相談室の「場を整える」ことの臨床的意義
Get PDF本小論は、養成大学院における相談室の掃除を素材に、心理臨床における相談室の「場を整える」ことの臨床的意義を考察した。はじめに、心理療法が行われるための「場」を整えることが、心理療法という営みの本質に関連するものであることを述べた。次に、「場」を整えるにあたり、庭の手入れから着想を得、そこから導かれる相談室の手入れについて、面接担当者による日々の手入れや大学院生による相談室の大掃除の実際を概観し、相談室の「場」を整える行為が、セラピストの内面を整えることにも通じていることに言及した。最後に、外界と内界は切り離せないものであり、その「場」に居る人の思いが、「場」の力を生み出す重要な要素となっていると考えた。departmental bulletin pape
施設利用高齢者に対しての介護職員によるライフレビュー面接の検討－ライフレビュー・プロセスと聴き手としての介護職員の体験に着目して－
Get PDF本研究では，施設利用高齢者が語り手となり，介護職員が聴き手となるライフレビュー面接を実施した。研究1では，面接を事例研究的に検討し，5回の面接を通した全体的なライフレビュー・プロセスと，各回での語り手と聴き手のやりとりについて検討した。研究2では，聴き手となった介護職員へのインタビュー調査を質的研究法によって分析し，聴き手が面接で何を感じたり考えたりしたか，また，それらが面接での語り手に対する態度や応答にどのような影響を与えたかということについて検討した。最終的には，語り手と聴き手の言葉でのやりとりと，面接を通しての聴き手のこころの動きを総合することにより，ライフレビュー面接における語り手と聴き手の相互交流のありようを考察した。本論文は，香川大学大学院医学系研究科臨床心理学専攻において令和3 年度に提出した修士論文，「施設利用高齢者に対しての介護職員によるライフレビュー面接の検討－ライフレビュー・プロセスと聴き手としての介護職員の体験に着目して－」をもとにしたものです。departmental bulletin pape
Impaired Hippocampal Long-Term Potentiation and Memory Deficits upon Haploinsufficiency of MDGA1 Can Be Rescued by Acute Administration of D-Cycloserine
Get PDF香川大学Kagawa University博士（医学）The maintenance of proper brain function relies heavily on the balance of excitatory and inhibitory neural circuits, governed in part by synaptic adhesion molecules. Among these, MDGA1 (MAM domain-containing glycosylphosphatidylinositol anchor 1) acts as a suppressor of synapse formation by interfering with Neuroligin-mediated interactions, crucial for maintaining the excitatory-inhibitory (E/I) balance. Mdga1-/- mice exhibit selectively enhanced inhibitory synapse formation in their hippocampal pyramidal neurons, leading to impaired hippocampal long-term potentiation (LTP) and hippocampus-dependent learning and memory function; however, it has not been fully investigated yet if the reduction in MDGA1 protein levels would alter brain function. Here, we examined the behavioral and synaptic consequences of reduced MDGA1 protein levels in Mdga1+/- mice. As observed in Mdga1-/- mice, Mdga1+/- mice exhibited significant deficits in hippocampus-dependent learning and memory tasks, such as the Morris water maze and contextual fear-conditioning tests, along with a significant deficit in the long-term potentiation (LTP) in hippocampal Schaffer collateral CA1 synapses. The acute administration of D-cycloserine, a co-agonist of NMDAR (N-methyl-d-aspartate receptor), significantly ameliorated memory impairments and restored LTP deficits specifically in Mdga1+/- mice, while having no such effect on Mdga1-/- mice. These results highlight the critical role of MDGA1 in regulating inhibitory synapse formation and maintaining the E/I balance for proper cognitive function. These findings may also suggest potential therapeutic strategies targeting the E/I imbalance to alleviate cognitive deficits associated with neuropsychiatric disorders.doctoral thesi

Experience of holistic understanding of cancer patient pain by expert cancer nurses

がん看護エキスパートナースが，がん患者の痛みの意味を捉えて痛みを全人的に理解する体験を明らかにし，がん患者の痛みの理解における新たな看護の示唆を得ることである．
方法
がん患者の痛みの意味を理解した体験のあるがん看護エキスパートナース3名の協力を得て，半構成的面接法で行った．データは，現象学的アプローチであるGiorgiによる方法論を用いて分析した．
結果
3名のがん看護エキスパートナースががん患者の痛みを全人的に理解する体験からは，1）エキスパートナースの患者に真摯に向き合う在りようが患者と心のつながりを生む，2）人間と人間の関係の中で，エキスパートナースが患者固有の痛みの理解を得る，3）エキスパートナースが自分を生きる患者を支える，これら3つの共通する構造が明らかとなった．
考察
エキスパートナースの痛みを理解しようとする在りようが患者との間に人間同士のつながりを生み，それはBuberの示す〈われ〉―〈なんじ〉の関係であった．〈われ〉―〈なんじ〉の関係のなかで，エキスパートナースは患者を唯一の存在として捉え，患者の抱える固有の痛みの理解を得ていた．
結論
がん患者の全人的苦痛を理解するためには，看護師が患者と人間同士の関係を築く必要があり，その関係構築に向けた看護師の在りようが重要であった．エキスパートナースの患者に真摯に向き合う在りようを目指すために，まずは看護師自身の人格を出した向き合い方が必要である．そして，①専門的知識と技術を学び高める努力，②タッチや傾聴する時の共感的姿勢の育成，③患者と共に痛みを感じる覚悟に向けた看護師の内面的な準備が必要であると示唆を得た．Objective: The present study aimed to reveal the experiences of expert cancer nurses as they interpret the meaning of pain felt by patients with cancer and gain a holistic understanding of their pain, while also aiming to obtain new nursing practice suggestions for understanding the pain of patients with cancer.
Method: Semi-structured interviews were conducted with the cooperation of three expert cancer nurses who had experience in understanding the meaning of pain felt by patients with cancer. Data analysis used the methodology of Giorgi’s phenomenological approach.
Results: The experiences of three expert cancer nurses holistically understanding the pain of patients with cancer revealed the following three common structures: 1） the expert nurse’s sincere attitude towards patients created an emotional connection with the patients; 2） through interpersonal relations, expert nurses gained an understanding of patient-specific pain; and 3） expert nurses supported patients living their own lives.
Discussion: The expert nurses’ attitudes trying to understand pain created a human connection with the patients, in essence, the ‘I’ - ‘Thou’ relationship described by Buber. In the ‘I’ - ‘Thou’ relationship, expert nurses perceived each patient as a unique individual and understood the unique pain that each patient was going through.
Conclusion: To understand the total pain experienced by patients with cancer, nurses need to establish interpersonal relationships with the patients, and the nurses’ attitudes toward building such relationships were found to be important. To acquire the expert nurses’ sincere attitude toward patients, there is a need for nurses to first adopt ways that show their own personalities. Furthermore, it was suggested that the following are needed:（1） efforts to learn and enhance specialized knowledge and skills;（2） nurturing empathetic attitudes for engaging in touch and active listening; and （3） internal preparation of the nurses toward their resolve to experience pain together with the patients.なお，本論文は香川大学大学院医学系研究科修士論文に加筆・修正したものである．また本論文の一部は34回日本がん看護学会学術集会にて発表を行った．departmental bulletin pape
