ANTIDIABETIC TREATMENT AND CANCER PREVALENCE IN DIABETIC PATIENTS
Background: There can be a possible relationship between diabetes mellitus and cancer development, according to the literature. The aim of the present study was to investigate the relationship between diabetes and prevalence of cancer.
Method: This study consisted of 306 patients with diabetes who referred to Haseki Training and Research Hospital Diabetes outpatient clinic between 2000–2002. Patients were followed up until October 2011 and medical records were evaluated. The prevalence of cancer was investigated retrospectively in these patients. Findings were evaluated statistically.
Results: The prevalence of was cancer was established to be 3.3% in the present study. The relation between the ages and HDL-cholesterol values of patients and cancer prevalence was found to be statistically significant, respectively p 0.024 and 0.037.
Conclusion: Cancer prevalence was found to be higher than that in the normal population. However, the number of cases was low, which may be the reason why results in other studies are not similar. There is agreement that cancer prevalence increases in diabetic patients
OFFSPRING BIRTH WEIGHT AND MATERNAL FASTING LIPIDS IN WOMEN SCREENED FOR GESTATIONAL DIABETES MELLITUS
Introduction: The most common neonatal complication of gestational DM is macrosomia. During early pregnancy, an accumulation of maternal fat depots occurs followed by increased adipose tissue lipolysis and subsequent hyperlipidemia which mainly corresponds to increased triglycerides in all circulating lipoproteins. In GDM women the enhanced insulin resistance and altered estrogen-progesterone ratio are responsible for the reported wide range of dyslipidemic conditions1. Association of high maternal glucose levels and fetus macrosomia have been documented. The prospective Amsterdam Born children and development cohort study, reported that high maternal TG levels in early pregnancy were associated with higher Birth weights and subsequently a higher occurrence of LGA births, whereas low TG levels were associated with accelerated postnatal growth2. Total cholesterol, HDL and lipoprotein concentrations are not significantly different between GDM patients and control subjects3.
Materials and Methods: It is a prospective observational study done on patients coming to the department of OBG, Father Mullers Hospital, Mangalore for antenatal checkup over the period of one year and nine months. All Antenatal mothers are advised to undergo OGCT test as part of universal screening for GDM according to their convenience. Minimum of 100 Patients with positive OGCT (> 140mg/dl) were selected. Detailed clinical evaluation including relevant history and physical examination was undertaken. They are asked to undergo OGTT test.
Results: Among the total of hundred GCT positive patients studied, 41 patients were GTT positive and 59 were GTT negative. Elevated triglycerides were found in 31(75%) of GTT positive patients. Elevated serum cholesterol levels were seen in 21(51.21%) of GDM mothers. Forty-one (69%) patients in the normal GTT group had elevated triglycerides and 31(52%) among the normal GTT group had elevated cholesterol. Among the 100 patients studied, there were 25 babies with birth weight more than 3.5 kg.
Discussion and Conclusion: Various studies have been conducted in different parts of the world to correlate between maternal glucose levels and macrosomia as well as lipid alterations with fetal weight changes in normoglycemic as well as diabetic patients. Our study was also aimed at finding whether any relationship existed between the development of macrosomia in diabetic or nondiabetic patients and relationship with maternal fasting lipids, mainly serum triglycerides. Our study shows elevated triglycerides and cholesterol both in gestational diabetic patients as well as nondiabetic controls.
Conclusion: Our study did not find any positive correlation between elevated triglycerides and GDM as well as between elevated total serum cholesterol and GDM. Elevated birthweight more than 3500 g also was not significantly found to be associated with either elevated serum triglyceride levels or serum cholesterol levels
EVALUATION OF TOXICITY PROFILE AND PROGNOSIS OF TREATMENT OF HPV ASSOCIATED HEAD AND NECK SQUAMOUS CELL CARCINOMA (HNSCC), WITH CHEMORADIOTHERAPY
Head and Neck cancer is generally referred as squamous cell carcinoma of Head and Neck and are located at the oral or nasal cavity. It has been one of the leading cause of death around the globe and has significantly increased epidemiological growth and rate of mortality over the last three decades. A significant subset of the 600,000 annual cases of HNSCC includes approximately 85,000 HPV associated (oropharyngeal) tumors, which means that the head and neck region is the second most common HPV+ tumor site. In 90% of the HPV associated tumors, HPV16 detection can be seen. The objective of the proposed study is to evaluate the effects of chemoradiotherapy and its toxicity profile for the treatment of HPV negative and HPV positive carcinomas. In a case-control study, 100 patients were enrolled and grouped into two groups with HPV negative and HPV positive carcinogenic tumor in the ratio of 50:50. All the patients are exposed to the treatment with concurrent chemoradiotherapy and radiotherapy. From the study, it can be concluded that HPV- positive tumors have less cumulative exposure to multiple risk factors (tobacco chewing, alcohol, smoking). The association of tumor HPV status with treatment response observed in our study showed a trend of better treatment outcome, consistent in the design and analysis of current and future clinical trials of treatments for head and neck cancer patients

Introduction: Frozen shoulder or adhesive pericapsulitis is a common condition estimated to be afflicting 2-5 % of the general population. It presents with pain and progressive limitation of both active and passive shoulder movement. Numerous treatment options have been described in the literature. Among them, one of the treatment methods commonly used is oral corticosteroid combined with non-steroidal anti-inflammatory drugs (NSAID). This treatment has been found effective in the early stage of frozen shoulder.
Objective: Evaluation of the outcomes in patients with a frozen shoulder after short course oral corticosteroid and NSAIDS.
Methodology: In this cross-sectional study, 50 patients having an early stage of frozen shoulder presenting to the outpatient department of Koshi Zonal Hospital from July to December 2017 were included. We included those patients who had already taken more than two courses of NSAID elsewhere but symptoms did not alleviate. We excluded those patients who were diabetic, pregnant and who had a history of trauma in and around the shoulder, neuromuscular disease, suspected pathology such as a tumor, rotator cuff rupture, infection, and arthritis. Oral steroid and NSAID tablets were given to all the patients under coverage of an oral proton pump inhibitor. The shoulder home exercise program was also instituted. The follow-ups were done on the1st,7th,14thand 21stday in the physiotherapy department to measure range of motion (ROM) and shoulder pain and disability index (SPADI) to evaluate the final outcomes. The data was analyzed by using Microsoft Excel program.
Results: We found the mean age 49.46 years ( range 30-60 years), duration of illness mean 15 weeks (range 10-19 weeks), considerable improvement of ROM from the baseline. PREVALENCE OF MORBIDITIES AMONG ELDERLY POPULATION OF JODHPUR, INDIA.
Aging of a population is a matter of great concern for the health sector. The elderly are on the whole less healthy than the non-elderly. The aged population has special health problems that are basically different from those of adult or young. The present study was conducted to examine the prevalence of morbidities among elderly population of Jodhpur, India, across their age and gender background. The treatment taken or not taken for one or the other morbidities were also recorded and delineate the common health conditions affecting the elderly respondent. For this purpose, data from 320 elderly respondents (160 men and 160 women) who were residing in Jodhpur city, India, were collected and analyzed with the cross- tabulation and ChiSquare Test using SPSS version 16.0. The majority (75.9%) of the elderly respondent were in the age group of 61-75 years of age. The findings revealed that the majority of elderly respondents were suffering from Hypertension and Cataract, followed by Diabetes, Heart diseases, and Osteoporosis. Treatment seeking behavior was more prevalent for Heart Diseases (97.5%), Chronic lung diseases (96%), Diabetes (92.5%) and Hypertension (90.9%) as compared to Arthritis (87.5%), Osteoporosis (78.5%) and Depression (65.5%). Awareness among the elderly population and their caretaker should be created for regular medical check-ups to ensure prevention and early detection of the chronic diseases. Introduction: Crimean-Congo Haemorrhagic Fever (CCHF) is a viral hemorrhagic fever caused by RNA virus (Nairovirus family: Bunyaviridae). CCHF can cause severe infection in humans with a fatality rate of 25%-30%. During the March 2017, two cases of Crimean-Congo hemorrhagic fever (CCHF) were observed in Kukesi district. The first blood sample using reverse transcriptase polymerase chain reaction (PCR) test was sent looking for HF, CCHF and HFRS, and leptospirosis. HFRS and leptospirosis resulted in negative.
