    Underlying cerebrovascular pathology causing secondary psychosis

    Case Background Psychosis can be classified as primary if it is caused by a psychiatric disorder or secondary if it is due to a specific medical condition. Despite there being multiple identified causes of secondary psychosis, making such a diagnosis can prove challenging. Case Details This case report presents a 57-year-old male who was brought by ambulance to the emergency department following an intentional jump from a first story window due to visual and auditory hallucinations and persecutory paranoid delusions. MRI findings on admission were consistent with cerebral autosomal dominant arteriopathy with subcortical infarcts and leukoencephalopathy (CADASIL). The patient’s background included an ischaemic stroke with haemorrhagic transformation five years previously. He experienced psychotic symptoms during the immediate recovery period. Prior to this he had no psychiatric history. Discussion Post-stroke psychosis and the condition of CADASIL are both entities that are relatively rarely described in the literature. However, this may be due to missed diagnosis and therefore not reflective of true prevalence. Conclusion Identifying a cause-effect relationship and establishing a diagnosis of secondary psychosis can be confounded by many factors. Nevertheless, this patient’s case demonstrates the importance of considering an underlying cause, particularly when the presentation is atypical and there is a temporal relationship between the proposed medical condition and the symptoms of psychosis.     The role of imperialism, as discussed in the previous paper, in determining contemporary patterns of development and under­development, cannot be overemphasised. The primary motivation was economic, as industrialising Europe needed, firstly, supplies of raw materials, and secondly, markets for it\u27s products. Raw materials were exploited in the colonies, processed in Europe, and often returned to the colonies as finished products at a profit. Strategic reasons, religious fervour, idealism, and racism were all secondary considerations but the primary aim and ultimate consequence was the creation of integrated world economy, dominated and controlled by the European nation states. Since the Second World War former colonies have progressively gained political independence, however tenuous, and the blatant domination of the colonial era has been replaced by more subtle but no less effective form of dependency

    It is generally recognised that soil moisture storage plays an important role in the generation of surface water run-off (Mein et.al. 1988, Burch et al. 1987, Wheater and Weaver 1980). Following clearing on a forest watershed in West Virginia, Patrick and Reinhart (1971) attributed. yield increases to an increase in soil moisture storage. On the Coranderrk catchment\u27s Victoria, Duncan and Heeps (1980) found that soil moisture also increased following clearfelling. Similarly thinning of a Eucalyptus marginata forest in Western Australia resulted in an increase in soil moisture storage and Stoneman (1986) attributed the lack of a substantial streamflow increase to this factor. In separate studies of forest to pasture conversion in California and Western Australia, both Hibbert (1969) and Sharman et al. (1987) respectively observed an increase in soil moisture storage. Hibbert (1969) suggested that the greater availability of- moisture in the upper soil layers following clearing, where it could be exploited by the shallow rooted pasture grasses, might result in a continued high evapotranspiration rather than the decrease which might be expected to result. &nbsp

    Many and varied views of poverty exist, ranging from the uncaring \u27its got nothing to do with me\u27 stance, through to concern at the plight of the poor, to calls for a radical transformation of society so as to eradicate poverty entirely. Studies of poverty in western societies have a long tradition. Among some early discussions of living conditions is Engel’s The Condition of The Working Class in England (1845). One of the first detailed surveys in Britain: was Rowntree\u27s study in York in 1899. Since then, the prominence given to the living conditions of people in western countries has waxed and waned, both in popular discourse and in academic debate.&nbsp

    In September of 1982 I visited Crete, intending to stay there for just two weeks. Six months later I was still on the island. During this time I was able to see, at first hand, the type of lifestyle led by some of the Cretans, especially in the village of Myrtos, on the south coast. Although I have been back to Crete since then, most of this article is based on my own observations made during my first stay. &nbsp

    Bipolar disorder and early childhood trauma

    Case Background There is a reported 48.8 million case of bipolar disorder (BD) world- wide. It can also be one of the most challenging disorders to manage clinically as patients often have poor medical compliance and numerous relapses throughout their lifetime. The etiology of this illness is still poorly understood, but it is recognized to compromise both genetic and environmental factors. Regarding the latter, abuse and instability in childhood and adolescence was reported as an important area of consideration in regard to the development and prognosis of bipolar disorder. We report here the case of a female who exemplifies the significance of early childhood experience in this disorder. Case Presentation Isabelle is a 21 year old, female, with a background history of bipolar disorder. She presented to the Cork University Hospital accident and emergency 2 weeks after discharge after a 4 week admission for a depressive episode. Isabelle was referred from the home-based team with an acute episode of mania and was detained involuntarily under the Mental Health Act. Her history is significant for childhood neglect, physical, emotional and sexual abuse, along with multiple foster care placements. She was managed as an inpatient with antipsychotics and a mood stabilizer before being discharged after a lengthy, arduous, admission. Conclusion Childhood trauma is an important consideration in BD. Therefore, practitioner awareness of the role of such trauma with regard to its etiology and prognosis is of great importance. It is also essential to the practice of preventive medicine.
