    Cervical Proprioception in Parkinson's Disease and Its Correlation With Manual Dexterity Function

    ObjectiveaaCervical proprioception plays a crucial role in posture and movement control. This study aimed to determine the relationships of cervical proprioception, cervical muscle strength and endurance with manual dexterity and hand strength in individuals with idiopathic Parkinson’s disease (PD). MethodsaaTwenty individuals with PD (mean age: 63.9 years) and 20 healthy individuals as a control group (mean age: 61.9 years) were recruited. Cervical joint position error (JPE), static endurance of neck muscles, activation of deep cervical flexor muscles (Craniocervical Flexion Test, CCFT), manual dexterity (Purdue Pegboard Test, PPT), cognitive and motor tasks of the PPT, finger tapping test (FTT), pinch strength, and grip strength were assessed. ResultsaaCervical JPE was significantly higher in individuals with PD than in controls (p < 0.05). The strength and endurance of the cervical muscles were significantly decreased in individuals with PD (p < 0.05). Cervical JPE measurements were negatively correlated with PPT, cognitive and motor tasks of the PPT in individuals with PD (all p < 0.05). The endurance of cervical flexor muscles was negatively correlated with PPT and cognitive PPT scores in the PD group (p < 0.05). In addition, a significant positive correlation was found between cervical flexor endurance and hand strength in the PD group (p < 0.05). ConclusionaaCervical proprioception and the strength and endurance of cervical muscles decrease in individuals with PD compared to healthy individuals. Impairment of cervical proprioception appears to be associated with poorer upper extremity performance. Detailed evaluation of the cervical region in PD may help determine the factors affecting upper extremity function

    Relationship between Perceived Stress Level with Menstruation Symptoms and Quality of Life in Women

    Aim: The aim of the study is to investigate the relationship between perceived stress level, menstrual symptoms and quality of life in women aged between 18-50 years. Material and Methods: This study was carried out with the participation of 172 volunteers. The Perceived Stress Scale was used to evaluate the stress levels of the participants, the Menstruation Symptom Scale was used to evaluate the menstrual symptoms, and the Short Form-36 was used to evaluate the quality of life. Simple Linear Regression Analysis was used to examine the effects of demographic characteristics of the participants on menstrual symptoms, and Pearson's or Spearman's Correlation Analyzes were performed in accordance with the distribution of the data to determine the relationship between perceived stress level, menstrual symptoms, and quality of life. Results: The results of our study show that smoking increases menstrual symptoms (p=0.022). Perceived stress level was found to be correlated with all sub-parameters of the Menstruation Symptom Scale at a low level (p<0.05). It was found that there was a moderate and negative correlation with all Short Form-36 sub-parameters, except for the Short Form-36 physical function sub-parameter (p<0.05). Discussion: Our study showed that menstrual symptoms tend to increase as the perceived stress level in women increases. In addition, it has been revealed that the increase in stress level and menstrual symptoms negatively affects the quality of life of women. According to the results of our study, it is recommended that future studies examine the effects of stress reduction practices on women's menstrual symptoms and quality of life

    Antibiotic prescribing practices for prophylaxis and therapy of oral/dental infections in pediatric patients-results of a cross-sectional study in Turkey

    Aim: Antibiotics are often prescribed for the treatment of various infec-tions and prophylactic purposes in dental practice. Their improper use can cause microbial resistance to antibiotics, which poses a world-wide threat. The aim of this cross-sectional study was to evaluate the knowledge and attitudes of dentists and dentistry students about anti-biotic prescription practices for prophylaxis and the treatment of dental infections in pediatric patients.Methods: A questionnaire was e-mailed to 2,100 dentists and 300 senior dentistry students. The questionnaire was filled out by the par tic-ipants within a 2-month period (May-June 2020). A 30-point scoring system was developed to assess the knowledge levels of the dentists according to the guidelines. Descriptive statistical analyses were per-formed. One-way ANOVA test and the Chi-Squared test were used to compare qualitative variables.Results: The response rate was found to be 24.2% for dentists and 49% for senior dentistry students. 19.4% of the participants were found to be moderately knowledgeable and 80.6% of them were highly know-ledgeable. Students' knowledge scores were found to be higher than the general dentists and other specialists (p<0.05). There was no sig-nificant difference between students and pedodontists. Conclusion: Dentists were found to have sufficient knowledge about the usage of antibiotics in children, but there is still a lack of information about circumstances under which antibiotics should not be prescribed. Dentists and dentistry students should attend continuing education programs to keep their information up-to-date and should also prescribe antibiotics in adherence with the current guidelines to prevent antibiotic resistance

    Sternum Varyasyonları ile Primer Spontan Pnomotoraks Aarasındaki İlişkinin BT Bulguları ile Gösterilmesi

    Amaç: Yapısal akciğer bozukluğu, enfeksiyon veya travma gibi belirli etyolojik neden bulunmayan olgular “primer spontan pnömotoraks (PSPn)” olarak isimlendirilmektedir. Çalışmamızın birincil hedefi PSPn tanısı alan hastalarda pektus ekskavatum (PE) ve sternal varyasyonlar ile spontan pnömotoraks arasındaki ilişkiyi değerlendirmektir. Materyal-Metod: Retrospektif ve tek merkezli çalışmamıza Ocak 2016-Şubat 2019 tarihleri arasında Üniversitemiz acil servisine başvuran 30 PSPn olgusu, 30 kontrol grubu olarak toplam 60 olgu dahil edildi. Tüm görüntüler sternal varyasyonlar açısından 8 yıl toraks radyolojisi deneyimi olan, hastaların bilgilerini bilmeyen bir radyolog tarafından değerlendirildi. Haller indeksi, tüm olgular için mediasten penceresinde aksiyel toraks kesitleri üzerinde literatürde tanımlanan şekilde ölçüldü. Pectus excavatum tanısı konulan hastalar için Haller indeksinin 2’nin üzerinde olması kabul edildi. Tüm istatiksel analiz SPSS (version 20.0, SPSS Inc., IL, US) programı ile gerçekleştirildi. Bulgular: Haller indeksine göre hastalar 2 olan iki gruba ayrıldığında her iki grup arasında SPn açısından istatistiksel farklılık izlendi (p=0,009). Sternal varyasyon varlığı PSPn grubunun %50’sinde, kontrol grubunun ise %23’ünde ortaya konuldu. Uzun ksifoid poçes, PSPn grubunda en sık rastlanan sternal varyasyon olarak bulundu (%20). Sonuç: Uzun ksifoid proçes ve pektus ekskavatum deformitesi gibi sternal varyasyonlar, spontan pnömotoraks gelişiminde rol oynamaktadır. Uzun ksifoid proçesin tekrarlayan spontan pnömotoraks nedeni olup olmadığının araştırılması ve operasyon açısından değerlendirme yapılabileceği düşünülmüştür.Objective: Cases without a specific etiological cause such as trauma, infection or lung disorder are identified as “primary spontaneous pneumothorax (PSPn)”. The primary target of our study was to evaluate relationship between sternal variations such as pectus excavatum and SPn in patients diagnosed with PSPn. Material-Method: Total 60 patients (30 PSPn patients and 30 control group) who admitted to the emergency service of University hospital between January 2016 and February 2019 were included in our retrospective and single center study. All images were evaluated by the radiologist with 8 years of thorax imaging experience who is blinded to the patients’ information. Haller index was measured for all cases on axial thorax slice in the mediastinal window as described in the literature. Cases with a Haller index above 2 was accepted for patients diagnosed with pectus excavatum. SPSS software was used for statistical analysis. Results: When patients were separated into two groups as 2 according to Haller index, there was a statistical difference between two groups in SPn (p=0.009). It was revealed that sternum variation is observed in 50% of PSPn group and in 23% of control group. We found that the long xiphoid process is the most frequent variation of the sternum in PSPn group (20%). Conclusion: Sternum anatomic variations such as long xiphoid process and pectus excavatum deformity play an essential role in the development of spontaneous pneumothorax. It was thought that operative evaluation could be made especially for the long xiphoid process in recurrent spontaneous pneumothorax cases

    Bu araştırma profesyonel futbolcuların ses kullanım alışkanlıkları ile ses hijyeni ve ses hastalıkları hakkında bilgi düzeylerini incelemeye yönelik yapılmıştır. Tarama modeli ile yapılan bu araştırmada konuyla ilgili alan yazına ve uzman görüşlerine başvurularak, araştırmanın amaç ve süresine uygun bir veri toplama aracı olarak hazırlanan anket, 12 şehirde görev alan 100'ü erkek, 3'ü kadın toplam 103 profesyonel futbolcuya uygulandı. Ankette sonucunda profesyonel futbolcuların, ses kullanım alışkanlıkları açışından risk taşımadıkları, ses hijyenlerine yeterince önem verdiklerine, ses hastalıkları ve ses mekanizmasına zarar veren davranışlar hakkında bilgi düzeylerinin ortalamanın üzerinde olduğu tespit edildi. Futbol sahasında yoğun ses kullanıcısı olan profesyonel futbolcuların sağlıklı ses kullanımı açısından bilgilerinin artırılabileceği ve ses kullanımı açısından doğru bilgilendirildikleri taktirde ses hastalıklarının azalacağı öngörülmektedir.This research was conducted to examine the voice usage habits of professional football players and Their knowledge levels about voice hygiene and voice diseases. In this research, which was conducted with the screening model, the questionnaire, which was prepared as a data collection tool in accordance with the purpose and duration of the research, was applied to a total of 103 professional football players, 100 male and 3 female, working in 12 cities. As a result of the survey, it was determined that professional football players do not carry any risk in terms of vocal usage habits, they attach sufficient importance to vocal hygiene, and their knowledge level about voice diseases and behaviors that damage the voice mechanism is above the average. It is predicted that the knowledge of professional football players, who are heavy voice users in the football field, in terms of healthy voice use can be increased and voice disorders will decrease if they are properly informed in terms of voice use

    Oral Microbiota Signatures in the Pathogenesis of Euthyroid Hashimoto's Thyroiditis

    One of the most prevalent autoimmune illnesses in the world is Hashimoto's thyroiditis, whose pathogenesis is still unknown. The gut-thyroid axis is frequently examined, and although oral health affects thyroid functions, there are limited data on how oral microbiota is linked to Hashimoto's thyroiditis. The study aims to identify the oral microbiota from saliva samples taken from treated (with levothyroxine) and untreated female euthyroid Hashimoto's thyroiditis patients as well as healthy controls who were age- and sex-matched to compare the oral microbiota across the groups and to contribute preliminary data to the literature. This study was designed as a single-center cross-sectional observational study. Sixty (60) female patients with euthyroid Hashimoto's thyroiditis (HT) and eighteen (18) age- and gender-matched healthy controls were included in this study. Unstimulated saliva samples were collected. After DNA isolation, sequencing was performed by targeting the V3-V4 gene regions of the 16S rRNA on the MiSeq instrument. R scripts and SPSS were used for bioinformatic and statistical analysis. No significant differences were found in the diversity indices. However, Patescibacteria phylum showed a significantly higher abundance (3.59 vs. 1.12; p = 0.022) in the oral microbiota of HT patients compared to HC. In the oral microbiota, the euthyroid HT group had approximately 7, 9, and 10-fold higher levels of the Gemella, Enterococcus, and Bacillus genera levels than healthy controls, respectively. In conclusion, the results of our study demonstrated that Hashimoto's thyroiditis causes changes in the oral microbiota, whereas the medicine used to treat the condition had no such effects. Therefore, revealing the core oral microbiota and long-term follow-up of the HT process by conducting extensive and multicenter studies might provide some important data for understanding the pathogenesis of the disease

    The Groin Region Anatomy and Clinical Anatomical Approach to Athletic Groin Pain Review of Literature

    The anatomy of the groin region is complex and involves a confluence of structures. Groin pain is a chronic symptom located in the inguinal region. In this review, it is aimed to give an overview of inguinal pain in the literature, to examine surgical and rehabilitation interventions and to compare the time to return to sports. In addition, different diagnostic approaches are indicated. To create a map for further studies by compiling in detail the studies in the literature on the anatomy of the groin region and the definition, treatment and return to sports after the treatment of athletic groin pain. The results of this review showed a comparable return to play rate and return to play time between surgical and rehabilitative interventions. We believe that dealing with the reasons of groin pain in detail in the treatment process will affect the duration of treatment and return to play.Inguinal Bölge anatomisi karmaşıktır ve çeşitli yapıların organizasyonunu içerir. Pubalji kasık bölgesinde lokalize olan kronik bir semptomdur. Bu derlemede pubalji konusunda literatürde yer alan genel bilgilerin verilmesi, cerrahi ve rehabilitasyon müdahalelerinin incelenmesi ile spora dönüş sürelerinin karşılaştırılması amaçlanmaktadır. Ayrıca farklı tanı yaklaşımları belirtilmektedir. Inguinal bölgenin anatomisi ve atletik pubaljinin tanımı, tedavisi ve tedavi sonrası spora dönüş konularında literatürde yer alan çalışmaları detaylı bir şekilde derleyerek ileriki çalışmalar için bir harita oluşturmaktır. Bu derlemenin sonuçları, cerrahi ve rehabilitasyon müdahaleleri arasında spora dönüş oranı ve süresinin karşılaştırılabilir olduğunu göstermiştir. Tedavi sürecinde pubalji nedenlerinin detaylı olarak ele alınmasının tedavi süresini ve spora dönüş süresini etkileyeceği belirtilmştir

    İşitme kaybı ile birlikte; konuşma zincirinin işitsel geri bildirim mekanizmasında bozulmalar meydana gelir. Konuşmacı sesini konuşmayla eşzamanlı olarak dinleyip sesini, artikülasyonunu ve genel anlaşılırlığını kontrol edebilirken işitsel geri bildirim mekanizmasının bozulması sesin akustik özelliklerini ve anlaşılırlığını etkileyecektir. Postlingual İ.K'lı bireylerde daha önceden var olan işitsel geri bildirim, dil ediniminden sonra bozulmaya uğramıştır ve koklear implant ile birlikte işitsel geri bildirimin iyileştirilmesi sağlanmaktadır. Çalışmanın örneklemi 21 postlingual İ.K'lı implant kullanıcıları ve 21 normal işiten yetişkinlerden oluşmaktadır. Katılımcılara ses ve konuşma özelliklerinin incelenmesi amacıyla objektif ve subjektif değerlendirmeler yapılmıştır. Ek olarak ses ve konuşma özelliklerini etkileyecek implant kullanım süresi, cinsiyet, yaş gibi etmenlerin etkileri incelenmiştir. Akustik ses analizinde; temel frekans(F0), Jitter (%), Shimmer (%) ve HNR (Harmonik Gürültü Oranı) parametreleri incelenmiştir. Bu parametrelerde bozulmalar meydana gelse de istatistiksel açıdan anlamlı düzeyde görülmemiştir. Algısal değerlendirme amacı ile uzmanlar tarafından GRBAS değerlendirmesi yapılmış ve Grade (Disfoni derecesi)2 ait puan ortalamalarının çalışma grubunda istatistiksel olarak anlamlı düzeyde daha yüksek olduğu (0,95±0,87) saptanmıştır (p<0,05). Breathiness (Nefeslilik)2 parametresine ait sıra ortalamalarının da çalışma grubunda istatistiksel olarak anlamlı düzeyde daha yüksek olduğu (Sıra ort:25,10) saptanmıştır (p<0,05). Katılımcıların kendi seslerini değerlendirmesi amacıyla Ses Handikap Endeksi-10 (SHE-10) ve katılımcıların iletişimcilerinin, kişinin konuşma anlaşılırlığını değerlendirmeleri amacıyla da Bağlamiçi Anlaşılabilirlik Ölçeği (BAÖ) uygulanmıştır. SHE-10'a ait puan ortalamalarının kontrol grubunda istatistiksel olarak anlamlı düzeyde daha yüksek olduğu (8,24±7,95) saptanmıştır (p<0,05). BAÖ' ye ait sıra ortalamalarının kontrol grubunda istatistiksel olarak anlamlı düzeyde daha yüksek olduğu (Sıra ort: 29,81) saptanmıştır (p<0,05). Tüm veriler dikkate alındığında postlingual işitme kayıplı bireylerin koklear implant kullanımıyla birlikte işitsel geri bildirimi büyük oranda iyileştirilmiş olduğu görülmektedir. Ses ve konuşma özellikleri de büyük oranda normale yaklaşmış olsa da yeterli olmadığı, işitsel rehabilitasyon ve konuşma terapisi ile desteklenmesi gerektiği düşünülmektedir.Along with hearing loss; disruptions occur in the auditory feedback mechanism of the speech chain. While the speaker can listen to his voice simultaneously with speech and control his voice, articulation and general comprehensibility, the disruption of the auditory feedback mechanism will affect the acoustic properties and comprehensibility of the sound. In postlingual CI individuals, previously existing auditory feedback has been impaired after language acquisition and auditory feedback is improved with cochlear implant. The sample of the study consists of 21 postlingual CI users and 21 normal hearing adults. Objective and subjective evaluations were made to examine the sound and speech characteristics of the participants. In addition, the effects of factors such as implant usage time, gender, age on sound and speech characteristics were examined. In addition, the effects of factors such as implant usage time, gender, age that will affect voice and speech characteristics were examined. In acoustic sound analysis; fundamental frequency (F0), Jitter (%), Shimmer (%) and HNR (Harmonic Noise Ratio) parameters were examined.Although there were disruptions in these parameters, they were not statistically significant. For the purpose of perceptual evaluation, GRBAS evaluation was performed by two experts and it was found that the average scores of Grade (Dysphonia degree)2 were statistically significantly higher in the patient group (0.95±0.87) (p<0.05). The average ranks of Breathiness2 parameter were also found to be statistically significantly higher in the patient group (Rank avg:25.10) (p<0.05). For the purpose of evaluating their own voices, the participants were administered the Speech Handicap Index-10 (SHE-10), and for the purpose of evaluating the speech intelligibility of the person by their communicators, the Connected Speech Intelligibility Scale (CSIS) was also administered. The average scores of SHE-10 were found to be statistically significantly higher in the normal group (8.24±7.95) (p<0.05). The average ranks of CSIS were also found to be statistically significantly higher in the normal group (Rank avg:29.81) (p<0.05). The correlation between cochlear implant usage time, gender and age characteristics and the results was examined but was not found to be statistically significant. When all data is considered, although cochlear implant usage significantly improves auditory feedback and brings speech characteristics closer to normal levels for individuals with post-lingual hearing loss, it is deemed insufficient and needs to be supported by auditory rehabilitation and speech therapy

    Volumetric Analysis of Subacromial Space After Superior Capsular Reconstruction for Irreparable Rotator Cuff Tears

    PurposeSubacromial volume measurement on magnetic resonance images is relatively new. It has been shown that decreased subacromial volume increases after surgical repair of full-thickness rotator cuff tears. There is no study examining subacromial volume changes after superior capsular reconstruction (SCR). The purpose of this study was to compare subacromial volume changes on magnetic resonance images (MRI) after superior capsular reconstruction performed for primary irreparable rotator cuff tears.MethodsPatients who underwent an SCR procedure between 2017 and 2019 with a minimum 2-year postoperative follow-up were included in this retrospective study. Subacromial volume was measured on MRI using software. The preoperative and postoperative acromiohumeral distance, Constant Scores, graft thickness, and Hamada grades of the patients were evaluated.ResultsA total of 18 patients with a mean age of 59.7 years (range: 49-74 years) underwent an SCR for massive irreparable cuff tear. The mean preoperative subacromial volume was 3.54 +/- 0.39 cm(3) (range 2.88-4.36 cm(3)), which increased to 4.46 +/- 0.39 cm(3) (range 3.75-5.32 cm(3)) postoperatively (p = < 0.001). The increase in subacromial volume and acromiohumeral distance did not correlate with Constant scores and graft thickness. We observed a significantly higher subacromial volume increase among Hamada grade 1 patients, compared to those with Hamada grade 2 (p = 0.011).ConclusionsWe observed that subacromial volume significantly increased after superior capsular reconstruction. However, the increase in subacromial volume did not correlate with clinical scores, acromiohumeral distance changes, or graft thickness
