Chapter 1  
General introduction 
Fishmeal (FM) is a high protein and a palatable ingredient, so it has been used a major dietary 
protein source in carnivorous fish as yellowtail Seriola quinqueradiata.. However, FM is high demand 
but low supply resulting in it necessary to replace FM in feeds with economically viable and 
environmentally friendly plant protein sources.  It is inevitable to substitute alternative protein sources 
for FM in aquaculture feed. Among alternative protein sources available, defatted soybean meal 
(SBM) and soy protein concentrate (SPC) are some of the promisingly alternative feed ingredients to 
FM due to their ready availability, high protein content, good digestibility and relatively well-balanced 
amino acid profile. However, low feed intake and poor growth performance are obstacles when SPC 
has totally replaced fishmeal (FM) on yellowtail feed.  Therefore, in order to solve these problems, 
the study focused 
(1) To improve feed intake and growth of soy protein concentrate based non fishmeal diet.  
(2) To clarify responsibility of remaining alcohol soluble components in SPC. 
 
Chapter 2  
Improvement in feed intake by soy lecithin 
  One of the bottlenecks in a high substitution of FM by alternative protein source is a reduction of 
feed intake by fish. Thereby, feeding stimulants to enhance feed intake of fish has been studied in 
yellowtail by an electrical method or by a feeding method. In the latter study, L-proline, L-alanine and 
Inosine-5’ mono-phosphate of a synthetic compound mixture (FS) have been verified to enhance feed 
intake. However, a high inclusion of soy products with FS does not seem to have the same degree of

palatability to FM. This suggests that the reduction in feed intake of soy-containing diet is not only 
tasteless but may contain unpleasant taste. Isoflavone and saponins are commonly found in soy 
products and have been known has a bitter taste. Since fish avoid bitter taste ingredients such as tannin, 
low feed intake observed in soy product containing diet may be due to a bitter taste. On the other hand, 
phosphatidic acid, which is a fraction of phospholipid, has been reported to reduce a bitter taste through 
inhibition of a bitter taste receptor. In fact, enhancement of feed intake by a soy-origin phospholipid 
(soybean lecithin, SBL) supplementation has been reported in Japanese flounder and amber jack. In 
their studies, SBL was tested in the feed containing animal origin ingredients, while the efficacy of 
SBL on diet without animal protein sources has never been tested before. 
 In the present study, three experiments were conducted to investigate whether or not SBL enhanced 
feed intake of no-fish meal SPC-based diet in yellowtail, and also to clarify which component of SBL 
was effective. In experiment 1 and 2, ten fish (26.9 g, 53.8 g in exp. 1 and 2, respectively) in were fed 
each SBL level diet at 0 % to 5 % with FS supplementation for 17 days in duplication. These results 
revealed that daily feeding rate (DFR) of fish fed SBL at 2% was significantly higher than that of SBL 
at 0% and further increase of SBL in the diet did not increase the DFR. Moreover, the DFR of fish fed 
diet without FS was the lowest among the dietary treatments indicated that the SBL does not have a 
feed intake enhancement effects in yellowtail suggesting it may act as a masking agent against 
undesirable taste of SPC. In experiment 3 five fish (54.9 g in average BW) were distributed into 200L- 
tank for each treatment diet and fed PL fraction diet at 2% inclusion for 17 days in duplication. DFR of 
phosphatidylcholine and phosphatidylinositol concentrations in the products fed fish was superior to 
others and those were positively correlated to the DFR of fish.  
These results revealed that these two fractions: phosphatidylcholine and phosphatidylinositol are 
effective components of the 2% SBL inclusion to enhance feed intake of yellowtail with presence of 
FS supplementation. 
 
Chapter 3  
Determination of taurine requirement 
Taurine is a sulfur-containing amino acid naturally found in animal tissues, whereas terrestrial plants 
are devoid of it. Taurine is now considered as an essential nutrient in fish with low capacity such as 
yellowtail. Taurine deficiency is likely to cause low hematocrit value and low osmotic pressure in

yellowtail. These physiological defects were overcome by dietary taurine supplementation. Although 
the dietary taurine requirement in yellowtail is reported as 1% or 4.5%, there are large differences in 
these reported values. One of the possible reasons for such discrepancies may be a difference in dietary 
protein sources (SPC or FM). It has been suggested that soybean protein drains taurine-conjugated bile 
acids leading to reduction in taurine storage in the body. Thus, dietary taurine requirement increases to 
offset the loss. If dietary soy protein content influences dietary taurine requirement in yellowtail, 
taurine supplementation level should be adjusted according to the soy protein content in the diet. It is 
important to reevaluate taurine requirement in yellowtail not only for growth but also from the 
physiological perspective. 
The aim of the present study was to clarify the effects of graded taurine levels in a soy protein-based 
diet on yellowtail. Six dietary taurine levels were used to study to determine the optimum taurine level 
to prevent growth retardation and physiological abnormalities. Taurine supplementation diet at 0, 1, 2, 
3, 4, and 5 % inclusion of SPC based- non FM and FM diet as a control was conducted to identify 
taurine requirement of yellowtail. Nine fish (53.8 g in average BW) was distributed in each tank for 
each dietary treatment in duplication. Feed intake of all fish was 3.3 % BW for 10 weeks. The feeding 
amount was adjusted every two weeks. The final body weight, concentration of taurine and serine- an 
indicator of taurine sufficiency, in tissue and hematocrit value of fish was significantly improved by 
1 % dietary taurine, and improvement in plasma osmotic pressure and prevention of a green liver were 
successful by 2 % taurine. Plasma taurine concentration fluctuated according to the dietary treatments, 
in which the concentration of T0 and T1 was lower than others before postprandial 6h. Moreover, all of 
their concentrations reached a peak at postprandial 3h- 6h.  
The results revealed that inferior growth and physiological defects of SPC based non FM diet fed 
yellowtail was improved by taurine supplementation at 2 % in inclusion. 
However, its growth was still inferior to that of FM despite of presence of taurine supplementation. 
 
Chapter 4 
 Investigate the effect of soy protein on digestive enzymes 
SBM has been suggested ideal alternative protein sources to fish meal but a high inclusion of SBM 
has been reported as to reduce growth performance of yellowtail. On the other hand, alcohol extracted 
SBM improved performance of yellowtail. In the yellowtail study, regular SBM diet fed fish reduced

pancreatic enzymes (trypsin and lipase) and bile acid secretion, furthermore, FM with alcohol extracts 
of SBM also resulted in lowering secretion of pancreatic enzymes. These results suggest that regular 
SBM has negative effects on the pancreatic enzyme and bile acid secretions, whereas an alcohol 
washing process alleviates these negative impacts on fish. Thereby SPC is an attention soy protein 
product in aquaculture feed with highly refined ingredient since most of the anti-nutritional factors and 
extractive soluble of SBM has been removed during processing. However, the growth of SPC based- 
no FM diet fed yellowtail in chapter 2 and 3 was still inferior to that of FM diet fed fish. If the negative 
factors to decrease enzyme secretion are still remained in a SPC, lower enzyme secretion compared to 
FM will be expected. And this can be a reason for the low growth of totally replaced SPC based no fish 
meal diet fed yellowtail. 
Therefore, this study was conducted to examine effects of SPC on pancreatic enzyme and bile acid 
secretions to clarify responsibility of remaining alcohol soluble components in SPC. A total of 18 fish 
(243 g in average BW) was conducted for 3 dietary treatments: SPC and SBM based FM diets and FM 
as a control, in duplication for 18 feeding- days. Secretion of trypsin and lipase after 3h feeding in 
proximal intestinal digesta and faded color of rectum were observed in SPC fed fish and similarly to 
that of SBM fed fish in previous study. Therefore, the alcohol extractive components may remain in 
SPC and is responsibility for inferior growth of high inclusion of SPC in diet fed fish since inhibitory 
effect on secretion of pancreatic enzymes in this study. 
 
Chapter 5 
 General conclusion 
The present study could be summarized that SBL and taurine supplementation improved feed 
intake and growth of high inclusion of SPC in diet fed fish but presence of alcohol soluble components 
in SPC reduces secretion of digestive enzymes. Thereby, to expand usage of SPC in yellow tail feeds, it 
is necessary to determine alcohol extractive components and economical method to exclusion of this 
part from SPC.

      論  文  名  膵頭十二指腸切除術を受けた患者に対する術後早期からの 
            経腸栄養投与が及ぼす影響 
       
審査結果の要旨 
【目的と背景】 
 近年、術後早期から腸管を使用することの有用性が報告されている。しかしなが
ら、膵頭十二指腸切除術のように大きな侵襲を伴う手術後には、思うように経口摂
取が進まないことや、合併症などの理由から絶食とせざるを得なくなることもある。 
 そうした場合には中心静脈栄養（TPN）管理としたり、あるいは経腸栄養（EN）
管理とすることが一般的である。 
 Diamine oxidase（DAO）は主に小腸粘膜の絨毛上皮細胞に分布する酵素で、粘膜
障害の程度と血中 DAO 活性値は有意に相関するため、小腸粘膜の障害の程度を鋭敏
に反映する指標となる。 
 膵頭十二指腸切除術後の早い段階から EN 管理した方が腸管粘膜の萎縮が軽度で、
なおかつ合併症が少ないという仮説を立て、prospective pilot study を計画し、
その評価に DAO 活性を用いた。 
【対象と方法】 
 愛媛大学臨床倫理委員会にて承認(承認番号 1101006)を得た後、2011 年 4 月から
2013 年 11 月に当科で膵頭十二指腸切除術を施行した患者のうち、術前に参加同意
を得られた 41 症例を対象とし、最終的に 34 症例が解析対象となった。 
 術前に TPN 群と EN 群の 2 群に割り付け、術後栄養管理を行った。TPN 群は術後 1
日目を維持輸液とし、術後 2 日目よりエルネオパ®1 号液を 20 kcal/kg/day で開始
し、術後 3 日目から 2 号液に移行し 30 kcal/kg/day 程度の投与量となるよう調節
した。EN 群は維持輸液に加え、術後 1 日目より 5%糖液を手術時に造設した腸瘻か
ら 20ml/hr で投与開始し、術後 2 日目よりラコール®を 20 kcal/20 ml/hr (480 kca
l/day)で投与、3 日目に 40 kcal/40 ml/hr (960 kcal/day)とし、術後 4 日目より 6
0 kcal/60 ml/hr (1440 kcal/day)を投与した。投与カロリーは両群間で原則同等
とし、患者の年齢や体格、食事摂取量などの状態に応じ適宜調節した。両群とも飲
水は 3 日目、食事は 5 日目から開始した。 
 EN 群と TPN 群において、術後合併症や有害事象の発生率（膵液瘻、膵液瘻を除く
Clavien-Dindo 分類グレード 3 以上のもの、SSI）、術後在院日数、各種栄養学的指
標（アルブミン、総蛋白、プレアルブミン、レチノール結合蛋白）、炎症指標の推
移（CRP、サイトカイン）、血糖コントロール(インスリン使用量)、DAO 活性を比較
検討した。 
【結果】 
 EN 群の術後 5、14 日目の DAO 活性は TPN 群に比較し有意に高値であった。 
 また肝機能の指標である AST は術後 5 日目に、ALT は術後 5 日目と 7 日目に、ど
ちらも TPN 群において有意差を持って高値であった。 
 両群間で栄養学的指標、炎症指標、インスリン使用量、合併症や有害事象の発生
率、術後在院日数には有意差を認めなかった。 
【結語】 
 膵頭十二指腸切除術後、早期からの経腸栄養は、腸管粘膜の萎縮を抑制すること
が示唆されたが、合併症の発生率、術後在院日数などの臨床経過には有用性を認め
なかった。 
 
審査結果： 
 本研究に関する公開審査会は、平成２９年２月６日に開催された。申請者から研
究内容が英語で口頭発表された後に、審査委員から本研究に関する以下の質問がな
された。１）研究方法において、EN 群と TPN 群の間で成分組成（一方で脂肪が入
っている）に違いがあることについて、２）術後 5 日目以降に経口摂取を開始して
いるが、その摂取の程度と EN や TPN の補助の度合いについて、３）EN 群で脂肪成
分により膵液分泌が促進された可能性はないか、４）TPN 群で ALT、AST が上昇し
たのは高カロリー輸液の副作用ではないか、５）手術侵襲と小腸萎縮の関連性につ
いて、６）患者同意取得の説明と、EN 群と TPN 群の振り分け方法について、７）
症状や満足度の差について、８）DAO 以外の合併症の発生率や術後在院日数などの
臨床経過に有意差が出なかった要因、などの多方面からの試問が行われた。これら
に対し、申請者は質問の意図を十分に理解した上で、詳細かつ明解に答えた。 
 審査委員は、申請者が本論文関連領域に対して学位授与に値する十分な見識と能
力を有することを全員一致で確認し、本論文が学位授与に値すると判定した。

          論 文 名   肝細胞癌における ANP32B のアポトーシス制御作用と肝癌の予後に対す
る臨床的役割について

【背景と目的】 
 ANP32 (Acidic nuclear phosphoprotein 32 family member)は核内リン酸化蛋白であり、その
サブクラスである ANP32B がマウスの正常な発達において重要な役割を担っていると報告された。
しかしながら、癌における ANP32B の作用機序、腫瘍進展に対する生理的役割については不明であ
る。申請者らは肝細胞癌において、特に ANP32B に着目し、その肝細胞癌における役割についてヒ
ト肝細胞癌組織及び、ヒト肝癌細胞株を用いて検討した。 
 
【材料と方法】、 
1. 手術時に得られた 31 症例の肝細胞癌組織の癌部、非癌部における ANP32B の発現を、リア
ルタイム RT-PCR, ウェスタンブロット法、免疫組織染色を用いて検討した。 
2. 肝組織の癌部、非癌部より、mRNA を抽出した。各々の症例で癌部、非癌部の ANP32B mRNA
の発現比をとり、癌部/非癌部 ANP32B mRNA 比の低下群 (Tumor low ANP32B; N=16)と増加
群 (Tumor high ANP32B; N=15)に分けて、予後と治療後の再発、臨床背景因子との関連に
ついて検討した。 
3. 肝細胞癌で発現する ANP32B の役割について検討するため、ヒト肝癌由来細胞株である Huh
7, HLE を用いて、ANP32B siRNA により ANP32B 発現をノックダウンし、癌関連遺伝子の変
化について網羅的に解析した。作用していたアポトーシスについて検討するため、スタウ
ロスポリンを用いてアポトーシスを誘導した細胞株に、ANP32B をノックダウンすることで、
アポトーシスが増強、減弱するか Annexin V で染色しフローサイトメトリーで解析した。
さらにウェスタンブロット法にて活性型カスパーゼの発現変化を検討した。 
ANP32B ノックダウンにて修飾される細胞内関連遺伝子を PCR アレイや抗体アレイで検討し、
ウェスタンブロット法で同定した。アポトーシス誘導剤である ABT-737 を添加し、細胞内
関連遺伝子の変化を検討した。

4. ANP32B-GFP, GFP を発現したプラスミドをトランスフェクションし ANP32B を高発現させ、
GFP 陽性細胞をフローサイトメトリーでソーティングして回収し、各々の細胞におけるア
ポトーシスの変化、修飾される細胞内遺伝子の変化をウェスタンブロット法で検討した。
さらにレンチウイルスベクターを用いて ANP32B を持続的に高発現させ、ABT-737 を添加し、
アポトーシスの変化を調べた。 
5. ヒト肝細胞癌組織における ANP32B の発現を免疫組織染色で確認し、アポトーシスとの関係
について TUNEL 法による染色所見と比較検討した。 
【結果と考察】 
1. 肝組織の癌部、非癌部ともに ANP32B は発現し、症例により発現量の差異がみられた。 
2. 肝組織において Tumor low ANP32B 群は UICC stage が進行しており、予後不良であった。 
3. 肝癌細胞株における ANP32B ノックダウン後に PCR アレイで解析したところ、複数のアポト
ーシス関連遺伝子に変化がみられた。スタウロスポリンによるアポトーシス誘導下で、ANP
32B のノックダウンにより、Annexin V 陽性細胞が減少し、アポトーシス抑制効果がみられ
た。活性型カスパーゼ 8 の発現は変化なかったが、活性型カスパーゼ 9、活性型カスパー
ゼ 3 が低下し、ミトコンドリア経路に作用してアポトーシスを抑制することが示唆された。 
4. 修飾される細胞内関連遺伝子を検討したところ、ミトコンドリア経路を制御する Bcl-2 フ
ァミリーに属する Bad のリン酸化、Bak の発現に変化が見られた。スタウロスポリンによ
るアポトーシスを誘導下で ANP32B をノックダウンすると、Bad のリン酸化が増加し、Bak
の発現が低下した。ANP32B ノックダウン細胞株では、ABT-737 濃度依存的にアポトーシス
の抑制がみられた。 
5. プラスミドを用いて ANP32B を高発現したところ、ノックダウンと反対に Bad のリン酸化が
低下し、Bak の発現が増加した。しかし、Bax の発現低下が同時に見られ、アポトーシスは
誘導されなかった。レンチウイルスベクターによる ANP32B 発現誘導株においてもスタウロ
スポリン、ABT-737 の添加によるアポトーシス誘導亢進作用はみられなかった。 
6. ヒト肝細胞癌組織において、ANP32B 高発現例に比較して ANP32B 低発現例では TUNEL 陽性
細胞が減少していた。 
 
【結論】ANP32B は Bad リン酸化、Bak, Bax の発現に関連して、アポトーシスを抑制しており、
ヒト肝細胞癌組織における非癌組織に対する ANP32B mRNA の相対的低下は、肝癌の進行お
よび予後不良に関連していた。ANP32B は肝細胞癌に対する治療、予後予測マーカーとして
臨床上有用な指標になりうる可能性が示唆された。

 論  文  名  アルツハイマー病におけるセロトニントランスポーター遺伝子の多型、 
          白血球 mRNA 発現量、メチル化の関連研究とメタ解析 
 
【審査結果の要旨】 
 本論文はアルツハイマー病患者 43 名と健常者 47 名の血液試料の解析と PubMed で検索した
文献のメタ解析による研究である。 
 
【はじめに】 
アルツハイマー病（Alzheimer’s disease, 以下 AD）は、最も多い変性による認知症疾患
で、近時記憶障害をはじめとする認知機能障害や抑うつ、幻覚、妄想、アパシーといった精神
症状をきたす。AD では、アミロイド β の沈着（老人斑）やリン酸化タウの凝集（神経原繊維
変化）がみられるが、未だ詳細な病態や病因は不明であり、コリンエステラーゼ阻害薬などの
治療薬が使用されているものの、根治療法はみつかっていない。私が注目した神経伝達物質セ
ロトニンは、摂食行動や性行動、情動などの他、認知機能の調節にも関与していることが一般
的に知られている。向精神薬にはセロトニン神経系を標的にした薬剤が多く、特にセロトニン
トランスポーター（5-hydroxytryptamine transporter, 以下 5-HTT）は抗うつ薬の主要な標
的部位である。5-HTT 遺伝子転写調節領域には 5-HTT-length polymorphic region (以下 5-HT
T-LPR)として知られる遺伝子多型が存在し、主に Long allele (L アレル)と Short allele (S
アレル)に分類され、L アレルはさらに一塩基多型 rs25531 のバリアントによって LA アレルと
LG アレルに分類される。LG アレルは S アレルと同様にメッセンジャーRNA（以下、mRNA）発現
の低下と関与していると報告されており、日本人に多いＳアレルはストレスに反応してうつ病
を発症しやすいとの報告もある。 
一方で、AD とうつ病はしばしば合併し、うつ病が AD のリスクとなることも知られている。
AD 患者の死後脳において 5-HTT の機能低下を示唆する報告が存在するが、5-HTT 遺伝子と症状
や発症のリスクについて詳細に検討した研究はない。本研究の目的は AD と 5-HTT 遺伝子との
関係を明らかにすることであり、我々は、AD と 5-HTT 遺伝子の遺伝子やエピジェネティック
解析、mRNA 発現解析を行った。 
 
【方法】 
本研究の対象は AD 患者 43 名（男性 16 名、女性 27 名、年齢 79.8 ± 3.84 歳）と健常者 47
名（男性 16 名、女性 31 名、年齢 79.7 ± 4.41 歳）とした。AD 患者群と健常群における性別、
年齢に有意差はない。対象患者・家族（介護者）には、Mini Mental State Examination (MMS
E)、Clinical Dementia Rating (CDR)、Neuropsychiatric Inventory（NPI）を施行し、認知
機能と精神症状を評価した。すべて対象者には、本学倫理委員会で承認されたプロトコールを
用いて書面で了承をいただいている。末梢白血球から DNA を抽出し、5-HTT-LPR 多型および rs
25531 は、polymerase chain reaction（PCR）を行い、MspI 制限酵素処理後に、電気泳動によ
って PCR 産物のサイズを判別し、LA、LG 、S アレルに分類した。メチル化率については、プ
ロモーター領域の 9 つの CpG サイトを解析対象とし、末梢白血球 DNA をバイサルファイト処理
後に、パイロマーク Q24 を用いて解析を行った。末梢白血球から RNA を抽出後、mRNA 発現量
については、内部標準遺伝子として GAPDH を同時に測定し、タックマンプローブを用いたリア
ルタイム PCR を行い、定量化した。また、5-HTT-LPR 多型が AD 発症に関与するリスクを PubMe
d で検索した文献でメタ解析した[Yamazaki K et al. 2016:主論文]。 
 
【結果】 
解析の結果、5-HTT-LPR 多型と rs25531 のアレル頻度は、いずれも AD 患者群と健常群にお
いて差はなかった。それぞれの CpG サイトにおけるメチル化率にも、AD 患者群と健常群に差
はなかったが、末梢白血球での 5-HTT mRNA 発現量は、健常群に比較して AD 患者群で有意に増
加していた。認知機能、精神症状を併せて評価すると、遺伝子多型とそれぞれの CpG サイトに
おけるメチル化率については、認知機能や精神症状との関連はみられなかった。しかしながら、
5-HTT mRNA 発現量と NPI の得点において正の相関がみられた。NPI の下位項目で解析を行うと、
アパシーを伴う患者群で mRNA 発現量の増加がみられた。mRNA 発現量と抗認知症薬であるコリ
ンエステラーゼ阻害薬の内服の有無には、関連は確認できなかった。その他、5-HTT-LPR 多型
と AD のリスクについてメタ解析を行ったところ、アジア人では有意な関連はみられなかった
が、白人では、L/L アレルが AD のリスクを減少させるとの関連がみられた。 
 
審査会は平成２９年１月５日に開催された。発表後の質疑応答では、AD とセロトニンとの関
係に着目した理由、末梢血と中枢神経との関連、血中セロトニンと血液脳関門について、将
来的な実験計画とその展望等について多くの質問が出され、申請者はこれらに的確に回答し
た。審査委員は，申請者が本論文関連領域に対して学位授与に値する十分な見識と能力を有
することを全員一致で確認し，本論文が学位授与に値すると判定した。

【背景】近年の多列検出器型 computed tomography（CT）の登場と発展により、冠動脈 CT 
angiography（coronary CTA）を用いた非侵襲的な冠動脈形態診断が多くの施設で行われるよ
うになった。しかし、虚血性心疾患の治療方針決定には、冠動脈の形態的狭窄度だけでなく機
能的狭窄度（狭窄に不随する心筋灌流異常があるか）を評価する事が重要である。Myocardial 
perfusion imaging（ MPI）の 1 つである single-photon emission computed tomography
（SPECT）や cardiac magnetic resonance（CMR）に関しては既に多くのエビデンスが積み重
ねられており、非侵襲的な心筋灌流検査として日常診療で使用されている。また、近年では薬
剤負荷を併用した心筋 CT perfusion（CTP）が心筋灌流異常（虚血/梗塞）の検出に有用である
という報告がある。CTP においては、dynamic 撮影によって得られた心筋の time attenuation 
curve（TAC）を解析することで、myocardial blood flow（MBF）といった定量パラメータを算
出可能であり、従来の定性評価より詳細な心筋灌流評価ができるのではないかと期待された。
そこで、今回我々は dynamic 心筋 perfusion 画像から算出した CT-MBF の虚血性心疾患評価に
おける有用性を心筋 SPECT、MRI と比較して検討した。 
【方法】対象は虚血性心疾患が疑われ、dynamic CTPと、SPECT（n= 25）とCMR（n= 28）が施
行された計53症例とした。解析は16-セグメントベースで行い、正常心筋および異常心筋（虚
血/梗塞）の定義はSPECTもしくはCMRを用いて行った。負荷/安静SPECTの評価は、5-point sca
le (0, 正常; 1, 軽度灌流低下; 2, 中等度灌流低下; 3, 高度灌流低下; 4, 灌流欠損)を用い
て行い、負荷時にscore≧1以上の灌流異常を認める場合に異常心筋（虚血＋梗塞）、負荷時・
安静時ともにscore≧3以上の灌流異常を認める場合にviabilityのない高度梗塞と定義した。C
MRの評価は、負荷/安静perfusion画像、遅延造影画像を用いて行い、正常心筋（灌流異常なし、
遅延造影なし）、軽度虚血（壁厚25％未満の灌流異常あり、遅延造影なし）、高度虚血（壁厚
25％以上の灌流異常あり、遅延造影なし）、内膜下梗塞（壁厚50％未満の遅延造影あり）、貫
璧性梗塞（壁厚50％以上の遅延造影あり）に分類し、高度虚血以上の病変を異常心筋（虚血＋
梗塞）、貫璧性梗塞をviabilityのない高度梗塞と定義した。CT-MBFを用いた心筋灌流異常
（虚血＋梗塞）や高度梗塞検出における至適カットオフ値をROC解析を用いて算出し、診断能
（感度、特異度）を検証した。 
【結果】SPECT群では、全400心筋セグメント中、90セグメントが灌流異常（虚血＋梗塞）、19
セグメントが高度梗塞であった。CMR群では、全448心筋セグメント中、223セグメントが灌流
異常（虚血＋梗塞）、36セグメントが高度梗塞であった。SPECT群におけるCT-MBFは、正常領
域で1.52 mL/g/min、軽度灌流低下領域で1.19 mL/g/min、中等度灌流低下領域で0.98 mL/g/mi
n、高度灌流低下領域で0.78 mL/g/min、灌流欠損領域で0.25 mL/g/minと段階的に低下を認め
た。CMR群におけるCT-MBFは、正常領域で1.51 mL/ g/min、軽度虚血領域で1.45 mL/g/min、高
度虚血領域で1.12 mL/g/ min、内膜下梗塞領域で1.06 mL/g/min、貫壁性梗塞領域で0.94 mL/g
/minと段階的に低下を認めた。SPECT群における心筋灌流異常（虚血＋梗塞）病変の検出に関
しては、カットオフ値を1.23 mL/g/minにすると、感度80 %、特異度86 %であった。CMR群にお
ける心筋灌流異常（虚血＋梗塞）病変の検出に関しては、カットオフ値を1.25 mL/g/minにす
ると、感度82 %、特異度87 %であった。SPECT群におけるCT-MBFの高度梗塞病変の検出に関し
ては、カットオフ値を0.92 mL/g/minにすると、感度95 %、特異度72 %であった。CMR群におけ
るCT-MBFの高度梗塞病変の診断能に関しては、カットオフ値を0.98 mL/g/minにすると、感度7
8 %、特異度80 %であった。 
【結論】Dynamic 心筋CTPによって精度の高い心筋血流定量評価が可能であり、心筋灌流異常
の検出だけでなく、viability判定の一助となる可能性があった。
