zapadnim zemljama. U ovoj stu - diji, odredili smo nivoe cirkuli{u}ih miR-21, miR-142, miR-143, miR-146a i nivoe RNU 44 kao kontrole za ranu dijagnozu PCa. Metode: Nivoi cirkuli{u}e miRNA u uzorcima periferne krvi odre|ivani su PCR metodom kod 43 pacijenta sa otkri - venim PCa, 12 pacijenata sa metastatskim PCa (MET) i kao kontrolnom grupom kod 42 pacijenta sa benignom hiperpla zijom prostate (BPH) u ukupno 97 dobrovoljaca. Rezultati: Nisu prona|ene razlike u vrednostima DCT me|u grupama. U grupama PCa i PCaMet ekspresija miR21 i miR142 bila je ve}a u pore|enju sa BHP grupom. Nisu uo~ene druge razlike me|u ostalim grupama. Ekspresija miR-21 u grupi PCa bila je 6,29 puta lo{ije regulisana, dok je u grupama PCaMet regulacija bila pove}ana 10,84 puta. Kada se proceni ukupna ekspresija miR-142, ona je pokazala po - zitivnu korelaciju sa miR-21 i miR-146 (oba p<0,001). Tako|e ekspresija miR-146 pokazuje pozitivnu korelaciju sa oba miR-21 i miR-143 (oba p<0,001). Utvr|eno je da je ekspresija miRNA nezavisan dijagnosti~ki faktor kod pacijenata da Gleason skorom, i nivoima PSA i slobodne PSA

    Sınırlı Erişim, Early accessObjectiveThe most significant complication in familial mediterranean fever (FMF) patients is dysfunction and organ failure developing depending on amyloid deposition in organs. The golden standard for showing amyloid deposition is the biopsy; however, tissue stiffness was examined by shear wave elastography as a non-invasive method in a restricted number of studies conducted, and it is considered that amyloid deposition can be shown indirectly. In our study, we aimed to indirectly evaluate amyloid deposition in organs with Shear wave and Doppler ultrasonography and to reveal its relationship with MEFV gene mutation analysis. Method42 FMF patients with normal thyroid and renal function tests and 35 participants with no FMF symptoms were included in our study. FMF patients were grouped depending on their MEFV mutation analyses. Thyroid, salivary glands, and renal parenchymal tissue stiffness were evaluated by shear wave elastography. Thyroidal artery and both renal artery resistances were evaluated by Doppler ultrasonography. ResultsBoth parotis gland, thyroid and renal parenchymal stiffness and arterial vascular resistances in the patient group were found higher than the control group. A significant difference was not found in any parameters in classification based on gender. Tissue stiffness and vascular resistance values in the patient group with M694V homozygote mutation were found statistically significantly higher than the other mutation groups (p < 0.001). ConclusionOur study shows that identifying genetic mutation type in FMF patients will help determine possibly amyloidosis risk. Imaging of tissue stiffness by shear wave elastography and evaluation of vascular resistance by Doppler can be useful for routine screening of those patients

    miRNA-129-3P Expression in Synovial Fluid of Patients with Osteoarthritis

    Objective: The aim of this study was to investigate the miRNA-129-3p expressions in the synovial fluid of patients with a primary knee osteoarthritis (OA), and thereby contribution to the elucidating underlying molecular mechanisms in OA pathophysiology. Methods: Patient group included 31 individuals with an advanced knee OA. A total of 13 patients with anterior cruciate ligament rupture who had no cartilage damage, were chosen as a control group. Synovial fluid samples were collected during the total knee arthroplasty and an arthroscopic reconstruction. After the centrifugation, samples were stored in a -80 °C cooler. The miRNA-129-3p expressions were examined by reverse transcriptase-polymerase chain reaction using the RNU44 molecule as a reference. The 2-ΔΔCt method was used to calculate the expression of molecules. Statistical comparisons were undertaken by the Student’s t-test. Pearson’s chi-square test was used to determine the differences in the ratios or relationships between the categorical variables. Statistical significance was set at p<0.05 for all the cases. Results: In this study, the miRNA-129-3p, which we thought to be associated with the ciliogenesis, IL-17, and osteoprotegerin, was found 1.54 times higher in the synovial fluid of the patients compared to a control group (p<0.01). Conclusion: It is thought that the miRNA-129-3p may play a role in the OA pathophysiology. Extensive studies are required to use the miRNA-129-3p as a biomarker, or a treatment target for OA

    Iatrogenic colon perforation during colonoscopy, diagnosis/treatment, and follow-up processes: A single-center experience

    Objective: latrogenic colon perforation (ICP) is one of the most feared complications of colonoscopy and causes unwanted morbidity and mortality. In this study, we aimed to discuss the characteristics of the cases of ICP we encountered in our endoscopy clinic, its etiology, our treatment approaches, and results in the light of the current literature. Material and Methods: We retrospectively evaluated the cases of ICP among 9.709 lower gastrointestinal system endoscopy procedures (colonoscopy + rectosigmoidoscopy) performed for diagnostic purposes in our endoscopy clinic during 2002-2020. Results: A total of seven cases of ICP were detected. The diagnosis was made during the procedure in six patients and after eight hours in one patient, and their treatment was performed urgently. Whereas surgical procedures were performed in all patients, the type of the procedure varied; laparoscopic primary repair was performed in two patients and laparotomy in five patients. In the patients who underwent laparotomy, primary repair was performed in three patients, partial colon resection and end-to-end anastomosis in one patient, and loop colostomy in one patient. The patients were hospitalized for an average of 7.14 days. The patients who did not develop complications in the postoperative follow-up were discharged with full recovery. Conclusion: Prompt diagnosis and appropriate treatment of ICP is crucial to prevent morbidity and mortality

    Prasugrel-related hepatotoxicity

    Prasugrel, daha güçlü ve daha hızlı antitrombotik aktivasyonu nedeniyle, akut koroner sendrom sırasında perkütan koroner girişim uygulanan hastalarda tekrarlayan koroner tromboz riskini azaltmak için genellikle Klopidogrel'e tercih edilir. Pazarlama sonrası gözetimde hafif ila orta dereceli alanin transaminaz (ALT) ve gama glutamil transpeptidaz (GGT) artışları fark edilmesine rağmen Prasugrel'in neden olduğu hepatotoksisite hakkında çok az şey bilinmektedir. Burada Prasugrel'e bağlı hepatotoksisitesi olan ve Prasugrel'den Ticagrelor'a geçtikten sonra geri dönen bir hastayı sunuyoruz.Prasugrel is usually preferred over Clopidogrel to reduce the risk of recurrent coronary thrombosis in patients who undergo percutaneous coronary interventions during an acute coronary syndrome owing to its more potent and more rapid antithrombotic activation. Little is known about Prasugrel-induced hepatotoxicity, although mild-to-moderate alanine transaminase (ALT) and gamma glutamyl transpeptidase (GGT) elevations have been noticed in post-marketing surveillance. Herein, we report the case of a patient with Prasugrel-related hepatotoxicity that was reverted after switching from Prasugrel to Ticagrelor

    A decision support system for reducing the strategic risk in the schedule building process for network carrier airline operations

    This study addresses the evaluation of schedule time window of a new frequency for a network carrier airline. The ideal schedule for an airline can involve various criteria that consist of commercial and operational constraints. This study proposes a new integrated Best–Worst Method and Technique for Order Preference by Similarity to Ideal Solution based on heterogeneous decision making approach for determining the most suitable schedule. This approach combines the advantages of multi-expert multi-criteria decision analysis, which yields heterogeneous information, with a developed decision making model. In addition, a sensitivity analysis is performed to observe the robustness of the proposed approach. To illustrate the efficiency of the proposed approach, a real world problem at a network carrier airline in Turkey is presented. The results indicate that the flexibility and applicability of the proposed approach can address real-world problems

    Acute effect of static stretching, PNF and motor imagery on hamstring flexibility

J Strength Cond Res 23(2): 660-667, 2009-This study compared the efficacy of 4 different hamstring-stretching techniques. Flexibility can be achieved by a variety of stretching techniques, yet little research has been performed on the most effective method. The 2 basic types include active stretching, in which range of motion is increased through voluntary contraction, and passive stretching, in which range of motion is increased through external assistance. The 2 types of active stretching include neuromobilization and proprioceptive neuromuscular facilitation (PNF). Our study aims to determine which type of stretching technique is most effective in improving hamstring length. One hundred subjects between the ages of 21 and 57 were enrolled in the study. Intrarater reliability of hamstring length measurement was performed using 10 subjects. All 100 subjects were included in a randomized controlled trial of 5 different groups comparing different hamstring-stretching techniques. Outcome measures, including hamstring length and perceived level of hamstring tightness, were recorded on all subjects initially, at 4 weeks, and at 8 weeks. After 4 weeks of stretching, there was a statistically significant improvement in hamstring length (p < 0.05) using active stretches as compared with passive stretches. From weeks 4 through 8, hamstring length for the active stretching groups decreased. After 8 weeks of stretching, the straight leg raise (SLR) passive stretch group had the greatest improvement in hamstring length. There was no correlation between hamstring flexibility and age, initial tightness, or frequency of exercise per week. Improvement in hamstring flexibility was greatest for the SLR passive stretch. Also, using PNF in the 90/90 active stretch provided better knee range-of-motion improvements than the 90/90 passive methods did

    Synthesis and in vitro antitumor activities of novel thioamide substituted piperazinyl-1,2,4-triazines

    Triazines are in great interest for their potential to mimic nucleoside analog compounds. Three different isomers exist including 1,2,3-triazine, 1,3,5-triazine (s-triazine) and 1,2,4-triazine. All of these skeletons were investigated. Among them, 5,6-diaryltriazines were previously tested on MCF7 and other tumor cell lines. In this study, we have synthesized and characterized 9 novel 5,6-diaryl-1,2,4-triazine derivatives (4a-i) using (HNMR)-H-1, (CNMR)-C-13, and HRMS spectra. Contributing to the previous data, we tested our compounds on U87MG, C6 and SH-SY5Y cell lines along with MCF7. We evaluated if these compounds affect CNS tumors in vitro, which cell lines have different protein profiles from MCF7 and mainly nucleosides are used as their antiproliferative agents. Briefly, compounds showed good antitumor activity on the C6 cell line with up to 50% inhibition for 7 of 9 compounds (4b, 4d-i) at 50 mu M. Additionally, compounds showed moderate activity on U87MG glioblastomas, compound 4a was the most active with 29.68 mu M IC50. Compounds did not show significant activity on SH-SY5Y cells. Cytotoxicity evaluation gave the results that the synthesized compounds show moderate cytotoxicity on NIH-3T3 and severe cytotoxicity on HEK 293.     Evaluation of cognitive deficits in patients infected with COVID-19

    OBJECTIVE: COVID-19 infection can cause impairments in many cognitive areas. The aim of the present study was to evaluate the cognitive functions of patients who had been infected with COVID-19. PATIENTS AND METHODS: The demographic and infection-related characteristics of patients who had been infected with COVID-19 were determined. Their cranial magnetic resonance imaging (MRI) and electroencephalography (EEG) findings were recorded. The Mini-Mental State Evaluation (MMSE). clock drawing test, forward and backward digit span tests. visual memory test. and Frontal Assessment Battery were applied to the patients. Finger agnosia and ideomotor apraxia were also determined. RESULTS: The study included 176 patients [100 female (56.8%). 76 male (43.2%), mean age 66.09 +/- 13.96 years]. About half of the patients were hospitalized for symptoms of COVID-19 infection (n=82, 46.6%). One third of these patients required intensive care (n=26. 14.8%). While 50 (45.9%) of the 109 patients diagnosed with dementia before infection were hospitalized. 32 (47.8%) of the 67 patients without a diagnosis of dementia required hospitalization (p=0.46). The most common neurological finding during COVID-19 infection was insomnia (n=36. 20.5%). The MMSE and visual memory test scores of the patients who were hospitalized for severe respiratory distress were lower than those whose treatment at home was completed (respectively 17.92 +/- 7.69/20.59 +/- 7.01, p=0.02; 2.53 +/- 1.73/3.69 +/- 2.80. p=0.01). The patients with moderate to severe cognitive impairment had significantly higher CRP levels at admission than the others (37.52 +/- 43.09/20.93 +/- 31.74, p=0.01, respectively). CONCLUSIONS: Cognitive damage in COVID-19 infection may be caused by ACE receptor density in the pial, hippocampal, and amygdala areas. In addition, the reason why people with severe dementia have a milder infection might be explained by the atrophy in these areas

    Skolyoz, omurganın sagital ve frontal düzlemlerindeki farklılaşma ve omurganın 10 dereceden fazla eğilmesiyle kendini gösteren üç boyutlu bir anomalidir. Skolyoz, vertebraya ait yapısal problemler, nöromusküler problemler, ekstremite boy farkı gibi sebeplerle de meydana gelebilir fakat skolyozun büyük çoğunluğu (>%80) idiopatiktir. Bireyin idiyopatik skolyoz tanısı alması için ilk olarak skolyoza neden olabilen diğer muhtemel sebeplerin ekarte edilmesi gerekir. Skolyozun tedavi seçeneklerinde sıklıkla kullanılan yöntemlerden biri egzersizdir. Skolyoz Ortopedi ve Rehabilitasyon Tedavisi Derneğince skolyoza özgü egzersiz çeşitleri “Skolyoza Özel Fizyoterapi Egzersizleri” adı altında tanımlanır. Katharina Schroth tarafından geliştirilen Schroth metodu, 1921 yılından itibaren en yaygın şekilde uygulanan egzersiz tekniğidir. Bu egzersizlere 3 boyutlu denilmesinin sebebi tedavinin sagital, frontal ve horizontal düzlemlerde yapılan egzersizleri aynı anda içeriyor olmasıdır. Schroth egzersiz programı; özel solunum yöntemleri ile proprioseptif uyarıları ve sagital planda görsel geribildirim denetimini kullanarak gerçekleştirilen postür düzeltmelerinden oluşmaktadır. Literatürde, Schroth yönteminin idiopatik skolyozda Cobb açısında azalma ve hayat kalitesini iyileştirmede seviye II düzeyinde bilimsel kanıtları mevcuttur. Bu derleme makalede Schroth metodunun idiyopatik skolyozlu hastalar üzerindeki etkilerinin değerlendirilmesi, hastalar üzerinde çeşitli parametrelerdeki değişiklikler incelenmektedir.Scoliosis is a three-dimensional deformity characterized by a deviation of the spine from its normal sagittal and frontal planes, with the spine bending over 10 degrees. Scoliosis can also occur due to structural problems of the vertebrae, neuromuscular problems, short legs, but a large part (>80%) of scoliosis is idiopathic. In order to be called idiopathic scoliosis, first of all, other possible causes that may cause scoliosis should be ruled out. One of the most commonly used method in the conservative treatment of scoliosis is exercise therapy. Scoliosis-specific exercises are defined as “Scoliosis-Specific Physiotherapy Exercises” by the International Society on Scoliosis Orthopedic and Rehabilitation Treatment (SOSORT). The Schroth method, developed by Katharina Schroth, is the most frequently applied three-dimensional exercise technique since 1921. The reason it is called 3D is that the treatment includes exercises performed in the sagittal, frontal and horizontal planes simultaneously. This treatment program; consists of postural corrections using proprioceptive stimuli with special breathing patterns and mirror control in the sagittal plane. In the literature, there is level II scientific evidence of the Schroth method in reducing the Cobb angle and improving the quality of life in adolescent idiopathic scoliosis.
