TELBIVUDINE IN THE TREATMENT OF HEPATITIS B VIRUS REACTIVATION IN CANCER PATIENTS RECEIVING CHEMOTHERAPY
Background & Aims: Hepatitis B virus Reactivation (HBVR) in cancer patients receiving chemotherapy is potentially fatal condition and needs prompt diagnosis and treatment. Treatment of such patients has been attempted with lamivudine, entecavir, and tenofovir with variable success rates. Although a rapidly acting highly potent antiviral, telbivudine has not been studied in HBVR and our aim was to study the efficacy of telbivudine in such patients.
Methods: This prospective study of four patients with HBVR who were suffering from different malignancies was conducted in a tertiary care center. These patients were referred with transaminitis and on investigating proved to be having HBVR. After starting them on Telbivudine 600mg OD, these patients were followed very closely on OPD basis.
Results: 3 out of 4 patients developed HBsAg seroconversion and the drug could be stopped within 1 year of treatment in all three of them. In one patient, HBe seroconversion was achieved.
Conclusions: Telbivudine is an effective alternative to entecavir and tenofovir in cancer patients with HBVR
METFORMIN INDUCED LACTIC ACIDOSIS AND ACUTE RENAL FAILURE
Metformin is a biguanide agent and oral antihyperglycemic agent used in the treatment of type 2 diabetes mellitus. Metformin-associated lactic acidosis and acute renal failure is a very rare but life-threatening side effect of metformin. We present the case of a nondiabetic 41 years old woman, who attempted to commit suicide by ingesting more than 20 grams of metformin. She presented with lactic acidosis and acute renal failure to the emergency department. She was successfully treated by hemodialysis. The main treatment method is hemodialysis who presents with lactic acidosis and acute renal failure in metformin intoxication

AN INCIDENTIAL FINDING OF HYPERCALCEMIA IN A PATIENT WITH ACUTE MYLOID LEUKAEMIA
Hypercalcemia complicating leukemia is a rare, but well-recognized finding. Few case reports have been documented in various journals. The etiology in most cases is not fully understood; however, there is an established fact that there is a link with cytokine release. We report a case of a 65year-old man who presented with hypercalcemia, during the course of his illness. Possible causes of hypercalcemia such as hyperparathyroidism, vitamin D intoxication, solid tumors, and ectopic parathyroid hormone production were excluded. From the case report, it is concluded that leukemia may be complicated by hypercalcemia and physicians should bear this in mind as they manage patients with leukemia
CARCINOMA OF UTERINE CERVIX WITH MULTIPLE SKULL METASTASES AS THE FIRST PRESENTATION OF RECURRENCE: A CASE REPORT
Carcinoma of cervix recurring as multiple skull metastasis is a very rare presentation. A 49-yearold woman, a diagnosed case of carcinoma cervix [International Federation of Gynecology and Obstetrics (FIGO) stage IB1] treated with radical hysterectomy presented with generalized seizures to us after 40 months of initial diagnosis. Contrast-enhanced computed tomography showed metastasis to left temporoparietal bone and left frontal bone. Biopsy from the left temporal lesion showed non-keratinizing squamous cell carcinoma. She was thus diagnosed as recurrent carcinoma cervix with multiple calvarial metastases with the controlled primary site. She received palliative radiotherapy to the entire skull. She further received 4 cycles of palliative chemotherapy with paclitaxel and carboplatin repeated every 3 weeks. However, she could not tolerate chemotherapy after 4 cycles owing to significant deterioration of performance status and expired after 12 months of diagnosis of metastasis

THE DETERMINANTS AND CONSEQUENCES OF INTIMATE PARTNER VIOLENCE AMONG PREGNANT WOMEN IN TERTIARY HEALTH INSTITUTIONS IN ABIA STATE, NIGERIA
Background: It has been documented that pregnant women who are exposed to intimate partner violence (IPV) are prone to the risk of complications both for the mother and the baby. There is a need to identify the reasons for this abuse.
Objectives: To identify the prevalence, determinants, and consequences of Intimate Partner Violence (IPV) among pregnant women in Tertiary Health Institutions in Abia State.
Materials and Method: This was a cross-sectional Study of 214 pregnant women attending the antenatal clinic in the two Tertiary Health Institutions in Abia State using pretested questionnaires administered to attendees who consented to participate. Analysis of data was done using SPSS Version 16.
Results: The mean age of the 214 participants was 30.15 ± 5.88 years. The Prevalence of intimate partner violence among pregnant women in our study was 32.7 %( 70/214). We did not establish any statistical association between age of women or that of her partner, duration of the marriage, her religion, parity or educational status of the partner and IPV (p>0.05). We established a statistically significant association between social habits of the partner and IPV (p< 0.05). About 31 %( 22) of the IPV victims had sustained an emotional/ physical injury while 20 % (14) had been hospitalized. Social habits of the spouse (13%) were the highest trigger for the abuse followed by financial problems (10%). Majority of victims 68.6 %( 48) did not feel the abuse should be reported.
Conclusion: This study demonstrates a high prevalence of IPV among pregnant women
