Retained Foreign Body Causing Actinomycosis and Osteomyelitis of Second Metatarsal in Paediatric Foot
Introduction: Injuries lead to cellulites or abscess formation, if neglected or improperly managed results in osteomyelitis or septic arthritis of foot structures. This is the report of isolated second metatarsal osteomyelitis following neglected foreign body in a pediatric foot.Case report: A 6 year old patient presented in the outpatient department of orthopaedics with swelling on the dorsum of the left foot from 10 days. The child gave a history of foreign body injury to the leg six months back .The diagnosis of left second metatarsal osteomyelitis was confirmed with plain radio-graphs. The ultrasound of foot showed minute wooden pieces. Patient was treated with complete removal of foreign body (wooden pieces), surgical debridement .As the histopathology and culture sensitivity of the foreign body and pus respectively showed actinomycosis with fungal elements a combination of flucanozole and cefixime was started for 1 week postoperatively. Conclusion: The neglected foreign body can result in osteomyelitis of small bones of foot in paediatric patient. The ultrasound is more useful tool in localizing foreign bodies for those which are not radio-opaque. Early diagnosis with adequate treatment of established osteomyelitis by complete removal of foreign body, curettage and antibiotics can give good results
Seroprevalence of HIV and Hepatitis-C infection among blood donors in Vijaypur district Karnataka, India-A Retrospective study
This retrospective study was undertaken to known the Seroprevalence of HIV and HCV viral infections among blood donors of both sexes over a period of 2 years from January 2011 to December 2012. A total 3535 blood donors of both sexes of were selected. 5ml blood was collected in a plain, sterile bottle. Sera separated by centrifugation analyzed by enzyme-linked immunosorbent assay and alternatively by rapid assays at Blood Bank of BLDE Shri B M Patil Medical college, Hospital and Research Centre Vijaypur. We found that prevalent of transfusion transmitted infections about 2.43% Male predominant amongst male donors i.e.1.78% of HBsAg compared to HIV 0.14% and , HCV 0.23% respectively. Amongst female donors i.e. 0.17% of HBsAg compared to HIV 0.17% and , HCV 0.35% respectively. No donor was found with positive for syphilis and malaria. By thisstudy we conclude that it would be possible screen a large population of donors for prevalence of HIV and HCV, and creating awareness among population about transfusion transmissible infection would lead to an effective control of transfusion transmissible infections
Colposcopic Evaluation of STD’s
Objectives. To differentiate between suspicious lesions requiring biopsy and insignificant lesions thus reducing the number of biopsies.Methods.Hundred STD outpatients from January to May 2008 were included in the present study. After microscopic examination of wet films, endocervical smear and PAP smear, colposcopic findings without and with green filter, acetowhitening, and Lugol’s iodine uptake were noted as normal, grade 1, 2 and 3. Biopsy was taken from suspicious sites. Colposcopic findings were correlated with PAP smear and biopsy reports. Observations. Dysplasias were common in sexually active multies and HIV positives. Leucorrhoea of erosion cervix, cervical polyps, cervical tears, candidiasis and bacterial vaginosis colposcopically did not require biopsy. Benign grade1 changes (41%) correlated with PAP smear reports (inflammatory36% mild dysplasia 18%). The premalignant grade 2(13%) & grade 3 (5%) colposcopy changes of chronic cervicitis histologically revealed chronic nonspecific cervicitis (5%), chronic nonspecific papillary endocervicitis (1%), TB cervicitis (1%),mild (3%), moderate (1%) and severe dysplasia(1%). Itching genitalia included candidiasis (grade1), trichomoniasis (grade2&3), leukoplakia vulva(grade2),kraurosis vulva(Grade2), senile vaginitis (Grade1), atrophic vaginitis (grade2 with moderate dysplasia on HPE).In Genital swellings decubitus ulcer and warts showed Grade 2changes.Ulcer genitalia (grade1), ulcer cervix (grade 3) was associated with diabetes mellitus.Conclusions.Colposcopy is of immense help in locating the site of biopsy of precancerous conditions. Insignificant colposcopic lesions correlated with PAP smear findings. STDs with persistant inflammatory smears, anogenital warts, trichomoniasis and HIV positivity presented with more dysplastic changes. Key message: Colposcopy is useful for early diagnosis of precancerous STDs and helps to locate the site ofbiopsy
A case report of a Glomus tumor in the nail bed of the middle finger
A 28-year-old male patient presented with a 5-year history of progressively intense pain, cold sensitivity, and severe tenderness to palpation of the radial side of the tip of his left middle finger nail. Subsequent diagnostic evaluation with magnetic resonant imaging revealed the presence of a glomus tumor in the tender area. Glomus tumors are benign, occurring in the vascular hamartomatous tubercles of the glomus body, which is a myoarterial apparatus typically found in the reticular dermis of the skin. Distal glomus tumors are relatively uncommon, and account for approximately 1% of all hand tumors. Most of them are located in the subungual area because of its high concentration of glomus bodies. We report a case of a glomus tumor with a typical triad of symptoms, yet with a rare location: on the pulp of the radial aspect of the distal phalanx of the left middle finger
