Study to Assess The Effectiveness of Structured Teaching Programme on Knowledge Regarding Immediate Care for Convulsion among Primary School Teachers in Selected Schools at Vadodara District
Aims1) Assess the existing knowledge regarding immediate care for Convulsion. Aim2) Evaluate the effectiveness of structured teaching program regarding immediate care for Convulsion. Aim3) Find out the association between the pre-test knowledge scores among primary school teachers with their selected demographic variable. METHODS 60 Primary school teachers were selected through non-probability convenient technique. Existing knowledge was assessed through a pretest after which STP was given on “Immediate care for Convulsion” to them. A post test was conducted after a week through the same tool as administered during pretest. The module covered aspects on Immediate care for Convulsion: Definition, types, causes, investigation, Immediate care for convulsion. The pretest score were compared and analyzed through descriptive and inferential statistics. RESULTS:Comparison between the pre & posttest scores obtained by paired “t” test. To test the hypothesis the level of significance was set at 0.05 level of significant. The value of “t” was calculated to analyze the difference in knowledge of the primary school teachers with their pre-test and post-test scores after calculation “t” calculated 18.87 is more than “t” table 1.69 at the 0.05 level of significance so it shows the very highly significant and association between pre and posttest knowledge score of primary school teachers. CONCLUSION The STP regarding “Immediate care for convulsion” was found to be effectiveness in increasing knowledge level in Primary school teachers
Anaesthetic Management of a case of Bilateral Pheochromocytoma with Cardiomyopathy
A 22year old male patient with bilateral pheochromocytoma presented with dilated cardiomyopathy, hypertension and tachycardia with history of Congestive cardiac failure (CCF). Confirmation of diagnosis was done by CTscan and raised catecholamines levels. Preoperative BP and HR were controlled with phenoxybenzamine, Angiotensin converting enzyme (ACE) inhibitors and Beta blockers. The anaesthetic technique used was general anaesthesia with epidural analgesia. Extubation was done after 12hrs with the help of Inj.Dexmedetomidine infusion
Vanishing lung syndrome: A Systemic Review
Vanishing lung syndrome is a rare condition, also known as idiopathic giant bullous emphysema. It is commonly seen in young smokers, alpha 1 antitrypsin deficiency and in persons with Marijuana abuse. It is often confused as pneumothorax and ICD being placed. It is progressive disease and signs and symptoms vary from asymptomatic presentation to exercise intolerance, worsening dyspnoea and spontaneous pneumothorax. CT scan of chest is the investigation of choice and limited bullectomy has shown better outcome
Stroke in Young Adults: A Study from a Tertiary Care Hospital, Guntur.
Background: Stroke is a worldwide health problem. Stroke in people under 45 years of age is less frequent than in older populations but has a major impact on the individual and society. Material & Methods: It was a retrospective, record based study of patients of stroke in the age group of 15-45 years admitted to Government General Hospital, Guntur. Hospital records were analyzed for a period of 2 years from January 2013 to December 2014. A total of 103 records were selected. A semi structured, pre coded proforma was used to collect information from the records. Results: Out of the 103 cases of stroke, majority 65 (63.2%) were ischemic stroke, three fourths (74%) were males and majority (56%) were from 31-45 years age group. High prevalence of risk factors was present in the study population. There were about 61% (63) smokers, 52% (53) alcoholics, 48% (49) diabetics and 68% (70) hypertensives. With regards to outcome about stroke, one thirds (33%) had good outcome and about 52% had disability. Conclusions: The present study concludes that ischemic stroke was more common in young adults. There was male preponderance with majority of cases occurring in 31-45 years which is the most economically productive period. High prevalence of risk factors was present which could have been prevented through health education and lifestyle modifications
Evaluation of Apoptotic and Proliferative Indices in squamous epithelial lesions of Uterine cervix (A light microscopic study)
Background; Apoptosis is a pathway of cell death induced by a tightly regulated intracellular programme, in which the cells destined to die, activate enzymes which degrade cell’s own nuclear and cytoplasmic proteins. The plasma membrane remains intact, but it’s structure is altered in such a way that Apoptotic cell becomes a target for phagocytosis. It is different from necrosis which is characterized by loss of membrane integrity, enzymatic cellular digestion and followed by host inflammatory reaction. Apoptosis is seen in various physiological and pathological conditions. Molecularly, a complex mechanism and regulation of apoptosis is involved (extrinsic & intrinsic pathways). The process is increased in cancers by activation of certain cancer genes. Biologically, cells having chromosomal lesions or mutated DNA may be eliminated by apoptosis, which is increased in these states. Aims and Objetives;1) To evaluate spectrum of cervical squamous epithelial lesions on light microscopy for apoptosis and to calculate Apoptotic Index (AI), Mitotic index (MI) and Turnover Index (TI) of the lesions. 2) To correlate three, AI, M I and T I with different grades of lesions. Methodology; The study was conducted in the Department of Pathology, MMIMSR, Mullana over a period of 18 months. All Cervical biopsies received, were processed conventionally, studied histologically and finally evaluated for apoptotic and mitotic counts. Conclusion; Apoptosis and mitosis both increase with increasing grades of squamous epithelial lesions in uterine cervix. Besides, squamous epithelial lesions of cervix, proliferative and apoptotic indices have also been found to be useful to distinguish between benign, premalignant and malignant lesions. Moreover, proliferative activity of a lesion is a reliable indicator of its malignant potential and together with apoptotic count gives an idea about net growth of tumor. So apoptotic index has also a great prognostic significance in cervical malignancy
Preliminary study to investigate the efficacy of Zoofa – e - Yaabis (Hyssopus Officinalis Linn) in stable cases of Bronchial Asthma
Bronchial asthma (Zeequn-nafs Shobi) is well known illness since ancient era. The word Asthma is derived from Greek language, which means “noisy breathing” or making blowing noise or panting. It is defined as "Chronic infective disease of airway that is characterized by increase responsiveness of the tracheobronchial tree to a multiplicity of the stimuli". It affects 5% to 10% of the world population. It is still a major challenge to treat in conventional stream of medicine. Only a symptomatic conservative treatment is available, thus as of today, a thrust area for research in every field of medical science. Unani physicians claimed to possesses many safe and effective herbs for management of asthma which have the properties of bronchodilator, expectorant and mucolytic. Hence preliminary an open perspective clinical trial was designed to evaluate the efficacy of Zoofae-Yaabis (Hyssopus Officinalis) in stable cases of bronchial asthma. 60 cases of either gender between the age group 20 - 60 years were treated for 90 days. Enrolled subjects got satisfactory relief in their signs and symptoms after 90 days. Hence, it can be concluded that test drug Zoofa- e-Yaabis (Hyssopus Officinalis) has the potential to significantly reduction in clinical signs and symptoms with considerable reduction in laboratory parameters
Study of Control of Primary PPH in Cases of Placenta Previa by Intrauterine Packing in GGS Medical College and Hospital Faridkot
Placenta Previa is a major cause of haemmorrhage in Obstetrics. There can be severe haemorrhage from the placental site after its removal. Various methods have been used to control bleeding .Intrauterine packing is very effective and life saving method in a situation of uncontrolled haemmorrhage due to placenta previa .This method of is effective and also peripartum hysterectomy can be avoided .Also for this much expertise is not required. In our series of cases, intrauterine packing done in cases of placenta previa was very effective and need for blood Transfusion was reduced
Uterine Rupture due to Placenta Percreta in early Pregnancy
Placental invasion anomalies are rare obstetrical complications. They cause severe third trimester hemorrhage, severe postpartum bleeding, and maternal morbidity and mortality unless they are diagnosed antenatally. We present a rare case with placenta percreta leading to spontaneous uterine rupture during the second trimester with an acute abdomen and hypovolemia
Ovarian hyperstimulation syndrome (OHSS) - our clinical experience
Introduction: Ovarian hyperstimulation syndrome (OHSS) is a serious complication of the luteal phase/early pregnancy, usually iatrogenic, after ovulation induction or ovarian hyperstimulation in the context of intrauterine insemination and in vitro fertilization (IVF). It is usually a self limiting disorder but may be more severe and persist longer than usual, if pregnancy is successful. Renal and hepatic dysfunction, thrombosis, hydrothorax, cerebral infarct and adult respiratory distress syndrome (ARDS) are the leading causes of morbidity and mortality seen in severe cases.
Materials and methods: This is a retrospective study of two years which included women with clinical signs and symptoms suggestive of OHSS. Diagnosis was confirmed by the investigations. Conservative medical management was the main stay of treatment with daily monitoring of the clinical and biochemical parameters. Surgical management like paracentesis, pleuracentesis, diagnostic laparoscopy and therapeutic termination of pregnancy was done wherever indicated. The women were discharged after the reversal in a satisfactory condition.
Results: 62.5% patients presented with early onset hyperstimulation. 31.25% patients presented with mild, 43.75% with moderate, 12.5% each with severe and critical OHSS. Polycystic ovarian disease was present in 60% women. 62.5% had pleural effusion. Intrauterine gestational sacs were one, two and three in 37.5%, 31.25 % and 12.5% patients respectively. 12.5% patients had haemoglobin more than 13 gm/dl and 25% had haematocrit more than 45%. 31.25% patients had decreased urine output. Pleuracentesis alone was done in 25% and paracentesis in 50% patients and both in 12.5%. The pregnancies continued in 87.5%.
Conclusion: Although OHSS cannot always be prevented; various parameters should be taken into consideration to prevent it. The patients with OHSS must be urgently treated and with multidisciplinary management. If left untreated, OHSS can result in serious health complications and even death.
 
Emphysematous pyelonephritis presenting with coexistent Pneumatosis intestinalis
Emphysematous pyelonephritis is a rare, life threatening, necrotizing infection of the renal parenchyma and perirenal tissues. It is characterized by gas formation in the kidney and its surrounding structures. Pneumatosis intestinalis is defined as gas in the bowel wall and is diagnosed radiologically on abdominal radiographs or computed tomography scans. Its etiology varies from benign conditions to fulminant gastrointestinal diseases. We report a case of emphysematous pyelonephritis presenting with fulminant colitis and having pneumatosis intestinalis, along with a brief review of literature. The patient, a diabetic male, presented with primary gastrointestinal symptoms and had rapid deterioration in his condition despite treatment. Radiological investigationsdiagnosed emphysematous pyelonephritis in addition to pneumatosis intestinalis. This is a rare clinical combination and due to the initial nonspecific symptomatology, may cause delay in the diagnosis of both potentially lifethreatening diseases
