    Endoscopic pilonidal sinus treatment (EPSIT) versus sinus laser therapy (SiLaT) for sacrococcygeal pilonidal sinus

    Introduction: New surgical treatment methods are being investigated in sacrococcygeal pilonidal sinus disease. Minimal invasive methods such as endoscopic pilonidal sinus treatment (EPSIT) and sinus laser therapy (SiLaT) have promising results and advantages of laparoscopic surgery. Aim: To compare the efficiency and safety of EPSIT with SiLaT. Material and methods: Seventy-three patients with pilonidal sinus who had undergone EPSIT or SiLaT within 2 years were evaluated retrospectively. Data of patients’ demographics, complications and postoperative course were collected and compared between the two groups. Results: We analyzed seventy-three patients who underwent EPSIT or SiLaT, with a mean age of 23.4 ±8.4 (12–46) years and body mass index (BMI) of 25.5 ±4.5 (18–38) kg/m2. Among them, 36 patients (26 males, ten females) underwent EPSIT and 37 patients (27 males, ten females) underwent SiLaT. Mean operative time was similar for both groups (32.3 ±14.8 vs. 31.0 ±14.8; p = 0.757). Early complications (minimal bleeding) were recorded in 2 patients in the SiLaT group. The duration of analgesic use was significantly lower in the EPSIT group compared to the SiLaT group (1.3 ±0.5 (1–3) vs. 1.9 ±1.1 (1–5); p = 0.005). The mean postoperative time of total wound healing was similar for both groups: 23.6 ±14.7 (12–90) vs. 25.2 ±14.5 (14–90) days (p = 0.385). There was no significant difference in the average time of return to total daily activity (3.4 ±0.9 (2–5) vs. 3.6 ±1.2 (2–7) days, p = 0.679). There were no significant differences between the groups regarding late postoperative complications (recurrence: 7). Conclusions: Both methods have similar early and late complications. The duration of need for analgesic usage was shorter in EPSIT patients. © 2023 Termedia Publishing House Ltd.. All rights reserved

    Quantifying the effect of amplitude scaling of real ground motions based on structural responses of vertically irregular and regular RC frames

    Increasing number of real ground motion (GM) record databases raised nonlinear dynamic analysis (NDA) as an attractive option to determine structural response statistics. Structural responses are sensitive to input motions and the appropriate selection and scaling of real GMs is one of the crucial topics in earthquake engineering. In this paper, the influence of amplitude scaling on important earthquake demand parameters (EDPs), namely, global drift ratio, inter-story drift ratio and maximum floor acceleration, were studied using different scaling approaches and scaling limits. Eurocode-8 was used as a reference seismic code and amplitude scaling effects on structural responses of vertically irregular and regular structures were quantified. Efficiency and sufficiency of amplitude scaling were assessed in terms of mean, dispersion and non-exceedance probability curves of the EDPs. Statistical distribution of GM characteristics and their dependence on GM amplitude scaling were also discussed. Evaluations have shown that similar mean responses can be obtained regardless of scaling limits, approaches, and building topology if spectral shape compatibility is ensured. Furthermore, results demonstrated that neither building regularity nor scaling of GMs influenced the statistical distribution of ground motion parameters and non-exceedance probability curves of the EDPs. In fact, it was revealed that record selection scenario including spectral compatibility of individual GMs had a dramatic impact on dispersion and exceedance probability of structural responses

    İnsan p60-katanin Proteininin, Bitki RIC1 ve İnsan Homoloğu Kiaa1432 Proteinleri ile Etkileşiminin Tespiti & Etkileşimin Mikrotubuller Üzerinde Fonksiyonel Analizi

    Mikrotubuller ihtiyaç durumuna göre uzayıp–kısalabilen, yeniden organize olabilen dinamik yapılı polimerlerdir. Mikrotubul organizasyonu, bölünen hücrelerde mitoz, post-mitotik nöronlarda akson/dendrit yapılarının oluşumunda görevlidir. Mikrotubul organizasyonuna imkan sağlayan mekanizmalardan birisi Katanin gibi mikrotubul kesici proteinlerce kesilmeleridir. Katanin, KATNA1 geni tarafından kodlanan ve enzimatik aktiviteye sahip p60, onun fonksiyonunu düzenleyen ve KATNB1 geni tarafından kodlanan p80 alt birimlerinde oluşmaktadır. Katanin’in bilinen regülatörleri, etkileşim partnerleri yok denecek kadar azdır. Arabidopsis thaliana bitkisinde yapılan çalışmalarda p60-katanin’in bir GTPase olan RIC1 proteini ile etkileştiği ve p60-katanin’in enzimatik aktivitesini arttırdığı gösterilmiştir. Bu sonuçtan hareketle, önerilen proje ile insan p60-katanin proteininin, bitki RIC1 ve insan homoloğu Kiaa1432 proteinleri ile fiziksel etkileşiminin tespiti ve etkileşimin mikrotubul kesimi bakımından fonksiyonel açıdan aydınlatılması amaçlanmıştır. Proteinlerin etkileşim bakımından incelenebilmesi adına ilk olarak bu proteinleri kodlayan cDNA’lar FLAG ve His eki içeren plazmidlere ayrı ayrı klonlanmışlardır. Arabidopsis thaliana p60-katanin ve RIC1 proteinleri Prof. Ying Fu tarafından temin edilmiş olup sadece ilgili plazmidlere aktarılmışlardır. İnsan p60-katanin yürürlükte olan 114Z971 projesi kapsamında FLAG ekli olarak elde edilmiş olup insan Kiaa1432 proteinini kodlayan cDNA klonlanmıştır. Klonlamayı takiben proteinler fiziksel etkileşim bakımından ko-immün çöktürme yöntemiyle test edilmiştir. Son olarak, His ve FLAG ekli olarak klonlanan proteinlerin cDNA’larını içeren plazmidler farklı kombinasyonlar halinde hücreye verilmek suretiyle olası etkileşimin mikrotubul ağına etkisi araştırılmıştır. Yapılan analizlerin sonucunda insan p60-katanin’in Arabidopsis thaliana RIC1 ve bu proteinin insan homoloğu Kiaa1432 proteinleri ile fiziksel etkileşimine rastlanamamıştır

    Iterative histogram equalization using discrete wavelet transform in low-dynamic range

    A progressive histogram equalization (HE) and contrast enhancement method is proposed in the frequency domain. This method has an iterative structure based on the 6-sigma rule in the low-dynamic range using discrete wavelet transform. The proposed method determines an automatic threshold for attenuating the high-frequency amplitude of a signal in the discrete wavelet domain, based on the standard deviation of the absolute power of the high-frequency components in the signal band generated from the probability density function (PDF). Then an iterative quantization procedure is used where the PDF values in the frequency domain are randomly quantized. Besides, the maximum Bhattacharyya coefficient, which matches the original input image's PDF, is used as a cost function to optimize the histogram smoothing output. For the random number generation, a 32-bit pseudorandom generator is employed to produce the same result for the output image at each runtime. The quantization factor parameter enables a controlled contrast enhancement rate to receive balanced equalization results in images. In the experimental studies, the proposed iterative HE method is compared with the well-known global, local, and brightness preserving algorithms. Experimental studies quantitatively and qualitatively display promising and encouraging results in terms of various state-of-the-art quality assessment metrics such as mean squared error, peak signal-to-noise ratio, structural similarity index measurement, contrast improvement index, absolute mean brightness error, and quality-aware relative contrast measure. © 2023 SPIE and IS&T

    The Factors That Affect the Fear of COVID-19 in Students of the Physiotherapy and Rehabilitation Departments: A Descriptive Study

    ABS TRACT Objective: To determine the level of fear of coronavirus disease-2019 (COVID-19) and the factors that affect the fear of COVID-19 in students of the physiotherapy and rehabilitation departments. Material and Methods: The study involved 253 volunteer students from 4 different universities in Türkiye. Utilizing an online data collection technology, the participant information form and the Fear of COVID-19 Scale were used to collect the study’s data. Results: Female gender (p=0.06), not working in any job (p=0.76), income source only (p=0.92), smoking (p=0.26), and not using alcohol (p=0.88), history of chronic disease (p=0.32), no previous COVID-19 infection (p=0.96), 12 weeks after COVID-19 infection (p=0.17), 3 doses of vaccination (p=0.63), using public transportation (p=0.49) were factors that increased the level of fear in physiotherapy and rehabilitation department students. In addition, the fear levels of the students who had a family history of chronic disease (p=0.007) and had the thought of freezing the school (p=0.001) were significantly higher than the others. On the other hand, there was no significant difference between the fear of COVID-19 and terms of living environment, number of theoretical lessons, number of practical lessons and class size (p>0.05). Conclusion: Physiotherapy and rehabilitation department students have low-to-moderate fear of COVID-19. Those with a chronic disease in their family have a high fear of COVID-19, and those with a high level of fear consider freezing the school. The number of practical and theoretical lessons and class size have no effect on the level of fear

    Sanal gerçeklik teknolojileri dijital dönüşümün neticesinde giderek kendini geliştirmekte ve birçok alanın içerisine dahil olabilmektedir. Bu teknolojinin gazeteciliğin içine girmesinin sonucunda da farklı anlatı türleri ortaya çıkmaktadır. Sarmal (immersive) gazetecilik, sözü edilen anlatı türlerinden biri olarak kendini göstermektedir. Immersive kelimesi sarmal, sarmalayan, üç boyutlu, sürükleyici gibi farklı anlamlarda kullanılmakta ancak gazetecilik ile bir arada değerlendirildiğinde tam olarak Türkçe karşılığı bulunmamaktadır. Çalışmada bu sebeple yaygın olarak tercih edilen sarmal gazetecilik terimi kullanılmıştır. Sanal gerçeklik teknolojilerinin sunduğu imkânlar doğrultusunda gerçekleştirilen bu gazetecilik türü günümüzde ağırlıklı olarak 360° ile izlemeye imkân tanıyan görüntülerden oluşmaktadır. Bu kapsamda çalışmada 360° görüntülerle haberlerini oluşturan Euronews ve The New York Times incelenerek her iki haber mecrasının YouTube üzerinden yükledikleri haber videoları içerik analizi yöntemiyle değerlendirilmiştir. Çalışmanın sonucunda 360° haber videolarının ağırlıklı olarak kültür-sanat ve seyahat konularında verildiği, mecraların 360° haber videoları yükleme sıklıklarının yıllar içerisinde farklılaştığı ve her iki mecranın da haberlerini benzer sürelerde sundukları saptanmıştır

    Sensitivity of Listeria innocua to high hydrostatic pressure at low temperature in Ringer's solution and milk

    The research was performed to investigate the effect of high hydrostatic pressure low temperature (HHPLT) on Listeria innocua in Ringer's solution (RS) and raw milk (RM). The physicochemical properties of RM were studied at 300 MPa and 10 degrees C. Compared with the control, protein and lipase activities were reduced at HHPLT in RM. The L -values (luminance) of LHPLT applied RM were significantly (P < 0.05) decreased for colour Pressure application at 300 MPa for 90 min totally inactivated L. innocua at -20 degrees C in both RS and RM. L. innocua showed resistance to low pressure (200 MPa) in RM. Overall, increasing the pressure with applications at -20 and -30 degrees C resulted in a higher (P < 0.05) inactivation of L. innocua than at 10 and 20 degrees C. HHPLT applications to frozen RM resulted in the retention of textural characteristics

    Pharmacist and child communication: A phenomenological multidisciplinary study from the perspectives of undergraduate students in pharmacy and child development

    Background: The present study is an interdisciplinary study about pharmacist-child communication exploring the perceptions and observations of students studying in two different but intersecting fields, which are pharmacy and child development. Objective: The objective of the study is to illustrate the perceptions and observations of undergraduate pharmacy and child development students about pharmacist-child communication. Method: The study is a phenomenological study and the phenomenon analyzed is “pharmacist-child communication”. Research study group was selected via criterion sampling method. The sample group consisted of 40 undergraduate pharmacy and child development students. “Demographic Information Form” was used as the data collection tool and “Focus Group Interview Guide” was prepared for focus group interview meetings. Ten open-ended questions aligned with the research objective were asked to the students in the focus group interview. The collected data were analyzed by descriptive analysis method and the experiences of these two different groups of students were explored. Results: At the end of the study, two main themes and five sub-themes were obtained. These themes and the sub-themes are as follows: adherence to drug therapy (Sub-themes: communication strategies relevant to the cognitive development at various ages of the child, rewarding children and reinforcement of good behavior, role of the parent in pharmacist-child communication) and physical characteristics of the pharmacy/pharmacist (Sub-themes: physical characteristics of the pharmacy, physical caharacteristics of the pharmacist). Conclusions: Each theme was illustrated in the study with comments of the students. The results showed that the observation and perceptions of the students studying in two different fields agreed with each other and those of other researchers. It is proposed that projects and practices can be developed by these two different disciplines, pharmacy and child development are two intersecting fields. As they complement each other, they could strengthen the pharmacist-child communication and as a result support the child's adherence to therapy. © 2023 The Author

    Immune Response and Therapeutic Vaccination against Helicobacter pylori

    Helicobacter pylori (H. pylori) is a gram-negative bacterium, considered one of the significant discoveries about 40 years ago, and was isolated and cultured from the human stomach. H. pylori has infected more than half of the human population, making it one of the most well-known human pathogens. The front line of immune response starts with innate recognition of H. pylori and its mediators and intracellular signaling by gastric epithelial cells in which they recognize and respond to bacterial products such as flagella, lipopolysaccharides, and peptidoglycan. The inflammatory response is followed by the recruitment of various cells of the innate and adaptive immune system. Cytokines including IL-12, IL-23, and TGF-& beta; direct the polarization of CD4+ T helper cells to Th1, Th17, and Treg, respectively. The clinical symptoms that may occur as a result of H. pylori infection linked to the virulence factors of the bacteria, the genetic factors of the host, and the immune responses. Specific antigens have been found as part of these crucial virulence factors. The specific antigens may play a role in the development of an effective vaccine to eradicate H. pylori infection. Innate and adaptive immunity and genetic factors have an important place in understanding the host response mechanisms, elucidating the pathogenesis of the disease, and developing new targetedtherapy approaches.     Foreign body aspirations in childhood: A retrospective review

    Objective: The aim of this study was to examine the childhood-age foreign body aspiration (FBA) retrospectively by focusing on symptoms, types, home interventions, hospitalization and complications. Method: All patients between 0–18 years admitted to pediatric surgical clinic and diagnosed with FBA were examined retrospectively from January 2021 to January 2022. Results: The study group consisted of 163 patients and their mean age was 17.8±12.7 months (1 months to 6 years). FBA events included aspiration of food (78%), coins (10%), batteries (3.7%), parts of toys (4%), buttons (2.4%) and other (2%). First aid treatment to children was performed at home by mothers (61%). Types of first aid treatment performed by mothers included tapping the back (31%); cleaning inside the mouth (24%); trying to remove the foreign body with fingers (12%), pushing the foreign body forward (5.9%) and forcing the child to vomit (9.5%) respectively. Conclusions: FBA is a potential life-threatening health problem during childhood. When the age of majority of patients being under one year old and prevalence of food in FBA types are taken into consideration, safe-feeding practices of mothers, feeding position and training about the safe-environment are the basic steps of prevention strategies. Practice implications: One of the most useful ways of preventing FBA cases is to provide planned and continuing education to parents, care givers and all the individuals responsible for the care of the child in order to increase their knowledge and practice levels. © 2023 Elsevier Inc
