    L'UTILIZZO DI CELLULE MESENCHIMALI AUTOLOGHE COLTIVATE IN VITRO NEL TRATTAMENTO DELLE DISTROFIE VULVARI FISIOLOGICHE E PATOLOGICHE

    La distrofia vulvare può presentarsi in condizioni fisiologiche come in menopausa e patologiche come nel lichen sclerosus, nella graft-versus-host disease vulvare o in relazione a trattamento ormonoterapico. Sintomi associati sono secchezza vaginale, bruciore dolore e dispareunia. Frequenti sono i processi infettivi a livello vaginale e a livello del tratto urinario, anch’esso coinvolto dal processo distrofico. Per il trattamento delle distrofie vulvari è già stato dimostrato l’effetto positivo del lipofilling. La possibilità di isolare una popolazione di cellule mesenchimali, definite ‘Adipose-Derived Stem Cells’ (ADSCs), dal tessuto adiposo autologo, ha enormemente amplificato le potenzialità terapeutiche a nostra disposizione. Presso l’Unità Operativa di Chirurgia Plastica e Ricostruttiva, abbiamo valutato l’efficacia dell’infiltrazione di ADSCs veicolate da acido ialuronico per il trattamento delle distrofie vulvari fisiologiche e patologiche. La tecnica utilizzata è stata: prelievo del grasso tramite liposuzione, isolamento e coltura delle ADSC in vitro e infiltrazione delle cellule nelle aree distrofiche. Abbiamo ottenuto un miglioramento del trofismo vulvare, netta riduzione del dolore e un alto grado di soddisfazione sessuale riferito dalle pazienti. L’infiltrazione delle ADSCs vuole rappresentare una metodica innovativa capace di restituire una normale anatomia della vulva e di rendere possibile visite ginecologiche e procedure di screening oncologico come il Pap-test, impossibili da eseguire prima del trattamento

    A New Method for the Validation of Ultraviolet Reactors by Means of Photochromic Materials

    The interest in ultraviolet (UV) water sterilization has grown significantly in recent years. The main difficulty in designing a UV reactor is to assess the UV dose delivered. In fact, that dose depends on both the UV radiation field and the flow distribution within the reactor. At the design phase, computational fluid dynamics (CFD) helps to predict the UV dose distribution, but it requires a validation; nowadays, such validation is possible only using the Lagrangian actinometry method. This promising technique, however, requires a complex and expensive equipment, which makes it difficult to apply in most of the real contexts. The purpose of this work is to develop a new method to make the validation of the UV reactor performance a faster, less expensive, and more sustainable procedure. To this aim, we used two photochromic materials, sensitive to the UV-C radiation. Each material has been characterized by relating its color variation with the absorbed UV dose. Samples of such materials, in some cases stuck on supports characterized by different densities, were then inserted within a pilot UV reactor under three different flow rates, to measure the dose distributions. These latter were then compared with the results obtained by the CFD simulations performed on the same reactor geometry, and by biodosimetry analyses. The best results, both in terms of average value and distributions of the UV dose, were obtained from the photochromic amorphous polypropylene samples, having a density similar to that of water. This method emerges then as a promising validation technique, able also to assess the dose distribution of a UV reactor

    Free Vibrations of Composite Oval and Elliptic Cylinders by the Generalized Differential Quadrature Method

    We use the generalized differential quadrature method (GDQ) and shell theories of different order to study free vibrations of laminated cylinders of oval and elliptic cross-sections. In the GDQ method partial derivatives of a function at a point are expressed as weighted sums of values of the function at several neighboring points. Thus, strong forms of equations of motion are analyzed. It is found that the computed frequencies rapidly converge with an increase in the number of grid points along the oval or elliptic circumference defining the cross-section of the mid-surface of the cylinder. For a clamped-free elliptic cylinder the converged frequencies match well with the corresponding experimental ones available in the literature. Furthermore, the lowest ten frequencies computed with either an equivalent single layer theory or a layer wise theory of first order and using shear correction factor are accurate

    La Sclerodermia, è una malattia infiammatoria cronica del tessuto connettivo caratterizzata da alterazioni morfofunzionali dei piccoli vasi san- guigni (vasculopatia diffusa) e da modificazioni legate alla deposizione nei tessuti di collagene e di altri componenti della matrice connettivale (fibrosi), in particolare a livello della cute e di vari organi interni. Le opzioni terapeutiche per il miglioramento del trofismo tissutale nei pazienti sclerodermici includono l’applicazione di emulsioni e creme idratanti, l’utilizzo di filler e biostimolazione ed in particolare il lipofilling. Una metodica innovativa è l'impiego di cellule staminali mesenchimali derivate da tessuto adipose e coltivate in vitro., i principali vantaggi di quest'ultima metodica, sono stati la possibilità di trattare pazienti con scarso pannicolo adiposo (BMI<20), la scarsa invasività e una buona compliance da parte delle pazienti

    Radial Basis Functions Based on Differential Quadrature Method for the Free Vibration Analysis of Laminated Composite Arbitrarily Shaped Plates

    The conventional strong form collocation approach known as Differential Quadrature (DQ) method has been applied in the past to a vast type of engineering problems. It is well-known that its application is strictly limited to regular regions where derivatives are approximated along mesh lines. Generally, its accuracy increases when the number of collocation points is large and the method tends to be stable. However, for some numerical problems several points are needed in order to obtain an accurate solution. Changing the basis functions another numerical technique was developed called Radial Basis Functions (RBFs) method, which has the advantage of approximating derivatives using irregular point distributions and the basis functions depend on the mutual radial distance of the grid points. In order to extend the idea of DQ method to a general case a Radial Basis Function based on Differential Quadrature (RBF-DQ) method has been recently developed. This method merges the advantages of both techniques. Furthermore, this work proposes the application of RBF-DQ when a domain decomposition technique is considered. In this way it will be shown that, using some kind of basis functions the number of grid points per element can be reduced compared to other classical approaches. Furthermore, once the shape parameter is fixed for one case, it is not needed to calculate it again for other applications

    Evaluation of a new hyaluronic acid dermal filler for volume restoration

    Objectives: The present study aimed to evaluate the effectiveness and safety of hyaluronic acid gel for the treatment of volume loss or contour deficiency. Methods: This 6-month, open-label study recruited sixty adults aged 40–65 years with bilateral moderate to severe volume loss or contour deficiency (Facial Volume Loss Scale [FVLS] score: 2–5). Eligible subjects were treated at the baseline visit with an injection of hyaluronic acid gel in one or more of three facial subregions: the zygomatic malar region, the anteromedial cheek and nasolabial folds. Results: A significant improvement from baseline was observed in the FVLS score at each study visit up to 6 months post treatment (p < 0.0125 by Wilcoxon test). A significant improvement was also observed in the Global Aesthetic Improvement Scale (GAIS) at each visit up to 6 months. The study showed that the GAIS score improved in 100% of subjects at month 3, while at the end of the study (month 6) the percentage of improved subjects decreased to 91. Treatment effect was maintained by 100% of subjects up to month 3, while 57% of subjects showed a persistence of effect up to month 6. Conclusions: Volume loss or contour deficiency may be safely and effectively corrected using the new dermal filler

    The origin of the muscolocutaneous latissimus dorsi flap dates back to 1906 when Igino Tansini, an Italian surgeon, described a procedure to reconstruct the mastectomy defect. After a detailed study of Tansini's original description and drawings, new insights about the pedicle of its compound flap have been found, showing that it has the same pedicle of the scapular flap. In the end, Tansini's flap should be more correctly considered as a compound musculocutaneous scapular flap
