    Retromolar Trigone Reconstructive Surgery: Prospective Comparative Analysis Between Free Flaps

    PURPOSE: Retromolar trigone (RMT) tumours are rare and aggressive malignancies, which require an aggressive surgical approach. The reconstruction oral cavity defects represent a challenge because of the critical role of this area both aesthetically and functionally. Free radial forearm (RF) or anterolateral thigh (ALT) flap are the first choice for the repair of intraoral defects. In reviewing the literature, there is lack of evidence pertaining to the differences between RF and ALT flaps in the reconstruction of patients with RMT tumours. This study evaluates the better microvascular reconstruction after RMT cancer resection. METHODS: Thirty patients with RMT cancer underwent oropharingectomy and microvascular reconstruction using the free RF flap (RF group) and the ALT perforator flap (ALT group). The two groups were homogeneous for sex, age, anatomic area, body mass index, and clinicopathologic profile. Viability, complications, scarring, cosmetic appearance, disorder of sensations, ROM, disease-specific items and satisfaction rate were analyzed, and statistical analysis was performed. STUDY DESIGN: Prospective study. RESULTS: There were differences between the RF and ALT groups in the morphofunctional outcomes, both short-term and long-term follow-up. These differences were statistically significant (p < 0.05) for donor site complications, cosmetic appearance, and scar evaluations. Manual dexterity was slower on the operated donor side than on the nonoperated side in the 33.3 % in the RF group. CONCLUSIONS: The study showed that the free ALT perforator flap provides better results in appearance and scarring than the RF flap for intraoral reconstruction after RMT cancer resection

    An integrated approach for demand forecasting and inventory management optimization of spare parts

    In this paper, we develop and test an advanced model, based on discrete-event simulation, whose purpose is to forecast the demand of spare parts during the whole lifetime of a complex product, such as, for instance, an industrial machine. To run the model, the relevant data of the product manufactured by a targeted company should be collected. With those data, the model provides an estimate of the optimal level of spare parts inventory the company should keep available. The data provided by the model are subsequently applied to a case example, referring to a hypothesised company, manufacturing industrial plants. The application is carried out considering two scenarios, i.e., a ‘traditional’ and an ‘advanced’ approach for demand forecasting, this latter reflecting the circumstance where the company makes use of the proposed forecasting method. The comparison of the outcomes obtained in the two scenarios highlights the efficiency and resolution capacity of the model developed

    The identification and characterization of open innovation profiles in Italian Small amd Medium-sized Enterprises

    Open innovation (OI) denotes a company's ability to draw on a range of external sources to meet their needs for innovation. Very few empirical analyses have been conducted on the extent to which this new paradigm is embedded in small and medium-sized enterprises (SMEs). Thus, the present study's objective is to identify and characterize different profiles of OI in SMEs, considering the openness of the innovation process phases, variety and breadth of collaborations, determinants, barriers, and motivations. A survey was carried out among Italian SME manufacturing firms, and a database of 105 companies was obtained. With regard to data elaboration, a factorial and cluster analysis was conducted, and three different OI profiles emerged: selective low open, unselective open upstream, and mid-partners integrated open. The different behaviors of the three clusters in terms of determinants, performance, contextual factors, barriers, and motivations were analyzed and discussed

    Improving the efficiency of public administrations through business process reengineering and simulation: a case study

    Purpose: this paper proposes a business process reengineering (BPR) approach to a public administration of Italy, to first assess the efficiency of the administration, then to redesign its internal processes, to improve the current performance. Design/methodology/approach: a detailed mapping of the AS IS processes of the public administration was initially carried out, together with the collection of the relevant data. Then, a simulation model was designed to support the BPR approach. In particular, the model was exploited to assess the performance of the AS IS scenario of the organization, then to investigate numerous TO BE process configurations and evaluate the achievable performance improvements. Findings: From the study, it emerged that the current efficiency level of the public administration examined has potentials to be significantly improved. For instance, by maintaining its current workforce, the public administration could consider the opportunity of providing additional services to the citizens or to serve citizens from the neighbouring municipalities. Otherwise, the organization could consider a reorganization and reduction of its current workforce, at the same time keeping the service level to its citizens almost unchanged. Research limitations/implications: results of this study cannot be fully generalised, since the whole analysis is grounded on specific public administration. Moreover, although the simulation outcomes of the TO BE processes show interesting improvements compared to the AS IS scenario, the TO BE configurations were not (yet) implemented in practice. Therefore, the results provided should be confirmed in future research activities. Practical implications: the case study allowed deriving some useful guidelines to improve the efficiency of the public administration examined, as well as to identify some TO BE configurations that could be implemented in practice. Originality/value: scientific literature includes a limited number of studies that evaluate the efficiency of public organizations in real contexts. Moreover, no studies target public administrations in Italy. Therefore, this case study represents an interesting addition to the literature
