    Tumores Neuroendócrinos: Requesitos Mínimos para o Diagnóstico Clínico

    Although neuroendocrine tumors of digestive tract are still rare, their incidence had increased 5-fold in the last 30 years. It seems that this burden is real and not only secondary to the improvement of diagnostic skills. Therefore, it is important to promote them near the medical community and the population in general, alerting for the symptoms, that although they are inicially vague, can lead to early detection if there is a high index of suspicion. Establishing minimal requirements for the clinical diagnosis can help to systematize and uniform the management of these tumors leading to a timely and correct treatment. Keywords: neuroendocrine, tumors, clinical, requirements Os tumores neuroendócrinos do tubo digestivo, embora continuem a ser raros, quintuplicaram a sua incidência nos últimos 30 anos, havendo cada vez mais certezas de que se trata de um aumento real e não apenas de um aumento de detecção derivado do apuramento da acuidade diagnóstica. Impõe-se por isso a sua divulgação, quer na comunidade médica, quer junto das populações, alertando para os sintomas, que embora na sua maioria sejam inicialmente vagos, podem sugerir o diagnóstico perante um elevado índice de suspeição. O estabelecimento e a divulgação de requisitos mínimos para um diagnóstico clínico é um dos primeiros passos para uma abordagem uniforme e sistematizada destes tumores, permitindo o seu diagnóstico precoce e um tratamento atempado e mais eficaz.     Dermatofibrossarcoma Protuberans – a propósito de um caso clínico

    Dermatofibrossarcoma protuberans (DFSP) is a rare malignant neoplasm, of slow growth, which rarely metastasizes and that has its source in the skin. This paper aims at presenting a clinical case of a 84 year old patient with DFSP, located in the thorax, with a 20 year evolution, who has had excision with Z-Plastic procedure. It also aims at presenting a literature review on the Dermatofibrossarcoma protuberans. O Dermatofibrossarcoma protuberans (DFSP) é uma neoplasia maligna rara, de crescimento lento, que metastiza raramente e tem origem na derme. Este trabalho pretende apresentar um caso clínico de uma doente de 84 anos com um DFSP localizado ao tórax com 20 anos de evolução, que foi submetida a excisão com plastia em Z. Pretende-se ainda apresentar uma breve revisão bibliográfica sobre o Derma- tofibrossarcoma protuberans.

    COLECISTECTOMIA LAPAROSCÓPICA 20 Anos Depois: Perspectivas e Opções Actuais – uma visão pessoal –

    Laparoscopic cholecistectomy was introduced in Portugal in 1991. 20 years after, it is considered the “gold standard” technique and its use has suffered many changes, either by its dissemination, or because of the technical developments which happened meanwhile. Nowadays, the options and ways of approaching the problem are not only different but also allow different choices. Those possible options and its reasoning are discussed in this paper. A colecistectomia laparoscópica foi introduzida em Portugal em 1991. Passados 20 anos, está implantada como técnica “gold standard” e a sua prática sofreu muitas alterações, por via da disseminação e por via do desenvolvimento técnico que, entretanto, aconteceu. Actualmente as opções e as formas de abordagem são não só diferentes como permitem várias escolhas. Essas possíveis orientações e suas justificações são abordadas neste texto.

    SAGES guidelines for the clinical application of laparoscopic biliary tract surgery

    Laparoscopic cholecystectomy (LC) has become the standard of care for patients requiring removal of the gallbladder. In 1992, a National Institutes of Health (NIH) consensus development conference concluded that ‘‘laparoscopic cholecystectomy provides a safe and effective treatment for most patients with symptomatic gallstones, laparoscopic cholecystectomy appears to have become the procedure of choice for many of these patients’’ [1]. The Society of American Gastrointestinal and Endoscopic Surgeons (SAGES) first offered guidelines for the clinical application of laparoscopic cholecystectomy in May 1990. These guidelines have peri- odically been updated, and the last guideline in November 2002 expanded the guidelines to include all laparoscopic biliary tract surgery. This document updates and replaces the previous guideline. The current recommendations are graded and linked to the evidence utilizing the definitions in Appendices 1 and 2.

    QUISTO DO COLÉDOCO - A propósito de um caso clínico

    Choledochal cysts are anatomical variations of the intra or extrahepatic biliary tree. Reported frequency rates range from 1 case per 100.000 to 1 case per 2 million with a female-to-male ratio in the range of 4:1. We report a case of a 22-year-old male, with right upper quadrant pain, carrying a gastroduodenoscopy compatible with antral gastropathy and duodenal procidence by external compression. The computed tomography (CT) scan showed a choledocal cyst with microlithiasis inside, compressing the second portion of the duodenum. The patient performed an endoscopic retrograde cholangiopancreatography (ERCP) and a cholangio-MR (MRCP), that confirmed the diagnosis. Surgery included cholecystectomy and complete excision of the cyst with sphincteroplasty. No intra or postoperative complications were observed. Keywords: Choledochal Cyst; Cyst excision; Bile ducts. Os quistos do colédoco correspondem a variações anatómicas da árvore biliar intra ou extra hepática. Trata-se de uma patologia pouco frequente com uma prevalência de 1/100.000 a 1/2.000.000 com um ratio entre sexo feminino e sexo masculino de 4:1. Apresentamos o caso clínico de um doente de 22 anos, sexo masculino, referenciado à consulta de Cirurgia por dor abdominal a nível do hipocôndrio direito, portador de endoscopia digestiva alta (EDA) que evidenciava gastropatia antral e procidência duodenal por provável compressão extrínseca. Realizou tomografia computorizada (TC) abdominal que revelou quisto do colédoco, com microlitíase no seu interior, que moldava a face interna da 2a porção duodenal. Realizou colangiopancreatografia retrógada endoscópica (CPRE) que evidenciou quisto do confluente bilio-duodenal e litíase múltipla intra-quística. Realizou ainda colangio-ressonância magnética (CPRM) cujo resultado confirmou o diagnóstico. Foi submetido a colecistectomia com excisão do quisto e esfincteroplastia, sem intercorrências intra ou pós-operatórias. Palavras-chave: Quisto do Colédoco; Quistectomia; Vias biliares.

    Hérnia de Amyand - Caso Clínico e Revisão da Literatura

    he acute appendicitis’s diagnosis sometimes is a challenge for the General Surgeon. The different positions where appendix may be found and the protean manifestations of acute appendicitis can make the diagnosis possible only intraoperative. The Amyand’s hernia - defined as the appendix included in the hernial sac – is frequently presented as an incarcerated right inguino-scrotal hernia. There is no standard treatment for the Amyand’s hernia. Factors as the presence of an inflamed appendix, the contamination of the operating field, the patient’s age and other anatomical variants must be considered when deciding the correct surgical techniques. The authors report the case of an 82-year-old male observed in the Emergency Department with a voluminous incarcerated right inguino-scrotal hernia. At surgical time it was found an Amyand’s hernia with a gangrenous appendicitis without perforation. An appendicectomy was performed followed by the hernia’s repair according to Rutkow-Robbins technique. The postoperative period was uneventful and the patient was discharged at day 3. Keywords: Amyand`s hernia, incarcerated inguinal hernia; Acute Appendicitis O diagnóstico de apendicite aguda pode constituir um desafio para o Cirurgião Geral. A possibilidade do apêndice ileocecal se apresentar em diferentes localizações e a eventual carência de clínica sugestiva podem levar a que o diagnóstico só seja possível intra-operatoriamente. A Hérnia de Amyand, definida como a presença de apêndice ileocecal em saco herniário, apresenta-se muitas vezes como uma hérnia inguino-escrotal direita encarcerada. Não existe um tratamento standard para a resolução da Hérnia de Amyand. Factores como a presença de apêndice inflamado, contaminação do campo operatório, idade do doente e condicionantes anatómicas são determinantes na opção da técnica cirúrgica. Os autores apresentam o caso clínico de um doente do sexo masculino de 82 anos de idade que recorre ao Serviço de Urgência com hérnia inguino-escrotal direita volumosa, com encarceramento. Tratava-se de uma Hérnia de Amyand, tendo sido realizada apendicectomia em apêndice gangrenoso, não perfurado, seguido de correcção do defeito herniário com prótese pela técnica de Rutkow-Robbins. O período de pós-operatório decorreu sem intercorrências relevantes, registando-se alta hospitalar ao 3o dia. Palavras-Chave: Hérnia de Amyand; Hérnia inguino-escrotal encarcerada; Apendicite Aguda

    Leiomiosarcoma do Intestino Delgado – um caso clínico

    Leiomyosarcomas (LMS) are rare mesenchymal tumours. LMS of the small bowel originate from smooth muscle cells from muscularis propria, with a peak of incidence around 60 years of age, without characteristic clinical signs, being, most of the time, detected when studying the cause of an occult haemorrhage or intestinal occlusion, with findings in CT scan or US. We report a case of a female patient, of 53 years of age, brought at our assistance with complaints of hipogastric discomfort, with a palpable mass in hipogastro and umbilical region, and that after performing an abdominal CT scan, was disclosed a mass, with 12 cm, appearing to have involvement of jejunal loops. In surgery, a jejunal mass, with extra-luminal growth was disclosed and excised, whose histology identified as being a Leiomyosarcoma. With a 24 month of follow-up, there is no recurrence. Keywords: leiomyosarcoma; neoplasia; small bowel; Objectivo: Apresenta-se o caso de doente com diagnóstico de leiomiosarcoma do intestino delgado e revisão da literatura. Apresentação do caso: Descreve-se o caso clínico de uma doente de 53 anos de idade, enviada à nossa consulta, por massa abdominal diagnosticada 2 semanas antes. Realizou TAC abdomino-pélvica nesse mesmo dia, que confirmou existência de “massa infra-mesocólica, com 12 cm de maior diâmetro, sólida e homogénea com áreas de necrose, parecendo envolver ansas do intestino delgado”. Foi submetida a laparotomia exploradora no dia 25/10/06, tendo-se confirmado a existência de massa abdominal, envolvendo uma ansa jejunal, cujo exame histopatológico revelou tratar-se de um leiomiosarcoma do intestino delgado. Conclusão: Os leiomiosarcomas do intestino delgado são raros, constituindo uma hipótese diagnóstica muito pouco frequente. A cirurgia mantém-se a única terapêutica curativa para esta patologia. Deve ser mantida uma vigilância apertada, visto apresentar um comportamento biológico imprevisível mas potencialmente muito agressivo, com metastização frequente.
