    Clinical, histological, and ultrasound follow-up of breast reconstruction with one-stage muscle-sparing “wrap” technique. A single-center experience

    BACKGROUND: Prepectoral implant placement and complete coverage with porcine acellular matrix after mastectomy is a new concept in breast surgery presented in few cases in the literature. This paper aimed to present our single-center experience in one-stage breast reconstruction muscle-sparing "wrap" technique by evaluating clinical and aesthetic outcomes, ultrasound and histological examination, and patient quality of life. METHODS: From January 2014 to January 2017, 52 patients (40 unilateral, 12 bilateral) underwent one-stage muscle-sparing breast reconstructions with Braxon® acellular dermal matrix and implant. In 3 patients, a surgical biopsy and histological examination with immunohistochemical analysis of the periprosthetic tissue were performed. All patients underwent breast ultrasound examination, and the occurrence of capsular contracture was assessed through the Baker classification and by measuring the mammary compliance scores with the Antoon Paar Mammary compliance system. Breast appearance was evaluated using a visual analogue scale (VAS) and in terms of quality of life using the EOCRT QLQ C-30 and QLQ BR-23 questionnaires. RESULTS: Early and late postoperative complications are reported. Histological and ultrasound evaluation showed a complete integration of the matrix. According to the VAS scale, the EOCRT QLQ C-30, and QLQ BR-23, patients' satisfaction resulted in a high score in terms of quality of life and aesthetic outcomes. CONCLUSION: The results of this new surgical technique in selected cases are promising in terms of effectiveness and low rate of postoperative complications, but further long-terms evaluations are required

    Evaluation of mechanical performance of cement–bitumen-treated materials through laboratory and in-situ testing

    Nowadays, cold recycling is considered as a common road construction/maintenance technique and no longer as an alternative technique to the traditional ones. The difficulties in simulating the production and construction processes in laboratory have led researchers to practise full-scale testing of mixtures. This paper presents a comprehensive evaluation of the mechanical performance of a cement–bitumen-treated materials (CBTMs) containing 80% of reclaimed asphalt. After a preliminary mix design, the mixture was produced in a mix plant and laid down on two trial sections with different layer thicknesses (15 and 20 cm) over a homogeneous subgrade. The mixture was sampled before laying, compacted and characterised through indirect tensile test and indirect tensile stiffness modulus test. The same tests were carried out on cores taken from the trial sections. Light weight deflectometer and falling weight deflectometer surveys were performed on the trial sections to evaluate the performances of the CBTMs

    Openness is a common feature in the contemporary design field. It may be found in both production and design processes, and enables the ability to spread information, create networks and connect people, skills or companies. In the last few year many new web platforms have based their business model on crowdsourcing, networking and on collective intelligence, envisioning new relations between the customer, the designer and the production system. Today not all of them are still on the market, and others have taken new directions. This paper presents a comparison of the path of three design companies: Opendesk, Quirky and Formabilio. The first company is still flourishing, the second changed its intent in part and the third closed after three years of activity. All these platforms have some points in common: they are design companies, they produce objects, their work is based on using internet and they create networks. The paper seeks to analyze these similarities, as well as the differences between these companies, by focusing the role of design, the methods of production and distribution, and the degree of openness in each of them

    Free vibration analysis of arbitrarily shaped Functionally Graded Carbon Nanotube-reinforced plates

    By means of Non-Uniform Rational B-Splines (NURBS) curves, it is possible to describe arbitrary shapes with holes and discontinuities. These peculiar shapes can be taken into account to describe the reference domain of several nanoplates, where a nanoplate refers to a flat structure reinforced with Carbon Nanotubes (CNTs). In the present paper, a micromechanical model based on the agglomeration of these nanoparticles is considered. Indeed, when this kind of reinforcing phase is inserted into a polymeric matrix, CNTs tend to increase their density in some regions. Nevertheless, some nanoparticles can be still scattered within the matrix. The proposed model allows to control the agglomeration by means of two parameters. In this way, several parametric studies are presented to show the influence of this agglomeration on the free vibrations. The considered structures are characterized also by a gradual variation of CNTs along the plate thickness. Thus, the term Functionally Graded Carbon Nanotubes (FG-CNTs) is introduced to specify these plates. Some additional parametric studies are also performed to analyze the effect of a mesh distortion, by considering several geometric and mechanical configurations. The validity of the current methodology is proven through a comparative assessment of our results with those available from the literature or obtained with different numerical approaches, such as the Finite Element Method (FEM). The strong form of the equations governing a plate is solved by means of the Generalized Differential Quadrature (GDQ) method

    The main aim of the present paper is to solve numerically the free vibration problem of sandwich shell structures with variable thickness and made of Functionally Graded Materials (FGMs). Several Higher-order Shear Deformation Theories (HSDTs), defined by a unified formulation, are employed in the study. The FGM structures are characterized by variable mechanical properties due to the through-the-thickness variation of the volume fraction distribution of the two constituents and the arbitrary thickness profile. A four-parameter power law expression is introduced to describe the FGMs, whereas general relations are used to define the thickness variation, which can affect both the principal coordinates of the shell reference domain. A local scheme of the Generalized Differential Quadrature (GDQ) method is employed as numerical tool. The natural frequencies are obtained varying the exponent of the volume fraction distributions using higher-order theories based on a unified formulation. The structural models considered are two-dimensional and require less degrees of freedom when compared to the corresponding three-dimensional finite element (FE) models, which require a huge number of elements to describe the same geometries accurately. A comparison of the present results with the FE solutions is carried out for the isotropic cases only, whereas the numerical results available in the literature are used to prove the validity as well as accuracy of the current approach in dealing with FGM structures characterized by a variable thickness profile
