    Comparative study of Manual Vacuum Aspiration and Suction Evacuation for first trimester abortion-Study of 200 cases

    Aim:-  To assess and compare the safety and effectiveness of Manual Vacuum Aspiration (MVA) and Suction Evacuation (SE) for first trimester abortion. Objective:- For first trimester abortion MVA and SE are used and aim of study was to assess and compare the safety and effectiveness of MVA and suction evacuation for first trimester abortion. Study Design:- A randomised prospective study was conducted in Obstetrics And Gynaecology Department, Government Medical College, Latur  from Feb 2008 to February 2010. 400 patients of first trimester abortion for various indications were randomly allocated to receive MVA (200) or SE (200). Patients were observed post operatively for any complications and statistical analysis was done using Standard Error of Proportion test. Result:- The group were comparable with maternal age, gestational age, operative time interval  and post-operative complication. First group showed significant decrease in post-operative complication like cervical injury, uterine perforation, incomplete evacuation, uterine atony and pv bleeding, infections with the use of MVA. No significant difference in operative time was noted. Conclusion :-This study shows MVA is much effective and safer method of termination of first trimester abortions with minimal post-operative complications

    Locking Compression Plate Fixation VS External Fixator for Distal Radius Fractures

    Purpose: To assess the outcome of distal radius fractures treated with locking compression plate and external fixator. Methods: 40 patients with fracture of distal radius were included in this study. Out of 40, 20 patients underwent open reduction and volar locking compression plate fixation and 20 patients underwent closed reduction and external fixation on random basis. The Demerit scoring system was used to assess the outcome of both groups. The average follow up period was 6 months. Results: Using Demerit scoring system of Gartland and Werley, the plate group had 9(45%) excellent, 9(45%) good, 2(10%) fair and no poor results and the external fixator group had 1(5%) excellent, 9(45%) good, 8(40%) fair and 2(10%) poor results. In plate group, 1 patient had tendon irritation. In external fixator group, 2 patients had pin tract infection and 1 patient had pin loosening. Conclusion: Open reduction and volar locking compression plate fixation group had stable fixation, early wrist range of motion and better Demerit score compared to external fixator group

    Approximately 8 million to 2.3 million people worldwide have impaired vision due to uncorrected refractive error and cataract1. Hence the World Health Organization (WHO) has initiated Vision 2020 global program – “The right to sight”. The main motto of Vision 2020 is  to correct refractive errors2. The author after years of trial and error has formulated unique Ayurvedic eye drops “ISOTINE”. This has a very precise and balanced combination of Ayurved content that not only corrects the refractive errors but also visual acuity and subjective symptoms. Number of scientific papers which have been published in various scientific journals explaining the mode action of individual contents of “ISOTINE” eye drops. The present study is a retrospective study of case series of 23 patients of myopia, hypermetropia and cataract who attended the OPD and follow up schedule at Dr. Basu Eye Hospital at Bareli, U.P. It was found that there was marked improvement in various parameters which were assessed before, during and after the study lasting for three months.Specific objective parameters of visual acuity and subjective parameters of signs and symptoms were recorded at each visit. The data was statistically analysed and the difference between parameters before and after the study was statistically found to be highly significant (p<0.0004,chi square test was applied).This indicates the highly  potent effect of "ISOTINE" eye drops for restoration of visual acuity

    Neuromuscular blocking characteristics of rocuronium bromide mixed with sodium bicarbonate

    Intravenous rocuronium produces intense discomfort at the site of injection hence leading to limb movements. Strategies to reduce or prevent pain on injection, along with the effect of adjuvant on the onset and duration of action were studied. 80 ASA physical status I or II patients, aged 18-65 years scheduled to undergo elective surgery with general anesthesia were enrolled for the study. Patients were randomly allocated into two groups to receive either rocuronium with 7.5% sodium bicarbonate or rocuronium with 0.9% isotonic saline. The patient’s response to rocuronium injection was verified through neuromuscular monitoring at the adductor pollicis muscle using train-of-four (TOF) response. The incidence of withdrawal movement after rocuronium administration was significantly lower in the sodium bicarbonate group along with shorter onset time and increased duration of action than the saline group. We concluded that the mixing of sodium bicarbonate with rocuronium reduces the pain on injection, enhances the potency, shortens the onset and prolongs the duration of action

It includes drugs which we regularly use in our diet, mainly as a supplement to our meals. ChaturvidhaSneha is especially meant to be used in raw or processed form to supplement our staple food. The aim of study was compilation of all Snehadravyasreferencesmentioned in Dravadravya Vidyaniya Adhyaya , Namarupatmaka, Gunakarmatmaka&Prayogatmaka study; classification according to Aahara, Vyadhi Hetu&Chikitsa.Compilation of literature of drugs mentioned in ChaturvidhaSneha was done from important commentaries of Ashtanga Hridaya such as Arundatta (Saravangsundara)&Hemadri (AyurvedaRasayana). Prayogavidnyana was studied under 3 sections as 1) ChaturvidhaSneha&Swasthavritta 2) ChaturvidhaSneha as Vyadhihetu. 3) ChaturvidhaSneha&Chikitsa. ChaturvidhaSneha used in form of various food cuisines and in the form of Chikitsa of different Vyadhi as Bhaya Chikitsa, Abhyantara Chikitsa or Panchakarma Prayoga in the form of various Yoga and Kalpanas so playing a very important role in  Ayurveda

    Back pain is the most frequent complaints in office working adults.  Back pain may have a sudden onset or can be a chronic pain, it can be constant or intermittent, stay in one place or radiate to other areas. It may be a dull ache, or a sharp or piercing or burning sensation. The pain may radiate into the arms and hands as well as the legs or feet, and may include symptoms other than pain. These symptoms may include tingling, weakness or numbness.  It is the fifth most common reason for physician visits, about nine out of ten adults experience back pain at some point in their life, and five out of ten working adults have back pain every year. Low back pain causes 40% of missed days of work as well it is the single leading cause of disability worldwide.In the present study effect of wet Hijamah has been studied in patients of backache 40 male 20-60year patients were included in the study. Wet Hijamah was done on Sunnah point monthly on 17th lunar date for three months. Patients who were having degeneration and disc prolapse were excluded and only patients having functional backache included in the study. Pain is measured by pain scale before during and after the study.It was found that wet cupping therapy was associated with clinically significant pain relief within 3-month follow-up. It can be concluded that Present study concluded that wet Hijamah on Sunnah points Relieves the FUNCTIONAL BACKACHE and further studies need to be conducted which will further supports the outcomes of this therapy

    Concept of Water Birth and Abzan Har (Hot Sitz Bath) for Delivery: A Correlation with Especial References to Unani System of Medicine

    Delivering in water has recently become popular, and more hospitals now offer water birth. However, water birth is still considered an alternative birthing method. Water birth refers to childbirth, usually human, that occurs in water. It is the process of giving birth in a tub of warm water. Water temperature in the tub is maintained at or around body temperature (98° F - 100° F). Warm water is soothing, comforting and relaxing. Similarly AbzanHar (Hot Sitz bath) is a type of bath in which the patient is made to sit in warm water orjoshanda of medicines (medicated sitz bath). The hip region is immersed in water in a tub. AbzanHar (Hot Sitz bath) activates the internal organs of the abdomen and pelvis by increasing blood circulation to the surface, and reduces congestion of abdominal organs and inflammation. It also helps to reduce cramps and lower back pain. Unani physicians were familiar about the use of warm water for delivery and it has been recommended by various eminent Unani physicians like Ibn-e-sina,ZakariyaRazi, Abu Al- Hasan Ali Ibn-e-Abbas, MohdKabiruddin,AkbarArzani and HkmMohd Azam khan etc . In both the techniques water birth and AbzanHar, warm water is being used to ease delivery. Though it has not been mentioned in Unani literature that delivery should also occur in water but it is clearly mentioned that when the time of delivery is near or the female is in labour pain make her sit in wamwater(AbzanHar –hot sitz bath).We cannot say that AbzanHar and water birth are same but we can definitely say that the use of warm water for delivery is in practice since ancient Unani times. Thus we can co relate the concept of water birth and AbzanHar  as in both the techniques use of  warm water is an important aspect of delivery

    Problems associated with preterm births and its management

    A child is the greatest treasure of mankind. His health, well-being, safety and future are in the hands of his parents from birth until he is on his own. A preterm infant is traditionally defined as a live-born infant born before the end of 37 weeks of gestation. The common factors associated with preterm births are low socio economic background, poor nutritional status, age of mother, infections, closely spaced pregnancies and obstetric complications like premature rupture of membrane and premature separation of placenta[1]. Some of the risk factors associated with prematurity such as asphyxia, hypothermia, sepsis, pulmonary problems, jaundice and retinopathy of prematurity can be prevented by avoiding early marriage and teenage pregnancy, providing pre-pregnancy health check-up, providing optimal and good quality antenatal care to all pregnant women, avoiding physical labor, emotional stress and sex during third trimester. Creating the awareness among the pregnant women about the early detection and management of prematurity is an important step in evading of the morbidity and mortality rate of preterm babies

    A Study of Small Volume Plasma Exchange in Guillain Barre Syndrome Patients Admitted in Gandhi Hospital

    Introduction: Guillain Barre Syndrome (GBS) or Acute Polyradiculoneuritis is an acute, diffuse post-infective- disorder of the nervous system involving the spinal roots, the peripheral nerves and occasionally the cranial nerves. Small volume plasma exchange was found to be as efficacious as large volume plasma exchange. It  is an acceptable mode of treatment in setting of limited resources. Methods & materials: In this study 40 adult patients (> 14years of age) diagnosed as Guillain Barre Syndrome (GBS) fulfilling the Cornblath criteria as modified by Asbury admitted in Gandhi Hospital were treated with small volume plasma exchange. Summary &results: Maximum  number of cases were in age 14-25 subgroup i.e 15,37%.  The male to female ratio of 3 : .The most common GBS subtype was axonal AMSAN variant.In this study 50% of patients reached peak deficit within 1 week of onset of illness,82.5% by 2 weeks.60% of our patients had cranial nerve dysfunction. Seventh nerve was most commonly involved. In those who presented in first week the paired t-test before and after plasma exchange was 4.817in upper limb,9.881 in lower limb& p value was 0.0001 which indicates that improvement in power was significant.  In patients who presented in second week  ,i.e,8 to 14 day subgroup ,paired t-test value was 3.734&6.492 in upper limb &lower limb respectively. The p value was 0.003 &0.000 which was significant.  In patients who presented late in third week, the paired t test value was 1.0&1.96 in upper limb& lower limbs respectively .the p value was 0.356 in upper limb& 0.096 for lower limb which was not statistically significant. These results suggest that small volume plasma exchange was most beneficial when initiated within two weeks of onset of illness to presentation

It is a frequent gyenaecological complaint that accounts for more then 1/4th gynaec .According to classical unani reference books sailanur raham is a discharge of fluid from the vagina .This fluid is produced in uterus due to weakness of absorptive faculty in the uterus also due to the natural evacuation or istifraghe-tabiee, where tabiyat diverts all the wastes of body to the natural manafiz, so these wastes comes in uterus to be evacuated. there is a classification made by the authors for sailanur raham, based on types of wastes upon colour and consistency of fluid it was demonstrated by hukma by asking patients to keep tampon is having blackish red colour, then the waste of sanguinic humour is there, orange red for bilious wastes, milky white of phlegmatic wasted and black colour indicates melancholic wastes.&nbsp
