    Get PDF
Background and aims: TAP block is an effective way to provide postoperative analgesia .TAP block using ropivacaine alone has not been consistently proven to be effective in alleviating pain after laparoscopic surgeries. The study aimed to compare the combination of dexmedetomidine and ropivacaine to ropivacaine alone in TAP block for laparascopic cholecystectomy patients for post operative analgesia. Materials and Methods: 60 patients of ASA health status I or II, were divided equally into 2 groups. Groups R received bilateral US- TAP blocks with 20 mL 0.375% Ropivacaine and Group RD received, 20 ml 0.375% ropivacaine with 50μg dexmedetomidine. Each patient was assessed in the PACU for first 24 hr for pain duration of analgesia, and need for rescue analgesic. Results: The mean of VAS pain score was least in group RD compared to group R and statistically significant (P < 0.05).The total rescue analgesia ( tramadol ) consumption in 24 h after surgery was (116.6±37.9) mg in group RD against (160±67.4) mg in group R, which was also statistically significant (P < 0.05). Duration of analgesia was greater in group RD (7.7 ± .53) compared to group R (4.5 ± .56)

Giant cell Granuloma: Histological comparison and a review

    Get PDF
Central giant cell granulomas (CGCG) constitute about 10% of benign jawbone lesions. Approximately one-third of CGCG exhibit local aggressive behavior with bone destruction and a tendency to recur.1 Jaffe (1953) was the first to distinguish central giant cell granulomas (CGCG) of the jawbones from other giant cell lesions of bones and originally called them“central giant cell reparative granulomas” since they were believed to be a reactive-reparative process that might heal spontaneously.1 Recently the World Health Organization has defined it as localized benign but sometimes aggressive osteolytic proliferation consisting of fibrous tissue with hemorrhage and haemosiderin deposits, presence of osteoclast-like giant cells and reactive bone formation. Many authors have established the differences with other lesions of giant cells. The clinical behavior of CGCG varies from a slowly asymptomatic swelling to an aggressive lesion that manifests with pain, cortical perforation, and root resorption.2 On the other hand the peripheral giant cell granuloma is a reactive exophytic lesion of the gingiva and alveolar ridge that usually occurs as a result of local irritating factors such as plaque, calculus, chronic infection, chronic irritation, tooth extraction, inproperly finished filling, unstable dental prosthesis, and impacted food which originates from the periosteium or periodontal ligament.&nbsp

    Get PDF
The Objective of this study is created a   novel derivative of the naproxen with the heterocyclic (1,2 and 4 the triazole) ring that reacted with benzoyl chloride derivatives, as well as preliminary antibacterial activity testing of the formed products. The derivatives are synthesized by esterifying naproxen hydrazine hydrate, then reacting with hydrated hydrazine and carbon disulfide, and finally reaction the benzoyl chloride derivatives (in the final step) with the triazole the derivatives of the naproxen including of amino group via a nucleophilic substitution reaction. In addition to naproxen, the antibacterial properties of the novel compounds (Va-Vd) were evaluated in agar at known doses (IN vivo). The antibacterial action of the compounds Va and Vd against staph was demonstrated in the topmost inhibited zone. In the case of E. coli, compound Vb has the smallest inhibitory zone. The findings of this study have implications to produce naproxen 1,2 and 4 the triazole-three-thiol derivatives, which reacted including the variety of benzoyl chloride derivatives. The potential of naproxen will be affected by the attraction of benzoyl chloride derivatives with para-groups on the benzen ring
