    Mean platelet volume as a measure of alveolar hypoventilation consequent to chronic nasal obstruction

    Nasal obstruction is a common presenting symptom in everyday rhinological practice. The chronic nasal obstruction causes upper respiratory tract resistance further leading to chronic hypoxia because of alveolar hypoventilation1, 2. MPV is associated with the function and activation of platelets and used as a marker of hypoxemia3. This prospective study conducted in the Department of Otorhinolaryngology and Head and neck surgery Vinayaka Missions Kirupananda Variyar Medical College, Salem on 30 patients with chronic nasal obstruction with 30 healthy control group. This study is to determine whether MPV values are elevated in patients with chronic nasal obstruction compared to healthy controls. The study also compares the Peak expiratory flow rate (PEFR) of nasal airflow between cases and controls. The diagnosis was based on anterior rhinoscopy and diagnostic nasal examination. In the present study deviated nasal septum was commonest followed by nasal polyps.MPV values are increased in all age groups compared to controls. The study also shows PEFR is decreased in cases compared to controls

    As per World Health Organization (WHO) data - India, China and USA are among the top three countries with a high number of diabetic populations. In 2015 there were about 69.2 million patients (7.8%) affected with type 2 diabetes mellitus in India. Thyroid dysfunction was more common in type 2 diabetes mellitus subjects as compared to nondiabetics. Both diseases influence each other in several ways. Treatment of diabetic patient becomes very difficult when both thyroid disorder and diabetes mellitus exist together. Present study is designed to estimate the prevalence of thyroid dysfunction in type 2 diabetes mellitus subjects along with other biochemical parameters. Among the diabetic group 109 patients and 120 patients in control group were found to be euthyroid. Higher prevalence of thyroid dysfunction was found in patients with type 2 diabetes mellitus, 19.26% patients in Group I and 9.77% patients in Group II were found to have thyroid dysfunction. A routine assessment of thyroid function test should be performed for patients with type 2 diabetes mellitus

    Myocardial bridging is a congenital anomaly in which a segment of a coronary artery takes a “tunneled†intramuscular course under a “bridge†of overlying myocardium. This causes vessel compression in systole, resulting in hemodynamic changes that may be associated with angina, myocardial ischemia, acute coronary syndrome, left ventricular dysfunction, arrhythmias, and even sudden cardiac death. India is being laden with increasing incidences of coronary heart disease out from clinical presentation through a simplified investigation profile their morbidity may be addressed in a distinguished manner which has a less malignant course of progression. The present study is designed with an objective to identify the clinical spectrum of presentation of coronary myocardial bridge and to evaluate such cases with simple conventional noninvasive and invasive tests. All cases of angina attending the medical and cardiology OPD for the first time at the central hospital, S.E.C. Railway, Bilaspur (CG) between April 2011 to March 2016. After basic investigations all angina cases were evaluated by resting ECG and Echocardiography. Cases with normal resting Echo study were subjected to Tread Mill Test (TMT) with Bruce protocol. Total 1362 (female =354) cases of angina attended for first time in OPD. Out of which 271 (72%) cases had atherosclerotic coronary artery disease (CAD), 61(16%) cases were having CMB and 43 (12%) cases had normal coronaries. 54 patients out of 61 of CMB remained free of symptoms where as 29 out of 43 cases of CAD on medical management. First-line therapy involves medical treatment with betablockers and non-dihydropyridine calcium-channel blockers. From the present study, the researchers observed better results with betablockers added with nikorandil. A prospective randomized trial is required to identify the best treatment strategy for patients with myocardial bridging

    FEASIBILITY STUDY OF SRIKANDI CLINIC DEVELOPMENT, SEMARANG - CENTRAL JAVA BASE ON SWOT AND SATISFACTION ANALYSIS

    Objective: The Primary Srikandi Clinic is a first-class health service unit that serves the community in the West Semarang. Business development health service, a feasibility study is needed so that it has a policy direction and business development. Method: The research data was obtained based on secondary data, library studies, and direct observation as descriptive. The study was conducted in the West Semarang sub-district and patients at the Srikandi Clinic in April to May 2019 with 80 respondents. Research Results: Based on the land and market suitability analysis, the Srikandi Clinic is in a very strategic location. The location of the clinic is in accordance with the Semarang City Spatial Plan and Regional Area. Based on the analysis of patient visits, the trend of patient visits at the Srikandi Clinic is increasing, while based on the analysis of the utilization of health insurance; patients at the Srikandi Clinic have a tendency to switch aid services for the poor to non-beneficiaries. The SWOT analysis for the Srikandi Clinic is a growth-oriented strategy with a strength-opportunity strategy. The Srikandi strategy approach of the clinical satisfaction analysis based on the tangible aspect parameters with the category was very satisfied with the highest score on the aspect of physician appearance 82.1%. The responsiveness level with the highest score was 94.6% in the very satisfied category, in the aspect of officer alertness in listening to patients the complaints aspect and the reliability of the highest score of 92.9% in the categorization was very satisfied. Conclusion: Srikandi Clinic can be upgraded to a type C hospital or main clinic based on Indonesian health regulation

    FAMILIAL CHRONIC MYELOCYTIC LEUKEMIA: THE OCCURRENCE IN FIRST COUSINS IN JOS, NORTH CENTRAL NIGERIA

    Introduction: Chronic myeloid leukemia is a leukemic myeloproliferative disorder that is rarely diagnosed in members of the same family. Aim: We report the occurrence of chronic myeloid leukemia in male first cousins in Jos. Cases report: Two cousins 37 years and 31 years old were referred to our department for leucocytosis in the first patient and priapism and leucocytosis in the second. Full blood count and bone marrow aspiration cytology in both cases revealed granulocytic leucocytosis with all the stages of myeloid maturation in peripheral blood and marrow myeloid hyperplasia in keeping with chronic myeloid leukemia. Treatment with tyrosine kinase inhibitor (TKI) was delayed in both, due to financial constraints for transportation to the treatment center in Nigeria. Cytoreduction with hydroxyurea was the initial treatment instituted. Even with the eventual commencement of imatinib mesylate, the outcome was still poor in the second patient who transformed and died within one year. Conclusion: Chronic myeloid leukemia has occurred in family members in our environment

    Pattern and predisposing factors of neurological disorders in a neurology outpatient clinic in Port Harcourt, Nigeria

    Background: Neurologic disorders are a major cause of morbidity and disability in childhood. With this in mind, understanding the present trends of neurologic disorders in any locality would help in the designing and implementation of preventive health strategies. Objectives; To determine the pattern and predisposing factors of neurologic disorders presenting at the Paediatric neurology outpatient department of the Rivers State University Teaching Hospital in Port Harcourt, Nigeria. Methodology: This was a prospective study carried out in the Paediatric neurology clinic of the Rivers State University Teaching Hospital from January 2015 to December 2017. Results: A total of 21,600 patients were seen in the Paediatric outpatient department in the period under review out of which 303 (1.4%) were children with neurologic disorders, 7 patients were excluded from the analysis. Their mean age was 3.5± 3.7 yrs with a M: F ratio of 1.5:1. Antenatal care was done in a hospital by 244 (82.4%) of the mothers while 78(26.4%) children were delivered by Traditional birth attendants (TBA) or unqualified attendants and 196(66.2%) were born at term via spontaneous vaginal delivery 229(77.4%). Single neurologic disorder was seen in 171(57.8%) children. The common neurologic disorders were Seizure disorders 152(51.4%), cerebral palsy 141(47.6%) and microcephaly 87(29.4). The identifiable predisposing factors were perinatal asphyxia 91(30.7%), meningitis 80(27%) and neonatal jaundice 37(13%). Conclusion. Seizure disorders and cerebral palsy are the commonest neurologic disorders in Port Harcourt, Nigeria. The predisposing factors are mostly preventable

    Diabetes is a condition defined by elevations in glucose. Historically, glucose measured in the fasting state or glucose measured two hours after a carbohydrate challenge (oral glucose tolerance test) have been the standard measures used to diagnose diabetes. Apart from this the researchers also reported various other shortcomings of measurement of blood glycemic value by HbA1c. A1C is a poor indicator of what occurs in the postprandial state. A1C captures only chronic hyperglycemia. Developed societies in which diabetes diagnosis is made with A1C and less developed societies (between and within countries) in which diabetes diagnosis is made with plasma glucose: such a division should be avoided. It would add to the inequities in health and health care. The objective of this article is to elaborate the uses of some nontraditional markers of hyperglycemia like 1. Fructosamine 2. Glycated albumin 3. 1,5-anhydroglucitol 4. Continuous glucose monitoring to be taken into consideration for estimation of blood glucose level as an alternative for HbA1c level determination

    MRSA is often used to describe multi-drug resistant Staphylococcus aureus. Resistance is mediated by a gene (mecA) that encodes the production of an altered penicillin-binding protein (PBP2a), which does not allow for the binding of ß-lactams to the bacterial cell wall, because ß-lactams exert antibacterial activity by binding and inhibiting enzymes necessary for bacterial cell wall synthesis, so these antimicrobials are not effective against MRSA.
