    Is Online Video a Suitable Source to Obtain Sufficient and Useful Information About Peri-Implantitis?

    Purpose: Recently, with the increase in cases of peri-implantitis, the amount of data patients can receive via social media about the condition has increased. The study presented here aims to examine what online videos (YouTubeTM, Google LLC, San Bruno, California) offer patients about peri-implantitis and to evaluate the quality of the information presented.Materials and Methods: In this cross-sectional study, a systematic search was conducted into online videos containing information about peri-implantitis using the keyword "peri-implantitis" by an experienced periodontologist. The source of videos, video type, duration, number of days since upload, number of views, comments, likes, dislikes, and interaction index of each video were recorded. Videos were scored according to content; Score 0 (low content): videos with no description of clinical presentations, risk factors, or management options; Score 1 (moderate content): videos that described 1 clinical presentation and 1 etiologic factor and did not describe management options; Score 2 (high content): videos with the description of at least 2 clinical presentations, 2 etiologic factors, and 1 management option. The quality of each video was evaluated using the Global Quality Scale and DISCERN. Data were analyzed using the Kruskal-Wallis test, Spearman's test, and multinomial logistic regression analysis. Intraobserver agreement was calculated as the intraclass correlation coefficient.Results: The first 200 videos were screened and, after exclusions, 103 videos were included for further analysis. United States-based videos were the majority (14 videos), 46.6% of the videos were uploaded by healthcare professionals, and 99% were in the educational videos category. Video content score 2 videos have higher mean values with 4,871.945 views, 6.002 comments, 50.729 likes, 7.751 viewing rate, and 0.023 interaction index than score 0 videos (P < .05). Statistically significant positive correlations were found between the video content score, Global Quality Scale, DISCERN, and the data showing the popularity and visibility of the videos (P < .01).Conclusions: Overall, half of the top-ranked online videos are valuable sources of information about peri-implantitis.     The effect of smartphone addiction on vessel wall thickness, which is a predictor of atherosclerosis

    Aim:The increasing global popularity of smartphone usage has increased concerns about the negative effects of smartphone addiction, such as lack of sleep, sedentary life, bad eating habits, anxiety, stress, and depression, especially among the young population. These problems caused by smartphone addiction are also well-known risk factors for atherosclerosis. However, according to our observation, there is no research in the literature that directly shows the relationship between smartphone addiction and subclinical atherosclerosis. In this study, carotid intima-media thickness, an important surrogate marker in demonstrating subclinical atherosclerosis, was used to examine the relationship between smartphone addiction and subclinical atherosclerosis. Materials and method:This cross-sectional study was conducted on 96 high school students aged between 13 and 22 years. A smartphone addiction questionnaire consisting of 33 questions was applied to measure smartphone addiction. Along with the socio-demographic characteristics of the patients, factors such as eating habits, sleep patterns, and activity levels were also questioned. The mean carotid intima-media thickness was measured by an experienced sonographer according to the published and accepted methods. Results:When we set the threshold for smartphone addiction at over 66 points, we discovered that the group with smartphone addiction had considerably thicker carotid intima-media (0.68 & PLUSMN; 0.2 versus 0.45 & PLUSMN; 0.1; p < 0.001). In addition, logistics regression analysis had shown that smartphone addiction level independently affects the carotid intima-media thickness (odds ratio:1.111; %95 GA:1.057-1.168, p < 0.001). Conclusion:Smartphone addiction may help prediction of subclinical atherosclerosis via carotid intima-media thickness among teenagers

    Could Long Non-Coding RNA MEG3 and PTENP1 Interact with miR-21 in the Pathogenesis of Non-Alcoholic Fatty Liver Disease?

    NAFLD is the most common cause of chronic liver disease worldwide. The miRNAs and lncRNAs are important endogenous ncRNAs families that can regulate molecular mechanisms. The aim of this study was to analyze the miRNA and lncRNA expression profiles in serum samples of NAFLD patients with different types of hepatosteatosis compared to healthy controls by the qPCR method. A total of180 NAFLD patients and 60 healthy controls were included. miRCURY LNA miRNA miRNome PCR human panel I + II kit and LncProfiler qPCR Array Kit were used to detect miRNA and lncRNA expression, respectively. DIANA miRPath and DIANA-lncBase web servers were used for interaction analysis. As a result, 75 miRNA and 24 lncRNA expression changes were determined. For miRNAs and lncRNAs, 30 and 5 were downregulated and 45 and 19 were upregulated, respectively. hsa-miR-21 was upregulated 2-fold whereas miR-197 was downregulated 0.25-fold. Among lncRNAs, NEAT1 was upregulated 2.9-fold while lncRNA MEG3 was downregulated 0.41-fold. A weak correlation was found between hsa-miR-122 and lncRNA MALAT1. As a conclusion, it is clear that lncRNA-miRNA interaction is involved in the molecular mechanisms of the emergence of NAFLD. The lncRNAs MEG3 and PTENP1 interacted with hsa-miR-21. It was thought that this interaction should be investigated as a biomarker for the development of NAFLD

    Total larenjektomili bireylerin yaşam kalite düzeylerinin incelenmesi = Examination of quality of life levels of individuals with total laryngectomy

    Total larenjektomi (TL) tüm larengeal yapıların ve üst trakeanın bir bölümünün cerrahi olarak alınmasını içeren bir ameliyat olarak tanımlanmaktadır. TL'li bireyler için başarılı ses restorasyonu üç şekilde gerçekleşmektedir: Özofageal konuşma (ÖK), elektrolarinks ve trakeoözofageal konuşma (TK). TL ameliyatı sonrasında bireylerin "yeni" seslerine alışma süreçleri zaman alabilmektedir. İletişim becerilerine ek olarak TL'li bireyler farklı alanlarda birtakım sorunlar yaşabilmektedir. Bu sorunları ne şiddette yaşadıklarını incelemek için araştırmacılar TL'li bireylerin yaşam kalitesi (YK) düzeylerini değerlendirmek üzere birtakım ölçüm araçları kullanmaktadır. Bu çalışmada literatürde yaygın olarak kullanılan ve YK düzeylerini inceleyen iki araç (TR-SECEL ile TR-UW-QOL) 39 TL'li bireye uygulanmıştır. Demografik (yaş ve eğitim), alerengeal iletişim aracı (ÖK ve TK) ve TL sonrası geçen süre değişkenlerine göre TR-SECEL ve TR-UW-QOL puanları karşılaştırılmıştır. TR-SECEL puan ortalamaları değerlendirildiğinde yaş ve eğitim grupları arasında anlamlı farklılık olmadığını, alerengeal iletişim aracı açısından Genel alt alan puan ortalamalarının TK kullanan bireylerde anlamlı olarak daha yüksek gözlendiğini, TL sonrası geçen süre açısından farklı üç grup arasında TR-SECEL puanlarının farklılaşmadığını, yaş değişkeni ile Genel alt alan puan ortalamaları arasında pozitif ve orta düzey bir ilişki olduğunu göstermiştir. TR-UW-QOL puan ortalamaları incelendiğinde, yaş gruplarında 60 yaş ve üzerindeki katılımcıların Moral maddesine ait puan ortalamalarının diğer yaş grubundan anlamlı olarak daha yüksek olduğunu, eğitim gruplarında 9 yıl ve üzeri eğitim alan katılımcıların Endişe maddesine ait puan ortalamalarının diğer eğitim grubundan anlamlı olarak daha yüksek olduğunu, alerengeal iletişim aracı açısından Görünüş ve Omuz maddelerine ait puan ortalamalarının sırasıyla TK ve ÖK kullanan katılımcılar lehine yüksek gözlendiğini, TL sonrası geçen süre açısından Görünüş maddesinde TL sonrası 121 ay ve üzeri geçen katılımcılara ait puan ortalamalarının 8-63 ay aralığındaki katılımcılara ait değerlerden anlamlı olarak daha yüksek olduğunu ortaya koymuştur. Sonuç olarak, herhangi bir alerengeal iletişim aracının diğerinden daha öncelikli ya da avantajlı olduğunu ifade etmek çok mümkün görünmemektedir.Total laryngectomy (TL) is defined as an operation involving surgical removal of laryngeal structures and part of upper trachea. Oesophageal speech (OS), electrolarynx and tracheoesophageal speech (TS) are among options for voice restoration in TL. After TL surgery, it may take time for individuals with TL (IwTL) to adapt to their "new" voice. IwTL may experience problems in different areas including communication. Researchers use measurement tools to assess quality of life (QoL) levels of IwTL to examine the severity of these problems. In this study, two instruments (TR-SECEL and TR-UW-QOL), widely used in literature to examine QoL, were applied to 39 IwTL. The scores were compared according to demographic variables (age and education), medium of alerengeal communication (SC and TC) and time after surgery. Findings of TR-SECEL showed there was no significant difference between age and education groups, General subdomain score means were significantly higher in IwTL who used TS, scores did not differ between three groups in terms of time after surgery, there was positive and moderate relationship between age and General subdomain scores. Regarding TR-UW-QOL, mean scores of Mood item among IwTL aged 60 years and over were significantly higher than other group, and those of Anxiety item of IwTL with 9 years of education or more in the education group were significantly higher than other group, the mean scores of the Appearance and Shoulder items were higher in favour of IwTL who used TS and OS, respectively, and in terms of time after surgery, mean scores of IwTL with a duration of 121 months or more after TL in Appearance item were significantly higher than values of those in the range of 8-63 months. In conclusion, it does not seem possible to state any alaryngeal communication medium is more prioritised or advantageous than the other

    Bu araştırmada, okul öncesi dönemde OSB'li olan çocukların iletişimsel katılım becerilerinin incelenmesi amaçlanmıştır. Çalışmada, otizm spektrum bozukluğu olan çocuklar ile tipik gelişim (TG) gösteren çocukların iletişimsel katılım becerileri karşılaştırılmıştır. Bu doğrultuda çalışmanın önemi; OSB'li olan çocukların iletişim becerilerinde yaşanan sorunları belirlemek ve saptanan problemlerin dil ve konuşma terapilerinde giderilmesine kaynak oluşturmaktır. Araştırma, yüz yüze ebeveyn ve klinisyen görüşmeleriyle gerçekleştirilmiştir. Katılımcı deney grubunu; özel eğitim ve rehabilitasyon merkezinde eğitim alan 20 -72 aylık OSB tanısı olan çocuklar ve ebeveynleri oluşturmaktadır. Katılımcı kontrol grubunu; kreş ya da anaokuluna gitmeye devam eden 20-72 aylık tipik gelişim gösteren çocuklar ve ebeveynleri oluşturmaktadır. Çalışmaya OSB tanılı 66 çocuk ve Tipik Gelişim (TG) gösteren 68 çocuk dahil edilmiştir. Araştırmada kullanılması için, "Gönüllü Onam Formu", "Demografik Bilgi Formu" ve "FOCUS- 34-TR" ölçeği olmak üzere 3 adet form kullanılmıştır. Araştırmaya katılan ebeveynler 3 formu da doldururken dil ve konuşma terapistleri ve anaokulu öğretmenleri ''Gönüllü Onam Formu'' ve ''FOCUS-34-TR'' ölçeği doldurulmuştur. Nitel veriler sayı ve yüzde, nicel veriler ise ortalama, standart sapma, ortanca, minimum ve maksimum istatistikleri ile özetlenmiştir. Bağımsız iki örneklem karşılaştırmaları yeterli örneklem büyüklüklerinde t testi ile aksi halde Mann Whitney U testi ile çözümlenmiştir. Bağımsız ikiden çok grup karşılaştırmaları ise Kruskall-Wallis H testi ile yapılmıştır. Anlamlılık düzeyi için 0.05 kabul edilmiştir. İstatistiksel çözümlemeler SPSS (versiyon 26) programı ile yapılmıştır. Araştırmanın bulgularına göre, OSB grubu çocukların dil gelişiminde TG grubuna göre genel olarak daha geri olduğu bulunmuştur (p<0,05). Bu durum, OSB'nin dil gelişimi üzerinde olumsuz bir etkisi olduğunu göstermektedir. Araştırmanın bulguları, OSB ve TG olan çocukların dil gelişimini etkileyebilecek faktörleri de ortaya koymaktadır. Bu faktörler arasında ebeveyn eğitimi, doğum şekli ve hamilelik sırasında ilaç kullanımı sayılabilir.In this study, it was aimed to examine the communicative participation skills of preschool children with ASD. In the study, the communicative participation skills of children with ASD and children with typical development (TD) were compared. In this direction, the importance of the study is to determine the problems experienced in the communication skills of children with ASD and to provide a source for the elimination of the identified problems in speech and language therapies. The research was conducted through face-to-face parent and clinician interviews. Participant experimental group consists of children that diagnosed with ASD between the ages of 20 and 72 months who receive education in a special education and rehabilitation center, and their parents. Participant control group consists of typically developing children aged 20-72 months who continue to attend nursery or kindergarten and their parents. The study included 66 children diagnosed with ASD and 68 children with typical development (TD). Three forms, namely "Voluntary Consent Form", "Demographic Information Form" and "FOCUS-34-TR" scale were used in the study. While the parents participating in the study filled out all 3 forms, speech and language therapists and kindergarten teachers filled out the "Voluntary Consent Form" and the "FOCUS-34-TR" scale. Qualitative data were summarised with numbers and percentages, and quantitative data were summarised with mean, standard deviation, median, minimum and maximum statistics. Independent two- sample comparisons were analysed with t-test at adequate sample sizes, otherwise Mann Whitney U test was used. Independent comparisons of more than two groups were analysed by Kruskall-Wallis H test. 0.05 was accepted for significance level. Statistical analyses were performed with SPSS (version 26) programme. According to the findings of the study, it was found that ASD group children were generally more retarded in language development than the TD group. This shows that ASD has a negative effect on language development (p<0,05). The findings of the study also reveal the factors that may affect the language development of children with ASD and TD. These factors include parental education, mode of delivery and medication use during pregnancy

    Is the Acetabular Cup Orientation Different in Robot-Assisted and Conventional Total Hip Arthroplasty With Right-Handed Surgeons Using an Anterolateral Approach?

    Introduction Total hip arthroplasty (THA) is one of the most successful orthopaedic procedures. Survival rates from 90% at 10 years to 93% at 20 years have been reported in different studies. Differences in implant and patient characteristics can undoubtedly explain some of this variability observed in prosthesis durability, but the effect of surgical technique and implant orientation cannot be ignored. Therefore, many intraoperative methods (anatomic landmarks, intraoperative x-ray, fluoroscopy, navigation, and robotic surgery) have been attempted to avoid acetabular component malpositioning. Although postoperative computed tomography (CT) is accepted as the gold standard for the measurement of acetabular anteversion, it remains controversial in respect of costs and radiation exposure. The aim of this study was to examine how acetabular component orientation was affected in robotic and conventional THA operations performed by two surgeons with right-hand dominance. Material and methods The study included 113 primary THA operations performed on 113 patients between 2017 and 2022 in two groups: (i) robotic THA (Mako, Stryker Corporation, Kalamazoo, Michigan, United States) (55 patients) and (ii) conventional THA (58 patients). The patients comprised 51 males and 62 females. THA was performed on 54 right-side hips and 59 left-side hips. The operations were performed by two orthopaedic surgeons, each with 20 years of arthroplasty experience, on all the patients in the lateral decubitus position with an anterolateral approach. In all the cases, the orientation of the acetabular component was 40° inclination and 20° anteversion. Difficult THA procedures (patients with developmental dysplasia of the hip (DDH), a history of hip surgery, revision THA, defect or deformity of the acetabulum, a history of scoliosis or lumbar posterior surgery, or those requiring proximal femoral osteotomy) were excluded from the study. Using the Liaw and Lewinnek methods, the acetabular component anteversion was measured on the radiographs taken in the optimal position postoperatively and the acetabular cup inclination angles were measured on the pelvis radiographs. The groups were compared using the Kolmogorov-Smirnov, Pearson Chi-square and Mann-Whitney U statistical tests. The limits were accepted as 40±5° for inclination and 20±5° for anteversion. Results No statistically significant difference was determined between the groups in respect of age, gender, or operated side. No statistically significant difference was determined between the optimal acetabular cup inclination angles of the robotic and conventional THA groups (p = 0.79). No statistically significant difference was determined between the optimal acetabular cup anteversion angles of the left and right conventional THA groups. Statistically significantly better results were determined in the robotic group in respect of acetabular cup anteversion (p<0,001). Conclusion The optimal orientation of the acetabular component is a key factor for successful THA. Otherwise, revision surgery is inevitable for reasons such as instability, impingement, or increased wear. The results of this study demonstrated that robotic surgery was superior to the conventional method in the placement of the acetabular component in the desired orientation

    Ekran maruziyetinin dil gelişimi üzerindeki etkilerinin incelenmesi = Examining the effects of screen exposure on language development

    Gelişimsel dil ve konuşma bozukluğu her 14 çocuktan birini olumsuz etkileyen en yaygın gelişimsel bozukluklardan biridir. Bu çalışmanın amacı tipik gelişim gösteren ve gelişimsel dil bozukluğu olan çocukların ekran maruziyetine bağlı dil ve konuşma gelişimlerini değerlendirmektir. Çalışmaya 24 – 48 ay arasında 36 gelişimsel dil bozukluğu olan (çalışma grubu, yaş ortalaması 37,53±8,77 ay) ve 24 tipik gelişim gösteren çocuk (kontrol grubu, yaş ortalaması 41,79±8,57 ay) ile bu çocukların ebeveynleri (59 kadın, 1 erkek olmak üzere toplam 60 ebeveyn) dâhil edildi. Tüm çocukların alıcı ve ifade edici dil becerilerini değerlendirmek için Türkçe Erken Dil Gelişim Testi (TEDİL) uygulayıcı formu ve ebeveynlerin ekran maruziyetinin dil gelişimine olumsuz etkilerine yönelik tutum ve farkındalıklarını belirlemek için Problemli Medya Kullanım Ölçeği (Kısa Form) kullanıldı. Çalışmaya katılan çocukların alıcı dil (standart puanları, eş değer yaşları, yaş farkları), ifade edici dil (standart puanları, eş değer yaşları, yaş farkları) ve problemli medya kullanım ölçeği sonuçlarının karşılaştırılmasında ölçüm ortalamaları bakımından gruplar arası fark istatistiksel olarak anlamlı bulundu (p<0,001). Ekrana maruz kalma süresinin her 1 saat artımında çocukların alıcı dil standart puanında ortalama 7,407 puan düşüş gözlendi. Dijital medya ve ekranlar artık çocukların hayatlarının her alanında ve her anında yer almaktadır ve kullanımları her geçen gün artmaktadır. Elde edilen bulgulara göre ekrana maruz kalmak çocukların alıcı ve ifade edici dil becerilerini olumsuz etkilemektedir. Dolayısıyla ekrana maruz kalmanın önerilen sınırlar içerisinde tutulması ve çocukların dil gelişimlerinin desteklenmesi gerekmektedir. The aim of this study was to evaluate the speech and language development of typically developing children and children with developmental language disorder due to screen exposure. The study included 36 children between 24 and 48 months of age with developmental language disorder (study group, mean age 37.53±8.77 months) and 24 typically developing children (control group, mean age 41.79±8.57 months) and their parents (59 females and 1 male, total 60 parents). The Turkish Test of Early Language Development (TEDIL) practitioner form was used to assess the receptive and expressive language skills of all children, and the Problematic Media Use Scale (Short Form) was used to determine parents' attitudes and awareness of the negative effects of screen exposure on language development. In the comparison of the results of receptive language (standard scores, equivalent ages, age differences), expressive language (standard scores, equivalent ages, age differences) and problematic media use scale of the children who participated in the study, the difference between the groups in terms of measurement averages was statistically significant (p<0.001). For every 1 hour increase in screen exposure time, an average decrease of 7.407 points was observed in children's receptive language standard score. Digital media and screens are now present in every aspect and every moment of children's lives and their use is increasing day by day. According to the findings, screen exposure negatively affects children's receptive and expressive language skills.     Integrated optimisation model for airline bank structure and fleet assignment problem

    The airline decision-making process is intended to solve complex operations planning problems sequentially. As computational power increases so do integrated approaches to solving major airline optimisation problems. In general, network carriers encounter more complex integrated problems of fleet planning, aircraft routing and increasing connectivity in the airline network. Bank optimisation problem addresses creating competitive advantage for airline network carriers, by providing better connectivity to passengers via improving their flight schedules in the airline bank structure. The aim of this study is to determine the optimal fleet to cover all flights in a flight schedule which is designed to improve connectivity in the network. The objective function takes account of the overall fleet allocation profitability on the flight schedule, the total worth of passenger connections, route profits, and the penalty cost for using excess slot. We propose a mixed integer linear programming model for the integrated bank optimisation-fleet assignment problem which can be defined as a specific flight scheduling model. To test the model, we have utilised a dataset from a Turkish carrier. The results from the integrated model show that small changes in the bank structure create more efficient schedule and fleet assignment could increase profitability at the same time. The findings imply that network and schedule planners are able to solve two major problems, simultaneously. Advances in the optimisation techniques will increase the application of the integrated models in the other parts of airline planning world, such as operations, crew planning, revenue management
