A case of pregnancy and delivery complicated with osteogenesis imperfecta
先天性骨形成不全症(Osteogenesis Imperfecta : OI)は、Ⅰ型コラーゲン異常により骨脆弱性をきたし、胎児期から成長期に骨折や骨格異常を発症するため、頻回の手術を要す疾患である。この疾患の発症要因として、ほとんどが常染色体優性遺伝であることが知られている。症例は31才、1経妊0経産、家族歴なく出生時に両大腿骨骨折、両上腕骨骨折を指摘され、小児科でOIと診断された。その後、頻回の整形外科手術を要し、今回の妊娠前の状態は、身長100cm、体重20kg、起立および少しの歩行は可能であったが、主な移動手段は電動車椅子であった。自然妊娠が成立し、当院を紹介受診した。胎児の発育に問題はなく、胎児の骨折は認めなかった。妊娠29週0日、管理目的に入院後、妊娠29週4日より切迫早産の診断で治療を開始した。妊娠30週0日の時点で骨盤レントゲン計測にて、児頭骨盤不均衡と診断した。脊椎の彎曲の為、全身麻酔で帝王切開を予定したが、気道の偏位の為に気道確保が困難であることが予想された。妊娠子宮による母体呼吸・循環への影響を考慮し、さらに緊急手術となることを回避するため、胎児の肺成熟が期待できる妊娠35週0日に帝王切開を施行した。狭骨盤のため、子宮下部の視野が確保できず、子宮底部横切開で児を娩出した。児はOIの所見なく、正常に発育している。OI合併妊娠は様々な合併症が報告されているが、本症例は大きな合併症なく妊娠35週まで妊娠継続が可能であった。狭骨盤の為、帝王切開術を要することが多く、本症例の様な極端な症例では、術式に工夫を要すことも必要となる。麻酔に際しては、脊椎変形の為、脊椎麻酔が困難な事が多く、また気道確保にも注意を要する。journal articl
Effects of D-allose on ATP production and cell viability in neonatal rat cardiomyocytes
2-Deoxy-d-glucose (2DG) induces anticancer effects through glycolytic inhibition but it may raise the risk of arrhythmia. The rare monosaccharide d-allose also has anticancer properties, but its cardiac effects are unknown. We examined the effects of d-allose on adenosine triphosphate (ATP) production in neonatal rat cardiomyocytes. We showed that 25 mM d-allose selectively reduced glycolytic ATP, but had minimal impact on mitochondrial ATP, while 1 mM 2DG strongly inhibited both. Furthermore, d-allose had less impact on cell viability and was less cytotoxic than 2DG; neither compound caused apoptosis. Thus, d-allose selectively diminished glycolytic ATP production with no apparent effects on cardiomyocytes.journal articl

ケアリング教育の統合 : 日本とアメリカにおける異文化的視点
This study examines Japanese Moral Education’s cross-cultural adaptability and effectiveness and two American approaches : Differentiated Instruction by Carol Ann Tomlinson and the Responsive Classroom Approach by Rimm-Kaufman. The research aims to understand how these strategies foster caring education, which nurtures empathy in students. Employing content analysis, the study compares these approaches’ pedagogical principles and practices to identify effective strategies for diverse educational settings. Results highlight that while some strategies are universally effective, others require significant modifications to address cultural nuances. The study underscores the importance of continuous teacher professional development, personalized learning, and integrating technology in education. Key findings suggest that adopting flexible and culturally sensitive teaching approaches can enhance moral education in Japan and globally, promoting academic success, emotional growth, and social responsibility among students. The research also explores future challenges in Japanese moral education, providing practical suggestions for educators. By integrating successful international practices, such as Differentiated Instruction and the Responsive Classroom Approach, the study offers actionable recommendations to bridge educational gaps and improve the efficacy of moral education. Ultimately, this research contributes to the broader discourse on how caring as a core educational strategy can facilitate comprehensive student development and prepare them to meet societal challenges effectively.departmental bulletin pape
