The frequency of potentially inappropriate medication usage in community-dwelling oldest-old people
Background/aim: It is critically important to protect the health of the oldest-old people, as their hospitalization and death rates are high. The objective of this study was to analyze the prevalence of potentially inappropriate medication use among the community-dwelling oldest-old people and its association with their demographic characteristics. Materials and methods: Data were collected from real-world settings using the observational method for this descriptive study. An older adult aged >= 85 years old was defined as the oldest-old. The participants were visited in their homes. The generic names of the medications used, and the age, sex, and province of residence were recorded. The medications were analyzed according to the 2019 Beers criteria, and their prevalence of use among the oldest-old people was determined. Results: Data were collected from 549 of the oldest-old people. The median age of the participants was 88.0 years (88.8 +/- 3.5; min = 85.0, max = 102), and 61.3% (n = 336) of them were female. The study findings showed that 65.0% of the community-dwelling oldest- old people used potentially inappropriate medications, with a median number of 1 (min = 0, max = 6). The prevalence of potentially inappropriate medication use increased linearly with the number of drugs used (p = 0.001). The median number of medications was significantly higher in the potentially inappropriate medication user group (5 vs. 2, p = 0.001). Diuretics, proton pump inhibitors, and nonsteroidal antiinflammatory drugs were the most frequently used potentially inappropriate medications. Conclusion: The prevalence of potentially inappropriate medication use was high among the oldest-old people in Turkiye. There were no differences in frequency of use according to age, sex, or geographical region. It is important to prevent the use of potentially inappropriate medications that should be avoided and to monitor the oldest-old group that uses potentially inappropriate medications that should be used with caution
Physician Preferences in Using Novel Digital Devices for the Management of Atrial Fibrillation-A DAS-CAM III Survey
Aim: A recent European Heart Rhythm Association (EHRA) practical guide provides guidance on the use of novel digital devices for heart rhythm analysis using either electrocardiogram (ECG) or photoplethysmography (PPG) technology for the diagnosis of atrial fibrillation (AF). This survey assesses physicians' preferences to use digital devices in patients with possible AF and their impact on clinical decision-making.
Methods and results: Participants of the DAS-CAM III initiated and distributed an online survey assessing physician preferences in using digital devices for the management of AF in different clinical scenarios. A total of 505 physicians (median age: 38 [IQR 33-46] years) from 30 countries completed the survey. A third of respondents were electrophysiologists, the others were cardiologists, cardiology residents, or general practitioners. Electrophysiologists were more likely to have experience with both ECG-based (92% vs. 68%, p < 0.001) and PPG-based (60% vs. 34%, p < 0.001) digital devices. The initial diagnostic approach to each scenario (symptomatic low-risk, symptomatic high-risk, or asymptomatic high-risk patient) was heterogeneous. Electrophysiologists preferred intermittent single-lead ECG monitoring to traditional Holter ECGs to screen for AF. Both electrophysiologists and non-electrophysiologists would rarely use PPG-based devices to diagnose and screen for AF (8.2%-9.8%). Electrophysiologists and non-electrophysiologists use ECG-based technology to confirm PPG-documented tracings suggestive of AF.
Conclusion: While PPG-based digital devices are rarely used for diagnosis and screening for AF, intermittent ECG-based digital devices are beginning to be implemented in clinical practice. More education on the potential of novel digital devices is required to achieve diagnostic pathways as suggested by the EHRA practical guide
A new service composition method in the cloud-based Internet of things environment using a grey wolf optimization algorithm and MapReduce framework
Cloud computing is quickly becoming a common commercial model for software delivery and services, enabling companies to save maintenance, infrastructure, and labor expenses. Also, Internet of Things (IoT) apps are designed to ease developers' and users' access to networks of smart services, devices, and data. Although cloud services give nearly infinite resources, their reach is constrained. Designing coherent and organized apps is made possible by integrating the cloud and IoT. Expanding facilities by combining services is a critical component of this technology. Various services may be presented in this environment based on the user's demands. Considering their Quality of Service (QoS) attributes, discovering the appropriate available atomic services to construct the needed composite service with their collaboration in an orchestration model is an NP-hard issue. This article suggests a service composition method using Grey Wolf Optimization (GWO) and MapReduce framework to compose services with optimized QoS. The simulation outcomes illustrate cost, availability, response time, and energy-saving improvements through the suggested approach. Comparing the suggested technique to three baseline algorithms, the average gain is a 40% improvement in energy savings, a 14% decrease in response time, an 11% increase in availability, and a 24% drop in cost
The effect of cerebellar lesıons on language in Turkısh speakıng indıvıduals
BackgroundThe cerebellum is approximately 10% of the brain volume and is responsible for attention, language, emotion and cognitive functions. Linguistic processing difficulties and cognitive impairments may be observed in individuals with cerebellar lesions depending on the location and severity of the damage. There is no study evaluating language in Turkish-speaking individuals with cerebellar lesions.ObjectiveTo determine the differences in linguistic and visuospatial skills of Turkish speaking individuals with cerebellar lesions compared to control groups, including a right hemisphere lesion group and a healthy control group.MethodFifteen patients with cerebellar lesion (without hemispatial neglect and dysarthria), 15 patients with right hemisphere lesion due to right arter cerebri media (without hemispatial neglect) and 15 healthy control were included. To clarify the language impairment specific to cerebellar lesions, we also included individuals with right hemisphere lesions, which are typically assumed not to present with aphasia. The Language Assessment Test for Aphasia (ADD), Benton Judgment of Line Orientation Test, Benton Facial Recognition Test, Single Letter Deletion Test were used. The "Accident Scene" picture was used for collecting narrative speech samples. After a three group comparison a post-hoc pairwise analysis was performed.ResultsAmong the microstructural parameters in the narrative language analysis, the effort ratio was significantly higher in the cerebellar lesion group compared to the healthy control group. Among the non-verbal cognitive tests, the Benton Facial Recognition Test score was significantly lower in the cerebellar and right hemisphere lesion group compared to the healthy control group. Total score, naming and grammar scores in the ADD test were significantly lower in the cerebellar lesion group compared with the healthy control group. There was no difference between cerebellar and right hemisphere lesions in terms of ADD score, ADD subtests and narrative parameters.ConclusionIn Turkish-speaking individuals with cerebellar lesions, verbal productivity may be interrupted by effort behaviours, and standard aphasia assessment tests may reveal impaired grammar and naming performance without a diagnosis of aphasia. In addition to reduced efficiency in verbal production, facial recognition may be impaired. We found no difference between individuals with lesions in the cerebellum and right hemisphere. This suggests that ischemic damage may affect different components of language independently of the diagnosis of aphasia
Kekemeliği olan bireylerde 5 gün art arda uygulanan bihemisferik tdas'ın konuşma akıcılığı üzerindeki etkisinin incelenmesi / An investigation of the effect of 5 consecutive days of bihemispheric tdcs on speech fluency in individuals with stuttering
Stuttering is a neurodevelopmental disorder that affects the lives of individuals in many aspects by causing frequent disruptions in speech flow, which is seen in 1% of adults. In recent years, with the increasing interest in neurological interventions in the field of stuttering, the effect of brain stimulation on speech fluency in order to reduce stuttering symptoms and to better understand stuttering has been investigated, and it is seen that tDCS has the potential to improve fluency. In this study, it was aimed to investigate the effect of 5 consecutive days of bi-hemispheric (anodal to left IFG, cathodal to right IFG) stimulation on speech fluency in individuals with stuttering. The hypothesis of the study is that bihemispheric stimulation applied consecutively for 5 days will provide a significant improvement on speech fluency by creating a cumulative effect. 36 individuals with stuttering (32 males, 4 females) completed a randomised, double-blind, sham-controlled study of bihemispheric transcranial direct current stimulation. Nineteen participants received 1 mA stimulation for 20 min daily while temporarily achieving speech fluency with metronome-timed speech, while the other seventeen participants received the same fluency intervention with sham stimulation. Speech fluency was assessed at baseline, before and after the intervention on each day of the 5-day intervention, and 1 week after the end of the intervention during reading and speaking. According to the change from baseline to the end of the intervention and 1 week after the intervention, the scores of the group receiving tDCS did not show a significant improvement compared to the sham group. However, daily pre- and post-intervention measurements showed significant differences in reading and speaking fluency and SSI-IV score on day 3 of the intervention. Although the results of this 5-day study showed that the tDCS did not lead to a significant improvement in speech fluency, it provides some indications that further research is needed. Consideration of these factors may shed light on future studies to determine appropriate tDCS protocols for the treatment of developmental stuttering
Therapeutic Drug Monitoring Characteristics in a City Hospital for a Year
Background: Therapeutic drug monitoring (TDM) involves the measurement of drug concentrations in serum, plasma, whole blood, or other biologic fluids. This study focused on evaluating the TDM requests of a city hospital over a period of one year, retrospectively. Methods: The study retrospectively analyzed TDM requests for carbamazepine, cyclosporine-A, digoxin (DIGOX), lithium (LITH), methotrexate (MTX), phenitoin, tacrolimus, and valproic acid (VALP) from June 1, 2022, to June 1, 2023. Parameters such as the age and the gender of patients, the requesting departments, the measurement results, and the turnaround time (TAT) were assessed. Drug concentrations below the reference values were classified as subtherapeutic, whereas concentrations above the reference values were considered supratherapeutic.
Results: In total, 10,913 drug concentration measurement records were analyzed. The gender distribution was 51.6% male and 48.4% female. Pediatric samples comprised 6.2% and elderly samples 8.6% of the total. Notably, DIGOX, LITH, and VALP levels showed a significant correlation with age (p = < 0.0001, p = < 0.0001, and p = 0.0002, respectively). TAT was maintained at 360 minutes (6 hours) for all tests.
Conclusions: The study found significant correlations between age and DIGOX, LITH, and VALP levels. TDM plays a critical role in the elderly population, necessitating careful management of these drugs
Çevrimiçi klinik pilates temelli egzersiz programının primer dismenoreli kadınlar üzerine etkisi / The effect of an online clinic pilates-based exercise program on women with primary dysmenhorrea
The aim of our study was to investigate the effect of a clinical pilates-based exercise program on pain level, lumbopelvic muscle strength and dysmenorrhea symptoms in women with primary dysmenorrhea. 34 women diagnosed with primary dysmenorrhea were included in the study. The control group was only informed about the symptoms of primary dysmenorrhea and did not receive any intervention. The clinical pilates group was informed about the symptoms of primary dysmenorrhea and an online exercise program was applied for 8 weeks, 2 times a week for a total of 16 sessions, each session lasting 50 minutes. Sociodemographic information and menstrual characteristics of the participants were recorded. Menstrual attitudes were assessed with the Functional and Emotional Measure of Dysmenorrhea (FEMD) , menstrual symptoms with the Menstruation Symptom Questionnaire (MSQ), pain levels with the Visual Analog Scale (VAS) and Oswestry Low Back Pain Disability Questionnaire, and trunk muscle endurance with the Trunk Endurance Tests at baseline and at the end of two consecutive menstrual cycles. SPSS v.20 program was used for data analysis. All data were analyzed with the Shapiro-Wilk test to determine the distributional characteristics. 'Paired Sample T-test'was used to compare the values before and after the treatment within the group according to the distribution of the data. In intergroup comparisons, 'Independent Samples T-test'was used for numerical data with normal distribution. The significance level was accepted as p<0.05 for all analyzes. A significant difference was found in the post-treatment FEMD, MSQ, VAS, Oswestry Low Back Pain Disability Questionnaire, and Trunk Endurance Test scores of the intervention group (p<0.05). A significant difference was found in the trunk extensor and lateral flexor muscle endurance test scores of the control group after treatment (p<0.05). Statistically significant differences were found between the intervention and control groups in FEMD, MSQ, VAS, Oswestry Low Back Pain Disability Questionnaire and Trunk Endurance Test scores after treatment (p<0.05). An online clinical pilates-based exercise program reduces pain levels and menstrual symptoms in women with primary dysmenorrhea. The biggest barrier in women's health exercise programs is the obligation created by going to a clinic. For this reason, an online clinical pilates-based exercise program in primary dysmenorrhea symptom management can be considered as a non-pharmacological approach in terms of ease of application. For women whose work and school life is negatively affected by primary dysmenorrhea pain, symptom management approaches can be addressed in schools and workplaces with a team of physiotherapists, gynecologists and psychologists
The impact of non-invasive vagus nerve stimulation on recovery following aerobic exercise in individuals with post-COVID syndrome: A randomized controlled clinical trial
BACKGROUND AND AIM: Transcutaneous auricular vagus nerve stimulation (taVNS) has gained popularity recently and has been used to enhance recovery in various diseases, showing beneficial physiological and neurological effects. This study aimed to evaluate the efficacy of taVNS in post-exercise recovery for individuals experiencing Post-COVID Syndrome (PCS). METHODS: Individuals aged 18-45 years with PCS were assigned to either taVNS group (n=22 patients) or placebo VNS group (n=22 patients) and engaged in an aerobic exercise regimen. This included a 5-minute treadmill warm-up, followed by 15 minutes of brisk walking and 15 minutes of jogging. Evaluations were conducted before and after the exercise using the H10 Polar autonomic device and a lactate meter. Participants in Group 1 received real taVNS, while those in Group 2 received placebo VNS, with a subsequent third evaluation. RESULTS: There were statistically significant differences in the post-exercise evaluation parameters between taVNS and placebo VNS groups. . Parasympathetic nervous system (PNS) activity (p=0.009) and blood lactate levels (p=0.006) increased significantly in both groups, while the Root Mean Square of the Successive Differences (RMSSD) index (p=0.028) decreased. Both sympathetic nervous system (SNS) activity (p=0.01) and blood pressure (p=0.01) decreased similarly across the groups. CONCLUSIONS: The study found that taVNS increased cardiac vagal control but did not significantly affect blood lactate levels or blood pressure. Further randomized controlled trials are required to investigate the effects of taVNS on recovery in different treatment groups
The effects of progressive muscle relaxation exercise on dementia caregivers
While progressive muscle relaxation has demonstrated effectiveness in reducing anxiety, stress, and depression in general populations, its impact on caregivers of dementia patients remains understudied. This study investigated the effectiveness of Progressive Muscle Relaxation (PMR) in improving the well-being of dementia caregivers. Caregivers of individuals with dementia (n = 57) participated in a 3-month intervention, engaging in PMR thrice weekly. Participants completed assessments of depression, anxiety, stress, and caregiver burden at pre-test and post-intervention period. The intervention led to significant reductions in anxiety, stress, depression, and caregiver burden among participants. Female caregivers reported to have higher levels of anxiety and stress compared to males. Caregivers under financial stress showed high levels of distress and increased caregiver burden. These promising findings suggest the need for further research with larger, multicenter samples. Notably, PMR is a readily implementable intervention by nurses, requiring minimal equipment and offering a cost-effective approach to supporting caregiver well-being. (c) 2024 Elsevier Inc.
