    Temporomandibular Joint Space Changes in Skeletal Class III Malocclusion Patients with Orthognathic Surgery

    The purpose of this retrospective study was to evaluate changes in the temporomandibular joint spaces (TMJSs) in skeletal Class III adult patients with orthognathic surgery using cone-beam computed tomography (CBCT). CBCT images taken from 26 orthognathic surgery adult patients (15 females, 11 males, average 19.6 ± 2.8 years at pretreatment, range 15.8–26.8 years) with skeletal Class III malocclusion (ANB \u3c 1°) were used for this study. TMJSs (AS, anterior space; SS, superior space; PS, posterior space; MS, medial space; CS, central space; LS, lateral space) were measured at each stage of treatment (T0, pretreatment; T1, presurgery; T2, postsurgery; T3, posttreatment, and T4, retention) and were compared according to gender, side, vertical skeletal pattern, number of surgery sites, and amount of mandibular setback. At T0, TMJSs were significantly greater in SS than in AS and PS. The ratio of AS to SS to PS was 1.0 to 1.5 to 1.1. TMJSs were significantly greater in MS and CS than in LS. The ratio of MS to CS to LS was 1.0 to 1.0 to 0.8. All TMJSs in males were significantly greater than in females except in PS. TMJSs on the left side were significantly greater than on the right side only in PS. TMJSs were not significantly different depending on the SN-MP, number of surgery sites, and amount of setback. From T0 to T4, there were no significant changes in TMJSs or their ratios according to gender, side, sella to nasion (SN), mandibular plane (MP), number of surgery sites, and amount of setback. Exceptionally, at T4, SS and CS were significantly greater in the small amount of setback group than in the large amount of setback group. There were no statistical changes in TMJSs throughout all stages when skeletal Class III patients were treated with surgery

    Resolvin D1 modulates periodontal ligament fibroblast function

    Background: The resolution of inflammation is an active process mediated by specialized lipid mediators called lipoxins and resolvins. Periodontal ligament fibroblasts (PDLFs) play a significant role in periodontal regeneration. The purpose of the current study was to determine the impact of resolvin D1 (RvD1) on human PDLF cell wound healing and proliferation, receptor expression (G-protein-coupled receptor 32 [GPR32] and formyl peptide receptor 2 [ALX/FPR2]), and cytokine expression and release. Methods: PDLFs were stimulated with interleukin-1β (IL-1β) (500 pg/ml) with and without RvD1 (100 nM). RvD1 receptor expression was determined by quantitative real-time polymerase chain reaction (qPCR), immunofluorescence microscopy, and fluorescence-activated cell sorting. Wound closure was measured by a scratch assay, and proliferation was determined by bromodeoxyuridine incorporation. Interleukin-6 (IL-6), interleukin-8 (IL-8), monocyte chemoattractant protein-1, cyclooxygenase-2, matrix metalloproteinases-1, -2, and -3 (MMP-1, -2, and -3), tissue inhibitors of metalloproteinases-1 and -2 (TIMP-1 and -2), prostaglandin E2, and osteoprotegerin (OPG) gene expression and production were measured using qPCR and Western blotting, multiplex immunoassay, and enzyme-linked immunosorbent assay. Results: PDLF expressed GPR32 and ALX/FPR2. RvD1 reversed IL-1β-induced inhibition of wound healing and proliferation of PDLF. IL-1β also induced the production of proinflammatory cytokines and MMPs. This effect was reversed by RvD1. RvD1 reversed IL-1β‒induced inhibition of TIMP-1, TIMP-2, and OPG. Conclusion: The data suggested that RvD1 has a pro-wound healing, proliferative, and anti-inflammatory impact on the PDLF that favors periodontal regeneration

    Effects of over-the-counter at-home whitening products with LED light on surface roughness of partially-and fully crystalized CAD/CAM lithium disilicate ceramics

    The aim of this study is to evaluate the effect of over-the-counter (OTC) at-home whitening products with LED light on partially-and fully-crystalized CAD/CAM lithium disilicate ceramics. Two partially-crystalized CAD/CAM lithium disilicate ceramics, Amber Mill and IPS e.max CAD, and one fully-crystalized CAD/CAM lithium disilicate ceramic, n!ce Straumann, were used. The specimens were divided based on treatment with OTC whitening products: no treatment provided, Colgate Optic, Crest 3D and Walgreens Deluxe. The surface roughness of the specimens was evaluated with an optical profilometer and scanning electron microscopy. The three LED whitening products significantly increased the surface roughness and changed surface morphology of Amber Mill and IPS e.max CAD but no differences for n!ce Straumann. OTC at-home whitening products with LED light can significantly increase the surface roughness of restorations fabricated with these partially-crystalized CAD/CAM lithium disilicate ceramic restorations. However, these products do not increase the surface roughness of restorations fabricated with this fully-crystalized lithium disilicate ceramic

    How acculturation contributes to a rise of diabetes in Hmong Americans

    Abstract This article explores the reasons for the high prevalence of diabetes among Hmong Americans, who are at greater risk for diabetes compared with non-Hispanic White patients and have a higher prevalence of diabetes than other Asian Americans and other races. Knowledge of this issue may allow medical practitioners to identify, screen, and treat Hmong Americans more frequently and at earlier ages. Although better integration of healthcare services, access, and education are needed, earlier screening may help start the diabetes education process sooner; reduce hospital, ED, and office visits; reduce costs; and prevent end-organ damage

    Physician Assistant Students’ Perception of Online Didactic Education: A Cross-Sectional Study

    Abstract Purpose: This study describes physician assistant students\u27 perception toward online didactic education and highlights relationships between student characteristics and their preference for online learning. Methods: A previously validated survey questionnaire was administered online to physician assistant students enrolled in traditional, in-person training programs across the United States. The survey consisted of five Likert-scale statements measuring perceptions of online learning and was rated on a seven-point Likert scale. Students also reported their age, gender, history of taking an online course, and preferred learning style. Mean scores were reported for agreement with each Likert-scale statement; Pearson correlation coefficients, one-way ANOVA with post hoc Tukey tests, and independent samples t-tests were used to determine relationships between student characteristics and their preference for online learning. Results: A total of 391 completed surveys met the inclusion criteria for the study and were used in data analysis. The average age of respondents was 25.98 years, 81.1% (n = 317) were female, 96.2%, (n = 376) reported taking an online course previously, and preferred learning styles were reported as 36.1% (n = 141) visual, 7.7% (n = 30) auditory, 15.6% (n = 61) reading/writing, and 40.7% (n = 159) kinesthetic. Nearly a quarter of respondents indicated they preferred online courses, particularly students with a preferred learning style of reading/writing. No relationships were observed between age, gender, or history of taking an online course and preference for online education. Conclusion: Most physician assistant students prefer in-person learning. However, a substantial number prefer online learning, and a significant number of these students reported a preferred learning style of reading/writing. More research is necessary to give educational institutions the ability to make data-driven, student-centered program development decisions. However, data in this study indicate a need for continued development of online/hybrid physician assistant programs to better align with current student preferences.     Transversus abdominis plane blocks in laparoscopic inguinal hernia repair: a review

    Introduction: Throughout its history, there have been significant advances in pain control of inguinal hernia repairs. One of the most recent developments is locoregional pain blocks. There is a multitude of literature available on laparoscopic inguinal hernia repair and transversus abdominis plane (TAP) blocks. Objectives: This paper seeks to provide a thorough and systematic literature review on the role of TAP blocks in laparoscopic inguinal hernia repairs. Methods: PubMed and Google Scholar were searched for relevant literature using predetermined medical subject heading (MeSH) terms: “(TAP block)” AND “(Laparoscopic inguinal hernia repair)”. Results: A total of 166 publications were identified, from which 18 publications were included in the final review after eligibility criteria were applied. Conclusion: The majority of studies conclude that TAP blocks performed in the setting of laparoscopic inguinal hernia repair improve post-operative pain and mobility, decrease opiate analgesic usage, and are superior in pain control compared to other modalities of regional anesthesia. Thus, to improve post-operative outcomes and patient satisfaction, TAP blocks should be heavily considered for routine use in surgical practice for laparoscopic inguinal hernia repair

    Can Time to Treatment Reduce In-Hospital Mortality and Morbidity Among Surgical and Nonsurgical Elderly Patients With Sepsis

    Sepsis mortality remains high and efforts to reduce it are continuing. We collected data from our patients presented to the emergency department (ED) with sepsis and performed a retrospective analysis of 1079 patients seen in the ED with sepsis during 2018 and 2020, before and after implementation of the new CDC protocol. Statistical analysis was performed using Student’s t-test and chi square test as well as Cox regression analysis. The patients were divided into pre-protocol (group 1) and post-protocol (group 2). A total of 1079 patients were included in the study. The mean age was 65 + 16.86 years, divided equally between gender (male 49%, female 51%). Patients with certain comorbidities showed statistically significant survival rate in the protocol group. The current protocol for sepsis when implemented will improve patients’ survival, in both surgical and medical patients and significantly in those with comorbid conditions

    Effect of the COVID-19 pandemic on racial disparities in postpartum visits at a Texas level IV maternal center

    Background: The primary aim of our study was to determine the attendance of postpartum visits stratified by race and if the COVID-19 pandemic affected racial disparities in postpartum visit attendance. Methods: We searched our labor and delivery records from July 1, 2019 to December 31, 2019 and from July 1, 2020 to December 31, 2020 and included patients who delivered liveborn infants. The final analysis was restricted to patients who identified as White or Caucasian only, Black or African American only, or Hispanic. We then performed joint tests on the logistic regression with an interaction term of race and year of delivery to determine the final model. Results: The odds ratio of Black or African American and Hispanic patients attending a postpartum visit was 0.589 (95% CI 0.456, 0.760; P \u3c.001) and 0.836 (95% CI 0.676, 1.034; P = 0.099), respectively, compared to White or Caucasian patients. The interaction term of race and year of delivery was not statistically significant. Conclusion: Black or African American patients at our hospital had a clinically and statistically significant lower utilization of postpartum visits compared to White or Caucasian patients and this disparity was not exacerbated by the COVID-19 pandemic
