EXTRAGASTROINTESTINAL STROMAL TUMOR WITH RETROPERITONEAL LOCATION: A RARE CASE REPORT
Gastrointestinal stromal tumors (GISTs) are the most common mesenchymal tissuederived tumors of the gastrointestinal tract. They occur rarely outside the digestive tract, and they are defined as extragastrointestinal stromal tumors (EGISTs). As it is the case for GISTs, the cases with inoperable EGIST may also respond dramatically to the treatment with oral imatinib mesylate. Herein, we reported a 70-year-old patient admitted with a giant retroperitoneal mass, diagnosed with inoperable EGIST and responded dramatically to imatinib treatment
PREVALENCE OF CORONARY ARTERY DISEASE IN PATIENTS WITH END STAGE RENAL DISEASE WITH HIGHER CONCENTRATION OF TOTAL HOMOCYSTEINE CONCENTRATION IN BLOOD PLASMA
The chronic kidney disease stage 3 to 5 (Glomerular filtration Rate GFR 59 to <15), Renal tubular dysfunction and decreased GFR which leads to decreased renal catabolism of homocysteine. Cardiovascular disease (CVD) is a major cause of death both in general population and in End Stage Renal Disease (ESRD), it accounts approx 40%. An analytical cross-sectional study is designed to find out the prevalence of coronary artery disease in patients with hyperhomocysteinemia in end renal disease patients and to discover the correlation between them. The homocysteine concentrations were found 34.66 ± 1.69 µmol/L and 17.6 ± 1.4 µmol/L for the group of patients with and without evidence of Coronary Artery Disease (CAD) respectively. Whereas the mean homocysteine concentrations of the total population were found 24.8 ± 1.85 µmol/L. This showed that patients having higher values of homocysteine concentration in blood plasma have higher chances of having CAD (p<0.050)

A COMPARATIVE EVALUATION OF ORAL HYGIENE PRACTICES AND HABITS AMONG MEDICAL & DENTAL STUDENTS IN GHAZIABAD DISTRICT
Introduction: Oral health is as an essential component of general health & is determined by knowledge of oral health behaviors. Factors like tobacco smoking, alcohol, nutritional status, and stress are associated with a wide range of oral diseases. Many oral conditions are intimately related to systemic diseases & total health care requires the combined efforts of the medical and dental professions. It is believed that dental professionals/ students get a better insight into good oral hygiene practices.
Aim: The present study was carried out to assess & compare the oral hygiene practices and habits among medical & dental students.
Materials and Methods: A structured questionnaire was prepared to inquire about oral hygiene practices, adverse oral habits, existing dental problems, dental service utilization patterns and attitude toward dental treatment& was distributed among 200 dental and 200 medical students of the second year.
Observations: Medical students had much better knowledge of some of oral hygiene practices as compared to dental students. Adverse oral habits & dental problems were comparatively more in dental students. However, the habit of regular dental checkup was more in dental students. The idea of early oral prophylaxis was lacking in both groups.
Discussion: To improve an individual\u27s overall health and well-being, oral health promotion programs providing education regarding proper eating habits, effective maintenance of oral hygiene, and avoiding tobacco use can go a long run in improving oral health among the students
DEVELOPMENT OF DELAYED RELEASE PULSATILE DELIVERY SYSTEM FOR NOCTURNAL HYPERTENSION
The aim of the present study was to develop enalapril maleate delayed release polymer-coated pellet formulation as a time-dependent delivery for treatment of hypertension having a close relationship to the chronobiology. Extrusion spheronization technique has been used with box Behnken response surface methodology to study three different factors effect on the robustness of formation i.e. maximum drug release and minimum friability. The concentration of binder showed significant impact on pellet robustness. A further novelty of this work is the use of the combination of pH and time-dependent polymers as a single coating for the development of delayed release pellet formulation. Coating of drug matrix core pellets was optimized by applying a 2-factors 3-levels full factorial design. Continuous dissolution studies were carried out in simulated gastric, intestinal, and colonic fluid with pH 1.2, pH 4.5 and pH 6.8 media respectively. The lag time prior to the drug release was highly affected by a combination of two factors, i.e. the percentage of Eud. RL in coating polymer mixture and coating % level. This experiment demonstrates that the enalapril polymer-coated pellets could be successfully prepared by the design of pH- and time-dependent modified polymer coating to get chronopharmaceutical formulation
ASSESSMENT OF LEFT VENTRICULAR FUNCTIONS BY ECHOCARDIOGRAPHY TO STUDY THE EFFECTS OF 8-12 WEEKS OF TREATMENT IN THYROID PATIENTS
Thyroid disease diagnosed by an imbalance of thyroid hormones. Increased levels cause hyperthyroidism whereas decreased levels cause hypothyroidism. Both conditions have profound cardiovascular manifestations. Left Ventricular (LV) dysfunction can be demonstrated by various echocardiography measurements and can explain such abnormalities with thyroid disease. The present study was planned to assess the effects hyper and hypothyroidism on LV dysfunction. Also the effect of thyroxine replacement and antithyroid therapy for 8-12 weeks, on LV abnormalities of hypothyroidism and hyperthyroidism respectively; were studied. Patients with hypothyroidism were reported with decreased E/A ratio which is significantly improved by thyroxine replacement therapy for 8-12 weeks
STATINS RELATED ACUTE RHABDOMYOLYSIS IN EMERGENCY DEPARTMENT STOPPING OR CHANGING STATIN DECISION
We report the case of a 72-year-old female patient, type IIB dyslipidemia, admitted in the ED for flu-like diffuse muscle pain, recently installed, progressive, apparently without a clear cause. For 3 months, the patient was being treated with gemfibrozilum and atorvastatin. The patient had suffered a myocardial infarction three years ago and right adrenalectomy as a cure for secreting adenoma. Rhabdomyolysis and important metabolic acidosis led to the decision to replace the statin and dynamic supervision of the iatrogenic myopathy. After 10 days, the clinic was improved without further reported incident. The rhabdomyolysis trigger event seems to be, apparently, associated with adding gemfibrozil to the treatment, statin being previously tolerated without incidents. However, keeping gemfibrozilum and changing the statin type, the myopathy showed short-term improvement, which raises the issue of whether changing the treatment scheme is urgently needed or just observation might be enough. Conclusions: Type of calcium channel blockers associated with the treatment should be considered when choosing a statin. Routine monitoring renal function, muscle damage biomarkers, and serum potassium level should be considered in this circumstances It seems preferable for ED to recommend changing the statin, management, and myopathy follow up, against stopping statin administration. It is likely that the interaction between these two classes of drugs to be influenced by the previous adrenalectomy and low muscle glycogen reserves due to diabetes
ELECTROCUTION IN A FOOTBALL VIEWING CENTRE IN CALABAR, SOUTH NIGERIA
Electrocution is the passage of a substantial electrical current through tissues with resultant skin lesions, organ damage, and death. Deaths from Electrocution may result from a person exposed to a lethal amount of electricity which may be due to high or low voltage. It is usually accidental in nature and homicidal electrocution is rare in Nigeria but cannot be ruled out. The maintenance of electrical installation is the poor and illegal construction of buildings such as football viewing centers, stores and an even residential apartment under high tension cables is a common occurrence. These buildings are usually marked for demolition but the occupants are adamant. We report a case of seven persons ranging in age from 13 to 65 years and comprising of a female and six male with a ratio of 1:6; who sustained various degrees of electrical burns to their upper, lower limbs, trunk resulting in death. The cause of death was sudden and determine by autopsy to be ventricular arrhythmias
THE INFECTIONS AND ANTIBIOTIC THERAPY IN PATIENTS WITH SOLID TUMORS IN PALLIATIVE CARE
Background: Patients in the palliative stadium of cancer disease are frequently exposed to infections. Introducing the antibiotic therapy to those patients is a dilemma: what benefit do they have from antimicrobial therapy regarding their short lifespan and the resource consumption of these medicines.
Objectives: We researched the most common sites and causes of infections and estimated the price and the efficiency of antibiotic therapy with respect to difficulty reduction caused by infections.
Methods: The prospective study was performed on patients hospitalized at Palliative care department in the year 2015. The antibiotic therapy, whose efficiency was estimated by the loss of symptoms related to infection, is included to 78 patients who underwent laboratory testing (Le and CRP) because they had verified clinical signs of infection.
Results: 109 causes of infection were found. Most of them were Escherichia coli, Klebsiella pneumonia, Proteus mirabilis, Pseudomonas aeruginisa and Staphylococcus aureus. The most used antibiotics were gentamicin (21.1%), meropenem (20.2%), trimethoprim-sulfamethoxazole (11.9%), amikacin and Cefepime (11.1%). The success of antibiotic therapy was 61.5%. The mortality is registered at 16.7% patients during antibiotic treatment. For antibiotic treatment, 5329.69 € (68.33 €/ pat.) is spent. The parenteral antibiotic application was used in 75.2% of cases, and peroral in 24.7% of cases. Side-effects of anti-infective therapy were recorded in 3 (3.85%) of 78 patients treated with antibiotics.
Conclusion: Antibiotic therapy reduces the intensity of difficulties in patients with bacterial infections with rare side-effects. Therefore, the main principles of palliative treatment – reduce the suffering and not to harm patients, are satisfied
EPILEPSY COMORBIDITY IN AUTISM SPECTRUM DISORDER IN ALBANIA
Introduction: The association between epilepsy and autism has been extensively described and documented during the past decade due to its therapeutic approach and clinical importance.
Objectives: This study aims to describe the comorbidity of epilepsy in autism spectrum disorder in Albania with a special focus on the effect of epilepsy on regression age and intellect (IQ).
Methods: In this retrospective study we reviewed 164 patients diagnosed with ASD at our clinic during 2011-2014. The chief complaints were developmental problems (speech delay, behavior problems, and developmental delay) and seizures. The children were evaluated by the child neurologist and the psychiatrist for children and adolescents.
Results: The comorbidity of epilepsy and ASD was found in 40 children (24%). In the comorbidity group, there was a significant positive correlation between lower IQ and early age of regression (r=0.817, n=49, p<0.001). Patients in the comorbidity group had a lower mean IQ than patients with ASD only (88.10±6.94 vs. 65.54±8.72; p<0.001).
Conclusion: Epilepsy in autism spectrum disorders is one of the additional problems to be to be dealt with in these patients. Major risk factors for seizure occurrence are mental retardation and additional neurological disorders, as well as some specific associated medical conditions
