    The effect of Orem's Self-Care Deficit Theory-based care during pregnancy and postpartum period on health outcomes: A systematic review and meta-analysis

    Aim: To examine the effect of the provision of care on health outcomes when provided based on Orem's Self-Care Deficit Theory during perinatal period. Background: The perinatal period is a process with multidimensional care needs for the mother, baby and family. Care based on nursing theories can improve the quality of perinatal care. Design: A systematic review and meta-analysis. Review methods: Studies on this topic from 2006 to 2020 have been accessed by nine database searches. The methodological quality of the studies was assessed using the Critical Appraisal Checklists for experimental and quasi-experimental studies, developed by the Joanna Briggs Institute. This study was conducted by following the Preferred Reporting System for Systematic Reviews and Meta-Analyses. Results: This systematic review included nine studies with a total sample size of 839 women. These studies showed that care based on Orem's theory significantly helped prolong the mean duration of pregnancy, reduced the incidence of preterm labor, improved hygiene behaviour and increased empowerment. Concerning the perinatal period, this intervention significantly improved self-care and readiness for hospital discharge, adaptation and breastfeeding self-efficacy and reduced nipple pain and pain due to abdominal distension. Conclusions: The implementation of this theory into care during the perinatal period can contribute to improved health outcomes. Summary statement: What is already known about this topic? The quality of pregnancy and postpartum care is important for the maternal health. The use of nursing theories-which nursing knowledge is produced-improves care quality and ensures that care is organized systematically. What this paper adds? There is insufficient research that supports Orem's theory of self-care deficit in pregnancy and postpartum care and also no systematic review about this topic, but the current meta-analysis showed that postnatal and prenatal treatments based on Orem's theory have a positive impact on the self-care agency. The implications of this paper: The use of Orem's theory helps nurses understand women and their needs during pregnancy and postpartum periods. Integrating Orem's theory of self-care deficit into pregnancy and postpartum nursing care services can help increase women's self-care power

    Nature-inspired algorithms revolve around the intersection of nature-inspired algorithms and the IoT within the healthcare domain. This domain addresses the emerging trends and potential synergies between nature-inspired computational approaches and IoT technologies for advancing healthcare services. Our research aims to fill gaps in addressing algorithmic integration challenges, real-world implementation issues, and the efficacy of nature-inspired algorithms in IoT-based healthcare. We provide insights into the practical aspects and limitations of such applications through a systematic literature review. Specifically, we address the need for a comprehensive understanding of the applications of nature-inspired algorithms in IoT-based healthcare, identifying gaps such as the lack of standardized evaluation metrics and studies on integration challenges and security considerations. By bridging these gaps, our paper offers insights and directions for future research in this domain, exploring the diverse landscape of nature-inspired algorithms in healthcare. Our chosen methodology is a Systematic Literature Review (SLR) to investigate related papers rigorously. Categorizing these algorithms into groups such as genetic algorithms, particle swarm optimization, cuckoo algorithms, ant colony optimization, other approaches, and hybrid methods, we employ meticulous classification based on critical criteria. MATLAB emerges as the predominant programming language, constituting 37.9% of cases, showcasing a prevalent choice among researchers. Our evaluation emphasizes adaptability as the paramount parameter, accounting for 18.4% of considerations. By shedding light on attributes, limitations, and potential directions for future research and development, this review aims to contribute to a comprehensive understanding of nature-inspired algorithms in the dynamic landscape of IoT-based healthcare services.     520 nm and 660 nm light-emitting diodes modulates pancreatic development and beta cell functions in zebrafish embryos

    Green and Red LEDs increase insulin production, but their comparative effects on pancreatic and beta cell development are unclear. Zebrafish embryos were divided into three groups: Control (n = 60), Green (G) (n = 60), and Red (R) (n = 60), then irradiated for three days (14 hours/day) with 0.5 W/cm(2) G (lambda peak = 520 nm, 180 mA) and R (lambda peak = 660 nm, 210 mA). At the end of 72 h, pancreatic and beta cells, circadian rhythm, and oxidative stress gene were analyzed using RT-PCR. Malondialdehyde, nitric oxide, superoxide dismutase, and glutathione levels were also evaluated. In the Red group, pancreatic area increased by similar to 97.13% compared to the Control group and by approximately similar to 62.16% compared to the G group (both p < 0.0001), and no significant difference in beta cell area (p = 0.964). G group insulin expression increased 2.31-fold compared to R group (p < 0.0001). Red LED treatment increased MDA levels (p < 0.001), oxidative stress (fth1b, nqo1) (p < 0.0001), and per1b during the photophase (p < 0.0001) compared to G group. R LED treatment increases oxidative stress and disrupts circadian rhythm, leading to reduced insulin secretion. The positive effects of G LED treatment have potential for metabolic syndrome, diabetes, and pancreatic diseases

    Micro- and Macrostructural Language Features in Vertebrobasilar or Carotid System Stroke Without Diagnosis of Aphasia

    Purpose: This study aimed to investigate the macro- and microstructural features of language in patients with ischemic stroke without aphasia. Method: Participants were grouped according to arterial system damage and given the Aphasia Language Assessment Test (ADD) to detect aphasia. A narrative sample was obtained and analyzed for macrostructural and microstructural features of the language. The study sample consisted of 31 participants with ischemic stroke (15 vertebrobasilar system [VBS] involvement and 16 carotid system [CS] involvement) and 31 healthy participants, totaling 62 individuals. Results: The healthy control group scored higher than the stroke group on the microstructural feature type-token ratio and mean length of utterance in the narrative analysis and on the auditory comprehension, repetition, naming, grammar, speech act, and writing subtests in ADD. Effort behavior, errors, edits, repetitions, and pauses among microstructural features and uncertainty, filler expression, and anomia among macrostructural features were significantly higher in the vertebrobasilar and CS groups than in the healthy control group. The total ADD score and speech fluency and reading subtest scores were significantly higher in individuals with VBS damage than in individuals with CS lesions (p < .05). Conclusions: Language components may be impaired differently in patients with carotid and vertebrobasilar lesions. Speech and language disorders in individuals who have experienced cerebrovascular accidents should be evaluated in the subacute and chronic phases, and the therapeutic needs of patients with ischemic stroke should be determined, regardless of the presence of a clinical aphasia diagnosis

    Ameliyat sonrası, bireylerin %80 ‘inden fazlası orta ila şiddetli düzeyde akut ağrı yaşar. Kontrol edilemeyen postoperatif ağrı, iyileşmeyi sınırlandırır ve kısa ve uzun vadeli morbidite ile ilişkilidir. Hekimler geleneksel olarak postoperatif ağrı yönetiminden sorumluyken, cerrahi hastalara bakım sağlayan tüm klinisyenler ortak farmakolojik ve girişimsel ağrı yönetimi stratejileri konusunda temel bir anlayışa sahiptir. Bu derlemede, akut ağrının sonuçları, ağrı değerlendirme yaklaşımları ve postoperatif ağrıyı yönetmek için yaygın olarak kullanılan tedavilere genel bir bakış ve hemşirelerin postoperatif ağrı yönetiminde rolü ve uygulamaları ele alınmıştır.After surgery, more than 80% of people experience moderate to severe acute pain. Poorly controlled postoperative pain limits healing and is associated with detrimental short-term and long-term morbidity. While physicians have traditionally been responsible for postoperative pain management, all clinicians caring for surgical patients have a basic understanding of common pharmacological and interventional pain management strategies. In this review, an overview of the outcomes of acute pain, pain assessment approaches and commonly used treatments to manage postoperative pain, and the role and practices of nurses in postoperative pain management are discussed

    Yaşlılık dönemindeki yetişkinlerin kelime bulmada patolojik bir durumları olmasa bile üretilen kelimenin doğruluğunda azalma olduğu ve öğeleri adlandırma için gereken sürenin arttığı bilinmektedir. Hızlı otomatik adlandırma (HOA), bireylerin nesneleri, renkleri, harfler ve rakamları sesli bir şekilde ne kadar hızlı adlandırabildiğini ölçen bir görevdir. Hızlı otomatik adlandırma becerisi leksikal (sözcüksel) erişim hızı, leksikal erişim, görsel algılama, semantik kategorizasyon ve motor çıktı aşamaları arasındaki işlemleme süresi, dikkatte sürdürülebilirlik gibi dil-bilişsel beceriler gibi çok faktörlü talepleri yansıtır. Artan yaşla birlikte en çok bildirilen şikayetlerden biri de adlandırma zorluğu çekilmesidir. Bu çalışmada genç yetişkin, orta yaşlı yetişkin ve yaşlı yetişkinlerden elde edilen verilerle hızlı otomatik adlandırma becerilerinin (adlandırma doğruluğu, adlandırmaya başlama süresi, adlandırma sırasında revizyon ve her bir alt görev için toplam adlandırma süresi) yaş ve eğitim düzeyine bağlı değişimi incelenip yaş ve eğitim düzeyi gruplarına göre kıyaslanmıştır. Çalışmanın katılımcıları, 20-65 yaşları arasında sağlıklı 157 bireyden oluşmaktadır. Çalışmaya dahil edilen katılımcılar; herhangi bir nörolojik ve psikiyatrik hastalığa sahip olmayan, anadili Türkçe olan, okuma- yazma bilmen, renk körü olmayan ve ileri düzeyde görme kaybı yaşamayan bireylerden oluşmaktadır. Katılımcılara Gönüllülük Onam Formu doldurulduktan sonra yaş, eğitim demografik bilgileri alındıktan sonra katılımcılara Montreal Bilişsel Değerlendirme Ölçeği, Edinburgh El Tercihi Envanteri, Diadokinetik Hız, Ishihara Testi ve Hızlı Otomatik Adlandırma görevleri uygulanmıştır. Yapılan istatistiksel analizlerde yaşa bağlı hızlı otomatik adlandırma becerilerinde farklılıklar olduğu saptanmıştır. Hızlı otomatik adlandırma becerisi; adlandırma doğruluğu, adlandırmaya başlama süresi, adlandırma sırasında revizyon ve her bir alt görev için toplam adlandırma süresi parametleri gruplara göre karşılaştırılmıştır. Artan yaş ve eğitim düzeyinin hızlı otomatik adlandırma becerisinin tüm alt görevlerinde adlandırma süresini etkilediği bulunmuştur. Adlandırma hataları, revizyon ve reaksiyon süresi açısından hızlı otomatik adlandırma hemtekli hem de çoklu alt görevler ile hem de sembolik olan ve sembolik olmayan görevlerde de gruplar arasında farklar gözlemlenmiştir.Even if older adults do not have a pathological condition in word finding, it is known that the accuracy of the word produced decreases and the time required to name items increases. Rapid automatic naming (RAN) is a task that measures how quickly individuals can name objects, colours, letters and numbers aloud. Rapid automatic naming skill reflects multifactorial demands such as lexical access speed, lexical access, visual perception, processing time between semantic categorisation and motor output stages, and languagecognitive skills such as attentional sustainability. One of the most commonly reported complaints with increasing age is naming difficulties. In this study, the variation of rapid automatic naming skills (naming accuracy, naming initiation time, revision during naming, and total naming time for each subtask) with respect to age and educational level was analyzed and compared across age and educational level groups with data obtained from young adults, middle-aged adults, and older adults. The participants of the study consisted of 157 healthy individuals between the ages of 20-65. The participants included in the study consisted of individuals who did not have any neurological and psychiatric diseases, whose mother tongue was Turkish, who could read and write, who were not colour-blind and who did not have severe visual loss. After the Consent Form was filled in and demographic information about age and education were obtained, Montreal Cognitive Assessment Scale, Edinburgh Hand Preference Inventory, Diadokinetic Speed, Ishihara Test and Rapid Automatic Naming tasks were applied to the participants. Statistical analyses revealed that there were age-related differences in rapid automatic naming skills. Rapid automatic naming skill; naming accuracy, naming initiation time, revision during naming and total naming time for each subtask were compared between groups. It was found that increasing age and education level affected the naming time in all subtasks of the rapid automatic naming skill. Differences were observed between the groups in terms of naming errors, revision and reaction time in both single and multiple subtasks of rapid automatic naming, as well as in symbolic and non-symbolic tasks

    A Case of Persistent Erythema Multiforme Treated with Dapsone

    Erythema multiforme is a clinical entity that presents in three different clinical forms, namely classic, recurrent, or persistent. Persistent erythema multiforme (PEM) is a rare variant that may be localized in different regions compared with the classical erythema multiforme, has a continuous course, and can present in inflammatory clinical forms. PEM was defined as the uninterrupted persistence of typical or atypical target-like lesions. Data about its etiology and treatment is obtained from case reports. Herein, we present the case of a 37-year-old male patient who presented to our outpatient clinic with palmar lesions and with the presence severe oral bullae and ulcers treated with systemic steroids and dapsone. Due to the rarity of the disease, reporting this case is expected contribute to the literature and provide evidence that 50 mg dapsone treatment may be used to control PEM

    Comparison of the efficacy of auricular vagus nerve stimulation and conventional low back rehabilitation in patients with chronic low back pain

    Background: In recent years, human and animal studies have provided increasing evidence that vagus nerve stimulation (VNS) can produce analgesic effects as well as alleviating resistant epilepsy and depression. Our study was designed to compare the efficacy of transcutaneous auricular vagus nerve stimulation with conventional low back rehabilitation in patients with chronic low back pain (CLBP). Methods: Sixty patients with LBP were randomly divided into two groups. Group 1 received conventional rehabilitation and home exercise, and Group 2 received transcutaneous auricular VNS and home exercise. Both groups received treatment five days a week for three weeks. Trunk mobility (Modified Schober test, fingertip -tofloor test), muscle strength (CSMI-Cybex Humac-Norm isokinetic dynamometer and Lafayette manual muscle strength measuring device), trunk endurance, balance tests, Visual Analog Scale, Beck Depression Scale, Pittsburgh Sleep Quality Index, Oswestry Disability Index were evaluated. Results: At the end of three weeks, within -group assessment results showed positive effects on mobility, functional status, depression and sleep in all groups (p < 0.05). Pain level, endurance time and flexion trunk muscle strength results showed more improvement in Group 2 (p < 0.05). Some parameters of isokinetic lower extremity quadriceps muscle strength and fall risk scores showed a significant improvement in Group 1 (p < 0.05). Discussion: VNS has been observed to be more effective on pain, trunk muscle strength and endurance duration and sleep status. Auricular VNS may be included in the treatment of patients with CLBP in whom conventional physical therapy is inadequate or not applicable

    Bu çalışmanın amacı, hastanede yatan hastalarda malnütrisyon riskini belirlemek ve malnütrisyon varlığı ile depresyon düzeyleri ve kan parametreleri arasındaki ilişkiyi incelemektir. Hem tedavi yan etkileri hem de hastalığın yaşattığı komplikasyonlar sonucu kanser hastalarında sıklıkla malnütrisyon görülmektedir. Malnütrisyon; yetersiz beslenme ve depresyon ile ilişkilendirilmektedir. Malnütrisyon, hastanede yatan hastaların prognozunu gösteren en önemli faktörlerden biridir. Bu çalışma, 01.02.2024 – 30.04.2024 tarihleri arasında Bezmialem Vakıf Üniversitesi Tıp Fakültesi Hastanesi Onkoloji servisinde yatan kanser tanısı almış 85 hasta ile yapılmıştır. Katılımcılara anket formu ile kişisel bilgi formu, 3 günlük besin tüketim kaydı, nutrisyonel risk taraması (NRS-2002) ve beck depresyon envanteri uygulanmıştır. Hastanede rutin bakılan bazı kan parametreleri değerlendirilmiştir. Katılımcıların 44'ü kadın (%51.8) ve 41'i erkekdir (%48.2). Yaş ortalaması 62.49 (±13.73) yıl olan katılımcıların yaş aralığı 19 ile 84 arasında değişmekte olup, %41.2'si ilkokul mezunudur. BKİ ortalaması 26.25 (±5.65) kg/m²'dir. NRS ortalaması 2.21 (±1.31) olarak belirlenmiş, puanlar 0 ile 5 arasında değişiklik göstermiştir. Total skor ≥3 olan hastaların nutrisyonel risk altında olduğu kabul edilmiştir. Depresyon puanlarının ortalaması ise 16.74 (±9.17) olup, minimum 3 ve maksimum 41 puan arasında değişmiştir. Beck Depresyon punı 17 ve üzeri alındığında tedavi gerektirebilecek depresyonu %90 üzerinde bir doğrulukta ayırt edebildiği belirlenmiştir. Beslenme alışkanlıkları incelendiğinde, katılımcıların %62.4'si günde üç ana öğün tüketmektedir. BKİ arttıkça NRS'nin azaldığı saptanmıştır (p<0.01). Depresyon ile NRS arasında istatistiksel olarak pozitif ve anlamlı bir ilişki bulunmuştur. Malnütrisyonlu hastaların, depresyon durumu ve BKİ ile arasında istatistiksel olarak anlamlı bir ilişki bulunmuştur. Malnütrisyon riski taşıyan hastalarda besin alımı ve biyokimyasal parametreler yakından takip edilmelidir. Bu konuda daha büyük örneklem grupları ile çalışmalar yapılması önerilmektedir.The aim of this study is to determine the risk of malnutrition in hospitalized patients and to examine the relationship between the presence of malnutrition, depression levels, and blood parameters. Malnutrition is frequently observed in cancer patients as a result of both the side effects of treatment and complications caused by the disease. Malnutrition is associated with inadequate nutrition and depression. It is one of the most important factors indicating the prognosis of hospitalized patients. This study was conducted between February 1, 2024, and April 30, 2024, with 85 cancer-diagnosed patients hospitalized in the oncology ward of Bezmialem Vakıf University Faculty of Medicine Hospital. Participants were administered a personal information form, a 3-day food consumption record, nutritional risk screening (NRS-2002), and the Beck Depression Inventory. Some routinely monitored blood parameters were also evaluated. Of the participants, 44 were female (51.8%) and 41 were male (48.2%). The participants' average age was 62.49 (±13.73) years, ranging from 19 to 84, with 41.2% being primary school graduates. The mean BMI was 26.25 (±5.65) kg/m². The mean NRS score was determined to be 2.21 (±1.31), with scores ranging from 0 to 5. Patients with a total score of ≥3 were considered to be at nutritional risk. The average depression score was 16.74 (±9.17), ranging from a minimum of 3 to a maximum of 41 points. A Beck Depression Score of 17 and above was found to distinguish depression requiring treatment with over 90% accuracy. When examining eating habits, 62.4% of the participants consumed three main meals per day. It was found that as BMI increased, NRS decreased (p<0.01). A statistically positive and significant relationship was found between depression and NRS. There was a statistically significant relationship between the nutritional status, depression, and BMI of malnourished patients. In patients at risk of malnutrition, food intake and biochemical parameters should be closely monitored. It is recommended that larger sample group studies be conducted on this subject

    Çalışmamızın amacı bronşektazinin hastalarda motor imgeleme yeteneği, reaksiyon zamanı ve kognitif fonksiyonlar üzerine etkilerini araştırmaktı. Atlas Üniversitesi Sağlık Bilimleri Fakültesi'nde 33 katılımcı çalışmaya alındı. İstinye Üniversitesi Tıp Fakültesi Hastanesi Çocuk Sağlığı ve Hastalıkları Ana Bilim Dalı Çocuk Göğüs Hastalıkları Bilim Dalında takip edilen 13 hasta bronşektazi grubu, 20 sağlıklı çocuk ise kontrol grubu olmak üzere iki gruba ayrıldı. Tüm katılımcıların motor imgeleme yeteneği Hareket İmgeleme Anketi-3 ve Hareket İmgelemesinin Netliği Anketiyle, reaksiyon zamanı Nelson El-Ayak Reaksiyon Testiyle, kognitif fonksiyonlar İz Sürme Testi ve Stroop Testi ile, solunum fonksiyonları Solunum Fonksiyon Testi ile, fonksiyonel kapasite 6 Dakika Yürüme testi ile, dispne düzeyi Dalhousie Dispne ve Algılanan Efor Ölçeği ile, yaşam kalitesi Çocuklar İçin Yaşam Kalitesi Ölçeği ile ve hareket korkusu Tampa kinezyofobi ölçeği ile değerlendirildi. Tüm değerlendirmeler sonucunda grupların demografik özellikleri benzerdi (p>0.05). Spirometrik ölçümde kontrol grubunda solunum fonksiyonunun FEV1/FVC (%), PEF (lt/sn), PEF (Pred %), FEF25-75 (lt/sn), FEF25-75 (Pred %) parametrelerinde, 6DYT'nin ΔKalp hızı (atım/dk), ΔDispne (Borg) parametrelerinde, motor imgeleme yeteneğinin değerlendirildiği Hareket İmgeleme Anketi-3 (İçsel), Hareket İmgeleme Anketi-3 (Dışsal), Hareket İmgeleme Anketi-3 (Kinestetik), Hareketi İmgelemenin Netliği Anketi (Gözler Kapalı), Hareket İmgelemenin Netliği Anketi (Toplam Puan) parametrelerinde, reaksiyon zamanı değerlendirmesinin Nelson Ayak Reaksiyon Testi'nde, kognitif fonksiyonun değerlendirildiği İz Sürme Testi- A (Kısa Form), ), Stroop Testi'nde "Dörtgenin rengini söyleme" ve "Renkli kelimeleri okuma" bölümünde hata ve düzeltme, "Renkli kelimelerin rengini söyleme" bölümünde hata değerlerinde elde edilen sonuçlar iki grup için istatiksel olarak anlamlı biçimde farklıydı(p0.05). In spirometric measurement, respiratory function in the control group was measured in FEV1/FVC (%), PEF (lt/sec), PEF (Pred %), FEF25-75 (lt/sec), FEF25-75 (Pred %) parameters, 6MWT's ΔHeart rate (beats/min), ΔDyspnea (Borg) parameters, Movement Imagination Questionnaire-3 (Internal), Movement Imagination Questionnaire-3 (External), Movement Imagination Questionnaire-3 (Kinesthetic), Vividness of Movement Imagination Questionnaire (Eyes Closed), , Vividness of Movement Imagination Questionnaire (Total Score) parameters where motor imagery ability is evaluated, reaction time evaluation in Nelson Foot Reaction Test, cognitive function evaluation in Trail Making Test-A (Short Form), The results obtained the errors and corrections in subparameters Stroop Test in "Saying the color of the rectangle" and "Reading colored words" and the results obtained in the error values in the "Saying the color of the colored words" section were found to be statistically significantly different between the groups (p<0.05). Evaluation of motor imagery ability, cognitive functions and reaction time may provide input for treatment programs aimed at improving the performance of patients with bronchiectasis
