45 p.ill.,Le modèle mathématique de projection des effectifs médicaux utilisé par la Commission de planification de l’offre médicale a été analysé par le Centre fédéral d’Expertise des Soins de Santé (KCE) et soumis à une évaluation critique par les différentes parties prenantes (stakeholders). Il en ressort que le modèle est techniquement assez performant – un des meilleurs d’Europe – mais qu’il tirerait avantage à être mieux articulé avec les évolutions sociétales et les orientations politiques.1. Lombalgie et douleur radiculaire : Eléments-clés d’un itinéraire de soins – Synthèse

Face à la grande variété de solutions proposées à ceux et celles qui souffrent de ce « mal du siècle », le KCE avait déjà publié en mai dernier un Guide de pratique clinique basé sur les plus récentes connaissances scientifiques. En guise de prolongation, voici un itinéraire de soins qui définit, pour un patient donné et en fonction du type et de la durée de sa douleur, les examens à effectuer et les traitements à proposer pour progresser de la façon la plus cohérente possible. C’est la première fois qu’un itinéraire de soins est réalisé en commun avec des représentants de toutes les professions de soins concernées et avec les patients. Il englobe à la fois les douleurs lombaires (mal de dos simple) et radiculaires (de type sciatique). Vers un système intégré de prise en charge des traumatismes graves : Synthèse
38 p.ill.,On observe actuellement une tendance internationale à réorganiser les soins aux blessés graves selon un modèle dit de « système intégré ». Dans de tels modèles, les blessés graves sont centralisés dans quelques hôpitaux stratégiquement répartis sur l’ensemble du territoire. Ces « Centres majeurs de traumatologie » répondent à des exigences extrêmement élevées en termes d’équipement, de qualification du personnel et de disponibilité (24h/24 et 7j/7).
Le Centre fédéral d’Expertise des Soins de Santé (KCE) a étudié ces exemples étrangers et analysé de quelle manière ils pourraient être transposés à la réalité belge. Il en ressort que notre système présente déjà beaucoup d’atouts, mais qu’il reste quelques lacunes, surtout au niveau de la coordination des différents acteurs. Comment accroître le recours à la chirurgie de jour en Belgique ? : Synthèse
28 p.il.,Les progrès médicaux permettent de faire de plus en plus appel à la chirurgie de jour et la Ministre de la Santé Maggie De Block souhaite en tenir compte dans le cadre de la réforme des hôpitaux. Le Centre fédéral d’Expertise des Soins de Santé (KCE) a dressé un état des lieux de cette modalité de soins en Belgique et analysé les conditions qui favoriseraient un glissement de l’hospitalisation ‘classique’ vers la chirurgie de jour. Aujourd’hui déjà, chez nous comme dans les autres pays, la plupart des personnes qui se font opérer de la cataracte peuvent rentrer chez elles le soir-même de l’intervention. Par contre, pour d’autres opérations comme l’ablation de la vésicule biliaire, il y a encore une large marge de progression possible. On observe également, pour beaucoup d’interventions, de très grandes variations entre hôpitaux. Le mode de financement actuel, extrêmement complexe, est assurément le principal obstacle au recours à ce type de chirurgie.
