    Effects of oxidative stress during human and animal reproductions: A review

    Given its high ability to damage important cellular components (lipids, proteins and deoxyribonucleic acid), oxidative stress is now recognized as one of the most common mechanisms associated with development of a variety of diseases and natural events such as pregnancy. During reproduction period, there is a change in the pro-oxidant and antioxidant balance due to the body and circulation modifications that are inherent to the pregnancy process. The present paper discusses the role of oxidative stress on the reproduction process. More effective defense strategies are needed to decrease the deleterious effects of oxidative-stress-induced gestation. This approach could be achieved by antioxidant status alteration. Further clinical and experimental studies are needed for better understanding of oxidative stress mechanism and the impact of antioxidant supplementation on reproduction

    Tireotoxicose factícia causada pelo uso de suplementos alimentares e produtos naturais

    Introduction: Overvaluation of the image leads to excessive consumption of dietary supplements and natural products such as multivitamins, herbs and substances used for weight loss. This consumption can lead to changes in the body, among them factitious Thyrotoxicosis. Objective: The goal is to describe what has been published in the scientific community concerning the effects of dietary supplements on the development of exogenous thyrotoxicosis, as well as the consequences for the organism. Methodology: Review of the literature non systematically based on databases Pubmed, Lilacs and Cochrane databases, where 136, 109 and 4 articles were respectively found. Among them 14 were selected according to the criteria of inclusion and exclusion. Development: Laboratory analysis concluded that many supplements contained excess of iodine or thyroid hormone. Most of the times, these substances were not included on product labels, making it difficult ANVISA and FDA inspections. The supplements that were analyzed contained extracts of gingko biloba, kelp, guggulu, xanthohumol and withamina somnifera related to increased thyroid function. The use of these products can lead to the development of factitious thyrotoxicosis that causes tachycardia, irritability, insomnia, cardiovascular disorders and hypertension. Drug interactions and nephrotoxic, hematologic and cardiac effects are also reported with the indiscriminate use of these substances. Conclusion: Literature shows that the use of supplements can cause damage in the body, such as factitious thyrotoxicosis and other systemic effects. However, further studies are needed comparing the relation of food supplementation and thyrotoxicosis as well as an efficient control

    Self-perception of food intake and dietary quality

    Introduction: Self-perception of dietary quality has been associated with weight and body composition, as well as dietary pattern. Both the gain and maintenance over the long term of excess weight have been frequently associated to dietary and life habits of individuals, which justifies the improvement of assessment methods. Methods: Convenience sample consisting of 14 women who took classes at Contours® gym, aged 18-45 years. Data collection was performed in two stages: the first consisted in the application of questionnaires designed specifically for the research on socioeconomic data, general information on health and the regular use of drugs, self-perception of dietary habits and a 24-hour recall. In the second stage, collection of the 24-hour diet recall was performed. The Revised Diet Quality Index (RDQ-I) was used in the assessment of dietary standards. Results: In the assessment of eating habits, 85.7% of the respondents were classified as healthy. Most of them, 78.57%, rated their dietary pattern as positive, 14.3% were not able to rate it and 7.13% rated it negatively. The Spearman correlation coefficient indicated that there was a moderate positive correlation between the RDQ-I (Revised Diet Quality Index) and self-perception. Conclusions: There is a statistical significant correlation of dietary pattern and dietary quality

    Interface da má absorção da frutose e depressão: revisão sistemática da literatura

    The present study aims to clarify whether patients with poorly absorption of fructose are more susceptible to develop depression. It is a systematic review of literature using the descriptors of health, in BVS Salud and Medline database. Inclusion criteria: studies with patients with or without fructose malabsorption, and with or without depressive symptoms, in all age, groups of both sexes and animal studies. In English, Spanish and Portuguese. Exclusion criteria: studies whose focus was not the fructose malabsorption affecting directly or indirectly in the emergence of depression. Nine articles were recovered, published in English between 1998 and 2014, two of them are reviews, one cross sectional study, five cohort and one clinical trial. The methodological quality was evaluated by the journal´s impact factor where the article was published; in addition to analyzing the quality of each by the CASP and Strobe, evaluated by EQUATOR. One positive relation between fructose malabsorption and depression was found in eight studies except one. The relation between fructose malabsorption and depression is hardly implemented in clinical / gastroenterological practice; so many cases are not being diagnosed. Nowadays, the prevalence of depression is high and the etiology is multifactorial. This is the first national study that correlates systematically the fructose malabsorption and depression. New studies are needed for the diagnosis and therapy

    Liraglutideas Adjunct Therapy in the Management of Obesity

    The obesity epidemic has reached a new level with some impressive numbers recently published in the United States of America. Even more alarming is the rapid increase in childhood obesity, which has been universally documented over the past few decades. The reasons underlying the problem of obesity can be simplified into three categories: i) routine consumption of bigger portion size meals; ii) confusing messages from food industry to consumers; and iii) physical inactivity is the “new normal”. Considering that the medical consequences of obesity are serious and directly affect morbidity and mortality, it has become necessary to act concomitantly to prevent and treat the increase in body weight and fat excess accumulation. The prevention of obesity is a formidable task that can only be accomplished with a concerted effort put together by several governmental agencies, especially those interested in agriculture, health and education, the food industry and health care providers, including physicians and nutrition specialists. Treatment of obesity with behavioral management, nutritional manipulations (“diets”) and, even bariatric surgery has had some success, but these strategies are accompanied by limited benefits and only to a select group of individuals. More importantly, the overall impact of these on the growing obesity pandemic is disappointing, at best. Some dietary recommendations with caloric restriction and adjustments in nutrient intake combined to pharmacotherapy have expanded our ability to manage obese patients.The recent approval by the U.S. Food and Drug Administration (FDA) agency of the Glucagon-Like-Peptide-1 (GLP-1) receptor analog liraglutide, at the dose of 3.0 mg once daily, has provided us with an additional tool to combat the disease and minimize its complications. In this particular study, a significant reduction in the conversion ratesfrom pre-diabetes to diabetes was also shown and represents an important findingof the trial. Despite the fact that nausea, vomiting and diarrhea were frequently reported during the 1-year period of observation, tolerance was acceptable and most subjects completed the study.These data clearly indicate that combination drug therapy with dietary adjustments can be successful in promoting weight reduction and further support routine utilization of adjuvant pharmacotherapy in the management of obesity

    Nutrologia: Análise e Avaliação da Composição e Cinética de Nutrientes em Diversos Compartimentos e Tecidos do Organismo Humano

    Nutrology is recognized medical specialty in Brazil by the Brazilian Medical Association and the Brazilian Health Authorities, which studies the nutrients functions / food on health, illness and disease prevention. The Nutrology clinical studies in medical practice also the important role of nutrients in clinical diagnosis, functional, biochemical and pharmacological treatment, food (dietary) of patients and prevention of nutritional diseases. Obesity, atherosclerosis, dyslipidemia, type 2 diabetes, hypertension, certain cancers, dyslipidemia, deficiencies of minerals and vitamins are some examples of diseases involving disorders in primary or secondary intake or metabolism of nutrients. During the course of these diseases occur characteristic nutritional disorders, specifically related to the intake and metabolism of nutrients. They can affect several or specific parts of our body and must necessarily involve a medical evaluation for clinical diagnosis and treatment the physician nutrition specialist, who is a specialist in the study of nutrients in health and disease. It is not possible in the clinic, in medicine, nutrology to treat diseases without clinical, biochemical and functional. The nutrologic analysis complete and specialized medical history beyond clinical nutrition, including specific data on family diet, the patient’s current and past since birth, relevant medical examination with emphasis on function and pathophysiology of nutrients in the transition from health to disease. It requires a series of functional tests, bioquímicos, nutrogenéticos / nutrogenômicos, histological and physiological effects of nutrients in different compartments of the human body. This analysis should always be seeks to integrate the processes of ingestion, absorption and metabolism of macro and micro-nutrients, as well as the utilization, storage and elimination of various products and nutritional metabolism in each patient. Also it is essential to establish the structural and functional properties of nutrients with emphasis on their relationship with the basic functions of the organs and tissues of the body. This type of integrated knowledge facilitates investigations then more specific and accurate physiological repercussions accompanying disorders of nutrients and modify biological processes essential to life. The end result of this nutritional clinical research allows an upgrade and development of more effective prevention and treatment of nutritional diseases, individual and community, which are directly or indirectly linked to disturbances of the processes of organic nutrients

    Consenso da Associação Brasileira de Nutrologia sobre manejo da dislipidemia secundária à obesidade infanto-juvenil

    Objetivo: Estabelecer consenso sobre o manejo da dislipidemia secundária à obesidade infanto-juvenil. Métodos: Foi realizada pesquisa bibliográfica nas bases de dados Medline, Scielo e LILACS. Com base nas evidências científicas, o grupo de trabalho estabeleceu as condutas recomendadas pelo Departamento de Nutrologia Pediátrica da ABRAN. Resultados: Os autores redigiram o consenso que foi aprovado pela Diretoria da Associação Brasileira de Nutrologia - ABRAN. Conclusões: São apresentadas, com base nas evidências científicas, as recomendações para a condução dos casos de dislipidemia associados à obesidade na infância e adolescência

    Nutritional support in a patient with Cytomegalovirus (CMV) enterocolitis in the acquired immunodeficiency syndrome (AIDS)

    Introduction: Several diseases of the gastrointestinal tract should be taken into account in patients with the human immunodeficiency virus (HIV) since diarrhea negatively affects the patient's quality of life. Given this condition, Cytomegalovirus (CMV) enterocolitis may present with fever, weight loss, intermittent diarrhea, and hematochezia. Objective: This study aimed to report the effect of enteral supplementation with L-glutamine and oligomeric enteral nutritional therapy (ENT) formula in patients with cytomegalovirus, and with refractory diarrhea associated with wasting HIV syndrome. Methods: The information contained in this report was obtained through a review of the medical record and a review of the literature. Results: This is the report of S. S. A, a female, 27 years old, who was admitted to Hospital Ernesto Dornelles, in Porto Alegre, from 11/15/2020 to 12/30/2020, diagnosed with HIV for a month, without undergoing treatment. The patient was hospitalized due to worsening abdominal pain starting 2 days ago, with diarrhea and mental confusion. On physical examination, she presented pain in the lower abdominal region. From anthropometry, the measured weight of 67kg, the self-reported height of 1.67m, and BMI of 24kg/m2. On 11/17, she presented PCR-CMV positive for CMV and underwent laparotomy for pneumoperitoneum with endarterectomy due to perforation of the distal ileum. In use of ganciclovir for CMV. On December 2, the upper digestive endoscopy with biopsy showed an esophagus with the Z-line located close to the diaphragmatic clamping, with esophageal mucosa with extensive, friable, serpiginous ulcers, being a valuable diagnosis in CMV enterocolitis. From 12/04 to 12/08, she used exclusive enteral L-glutamine 20g/day. On 12/09, she started an oligomeric formula via the low-flow enteral route with progression to the caloric and protein targets. On 12/30, the patient had improved diarrhea, with a depressible and painless abdomen, and had been discharged. Final Considerations: In the present report, it was obtained a beneficial effect with the nutritional intervention using L-glutamine and enteral oligomeric formula in the treatment of diarrhea. Thus, the improvement in the condition in just 5 days can be explained by the rhythm of the enterocyte cell cycle, which occurs in an average period of 3 days, and it is possible that the 4 day period of nutritional intervention is sufficient to increase endogenous levels of L-glutamine
