    Telerehabilitation and remote occupational therapy: research and practice in Japan and elsewhere

    Telerehabilitation refers to providing rehabilitation services from a remote location using information and communication technology. It is already in operation for certain patients who cannot easily access hospitals and other facilities, and it widens access to rehabilitation services. In Japan, the impact of COVID-19 has made it difficult to provide face-to-face rehabilitation, and the pandemic has drawn attention to the use and potential of telerehabilitation. In the field of occupational therapy, telerehabilitation has been practiced to people in various age groups with a variety of medical conditions in several countries. In this review article, we summarize efforts and highlight important research on telerehabilitation in Japan and elsewhere, with particular attention to its use in the field of occupational therapy.REVIEW ARTICLEothe

    Association Between Recovery and Participation in Meaningful Occupations Among Hospitalized Patients with Mental Disabilities

    〔目的〕近年、 精神科医療において "personal recovery" という考え方が注目されており、リカバリーに影響する様々な因子が検討されているが、個人にとって意味のある作業への参加状況(作業参加)との関連は十分に検討されていない。本研究では, リカバリーと作業参加の関連を明らかにすることを目的とした。〔方法〕入院する精神障害者30名を対象に、Recovery Assessment Scale (RAS)、Self-completed Occupational Performance Index (SOPI)、その他個人属性を調査した。〔結果〕RASの総合点は教育年数と有意な負の相関が、SOPIの総合点や「余暇活動領域」、「作業バランス」とそれぞれ有意な正の相関が認められた。重回帰分析の結果、RASには「教育年数」(β=-0.437, p<0.01)、「SOPI作業バランス」(β=0.457, p<0.01)が影響を与えていることが明らかになった。〔結論〕本研究によりリカバリーに作業参加が影響を与えることが明らかになった。精神障害者への支援の一つとして、個人にとって意味のある作業を見出し、それに取り組む支援を行う必要性があると考えられる。[Purpose] The concept of "personal recovery" has gained much attention in the domain of psychiatry in recent years. Various factors have been shown to affect recovery; however, the association between personal recovery and participation in meaningful occupations (occupational participation) remains unclear. In this study, we investigated the association between recovery and occupational participation. [Methods] The Recovery Assessment Scale (RAS) and Self-completed Occupational Performance Index (SOPI) scores and other personal attributes were analyzed in 30 hospitalized patients with mental disabilities. [Results] The overall RAS score was significantly negatively correlated with the number of years of education and significantly positively correlated with the overall SOPI score, the "leisure activity domain," and "occupational balance." Multiple regression analysis revealed that "years of education" (β=-0.437, p<0.01)and "SOPI occupational balance" (β=0.457, p<0.01) were associated with RAS. [Conclusion] This study highlights the effect of occupational participation on recovery. Identification of a meaningful occupation and provision of support to participate in it may be a useful strategy to support individuals with mental disabilities.Original Articledepartmental bulletin pape

    Catastrophic thinking and worsening subjective health of community-dwelling older adult women with chronic pain

    Objective: Catastrophic thinking in patients with chronic pain (CP) causes inactivity and depression. In this survey, we intended to clarify the cutoff value of the subscales (rumination, magnification, and helplessness) of the Pain Catastrophizing Scale (PCS) with the PCS scores of community-dwelling older adult women with chronic pain. Methods: The subjects were 121 women with CP among local residents who received a medical check conducted in Kaizuka City, Osaka Fu. The survey items included demographic data, pain-related surveys, physical and psychologic functions, and health-related indicators. Results: The subjective health was found to be significantly associated with the presence of catastrophic thinking (p < 0.05). Additionally, rumination and magnification, the subscales of the PCS, had moderate sensitivity and specificity. Furthermore, the cutoff value was clarified to be 7.5 for the rumination score and 3.5 for the magnification score.     大阪府貝塚市の地域在住高齢者における外出自粛状況とオンライン動画視聴の実態調査

    〔目的〕大阪府貝塚市在住者を対象に新型コロナウィルス感染症拡大に伴う緊急事態宣言時の外出自粛の状況とオンライン動画などの視聴状況と保有デバイスおよび心身状態を調査することとした。〔対象と方法〕対象は大阪府貝塚市にて2018年、 2019年に実施したヘルスチェック事業参加者504名とした。郵送による自記式の悉皆調査を実施した。返送があった285名(56.5%)のうち、64歳以下の者および記入に欠損が有った者の50名を除く235名(46.6%)、平均年齢75.6±5.7歳を分析の対象とした。郵送時期は1回目の緊急事態宣言解除後2ヵ月経過した2020年7月から8月とした。調査項目は、緊急事態宣言時の外出自粛の程度、現在の体調、オンライン(インターネット)の活用状況、保有デバイスの種類および心身状態とした。〔結語〕対象者235名における緊急事態宣言中の外出自粛した者は227名(96.6%)であり、現在の体調について「良い」、「まあまあ良い」、「普通」に該当した者が212名(91.6%)であった。次に、オンライン動画活用状況では「オンライン(インターネット)で体操や運動の動画を見ていますか」について、 "はい" と回答した者は全体で30名(12.8%)であり, デバイスの保有状況はスマートフォンが131名(55.7%)、タブレット18名(7.6%)、パソコン18名(7.6%)であった。オンライン活用群30名は身体状況としてロコモ25スコアが良い傾向であった。[Purpose] The purpose of this study was to measure degrees of refraining from going out and watching online exercise videos in community- dwelling older adults in Kaizuka city, Osaka, Japan under the declaration of emergency situation on the first COVID-19 epidemic in 2020. [Subjects and method] A total of 504 participants of the health check project conducted in 2018 and 2019 in Kaizuka City, Osaka Prefecture, were included. A self-administered survey was conducted via mail; data were collected from 285 people (56.5%). The analysis included 235 (46.6%) individuals with a mean age of 75.6 +- 5.7 years. Individuals aged 64 years or younger and those whose entries were incomplete were excluded. The mailing time was from July to August 2020, two months after the first emergency period ended. The survey items included the degree of refraining from going out when the emergency was declared, the current physical condition, the status of online (Internet) utilization, the type of device owned and physical and mental condition. [Conclusion] Of the 235 subjects, 227 (96.6%) refrained from going out during the state of emergency, and 212 (91.6%) were "good", "fairly good" or "normal" in their current physical condition. Next, in terms of online video utilization status, 30 (12.8%) of the respondents answered "yes" to "Are you watching gymnastics and exercise videos online (Internet)?" In addition, 131 (55.7%) of the devices used for watching internet videos were smartphones, 18 (7.6%) were tablets, and 18 (7.6%) were personal computers. The 30 (12.8%) people in the online use group tended to have a good GLFS 25 score, which indicated their physical condition.Original Articledepartmental bulletin pape

    Effects of early physiatrist and registered therapist operating rehabilitation (PROr) in patients with stroke

    The neurological effects of early post-stroke mobilization have recently been reported in animal and clinical studies. In this review, we summarize previous findings on the effects of early post-stroke mobilization. Focusing upon stroke patients, the A Very Early Rehabilitation Trial (AVERT) Phase II and Phase III trials examined the effects of early mobilization within 24 h of a stroke. Early mobilization, results suggest, could be both potentially harmful to and potentially beneficial to the post-stroke recovery process. However, while the effect of early rehabilitation on ADL in most previous studies has been examined by nurses and therapists, and only a small number of studies have reported similar assessments conducted by therapists specializing in post-stroke rehabilitation. Conversely, at our hospital, first assessment is by a physician, who then refers patients to a physiatrist, who in turn evaluates the specific needs for rehabilitation, and then refers them to a registered physical therapist and occupational therapist with the aim of providing early mobilization (physiatrist and registered therapist operating rehabilitation: PROr). Based on this background, we conducted two studies to examine the effects of PROr in patients that have had a stroke. Rehabilitation within 24 h of stroke onset did not increase the risk of mortality or recurrence, but several benefits were suggested.REVIEW ARTICLEothe

    Current source density and functional connectivity of frontal midline theta rhythm during craft activities: EEG-LORETA study

    Frontal midline theta rhythm (Fmθ) has been shown to help connect distributed neural systems into brain functional networks for task execution. In this study, we investigated the current sources and functional connectivity of Fmθ that appeared during manual arts and crafts activities using the low resolution brain electromagnetic tomography (LORETA) method. Current sources of Fmθ are located in the anterior cingulate cortex and frontal lobe, and there are many functional connections in the delta, theta, alpha, beta, and gamma bands.     Combination of occupational therapy and neuromodulation for psychiatric disorders: literature review

    Various clinical studies have been conducted in Japan and elsewhere on occupational therapy. Clinical studies in the psychiatric field in particular face various difficulties, and most reports are limited to case reports and qualitative comparative studies, and few objective and quantitative studies have undergone scientific verification. Conversely, there are various methods of neuromodulation, such as repetitive transcranial magnetic stimulation, transcranial direct current stimulation, electroconvulsive therapy, and vagus nerve stimulation, which have undergone remarkable development in recent years. These methods have been widely applied clinically, mainly in the fields of neurology and psychiatry. Although there have been many reports of intervention studies combining these neuromodulation methods with rehabilitation such as physical therapy and occupational therapy in the physical field, most of these studies have focused on motor function after cerebrovascular diseases. In psychiatry, however, only a few studies have combined neuromodulation and occupational therapy. Systematic research on combined therapy for psychiatric disorders is highly anticipated. In this article, we review clinical studies that used a combination of occupational therapy and noninvasive brain stimulation for psychiatric disorders, with discussion of the problems and prospects in this area.REVIEW ARTICLEothe

    Relationship Between Sleep and Chronic Pain in Elderly Community Dwellers

    【目的】地域在住高齢者を対象にアンケート調査を実施し、睡眠と慢性疼痛との関連性について調査することを目的とした。【対象及び方法】対象は、アンケート調査にて回答が得られた大阪府貝塚市に在住する地域高齢者 119 名(男性 16 名、女性 103 名)とした。アンケート項目は疼痛強度、破局的思考、疼痛罹患期間、ピッツバーグ睡眠質問表日本語版、老年期うつ病評価尺度を自己記入式で調査を実施した。また、慢性疼痛の有無とピッツバーグ睡眠質問表を用いて重症度別に 6 群に分類し、各項目で統計解析を実施した。【結果】6 群を比較したところ、疼痛強度や破局的思考では一部に有意な差が認められたが、慢性疼痛の有無による睡眠障害の重症度には有意な差は認められなかった。【結論】地域在住高齢者の睡眠障害と慢性疼痛との関連性は低いことが示唆された。articl
