    Hypoglycemia is a major limiting factor in achieving glycemic control in patients with diabetes. The American Diabetes Association recommends an HgA1C goal of less than 7% in most patients, and the American Association of Clinical Endocrinologists recommends an HgA1C less than 6.5%, if achievable without significant hypoglycemia. Landmark studies, such as the Diabetes Control and Complications Trial (DCCT) and the United Kingdom Prospective Diabetes Trial have clearly shown that tight glycemic control can prevent or delay the development of microvascular complications, such as retinopathy, nephropathy, and neuropathy, in type 1 and type 2 diabetes, but with aggressive glycemic targets comes an increase of hypoglycemia risk

    Endodontic management of maxillary canine with two root canals: A case report

    For a successful outcome of surgical or non-surgical root canal treatment an in-depth knowledge of the root canal anatomy is a must. Understanding the root canal is an essential part of root canal treatment. This article aimed to present a case of endodontic management of maxillary canine with two root canals in a single root. Cone Beam Computed Tomography (CBCT) was further used to confirm their presence. This article highlights the importance of a thorough knowledge of the root canal anatomy and their aberrations, emphasizing on the use of advanced diagnostic techniques to further aid towards the success of the root canal treatment

    Relationship between various indications and endoscopic findings in patients presenting to the tertiary care hospital in Peshawar

    Objective: To determine the relationship of various indications for upper GI endoscopy with the positive findings after endoscopic evaluation and biopsy. Material and methods: prospective analytical study, at the department of medicine Kuwait Teaching Hospital Peshawar from 1st January 2015 till 31st December 2015.  After approval from ethical research committee the study was conducted with patients recruited through non probability consecutive sampling presenting to the OPD. Results: Among the total 1499 patients examined 18% were females and 82% were males. The data was stratified in to various age groups and the indications for endoscopy were assessed accordingly, the highest no. 78.7% (1180)  presented with retrosternal burning, 38.8% (581) presented dyspepsia, 10.8% (161) presented with gastro esophageal reflux, 10.4%(156) has unexplained anemia, 10% (151) had persistent vomiting, (8.2%) 123 reported dysphagia, 6.3%(101) reported chronic diarrhea, and 2.2% (33) presented with hematemesis.  endoscopy showed 43.5 % (651) with normal findings, 27.2% ( 407), pan hemorrhagic gastritis, 8.3% (124) reflux esophagitis, 6.2% (93) had gastric adenocarcinoma, esophageal carcinoma was found in 2.9 % (75). other findings were,  H. Pylori gastritis in (33) 1.83%,  esophageal candidiasis in (29) 2% of the cases, gastric ulcer in 24 (1.6%) , duodenal ulcer in ( 25) 1.6%,  duedenitis in (12) 0.8%, achalasia in (9) 0.6%,  duodenal growth in  (5) 0.3% , Antral gastritis in (5) 0.3%, villous atrophy in 4 (0.26%)  Barrett`s esophagus in (2) 0.1%, and drug induced ulcer in 0.06% (1).  The result showed, age having no significant relationship with any of the indications whereas sex association showed Melena being common in males and GERD and unexplained abdominal pain in females. Results of Cohort study shows that symptoms like PMD, and Melena has a negative relationship with endoscopic findings suggesting a very limited role of these indications for a subsequent endoscopy ,Whereas dyspepsia, Anaemia, unexplained abdominal pain, Dysphagia, GERD and hematemesis are valid indicators as they bear a positive relationship with AEF. Conclusion: . Symptoms like post-meal distension and melena found are misleading indications, whereas nonresponsive dyspepsia, unexplained anemia, unexplained abdominal pain, dysphagia, and GERD, and hematemesis are all valid indications for upper GI endoscopy

This new virus and disease were unknown before the outbreak began in Wuhan, China in December 2019. On 11th March 2020, WHO declared the COVID-19 outbreak as a pandemic and reiterated the call for countries to take immediate actions and scale up the response to treat, detect and reduce transmission to save people’s lives. The COVID-19 global crisis has given rise to the new normal way of surviving, both personally and professionally. With individuals restricted within the four walls of their homes, mental health has taken a severe hit. Sixty-one percent of Indians are experiencing mental health-related issues because of the uncertainty and looming financial crisis during the lockdown as per the survey conducted by The Mavericks India. Women are struggling more than men as their workload has increased significantly with them carrying multiple responsibilities without any assistance from domestic helpers. It is crucial for the wellbeing of the people to lower down the effect of mental stresses to heal Psychosomatic diseases like mood, anxiety disorders, loss of memory, diabetes, obesity, hypertension, cardiovascular disease, thyroid, chronic back pain, arthritis, migraine, respiratory disorder, hepatitis, sleep disorders, kidney problems, allergies, muscular atrophy, etc. are more prevalent and the incidence of schizophrenia has increased. In the present study, it will be shown the effectiveness of Psycho Neurobics, innovative techniques designed by my Guru, and guide Dr. Chandrashekhar Tiwari, founder president of SIGFA Institute of Research and Development, Faridabad, Haryana, India, on psychosomatic disorders arising due to lockdown. The results will be statistically analyzed and the probability for complete healing will be discussed

    Hiccups are common and often don’t require treatment. Intractable hiccups, on the other hand, are often bothersome for the patient and require therapy. Formerly known as “psychogenicâ€, “functional†or “hysteriaâ€- “conversion disorder†usually present with either or both sensory and motor function deficiencies. Common in females and having a varied presentation, conversion disorders are difficult to diagnose. When an uncommon presentation of them like intractable hiccups present in sex with a lesser predisposition, the diagnosis may be missed and not appropriately managed

    Concrescence of erupted second molar and impacted third molar: A report of two cases and literature review

    Concrescence, a rare dental developmental anomaly, is characterized by adherence of cementum of two adjacent teeth apical to the cementoenamel junction. The incidence of this anomaly in the maxilla is greater than that in mandible. Numerous factors such as insufficient space, localized trauma, chronic periapical/ periodontal inflammation and excessive occlusal forces play an imminent role. However, the exact etiology is not known. Diagnostic 2D imaging modalities offer certain disadvantages such as overlapping of the surrounding structures as well as distortion of the image which interferes with the diagnosis of the condition. This may be overcome by considering Cone beam computed tomography modality. The present article reports two cases of concrescence in the posterior maxilla and highlights the importance of CBCT as a diagnostic imaging tool in the evaluation and management of the same

    Polycystic ovary syndrome (PCOS) is one of the most communal endocrine illnesses in women of reproductive age. The clinical and biochemical presentation is heterogeneous, but elevated serum concentrations of androgens are the most consistent biochemical abnormality and may be considered to be the hallmark of the syndrome. Many women with PCOS also have insulin resistance and hyperinsulinemia, which may contribute to the clinical and endocrine abnormality. Obesity, central obesity and insulin resistance are powerfully concerned in its etiology. Dietary weight loss is recommended as the primary treatment approach. Our aim is to control prevalence, etiology of PCOS, diagnosis and its association numerous factors in PCOS patients, so as to encourage young women to treat timely treatment and prevent long term complications

    Substantial evidence based on results of new physics experiments and physiological research affirm Starling’s law wrong. Here we demonstrate that the Revised Starling’s Principle (RSP) is also misleading. The article is a futile attempt to revive Starling’s law after it has long been dead and buried. A most recent article seriously criticized RSP. We hope the excuse for the authors is unawareness of new contributions rather than a desperate attempt to defend the indefensible. Nether Starling nor the authors who made the hypothesis a law were aware of the discovery of the precapillary sphincter that demonstrates capillary pressure induce suction not filtration as demonstrated in the G tube, and the wide intercellular slit pores of the capillary wall that allow the passage of plasma proteins, thus nullifying oncotic pressure in vivo. In addition to previously reported 21 reasons affirming Starling’s law is wrong we add two more that concern the main reports that transformed Starling’s hypothesis into a law. Physiologists and physicists are concerned about formulae and calculations while physicians are more concerned about the lives and safety of their patients. Hence physicians understand the importance of discarding Starling’s law; being wrong is responsible for the induction of the new volume kinetic shocks and the causation of the acute respiratory distress syndrome that kills hundreds of thousands of patients every year. Now, there is a replacement for it: The hydrodynamic phenomenon of the porous orifice (G) tube. It is time to say goodbye Starling’s law, hello G tube

    Effect of sublingual Vitamin D3 on chronic kidney disease.

    The present study was aimed to study the effect of Sublingual Vitamin D3 on Chronic Kidney Disease. This was a non-randomized clinical trial and interventional study. Total of 400 patients were enrolled for the study. All the subjects were screened for the serum 25(OH)D level. Subjects fulfilling the eligibility criteria with serum 25(OH)D level less than 20 ng/ml, having history of renal dysfunction were included in the study. At the end of the study, 49 patients (24 from Test and 25 from control group) dropped out of the study due to their personal reasons and finally 351 patients participated till the end of the study,176 patients in test group and 175 in control group. There was statistically significant difference in serum vitamin D3 level before 16.61±6.71 ng/ml and after 35.80±7.80 ng/ml after treatment with Sublingual Vitamin D3, whereas in the control group decreased from baseline 16.40±3.63ng/ml to 14.96±4.94 ng/ml. As per the information is taken from the patients, 38 participants in the test group (21.59%) and 41 in the control group (23.42%) had a history of renal dysfunction in our study. In our study there were no significant changes in the Renal Function Test on giving sublingual vitamin D3 further multicentred trials with large sample size are required to come to a conclusion. However, improvement in hypertension as a result of vitamin D3 could prove effective in treating CKD. further multicentred trials with large sample size are required to come to a conclusion
