    An intellectual disability is defined as an IQ below 70 and deficits in adaptive behavior or daily living skills (eating, dressing, communication, participate in a group activity). Children with intellectual disabilities learn slowly and have difficulty with abstract concepts. Children with intellectual disabilities have a wide range of needs and most exhibit behavioral problems. Around 7-15% of children with ID have severely challenging behavioral problems. The nature and severity of these behavioral problems vary with the degree of Intellectual Disability. Modification of behavioral problems in children is a great concern for parents in India. In the present study, it will be attempted to analyze the efficacy of Psycho Neurobics, innovative techniques designed by my Guru and guide Dr. Chandrashekhar Tiwari, founder president of SIGFA Institute of Research and Development, Faridabad, Haryana, India, for behavioral modification provided to 12 students with ID in Delhi, India.     MANUAL NUCLEAR DIVISION IN ANTERIOR CHAMBER WITH MVR BLADE FOR 4MM SICS – SAFETY AND EFFICACY

    Purpose: Critical evaluation with respect to intraoperative and postoperative complications, visual outcome and cost effectiveness in a 4mm small incision cataract surgery by phacofracture with MVR blade and conventional SICS. Methods: 300 patients were divided into two groups 150 each. Patients had grade I-IV nuclear sclerosis and were operated under peribulbar anesthesia. 4 mm scleral tunnel made 2mm behind limbus. Capsulorhexis done, Hydrodissection done. Nucleus prolapsed in anterior chamber. Viscoelastic injected behind and in front of nucleus. Wire vectis passed through the main incision below the nucleus to stabilize it 20G MVR blade was introduced from 11O’clock and pierced through nucleus substance.MVR blade pressed against wire vectis and nucleus was bisected into two halves. The fragmented nucleus halves removed through main incision. Cortical wash was given and foldable IOL was implanted. Results: Out of 300 patients 1.03% had grade I cataract, 20.05% had Grade II, 55.01% had Grade III and 24.04% had Grade IV. Commonest intraoperative complication was iridodialysis and extended rhesix, while post operatively striate keratopathy and cystoids macular edema were noted. There was no significant difference noted between the two groups. Also the surgically induced astigmatism was assessed post operatively and keratometric readings compared between the two groups. There was no statistical difference between the two groups in term of induced astigmatism. Conclusions: Micro MSICS is more cost effective, with no major complications, similar post operative astigmatism, provides early rehabilitation & good visual recovery with short learning curve

    The study explored the viability of psychoneurobics on memory among highschool youngsters matured 14 to 16 in JSS High School, Lakshmipuram, Mysore, Karnataka State, India. The study utilized one gathering pretest and post-test, pre exploratory structure. The population for the study involved poor entertainers of tenth standard understudies of various segments of High school in Mysore.     Introduction: Lower limb trauma is a common presentation in our day to day clinical practice. Distal diametaphyseal tibia being subcutaneous bone is one of the common fracture sites. Open reduction and internal fixation (ORIF) could further endanger their circulation adding iatrogenic trauma. Closed reduction and minimally invasive plate osteosynthesis (MIPO) with pre-contoured locking plate on basis of biological osteosynthesis (BO) is an alternative treatment option. The aim of this study is to estimate hospitalization period, fracture union period and complications of MIPO with pre-contoured locking plate on diametaphyseal distal tibia fracture. Objective: Evaluation of outcomes after MIPPO in distal diametaphyseal fractures managed by using distal tibial pre-contoured locking plate at Koshi Zonal Hospital. Methodology: Twenty-seven patients with close distal tibia diametaphyseal fracture were treated with MIPO technique between May 2016 to June 2018 in Koshi Zonal hospital. Results: There were 27 patients with mean age 43.83 years ( range: 18- 70 years). The average duration of hospital stay was 11.94 days (range: 6-25 days). Average fracture union duration was 21.05 weeks (range:17-27weeks); among them, two cases got a superficial infection. There was no angulation and no implant failure. Eight patient complained implant prominence and pain around ankle after fracture healed. Conclusion: MIPO with pre-contoured locking plating in distal tibia diametaphyseal fracture is a rational choice with short union time, few complications and early rehabilitation

    Background: Cerebral palsy is a heterogeneous condition associated with a non-progressive lesion causing permanent disorder of movement with limited mobility, and is generally associated with gross motor developmental delay. In moderate to severe cases of cerebral palsy, motor developmental milestones such as walking may never be achieved. Impaired cognition and delayed speech are also common.  Patients and methods: Seventeen patients with cerebral palsy are described in this paper including two female patients whose early treatment courses were included in previous publications, and 15 new cases (11 males and 4 females) observed during seven months period (May-November,2019). Their ages ranged from 10 months to 9 years. Ten patients had significant spasticity limiting their movements. All patients had developmental delays including delayed speech. Nine patients were unable to sit without support, including a patient with significant dystonia, and a patient who could stand and walk with support but was unable to sit without support. Only two patients were able to walk alone, but slowly and with difficulty. Two patients had history of birth asphyxia, and one patient had a genetic condition with 2 of his brothers being affected. The patients were treated based on our published experiences with individualized treatment plans providing a combination of various interventions including nutritional support, muscle relaxants, oral pyritinol, intramuscular piracetam, citicoline (oral and injectable), intramuscular cerebrolysin, and intramuscular nandrolone decanoate. The aims of these therapies include overcoming spasticity, repairing the brain and improving its function, and ultimately improving mobility and advancing development.  Results: All patients experienced improvement in motor development without the occurrence of any side effect. However, it was not possible to document the details of treatments and follow-up for all patients, but it was possible to provide an illustrated demonstration of improvement in seven patients.  Conclusion: Cerebral palsy is a heterogeneous condition, and the emergence of a single therapeutic agent that offers a comprehensive effect to improve its manifestations is very unlikely is the near future. Therefore, the use of evidence-based multi-factorial therapies is advisable. Adequate muscle relaxation is vital to prevent the complications of contractures which appear to cause a progressive disability

    Study of clinical and surgical association in patients having combined coronary artery and cardiovascular diseases.

    The association between coronary artery disease and non-coronary atherosclerosis is explained by cardiovascular risk factors. A large majority of patients with combined coronary and carotid disease live for more than 5 years. An increase in CIMT (carotid intima-media thickness) by carotid Doppler ultrasound examination is associated with the presence and extent of CAD. The present study is designed with an objective to study the association of coronary artery disease with various cardiovascular complications. This is a prospective study of patients with proven coronary artery disease or detected on workup for surgery along with significant non-coronary cardiovascular diseases. A total of 40 patients who met with the inclusion criteria were included as a subject for the study. Patients from both the genders from the age 31- 80 years were included. The youngest patient was a 38 yrs lady who presented with aorto bi iliac disease 2 years after undergoing CABG. All the selected subjects were assessed for the clinical presentation of the lesion and a maximum 30 patients were observed with Aortoiliac disease. Diabetes was the most common co-morbid factor which affected 36 patients among the selection. Post-operative observation revealed that 30% of patients were found to be diseased by Transient renal dysfunction. This evident that the patients with vascular disease have high concomitant coronary artery involvement with disease. Patients with significant associated carotid stenosis benefit by combined Carotid endarterectomy and CABG. Medically treated patients of ischemic VSD have grim prognosis; nevertheless, cardiogenic shock is an important predictor of surgical outcome

    Risk factors of female reproductive cancers in Indian inhabitants.

    Cancers of the female reproductive system are a major cause of mortality in females especially among rural areas where lack of awareness, poverty, and illiteracy still exist. Even though treatments are available, not every individual is capable of bearing expenses. A few preventive measures can be taken against risk factors that have suspect to cause female reproductive cancer in the future. These risk factors include early menarche, child marriages, early pregnancy, age at first childbirth, late menopause, alcohol consumption, obesity, occupational exposure to pesticides, smoking, use of oral contraceptive pills, and cumulative menstrual cycle lifetime. Preventive measures can be taken to avoid risk factors causing female reproductive cancers. Utmost efforts should be made to teach women in early cancer detection by creating awareness of risk factors and symptoms. Gynecological cancer screening helps in early identification and therefore improves the overall outcomes

    Assessment of knowledge, attitude, and practice of pharmacovigilance in a medical college of Nepal

    Introduction: Pharmacovigilance is the science that relates to the collection, detection, assessment, monitoring, and prevention of adverse drug reactions (ADR). The incidence of ADR is 2.4-6.5% in western countries, with only 6-10% reported worldwide. The under-reporting of ADR is due to inadequate knowledge, attitude, and practice among the prescribers about the system. Thus, medical students who are the future drug prescribers bear a crucial role in bridging this gap. Materials and Methods: This cross-sectional questionnaire survey was carried out among 261 medical undergraduates including interns at Maharajgunj Medical Campus in Kathmandu. A self-administered questionnaire was used for data collection. The responses were analyzed and descriptive statistics are presented as frequencies and percentages. Results: In this study, 94.3% of students responded that doctors, nurses, and pharmacists need to report ADR as they encounter them but only 13.8% responded to have reported ADR to the concerned authority. Also, 97.3% of respondents answered that ADR reporting should be taught in their medical college. Conclusion: In conclusion, medical students are not adequately aware of the ADR reporting system. A proper orientation to pharmacovigilance should be given to the medical students by incorporating it into the medical curriculum and providing training to future healthcare professionals
