MINIMAL INVASIVE FOR MAXIMAL RECOVERY IN DENTAL TRAUMA : A CASE REPORT
Introduction: Dentoalveolar fractures can be treated in a simple non-surgical procedure by repositioning the fractured fragments and immobilizing them. One of the factors affecting the success of dentoalveolar fracture treatment is the choice of material and immobilization technique.
Case: A 16-year-old girl came to the Emergency Department with complaints of bleeding from the mouth due to a traffic accident 5 hours ago. Her lip hit the handlebar of a motorcycle. There was no history of fainting, vomiting, and bleeding from the ears and nose. Extraoral examination revealed an asymmetrical face. Edema and laceration on the upper lip. Intraoral examination revealed gingival laceration of teeth 12-22 and avulsion of teeth 12,22. Panoramic x-ray revealed supra position of teeth 11, 21 and avulsion of teeth 12, 22.
Case Management: Wound debridement, indirect root canal treatment of teeth 12, 22, repositioning of teeth 12-22, fixation with wire and composite splinting, selective grinding, antibiotics, and analgesics were performed. Post-operative instructions included maintaining oral hygiene, a soft diet, and a return for splinting control.
Discussion: The goal of dentoalveolar trauma treatment is to restore the normal function and shape of the masticatory organs. The treatment in this case uses flexible splinting because it has ability to resemble the physiological mobility condition of the teeth which can help the healing of the periodontal ligament. The treatment results showed soft and hard tissue healing after one month of treatment.
Conclusion: Successful treatment of dentoalveolar fractures will be achieved if performed promptly and if accurate immobilization and accurate tools are used. Simple tools and techniques make flexible splinting a good treatment for simple dentoalveolar fractures
PERIODONTICS IN ARTIFICIAL INTELLIGENCE ERA : A LITERATURE REVIEW
Introduction: Artificial intelligence (AI) involves the creation of computer systems that imitate human actions, and it is progressively adopted as a supportive tool in aiding clinicians with disease diagnosis and treatment. One prevalent global ailment is periodontitis, which leads to the degradation and loss of the tooth-supporting tissues. The aim of this review is to evaluate existing literature that delineates the influence of AI on diagnosing and studying the prevalence of this condition.
Review: A Pubmed advanced search with narrative review was conducted of the past ten years using several search term such as “artificial Intelligences” and “periodontics”. Thorough searches were conducted on Pubmed in June 2023, encompassing studies where AI functioned as the independent variable for assessing, diagnosing, or treating patients with periodontitis. After eliminating duplicates, a total of 100 articles were recognized for preliminary abstract scrutiny. Of these, 76 documents were excluded, resulting in 24 texts for comprehensive evaluation.
Conclusion: The development of artificial intelligence in the field of dentistry requires more systematic reviews and meta-analyses to enhance the knowledge and scope of artificial intelligence applications. AI models for periodontal applications are still under development and in the future, they have the potential to support diagnostic accuracy capability
AESTHETIC IMPROVEMENT IN ONE VISIT PERIODONTAL PLASTIC SURGERY USING SCALPEL AND LASER SURGICAL TECHNIQUE (CASE REPORT)
Introduction: A charming smile is formed from good harmonization of teeth and gums. Short teeth clinical crown, dark gingival color, diastema between the maxillary central incisors can interfere the aesthetic of the smile. This case report aims to describe the treatment of periodontal plastic surgery in form of aesthetic crown lengthening, gingival depigmentation and frenectomy to improve function and aesthetic in one visit using scalpel and laser.
Case: A 23 year old male patient complained that the maxillary front teeth spaced, short and small, and the gum of the upper jaw is dark, which caused aesthetic discomfort. Objective examination showed high frenal attachment reaching the interdental papilla to palatal, diastema of the maxillary central incisors, short clinical crown and brownish maxillary gingiva. aberrant frenum labialis superior, altered passive eruption and gingival hyperpigmentation diagnoses was made in this case.
Case Treatment: Aesthetic crown lengthening treatment with scalpel, gingival depigmentation using scrapping technique and frenectomy using combination of laser and scalpel surgical technique was carried out in one visit.
Discussions: The advantages performing three periodontal plastic surgeries in one visit is require just once preparation of surgical materials and instruments, once surgical trauma, once prescription and less number of control visits.
Conclusions and Suggestions: Treatment of crown lengthening, gingival depigmentation and frenectomy in one visit are considered effective and efficient based on time, trauma, and costs required. The periodontal plastic surgery technique can be choosing based on clinical finding on patient, patient’s needed and clinical skills of operator
ERUPTION CYST OVERLYING THE CROWN OF UPPER RIGHT FIRST PREMOLAR: A CASE REPORT: KISTA ERUPSI YANG MENUTUPI MAHKOTA PREMOLAR PERTAMA KANAN ATAS: LAPORAN KASUS
Introduction: An eruption cyst is one of the odontogenic cyst involving the soft tissue, associated with the eruption of the primary or permanent tooth that lies behind the cyst. It often occurs in children; first permanent molars and maxillary incisors are the most prevalent teeth. Case: This article report an eruption cyst case in a 9-year-old female patient. The patient came with swelling on the gingiva overlying the crown of 14, and , clinically, the tooth has not erupted yet. Intraoral examination revealed the presence of smooth swelling, 1 cm x 0.8 cm, with bluish-black color. Case Management: Surgical incision under local anesthesia was performed to remove the cyst and exposed the tooth. Discussion: Eruption cyst is a soft tissue cyst. The exact aetiology remains unclear. It appears as a dome-shaped swelling, soft, transparent, bluish to blue-black color. It’s hard to distinguish eruption cyst on radiograph examination, because it’s a soft tissue cyst, no bone involvement. Histologically, it presents connective tissue covered with layer of non-keratinized squamous epithelium. A small asymptomatic eruption cyst does not require any treatment because it could rupture spontaneously. If it causes a problem, it must be removed by incision and drainage the cyst fluid contents. In this case, the cyst treated with simple incision, the tooth was exposed and could erupt normally. Conclusion: The latest knowledge and developments in diagnosis and treatment of eruption cysts are important for clinicians to understand in order to decide the right diagnosis and treatment plan
EARLY DETECTION OF VERRUCOUS CARCINOMA USING METHYLENE BLUE STAINING IN SUSPECTED LEUKOPLAKIA LESION: A CASE REPORT: DETEKSI DINI KARSINOMA VERUKOSA MENGGUNAKAN PEWARNAAN METILEN BLUE PADA LESI TERDUGA LEUKOPLAKIA: LAPORAN KASUS
Introduction: Verrucous carcinoma is one of squamous cell carcinoma in the oral cavity which can caused by smoking, alcohol consumption, HPV etc. Verrucous carcinoma can mimick as pre cancer lesion such as leukoplakia, so that early detection is mandatory. Vital staining can be done by toluidine blue, but it reported toxic in organ. Methylene blue had similar chemical structure and exhibited similar physicochemical properties to toluidine blue.The aim of this case report is to report the case of early detection verrucous carcinoma using methylene blue in leukoplakia among smoker and alcoholic patient. Case: A 46-year-old male patient came to RSIGM Sultan Agung Semarang complaint that there was a wound on the left cheek that has not healed for the last 3 years. The wound was not painful but sore when exposed to food. The wound had been treated using Albothyl but there was no change. The patient had been an active smoker since 20 years ago and consume as much as 2-3 packs of cigarettes/day and also consumed alcohol about 4 years ago. Intraoral examination nodular white plaques, unscrappable with pigmentation areas and painless. Clinical diagnosis was nodular leukoplakia. Case Treatment: Early detection using methylene blue 1% was obtained and a positive dark blue indicated a change in malignancy. Patients were given symptomatic therapy of triamcinolone acetonide in orabase 0.1% then an excisional biopsy was performed and a histopathological appearance shows thickening and changes in polarity of the epithelial cells and dysplasia. The final diagnosis is verrucous carcinoma. Conclusion: The persistent white plaque with the risk factors of smoking and alcohol is a suspicion toward malignancy, early detection using methylene blue staining can performed before biopsy
SPACE SEARCH FOR MINOR CROWDING CASES WITH EXPANSION PLATE IN PATIENTS WITH A HISTORY OF ANTIDEPRESSANT DRUG CONSUMPTION
Introduction: Gaining space in malocclusion cases using expansion plate removable orthodontic appliances. Depression is a mental health disorder that is treated with antidepressants. There is a correlation between a history of long-term antidepressant medication use and orthodontic treatment.
Case: A 21-year-old female patient with a history of antidepressant medication complained of slightly advanced maxillary front teeth and crowded mandibular front teeth. Clinical examination revealed the first permanent molar relation Angle Class I modified Dewey type 2 malocclusion with malposition of individual teeth. Arch discrepancy analysis showed a space deficiency of 4 mm in the maxilla and 2.85 mm in the mandible.
Case Management: The first stage used a removable orthodontic appliance for maxillary and mandibular expansion plates to obtain space deficiency. The expansion results showed that the maxillary dental arch was successfully expanded by 4.52 mm and the mandibular by 4 mm.
Discussion: The slow-type expansion plate functions to expand the dental arch.
Conclusion and Suggestion: Expansion plate removable orthodontic treatment effectively gains space in patients with a history of antidepressant drug use. Patients' cooperativeness in wearing the appliance influences the expansion plate removable orthodontic appliance treatment result
ANTIBACTERIAL COMPARISON BETWEEN MANUKA HONEY AND TUALANG HONEY AGAINST PORPHYROMONAS GINGIVALIS BACTERIA.
Introduction: Periodontal disease, namely gingivitis and periodontitis are an inflammatory condition of the periodontal tissue which involves interactions between bacteria, immune response, inflammatory reactions, and environmental factors. The main cause of periodontal disease is the proliferation of oral pathogenic biofilms. The main problems in the treatment of periodontal disease are antibiotic resistance and the side effects of using mouthwash which have increased over the last decades.
Review: Manuka honey and tualang honey are popular ingredients and have antibacterial, antifungal, antioxidant, antiseptic and anti-inflammatory properties, which can be used as alternative choices. The purpose of this paper is to conduct an integrative literature study on manuka honey and tualang honey against the periodontal pathogenic bacteria, namely Porphyromonas gingivalis. The material analyzed was the antibacterial properties of manuka honey and tualang honey by comparing them based on the inhibition zone, MIC and MBC values.
Conclusion: Manuka honey and tualang honey have antibacterial potential against Porphyromonas gingivalis, but manuka honey is more effective in inhibiting Porphyromonas gingivalis compared to tualang honey. Manuka honey and Tualang honey can be an alternative to avoid antibiotic resistance
IMPLEMENTATION OF THE MOBILE-BASED DENTALK APPLICATION AS A MEDIA FOR INCREASING PUBLIC KNOWLEDGE IN USING THE TELEDENTISTRY APPLICATION
Background: Teledentistry is defined as a method of using virtual conferencing technology to provide convenient diagnostics for remote care. The biggest challenge in the development of teledentistry is the understanding and knowledge of the public to use the service. Dentalk is an application designed as part of teledentistry that can provide easy access to dental health services, such as diagnosis, action plan, consultation, and follow-up.
Material and Method: The type of research used is quasi-experimental research with pre-test and post-test group design. Questionnaires were made to measure the sample's knowledge in using the teledenstistry application, in this case the dentalk application. The research sample is a subject taken from a population that meets the research criteria, namely 40 samples.
Result and Discussion: Based on the results of the comparison test, the value of 0.000 < alpha 0.05 means that there are differences in knowledge before and after using the dentalk application respondent.
Conclusion: The paired sample t-test results can be concluded that there is an increase in knowledge in using the dentalk application
IDENTIFICATION AND TREATMENT OF ERYTHEMA MULTIFORME INDUCED BY ALLOPURINOL
Introduction: Allopurinol is a xanthine oxidase inhibitor and has become established as the drug of choice for preventing and treating chronic gout. Erythema multiforme (EM) is an immune-mediated reaction that involves the skin and sometimes the mucosa. Many cases of EM are caused by drugs, one of which is allopurinol. The purpose of the case study is to explain the identification of allopurinol as a predisposing factor for EM and how it is treated.
Case. A 56-year-old woman came with complaints of scabs on her lips for 1 week ago, and had been treated with corticosteroid ointment for 3 days. Extraoral examination showed hemorrhagic crusts on the upper and lower lips with erosive surrounded by erythematous margins. On the extremity found a target lesion on the right hand. Based on the examination, the patient was diagnosed with Erythema Multiforme with a differential diagnosis of herpes labialis and herpes associated erythema multiforme.
Case Management. The therapy in this case was giving antihistamines for 1 week, NaCl 0.9% compresses on the lips for 30 minutes 3 times a day, after 2 weeks of treatment added with the application of 1% hydrocortisone cream as an anti-inflammatory and petroleum jelly on the lips as a moisturizer.
Conclusion. Identification of the etiology of this EM case was carried out through anamnesis and history of drugs consumed as well as administration of topical corticosteroids as anti-inflammatories and oral antihistamines to reduce allergic reactions
THE IMPACT OF STUNTING MALNUTRITION OF ORODENTAL HEALTH IN CHILDREN: A SCOOPING REVIEW
Introduction: Short children or commonly called stunting is a prevalent nutritional issue found among children globally. Indonesia has a notably high prevalence of stunting throughout the Southeast Asian region. Stunting malnutrition children will experience disturbances in the process of tooth growth, affecting the teeth during pre-eruption. Aim to provide an overview of the impact of stunting malnutrition on dental health in children.
Materials and Methods: This study uses the PRISMA method, with the steps: (1) Question framework (literature study); (2) Search for articles in databases (on Scopus, PubMed, and Google Scholar), and journal databases with articles published from 2020 to 2022; (3) Selection of relevant research; (4) Data extraction; (5) Synthesis with narrative method.
Results and Discussions: The study obtained 7 articles to be reviewed, proving that stunting malnutrition children experienced delayed tooth eruption in 3 articles; 4 articles analyzing the impact of stunting malnutrition on dental caries; and experiencing unhealthy dental health status in 1 article.
Conclusion: The impact of stunting malnutrition children on orodental health is delayed tooth eruption, and severe dental caries
