    Introduction: Gingiva is one of the oral mucosa that is most susceptible to injury, one of the factors is surgical procedures such as curettage which will be followed by a natural wound healing process. When a wound occurs, the cells that act as the building blocks of ground substance and the formation of collagen fibers in closing the wound are fibroblast cells. Many people think of earthworms (Lumbricus rubellus) only as animal feed or fish feed, but earthworms have many properties in antiperetic, antispasmodic, antidiuretic, antiasmatic, antihypertensive, antiallergic, anti-inflammatory, and have fibrinolytic activity. The purpose of this study was to determine the effect of the application of earthworm (Lumbricus rubellus) extract gel on the number of fibroblast cells in the gingival wound healing process of male Wistar (Rattus norvegicus) rats. Materials and Methods : This research method used in vivo laboratory experiments on wistar rats with three treatment groups, namely the group receiving 80% earthworm extract gel, and the control group giving placebo gel (CMC-Na 2%) which was observed on day 3. Results and Discussion: The results showed that there was a significant difference in the mean score of the number of fibroblasts on day 3 in the treatment group and the control group. The treatment group was given an 80% concentration of earthworm (Lumbricus rubellus) extract gel, which was 418.67, while the control group on day 3 was 270.33. Conclusion: It can be concluded that the gel extract of earthworm (Lumbricus rubellus) concentrate 80% is effective in increasing the number of fibroblast cells in the healing process of rat gingival wounds

    EFFECT OF KECEMCEM TREE (SPONDIAS PINNATA) BARK EXTRACT GEL TO INCREASE COLLAGEN POST WISTAR RATS CURETTAGE

    Introduction: Periodontal treatment such as curettage and requires adjuvant therapy for healing process. One of the adjuvant therapies used is hyaluronic acid. Recently, several scientific studies have focused on the antioxidant and anti-inflammatory properties of various natural products such as kecemcem tree bark (Spondias pinnata) having a protective effect against oxidative and inflammatory changes that occur during the development of mucositis. This study aims to determine the effect of 70% Spondias pinnata bark extract gel to increase collagen fiber density after curettage on wistar rats. Material and Methods: Wistar rats were divided into 3 groups, negative control (CMC-Na gel), positive control (hyaluronic acid gel), and treatment group (70% Spondias pinnata bark extract gel). Mice were sacrificed and then histological preparations were made for further observation in three fields of view using a 400x magnification light microscope. Results and Discussions:  The average collagen density scoring value was higher than the positive control group and the negative control group. The results of the Mann-Whitney U test showed that there was a significant difference (p<0.05). Flavonoids, phenols, and β-sitosterol in kecemcem bark extract (Spondias pinnata) have an effect on increasing the density of collagen fibers after curettage as indicated by the higher average scoring value in the treatment group. Conclusion: It can be concluded that 70% 0% Spondias pinnata bark extract gel has the effect of increasing the density of gingival collagen fibers after curettage in Wistar rats (Rattus norvegicus)

    THE EFFECTIVENESS OF CEMCEM (Spondiaspinnata (L.f) Kurz) LEAF EXTRACT IN INHIBITING THE GROWTH OF Streptococcus mutans

    Introduction: Dry socket is post-tooth extraction pain characterized by partial or complete loss of blood clots in the alveolar socket. One of the management of dry socket is by using antibiotic therapy. Inappropriate use of antibiotics in addition to being economically wasteful can also cause bacterial resistance to antibiotics. The use of herbal ingredients such as Cemcem (Spondiaspinnata (L.f) Kurz) is one solution. Cemcem leaves are thought to have antibacterial effects because they contain flavonoids, tannins and saponins. This study aims to determine the inhibition of cemcem leaf extract (Spondias pinnata (L.f) Kurz) against the growth of Streptococcus mutans. Materials and methods: The research method used is the Kirby Bauer method which is a sensitivity test with the agar diffusion method using the disc diffusion technique. In the Kirby Bauer sensitivity test using selective media, namely Muller Hinton Blood Agar media, the inhibition zone created is characterized by a clear zone around the disc. The data obtained were analyzed using the Kruskall Wallis test to determine whether or not there was a significant difference then continued with the Mann Whitney U - Test. Results: Cemcem leaf extract (Spondias pinnata (L.f) Kurz) with a concentration of 60% and 70% can inhibit the growth of Streptococcus mutans better than positive control and 80% concentration. Conclusion: 60% concentration of cemcem leaf extract is more effective in inhibiting the growth of Streptococcus mutans than 70% and 80% concentrations

    REGENERATIVE TREATMENT USING PERIODONTAL FLAP SURGERY COMBINED Ti-OSS®, PERICARDIUM MEMBRANE AND PLATELET-RICH FIBRIN IN CHRONIC PERIODONTITIS

    Introduction: Periodontal regenerative treatment with periodontal flap surgery followed by application of bone graft combined with platelet-rich fibrin (PRF) is a treatment using the concept of guided bone regeneration which is required to stimulate bone and periodontal tissue regeneration in chronic periodontitis with radiographic sign of alveolar bone vertical defect. This case report will describe the flap surgery with bone graft (Ti-OSS®, South Korea) and PRF treatment for correction of alveolar bone vertical defect in tooth with chronic periodontitis. Case: A 45-years old female complained of swollen and suppurated gingiva in the upper right area. Clinical finding showed deep periodontal pockets in tooth 13 and 15 and radiographic finding showed apical third alveolar bone loss in tooth 13 and 15. Case Treatment: Initial treatments included scaling and root planing, occlusal adjustment, and antibiotic prescription. Treatments were followed by periodontal flap surgery for debridement and elimination of granulation tissue on the root and alveolar bone surface of tooth 13 to 15, application of Ti-OSS® combined with PRF and pericardium membrane. Discussion: The follow-up evaluation 2 months after treatment showed clinical finding of decreased Clincial Attachment Level (CAL) and radiographic finding showed increased alveolar bone height around defect area of tooth 13 and 15. Conclusion and Suggestions: Periodontal flap surgery combined with application of Ti-OSS® and PRF may able to stimulate regeneration of alveolar bone and periodontal tissue in chronic periodontiti

    THE EFFECTIVENESS OF ORANGE PEEL EXTRACT TOOTHPASTE IN LIGHTENING TEETH AND PREVENTING BACTERIAL INFECTIONS LEADS TO HALITOSIS

    Introduction. Halitosis is a dental and oral illness that is common in society. Steptococcus salivarius is the bacteria that causes halitosis. The purpose of this study was to determine whether lime  peel extract toothpaste possesses any bioactive antibacterial properties against the strain of bacteria Streptococcus salivarius  and can brighten teeth. Materials and Methods. The research method used was True Experimental Design. This research design used Post Test Only Control Design. Where there are three groups, namely the treatment group, positive control group, and negative control group. The data obtained will be tested statistically using the one-way ANOVA analysis with a confidence level of 95% and followed by the Least Significance test. Dental lightening research is carried out on teeth that have been extracted. Brush the teeth with lime peel paste for 7 days and do it 2 times a day. Tooth color was recorded before treatment and after treatment with shade guide Chromascop Ivoclar Vivedent. Result and Discussion. The results of this study showed that toothpaste with lime peel extract was able to inhibit the growth of Streptococcus salivarius bacteria. The respective concentrations of lime extract in toothpaste F0 to F4 are 0%, 2.5%, 5%, 7.5%, and 10%. K+ about using toothpaste on the market, with an average inhibition zone of F1 (8.7), F2 (9.86), F3 (11.58), F4 (13.4), K+ (19.65), and K- (0), so it can be stated that the more the extract content, the higher the inhibitory power for the growth of Streptococcus salivarius. Besides that, lime peel toothpaste can brighten teeth several levels Conclusion: Lime peel extract toothpaste is able to inhibit the growth of Streptococcus salivarius bacteria and is able to brighten teeth several levels

    Introduction: Bruxism is a parafunctional habit in the oral cavity area. Bruxism is distinguished from awake and sleep bruxism. Bruxism is caused by various etiological factors. Smoking habits are considered is one of the exogenous factors that cause bruxism. This study aims to determine the effect of smoking habits on bruxism. Review: This study was conducted using the rapid review of articles reporting the effect of smoking on the occurrence of bruxism. Article searches were conducted through PubMed, Google Scholar, and EBSCO Host from October until November 2022. Article screening was carried out by checking for duplication, reading titles and abstracts, and reading the contents of the entire text. A total of 1257 articles were identified at the initial stage of the research with total of 6 articles included into rapid review criteria. Research on articles analyzed qualitatively shows that smoking habits affect occurrence of bruxism in most of the articles reviewed. As many as 2 articles prove smoking influences bruxism in general and as many as 2 articles proving that smoking affects awake bruxism. Conclusion: Smoking habits have an influence on the occurrence of bruxism in general

    CASE MANAGEMENT OF ANTERIOR TOOTH INTRUSION DUE TO TRAUMA

    Introduction: Dental trauma is damage that affects the hard tissue of the teeth and/or periodontal due to mechanical causes. One of the effects of trauma to the teeth is intrusive luxation, namely the movement of teeth into the alveolar bone, which can cause damage or fracture of the alveolar socket. Intrusive luxation causes the tooth crown to appear shorter. Case: An 11 year old male patient who was involved in a traffic accident came to the emergency department with complaints of being unable to walk, swollen lips and broken teeth. Case Management: The patient experienced intrusive luxation on tooth 21 and subluxation on teeth 11 and 22. The treatment carried out in this case was repositioning of tooth 21. Then splinting was carried out using wire on teeth 12, 11, 21, 22 for 1 month. Discussion: Immediate repositioning of luxated teeth is very important to accelerate tissue healing. Treatment in acute cases can be supplemented with acute management, namely administering antibiotics. In this case, antibiotics, anti-inflammatories and calcium are given to speed up tissue healing and reduce the risk of complications in the tissue. Conclusions and Suggestions: immediate repositioning, accompanied by wire splints and administration of antibiotics provide good results in the treatment of cases of intrusive luxation due to accidental trauma

    Introduction: To address the limitations resulting from the implementation of health protocols in providing healthcare services to patients, a solution was found in the utilization of information and communication technology to support healthcare services for patients in the form of telehealth, often known as telemedicine. Review: According to Article 1, Paragraph 21 of Law Number 17 of 2023 regarding Health, telemedicine is defined as the provision and facilitation of healthcare services, including public health, health information services, and self-care services, through telecommunications and digital communication technology. One of the aspects of telemedicine in the field of dentistry is teledentistry, which can be a solution for optimizing dental and oral healthcare services for individuals and the community. However, there are various issues in its implementation, especially related to legal issues such as licensing, patient electronic medical record privacy and confidentiality, clinical guidelines, and others that are highly vulnerable to data discrepancies, falsification, or digital patient data leakage. Conclusion: These issues can potentially lead to medical disputes that may enter the legal realm. On the other hand, teledentistry services also play a crucial role in improving public health, especially in dental and oral health in Indonesia, particularly during the COVID-19 pandemic. Therefore, the development of teledentistry services needs to be continuously improved, both in terms of systems and regulations, to provide security and legal certainty for the Indonesian population

    FRENECTOMY ON MAXILLARY LABIAL FRENULUM PENETRATING PAPILLA TYPE BY CONVENTIONAL SURGICAL TECHNIQUES: A CASE REPORT

    Introduction: The frenulum is a fold of mucosa that attaches the lips and cheeks to the alveolar mucosa, gingiva, and underlying periosteum. If the attachment is too high and thick to approach the gingival margin (papilla penetrating type) it causes a central diastema on the upper incisor or recurrence after orthodontic treatment, besides causing oral hygiene problems and gingival recession as well as denture stability. An abnormal frenulum should be removed by frenectomy. History and Clinical Finding: An 18-year-old female patient with a tall and thick maxillary labial frenulum as a cause of central diastema on teeth 11, 21. The patient had been treated with removable orthodontics starting 2 years ago. Since 4-6 months ago the diastema has relapse. The maxillary labial frenulum appears to be attached to the incisive papilla (penetration of the papillae). The Blanch test shows movement of the interdental papillae and blanching (ischemia) of the incisor papillae. Case Management: Frenectomy using a scalpel until all the papillae and fibrous fibers are removed. Control 14 days postoperative, the wound was healed. During the blanching test, there was no movement of the papillae and the color was normally. Discussions: The high maxillary labial frenulum causes the two central incisors to erupt far apart. Orthodontic treatment  must be accompanied by removal of the etiological factor by frenectomy for the treatment to be successful. Conclusions: Penetrating papilla type frenectomy of the maxillary labial frenulum with the conventional technique of using a scalpel effectively eliminates the causes of central diastema
