Deficiência de Vitamina B12 no pós-operatório de Cirurgia Bariátrica
A obesidade é uma doença crônica caracterizada por excesso de gordura corporal que se tornou a doença nutrológica mais comum em países desenvolvidos e em nosso meio. Com isso é crescente o número de pacientes submetidos à cirurgia bariátrica. No pós-operatório podem ocorrer deficiências de determinados micronutrientes, sendo um deles a deficiência de vitamina B12. Adeficiência de vitamina B12 pode levar a anemia grave e contribuir silenciosamente para problemas cardíacos, e neurológicos, desde os de ordem sensoriais até distúrbios psiquiátricos e da aprendizagem. Assim, as deficiências nutricionais servem para nos alertar sobre a importância da vigilância nutricional após a cirurgia bariátrica
Copper deficiency anemia after bariatric surgery
Copper is a trace mineral essential to hematopoiesis and to the structure and function of the nervous system. Copper acts as a ligand to ferroxidase II, which oxidizes iron, allowing it to be mobilized and transported from the hepatic stores to the bone marrow to be used in erythropoiesis..Copper deficiency is a rare cause of anemia, leukopenia, and myeloneuropathy. In this report we describe a case of a 49 year-old overweight Caucasian woman who was referred June 2008 to the Obesity Outpatient Clinic at UNIFESP for evaluation of muscular pain and weakness in both legs, fatigue and chronic anemia. The past medical history included Scopinaro Surgery for severe obesity in November 2004 when her weight was approximately 180 Kg (BMI 68.6 kg/m2). She lost considerable weight after surgery, with her weight being stable around 80 kg (BMI 30). Because of clinical manifestations of sub nutrition, protein and vitamin deficiencies and severe anemia, which lead to several hospital admissions, in February 2008 she was submitted to another surgery from conversion of the gastrointestinal surgical Scopinaro procedure to that performed by Capella. She also had a history of hypothyroidism, hyperuricemia and panic syndrome, requiring specific medications, beyond parenteral iron and B12 vitamin therapies for anemia. Physical examination revealed a bilateral edema and no other abnormality. Laboratory tests revealed hypoalbuminemia and hypochromic megaloblastic anemia with normal serum levels of B12 vitamin, iron and ferritin. Due to the persistence of the neurologic symptoms and anemia in spite of adequate iron and B12 vitamin therapies, the hypothesis of copper deficiency was considered and a low serum level of 40μg/dl was found. An oral supplementation with high doses of copper resulted in evident clinical and laboratorial improvements after eight weeks of therapy. The diagnosis of copper deficiency has to be part of the differential diagnosis in patients with unexplained neurologic symptoms, anemia, and leukopenia, especially in those who underwent to previous gastrointestinal surgery
Ecocardiografia para avaliação cardiovascular de crianças com sobrepeso/obesidade: uma revisão
In Brazil, the current data show that the prevalence of overweight and obesity in children is high and increasing. In this scenario it is necessary to detect early cardiovascular changes resulting from this framework. This review is a descriptive exploratory study that addresses the use of transthoracic echocardiography in the diagnosis of cardiovascular disorders and included publications on indexed scientific research bases, MEDLINE, SCIELO, and classical references during the period 2001-2013. The search terms used were “obesity”, “echocardiogram”, “cardiovascular disorders”. Transthoracic echocardiography accurately assesses in real time structure and cardiac function through echocardiographic parameters and therefore allows to detect early changes in diastolic function and left ventricular geometry
“Avaliação do uso de ergogênicos por praticantes de academia no Noroeste e Nordeste do estado de São Paulo”
This study aims to analyze the profile of physical activity practitioners that makes use of ergogenic dietary supplements, based on gender, age, body mass index ( BMI), education and nutritional supplements used. 229 practitioners were surveyed in 7 cities of the in Northeast and Northwest of São Paulo : Ribeirão Preto , Jaboticabal, Cajuru, Taquaritinga , Catanduva , São José do Rio Preto and Pindorama . Of these 229, 100 43.6%) used nutritional ergogenic , 69 (69%) men and 31 (31%) women . The average age was 26.8 years with a minimum and maximum values respectively, of 15 and 60 years old; The main BMI was 24.95; and prevalent schooling was complete or incomplete Higher Education (being composed of students). The most often used supplements were : rich in protein (73%), branched chain amino acids (36%), carbohydrate (35%) and creatine (29%); reminding that users could consume more tahn one nutritional ergogenic. In the male group , the most commonly used supplements were high in protein (80%), branched chain amino acids (39%), carbohydrate (39%) and creatine (36%); however the female group supplements were rich in protein (58%), fat burners (32%), branched chain amino acids (29%) and carbohydrate (26%). The general profile of the user points to a young - adult individual aged between 20-30 years old of higher education and BMI of 24.9 kg / m, showing both physical and nutritional concerns
A importância da idade na síndrome metabólica em pacientes portadoras de ovários policísticos
This study aims to evaluate the prevalence and individual characteristics of the metabolic syndrome (MBS) in sergipanas women with polycystic ovary syndrome (PCOS) using age as an association variable. It is a cross-sectional study involving 50 women aged 25-50 years old with PCOS in accord with Rotterdam criteria (2003). For the diagnosis of metabolic syndrome, the National Cholesterol Education Program's Adult Treatment Panel III (NCEP-ATP III) was considered. The prevalence of the MBS was 56%. As for the individual components of MS, was evident: abdominal circumference > 88 cm in 72%, triglycerides ≥ 150 mg/dL in 32%, HDL-cholesterol <50 mg/dl in 54%, fasting glucose ≥ 110 mg/dl in 26%, systolic blood pressure ≥ 130 mmHg in 52% and diastolic blood pressure ≥ 85 mmHg in 40%. Patients aged equal or over 35 years had a higher percentage of Abdominal Circumference (AC) > 88 cm (40%), modified triglyceride levels (20%), HDL-cholesterol to levels below 50 mg/dl (34%), altered levels of fasting glucose (22%), systolic blood pressure (SBP) equal to or above 130 mmHg (36%) and diastolic blood pressure (DBP) equal to or above 85 mmHg (26%). There was a statistically significant association (p <0.05) between age and the following variables: AC, triglycerides, fasting glucose and DBP. There was a significant prevalence of MS in more than half the women studied, indicating increased chances for developing cardiovascular disease (CVD) and pointing out the need for tracking cardiovascular risk among women with clinical manifestations of PCOS
Atualidades sobre o tratamento da Sarcopenia revisão de literatura
Sarcopenia is a syndrome characterized by progressive and widespread impairment of skeletal muscle of older people and causes a loss of its mass and function (strength and performance). Its etiology is multi-facted and also poorly understood. Courses with reduced mobility and quality of life, resulting in increased morbidity and mortality. Its prevalence is increasing as the population ages, growing demands for health resources. This makes it imperative to study more thoroughly this disease. Was carried a review of articles on the topic sarcopenia that were published since 2010 up to first half of 2015, especially focusing therapeutic interventions. It was concluded that treatment is based primarily on lifestyle change, i.e., physical activity and higher intake of protein. In addition, pharmacotherapy must be prescribed to patients for whom it is indicated, such as vitamin D and testosterone. Should be considered creatine and βhydroxymethylbutyrate among others therapies
Bariatric surgery in HIV-infected patients: review of literature
Obesity is now a common problem among HIV-infected patients receiving antiretroviral therapy (ART). Until recently, HIV infection has been considered a contraindication to bariatric surgery for various reasons. Insurance carriers have considered HIV a terminal disease, and surgeons have been reluctant to operate HIV-infected patients because of this, as well as the associated risk of infectious transmissions, although this has been changing. Gastric bypass surgery may be an option for some patients who have failed diet and therapeutic lifestyle changes, modification in ART or other treatment modalities for HIV/ART-related lipohypertrophy and obesity. However, few data are available regarding HIV-related outcomes after such surgery and its impact on ART tolerability. The aim of this study is to review bariatric surgery in HIV-infected patients
Perfil de crianças atendidas em programa de fornecimento de fórmulas infantis
Objective: To describe the profile of the children seen at the Nutrition Clinic at the municipal health network of Ribeirão Preto, through the initial protocol supply of infant formula. Material and Methods: A descriptive and retrospective study with a quantitative approach, carried out through documentary survey, the data of 924 infants who received infant formula in the 2009-2013 period. Results: It was found that 50.2% were male, 28% children were included at 2 months of age, prematurity was the biological cause in 42% of children and the formula was offered as the sole ration for 52.6% cases. Conclusion: Children who receive formula as a supplement should be greater investment target as the promotion of breastfeeding, with greater commitment of health teams in actions for breastfeeding, particularly in the immediate postpartum
Vitamina D em pediatria
Vitamin D (VD) is one of the fat-soluble vitamins. Its main sources in nature are diet and exposure to sunlight. Recent findings have shown that organs other than the kidney and liver are also able to synthesize VD metabolically active (calcitriol). Vitamin D deficiency (VDD) has been described almost 2.000 years ago and is classically related to diseases of the musculoskeletal system (especially rickets and osteomalacia). However, in recent years, VDD has been linked to disorders that affect other systems in the body, such as asthma and diabetes. In addition, an increase in the prevalence of VDD worldwide has been observed, even in countries with good insolation conditions. The most appropriate way to prevent VDD is by vitamin supplementation since there are no safe limits to sun exposure. The treatment of the VDD is mainly based on the replacement of the vitamin and it is indicated in the symptomatic forms (osteo-muscular symptoms) of its deficiency. VD intoxication is rare and observed in infants and young children who take high doses of VD in an acute way. This review article approaches these and other questions about VD, with particular emphasis on the particularities related to children and adolescents
A importância da microbiota no organismo humano e sua relação com a obesidade
This paper is a bibliographic review aiming to describe the importance of microbiota in the human organism and its relationship with obesity. Original scientific articles and reviews from indexed journals such as Brazilian Cardiology articles, Scielo PubMed/Medline, SeCs were compiled. The present work described that microbiota is a population of microscopic organisms that live into the human body. Intestinal microbiota influences gastrointestinal diseases and a great variety of immunological conditions such as, diabetes mellitus type I, cardiovascular diseases, autoimmune demyelinating diseases, allergy, asthma and obesity. There is a close association between intestinal microbiota and body weight due to a modification in bacterial composition of obese people which is different from those with normal BMI. The increase in the number of health promoting microorganisms in the gastrointestinal tract may be achieved by eating prebiotic and probiotic food. They change microbiota composition by creating a barrier against bad bacteria from the ecosystem. Such paper has demonstrated that when preserving intestinal integrity, it is possible to avoid the occurrence of diseases by preventing the entrance of pathogenic and antigenic microorganisms
