    Objective: In this study serum tumor necrosis factor alpha (TNF-alpha), transforming growth factor beta (TGF-beta), interleukin-10 (IL–10) and alanine transferase (ALT) levels were measured in naive chronic hepatitis C patients and healthy control group. The relationship between the values and the histological activity of the liver was evaluated to make predictions about liver histology without the need for biopsy. Methods: A total of 86 individuals including 43 patients and 43 controls participated in the study. Serum AST, ALT, TNF-alpha, TGF-beta and IL–10 values were analyzed in patient and control groups. The correlation between liver fibrosis degree (stage) and histological activity index (HAI) scores and the IL–10, TNF-alpha, IL-10, TGF-BETA values of the case and control group was evaluated. Results: A very strong correlation was found between HAI and TNF-alpha (rho=0965). A significant relation was found between HAI and TGF-beta (rho=0.446) and statistical correlation was not found between HAI and IL-10. A significant relation was found between HAI and serum ALT values in the patient group. Conclusion: Liver biopsy is an invasive procedure with complications which may have severe results. There is a need for biochemical or radiological markers which extrapolate to liver biopsy. In our study, measurement of serum cytokine and ALT values was observed as a noninvasive method which can help to meet this need. © 2023, DOC Design and Informatics Co. Ltd.. All rights reserved

    The role of social support on the relationships between internet use and sleep problems in adolescents during COVID-19 pandemic: a multicentre study

    BackgroundThis study examines the frequency of problematic internet use and sleep problems in adolescents aged 14-18 years during the COVID-19 pandemic and identifies the impact of factors such as sociodemographic characteristics, internet habits, changes in daily life, and perceived social support on these problems. MethodsThis multicentre study was a questionnaire-based online survey study. The questionnaire included the Young Internet Addiction Scale, the Pittsburgh Sleep Quality Index, and the Multidimensional Scale of Perceived Social Support, as well as questions about demographic information, internet habits, and changes in daily life during pandemic. Several multivariate Backward logistic regression models were run to determine the variables that predicted problematic internet use and poor sleep quality. ResultsIt was determined that the frequency of problematic internet use was 15.5%, and the frequency of poor sleep quality was 47.8%. Poor sleep quality was found 2.5 times higher in problematic internet users. The perceived social support was found insufficient in adolescents with problematic internet use and poor sleep quality. Various factors such as the excessive use of internet and social media, low school success, lack of physical activity, lack of rules for internet use at home, and worsening of relationships with parents were found to be predictive factors for these problems. ConclusionsProblematic internet use during the pandemic is associated with worsening sleep quality in adolescents. It is important to create special interventions for problematic internet use and sleep problems that develop in adolescents as a result of restrictions during the pandemic

    Perceived Need for Mental Health Services Among Healthcare Workers During the Coronavirus Disease-19 Pandemic in Turkey: A Multicenter Cross-sectional Study

    Background: The coronavirus disease-2019 pandemic has contributed to work-related psychosocial risks in healthcare workers.Aims: To evaluate the perceived need for mental health services and related factors in Turkish healthcare workers practicing in pandemic hospitals.Study Design: Cross-sectional study.Methods: Data were collected from face-to-face interviews with healthcare workers at 19 pandemic hospitals in 13 provinces between September and November 2021. The study survey included the evaluation of the perceived need for and utilization of mental health services in the previous year, as well as sociodemographic, health-related, and work-related characteristics, the General Health Questionnaire-12, the World Health Organization Quality of Life-BREF (WHOQoL-BREF) questionnaire, and the Fear of coronavirus disease-2019 scale (FCV-19S).Results: Of 1,556 participants, 522 (33.5%) reported a perceived need for mental health services, but only 133 (8.5%) reported receiving these services. Multiple logistic regression analysis of the perceived need for mental health services revealed significant relationships with lower age, female sex, being a current smoker, having a chronic disease, having a mental disorder, coronavirus disease-2019 contact within the last three months in settings other than the home or workplace, a positive coronavirus disease-2019 vaccination history, being a physician, being a non-physician healthcare professional, and coronavirus disease-2019 contact within the last three months at work. After adjustment for these characteristics, higher General Health Questionnaire-12 and FCV-19S scores and lower WHOQoL-BREF domain scores were related to the perceived need for mental health services in logistic regression analyses.Conclusion: The findings indicate a substantial need for mental health services amongst Turkish healthcare workers during the pandemic and outline participants' characteristics regarding high-priority groups for the intervention. Future research may focus on developing actions and evaluating their efficiency

    Effect of Intensity of Home Noninvasive Ventilation in Individuals With Neuromuscular and Chest Wall Disorders: A Systematic Review and Meta-Analysis of Individual Participant Data

    Introduction: Home noninvasive ventilation (NIV), targeting a reduction of carbon dioxide with a combination of sufficient inspiratory support and backup-rate improves outcomes in patients with chronic obstructive pulmonary disease. The aim of this systematic review with individual participant data (IPD) meta-analysis was to evaluate the effects of intensity of home NIV on respiratory outcomes in individuals with slowly progressive neuromuscular (NMD) or chest-wall disorders (CWD).Methods: Controlled, non-controlled and cohort studies indexed between January-2000 and December 2020 were sought from Medline, Embase and the Cochrane Central Register. Outcomes were diurnal PaCO2, PaO2, daily NIV usage, and interface type (PROSPERO-CRD 42021245121). NIV intensity was defined according to the Z-score of the product of pressure support (or tidal volume) and backup-rate.Results: 16 eligible studies were identified; we obtained IPD for 7 studies (176 participants: 113-NMD; 63-CWD). The reduction in PaCO2 was greater with higher baseline PaCO2. NIV intensity per se was not associated with improved PaCO2 except in individuals with CWD and the most severe baseline hypercapnia. Similar results were found for PaO2. Daily NIV usage was associated with improvement in gas exchange but not with NIV intensity. No association between NIV intensity and interface type was found.Conclusion: Following home NIV initiation in NMD or CWD patients, no relationship was observed between NIV intensity and PaCO2, except in individuals with the most severe CWD. The amount of daily NIV usage, rather than intensity, is key to improving hypoventilation in this population during the first few months after introduction of therapy.& COPY; 2023 SEPAR.     Evaluation of pro-atherogenic lipid profile and high atherohenic indexes in patients with Behcet's disease: A case-control study

    BackgroundBehcet's disease is a systemic auto-immune and auto-inflammatory chronic disease in which genetic and environmental factors play a role. Patients with Behcet's are at significant risk for developing many comorbidities, including cardiovascular diseases. AimsIt was aimed to investigate the relationship between serum lipid parameters and atherogenic indexes to evaluate the cardiovascular risk status in patients with Behcet's disease. Patients/MethodsThis study was designed as a single-center, retrospective case-control study. The study was conducted with 212 patients over 18 years of age, 106 in the case group and 106 in the control group. ResultsThere was a significant difference in lipid values between the patients with Behcet's disease and the control group. While the serum triglyceride, total cholesterol, low-density lipoprotein cholesterol (LDL-C), and non-high-density lipoprotein cholesterol (NHC) levels were significantly higher, the high-density lipoprotein cholesterol (HDL-C) level was low in patients with Behcet's disease. From atherogenic indexes, Atherogenic Index of Plasma (AIP) (0.03 +/- 0.27 vs. -0.07 +/- 0.23, p = 0.003), Castelli Risk Index I (CRI-I) (4.24 +/- 1.07 vs. 3.02 +/- 0.96, p < 0.001), Castelli Risk Index II (CRI-II) (2.65 +/- 0.81 vs. 1.84 +/- 0.59, p < 0.001) and Atherogenic Coefficient (AC) (3.24 +/- 1.07 vs. 2.02 +/- 0.96, p < 0.001) levels were significantly higher in patients with Behcet's disease. ConclusionOur study shows that patients with Behcet's have a higher pro-atherogenic lipid profile and atherogenic indexes at high risk. Patients with Behcet's have an increased risk of cardiovascular diseases associated with atherosclerosis

    Kanunen düzenlenme altına alınmamış olan hatır senedi ve bedelsizlik kavramları uygulama ve Yargıtay kararları ile ortaya çıkmışlardır. Taraflarca anlaşılarak bedelsiz kılınan ve alacaklı menfaatine düzenlenen senetler olarak tanımlanabilecek olan hatır senetlerinde bedelsizlik iddiası şahsi bir def’i olup, kural olarak yalnızca hatır ilişkisinin tarafları arasında ileri sürülebilmektedir. Uygulamada hatır senetlerinde bedelsizlik iddiası çoğunlukla hatır borçlusu tarafından açılan bedelsizliğe dayalı bir menfi tespit davası ile gündeme gelmektedir. Bununla birlikte uygulamada ve Yargıtay tarafından verilen bazı kararlarda hatır senetlerinde bedelsizlik iddiasına ilişkin bir takım hukuki sorunların öne çıktığı görülmektedir. Bu kapsamda özellikle hatır senetlerinde bedelsizlik iddiasına ilişkin olarak bile bile borçlu zararına hareket halinin tespiti ve malen kaydı bulunan hatır senetlerinde ispat yükünün kimde olduğu konularında öğretide farklı görüşler bulunmakla birlikte Yargıtay uygulamasında ise birbirinden farklı kararlara rastlanmaktadır

    The Use of New Hematological Markers in the Diagnosis of Alopecia Areata

    Introduction: Alopecia areata (AA) is a non-cicatricial inflammatory and autoimmune hair loss disease. In recent studies, it has been reported that hematological parameters can be used as oxidative stress markers in the diagnosis of many inflammatory diseases due to their low cost and widespread use.Objectives: In this study, it was aimed to reveal the significant cut-off points of hematological inflammatory markers in AA that can guide clinicians in clinical practice and determine how many times they increase the risk of disease.Methods: The present study is retrospective case-control type. Seventy patients with AA and seventy healthy controls were included in the study. The hematological parameters in both groups were evaluated retrospectively.Results: Hemoglobulin, monocyte, platelet, monocyte high-density lipoprotein cholesterol (HDL-C) ratio (MHR), monocyte lymphocyte ratio (MLR), platelet lymphocyte ratio (PLR) were high in patients with AA, while the number of lymphocytes was low. In ROC analysis, the optimal cut-off values for the diagnosis of AA were as follows: MLR 0.216, MHR 0.010, and PLR 111.715. In regression analysis, being above the following values of MLR 0.216, MHR 0.010, and PLR 111.715 increased the risk of developing AA by 6.3, 3.8, and 2.7 times, respectively.Conclusions: It was seen that MHR and PLR, especially MLR, can significantly increase the risk of developing the disease in AA and can also be used as diagnostic markers

    THE ROLE OF LONELINESS, COGNITIVE FLEXIBILITY, AND INTERPERSONAL PROBLEM-SOLVING IN PREDICTING DARK TRIAD PERSONALITY OF UNIVERSITY STUDENTS

    This study focuses on the variables that predict dark triad personality traits in university students. These variables are loneliness, cognitive flexibility, and interpersonal problem-solving. One hundred and sixty-six university students aged between 18-27 were recruited for this study. Data were collected using the dark triad scale, UCLA loneliness scale, cognitive flexibility scale, and interpersonal problem-solving inventory. Findings show that loneliness, cognitive flexibility and interpersonal problem solving together predict dark triad personality traits.The author observed that these variables explained 16% of the total variance in the machiavellian personality trait, 14% of the total variance in the narcissism personality trait, and 8% of the total variance in the psychopathy personality trait. More specifically; cognitive flexibility and lack of self-confidence predicted positively machiavellianism. On the other hand, constructive problem-solving predicts machiavellianism negatively and significantly.. Taking no responsibility and approaching the problem negatively predict narcissism positively and significantly. Finally, the psychopathy personality trait is only positively predicted by the self-confident approach variable. The findings were discussed in the light of the current study, and recommendations were made for future research

    Effects of Moderate Exercise Training on ApoE and ApoCIII in Metabolic Syndrome

    Objective: Metabolic syndrome (MetS) is an endocrinopathy with a combination of cardiovascular and metabolic compounds. In our study, it is expected to obtain results showing that mortality rate, loss of workforce, and treatment costs due to disorders caused by MetS can be reduced by physical exercise. The study analyses the effect of moderate exercise training on this Apolipoprotein E (ApoE), Apolipoprotein CIII (ApoCIII), adiponectin, resistin, interleukin-6 (IL-6), and tumor necrosis factor-alpha (TNF-a) which are thought to have a role in the deterioration of glucose and lipid metabolism in MetS. Methods: This clinical experimental study consists of 3 groups. The MetS+E (n=24) group, which included the participants who agreed to participate in the exercise program in addition to their medical treatment, the MetS (n=23) group who received medical treatment but did not exercise, and the Control+E (n=25) group, which included healthy volunteers who had the same protocol as MetS+E. ApoE, ApoCIII, adiponectin, resistin, IL-6, and TNF-a plasma levels of all participants were measured both at the beginning of the study and at the end of the protocol. Results: At the end of the study we reached the following findings; insulin and Homeostatic Model Assessment for Insulin Resistance (HOMA-IR) levels decreased in exercise groups (p=0,03). ApoCIII levels are increased all the groups after the study (p<0,01). IL-6 levels decreased in MetS+E (p<0,01) and Control+E (p=0,037). ApoE (p=0,01) and TNF-sigma (p=0,037) levels decreased only the Control+E group. Conclusion: Training showed metabolic, anti-inflammatory and physical improvements independent of ApoE and ApoCIII in those with MetS

    The role of Neuropsychological Assessment in the Differential Diagnoses of Late-Onset Depression, Dementia, and Mild Cognitive Impairment

    Objective: The present study aimed to compare the neuropsychological test parameters of dementia, depression, and MCI patients and determine the disease-specific test characteristics and their relationship with electroencephalography. Methods: Ninety-one patients who were admitted to the neurology outpatient clinic with forgetfulness complaints between October 2019 and March 2022 and whose neuropsychological tests were performed were included in the study. The files of these 91 patients were reviewed retrospectively and their sociodemographic data were recorded. Furthermore, the EEG results which were taken during the patients' evaluation period due to forgetfulness were evaluated. Neuropsychological tests were compared between patients with dementia, MCI and depression. It was also investigated whether there was a relationship between NPT test parameters and EEG in patients with EEG results. Results: The study was completed with 87 patients. Of these 87 patients, 54 were female and 33 were male. Twenty-four patients had depression, 16 MCI, and 47 dementia. All of the dementia patients had Alzheimer's type dementia. When dementia, depression, and MCI groups were compared, the age difference was statistically significant (P = 0.001). The mean age of the depression group was 66.5, the MCI group was 73.5, and the dementia group was 77. WMS-I, WMS-II, WMS-III, WMS-IV, similarities test, clock drawing test, trail making test, shape copying, language, and mood evaluation tests were statistically significantly different between the groups. There was no statistically significant difference between the groups regarding gender, education level, dominant hand, and occupation. EEG background activity frequencies were also examined between the groups, and there was no statistically significant difference. Conclusion: In conclusion, when evaluating patients who present with the complaint of forgetfulness, a detailed neuropsychological evaluation must be performed in addition to other diagnostic tests. Sensitive tests should be included to confirm the diagnosis, especially in cases where being in between for the differential diagnosis. Thus, further studies are needed on this subject
