MULTIDISCIPLINARY APPROACHES IN THE MANAGEMENT OF CHRONIC RHINOSINUSITIS WITH NASAL POLYPS: CLINICAL OUTCOMES AND TREATMENT OPTIMIZATION
Chronic rhinosinusitis with nasal polyps (CRSwNP) is a complex, persistent inflammatory disorder of the sinonasal mucosa characterized by polyp formation, nasal obstruction, and impaired quality of life. Despite advances in medical and surgical therapies, the disease remains challenging due to its heterogeneous pathophysiology and high recurrence rates. This study aimed to analyze the clinical outcomes of a multidisciplinary treatment approach incorporating pharmacotherapy, endoscopic sinus surgery, and biologic agents in patients with CRSwNP. Using a prospective cohort design, 150 patients treated at the Department of Otorhinolaryngology No. 2, Samarkand State Medical University, were followed over 24 months. Outcome measures included symptom scores, endoscopic evaluation, imaging findings, and recurrence rates. The integrated treatment protocol significantly improved symptom control, reduced polyp size, and minimized the need for revision surgery. The findings underscore the importance of personalized, multidisciplinary strategies in optimizing care for CRSwNP patients and provide a foundation for future research on targeted therapies. The article addresses the pressing issue of chronic suppurative otitis media (CSOM), a common and challenging otolaryngological disorder characterized by persistent inflammation and discharge from the middle ear. Despite advances in medical and surgical treatments, CSOM remains a significant cause of hearing loss and morbidity worldwide, particularly in developing countries. This study aims to evaluate the clinical presentation, diagnostic approaches, and treatment outcomes of patients with CSOM managed at the Department of Otorhinolaryngology No. 2, Samarkand State Medical University. The research involves a comprehensive analysis of 150 patients diagnosed with CSOM, utilizing both conservative medical management and surgical interventions including tympanoplasty and mastoidectomy. The study employs a combination of audiometric testing, otoscopic examination, microbiological cultures, and imaging techniques to assess disease severity and guide treatment decisions. Results reveal that early diagnosis and tailored treatment strategies significantly improve hearing outcomes and reduce complications. The article further discusses the challenges faced in managing CSOM, including antibiotic resistance, recurrence, and patient compliance. Emphasis is placed on the importance of multidisciplinary approaches involving otolaryngologists, audiologists, and microbiologists. This work contributes valuable data to the understanding of CSOM in the Central Asian population and offers recommendations for optimizing therapeutic protocols to enhance patient quality of life and reduce the burden of this chronic disease
THE CLINICAL AND AUDIOLOGICAL EVALUATION OF PATIENTS WITH SEROUS OTITIS MEDIA: DIAGNOSTIC APPROACH AND TREATMENT OUTCOMES
Serous otitis media (SOM), also known as otitis media with effusion, is a non-purulent inflammation of the middle ear characterized by the presence of fluid in the middle ear cavity without signs of acute infection. It is a frequent cause of hearing impairment in children but also affects adults.
Objective: To evaluate the clinical presentation, audiological findings, and effectiveness of various treatment approaches in patients diagnosed with SOM.
Methods: A prospective observational study of 47 patients (aged 3–45 years) diagnosed with SOM at the ENT department of Samarkand State Medical University over a 6-month period. Diagnostic tools included otoscopy, tympanometry, and pure-tone audiometry. Treatment included medical therapy and, when necessary, surgical intervention (myringotomy with or without grommet insertion).
Results: Tympanometry proved critical for diagnosis. Medical management was effective in 68% of cases, while 32% required surgical intervention. All patients reported hearing improvement post-treatment.
Conclusion: SOM is effectively diagnosed through audiological tests and otoscopy. Early identification and appropriate management significantly improve hearing outcomes and prevent complications such as speech delays and chronic middle ear disease
ЭНЗИМОТЕРАПИЯ ДЛЯ ПРОФИЛАКТИКИ ОСЛОЖНЕНИЙ ДЕСТРУКТИВНОГО АППЕНДИЦИТА
Destruktiv appendifit — bu appendifitning o‘rta shakli bo‘lib, peritonit va septikgi kabi jiddiy qiyinchiliklarga olib kelishi mumkin. Standart davolanish appendifektomiya va antibiotiklar bilan amalga oshiriladi, ammo operatsiyadan keyin yuzaga keluvchi qiyinchiliklarni oldini olish uchun, ayniqsa adgezionlarni oldini olish uchun, enzimoterapiya kabi qo‘shimcha davolanishlarga qiziqish bor. Adabiyot ko‘rinishida appendifit davolanishida enzimoterapiya qo‘llanishi bo‘yicha cheklangan to‘g‘ridan-to‘g‘ri dalillar topildi, ammo hayvonlarda amalga oshirilgan tadqiqotlar uni adgezionlarni kamaytirishini ko‘rsatadi. Insonlarda destruktiv appendifitda uni qo‘llanishi bo‘yicha maxsus tadqiqotlar mavjud emas. Shuning uchun, hozirgi vaqtda enzimoterapiya standart davolanish sifatida tavsiya etilmaydi, lekin unda yanada tadqiqotlar amalga oshirilishi mumkin

ASSESSMENT OF COGNITIVE IMPAIRMENT IN PATIENTS WITH CHRONIC CEREBROVASCULAR DISEASE: CLINICAL AND NEUROPSYCHOLOGICAL EVALUATION
Chronic cerebrovascular disease (CVD) often leads to progressive cognitive impairment affecting memory, attention, executive functions, and daily living activities. Chronic cerebrovascular disease (CVD) represents a significant public health challenge worldwide, often leading to progressive and debilitating cognitive impairment. This impairment manifests primarily in executive functions, memory, attention, and information processing speed, severely affecting patients’ independence and quality of life. Despite its prevalence, cognitive dysfunction in chronic CVD patients is frequently underrecognized and undertreated in clinical settings. This study undertook a comprehensive clinical and neuropsychological evaluation of 75 patients diagnosed with chronic cerebrovascular disease to characterize the patterns and severity of cognitive impairment. Utilizing standardized tools such as the Mini-Mental State Examination (MMSE), Montreal Cognitive Assessment (MoCA), Trail Making Test, Stroop Test, and Wechsler Memory Scale, the research aimed to detect subtle and overt cognitive deficits and correlate these findings with clinical variables including disease duration, vascular risk factors, and neuroimaging data. The results indicated a high incidence of mild to moderate cognitive impairment, with executive dysfunction and memory deficits being the most prominent features. These findings highlight the critical need for incorporating routine cognitive screening into the management of chronic CVD patients to enable early detection and timely intervention. The study advocates for multidisciplinary treatment approaches including cognitive rehabilitation, vascular risk factor control, and patient education to improve cognitive outcomes and enhance the overall quality of life for affected individuals.
