    Early-treatment cerebral blood flow change as a predictive biomarker of antidepressant treatment response: evidence from the EMBARC clinical trial

    Background Major depressive disorder (MDD) is one of the most prevalent and disabling illnesses worldwide. Treatment of MDD typically relies on trial-and-error to find an effective approach. Identifying early response-related biomarkers that predict response to antidepressants would help clinicians to decide, as early as possible, whether a particular treatment might be suitable for a given patient.Methods Data were from the two-stage Establishing Moderators and Biosignatures of Antidepressant Response for Clinical Care (EMBARC) trial. A whole-brain, voxel-wise, mixed-effects model was applied to identify early-treatment cerebral blood flow (CBF) changes as biomarkers of treatment response. We examined changes in CBF measured with arterial spin labeling 1-week after initiating double-masked sertraline/placebo. We tested whether these early 1-week scans could be used to predict response observed after 8-weeks of treatment.Results Response to 8-week placebo treatment was associated with increased cerebral perfusion in temporal cortex and reduced cerebral perfusion in postcentral region captured at 1-week of treatment. Additionally, CBF response in these brain regions was significantly correlated with improvement in Hamilton Depression Rating Scale score in the placebo group. No significant associations were found for selective serotonin reuptake inhibitor treatment.Conclusions We conclude that early CBF responses to placebo administration in multiple brain regions represent candidate neural biomarkers of longer-term antidepressant effects.</p

    Transcranial electrical stimulation (tES) is a non-invasive neural modulation technique known for its high safety, patient compliance, and portability. It holds promise as a potential non-pharmacological method for analgesia. However, challenges persist in utilizing tES for pain management, including inconsistent research findings and limited understanding of its analgesic mechanisms. Therefore, by summarizing the advances in the analgesic researches employing the 3 primary tES techniques, transcranial direct current stimulation (tDCS), transcranial alternating current stimulation (tACS), and transcranial random noise stimulation (tRNS), we reviewed the analgesic effects on both acute and chronic pain, as well as the neural mechanisms underlying the analgesic effect of each technique. Accumulating evidence suggests that the analgesic effects of tDCS are significant, but studies on analgesic effects of tACS and tRNS remain limited. And the exact mechanisms of pain relief through tES turned out to be not yet well established. Furthermore, we systematically discussed the limitations of analgesia-related studies employing tES techniques across various aspects, involving research design, stimulation protocol formulation, neural response observation, analgesic effect assessment, and safety considerations. To address these limitations and advance clinical translation, we emphasized utilizing promising stimulation techniques and offered practical suggestions for future research endeavors. Specifically, employing numerical simulation of electric field guided by magnetic resonance imaging (MRI) would reduce variability of outcomes due to individual differences in head anatomy. For this purpose, it is advisable to establish standardized head models based on MRI data from the Chinese populations and validate simulated electric field results in tES research to diminish confounding factors concerning anatomy. Meanwhile, novel techniques like multi-site brain stimulation and interferential stimulation (IFS) could broaden the range of stimulation sites in both scope and depth. Multi-site brain stimulation facilitates modulation of entire neural networks, enabling more sophisticated investigations into the complexity of pain. IFS can reach deep brain tissues without invasive surgical procedures, achieving more comprehensive modulation. Regarding neural response observations, establishing a tES-neuroimaging synchronized platform would enable revealing its mechanisms and personalizing protocols based on inter-subject neural response variability detected through recordings. By integrating tES with various neuroimaging techniques, such as functional MRI, electroencephalography (EEG) and magnetoencephalography, into one unified platform, researchers could examine brain activities in baseline before stimulation, dynamic changes in brain activities during stimulation, and sustained brain responses after stimulation. Additionally, collecting finer-grained data on participant characteristics and pain intensity would enhance the sensitivity of future studies. In designing clinical trials to evaluate chronic pain treatments and reporting the results, adopting the six core outcome domain measures recommended by the Initiative on Methods, Measurement, and Pain Assessment in Clinical Trials (IMMPACT) could prove beneficial. Lastly, safety considerations can never be overemphasized in future tES studies especially when combining tES with MRI and EEG techniques. These efforts may help to broaden the research scope, reconcile inconsistencies in findings and elucidate the analgesic mechanisms of tES, thus facilitating the development of pragmatic pain management strategies such as combination therapies and home therapies. Ultimately, these suggestions will maximize the clinical application value of tES in pain treatment to achieve pain relief for patients.</p

    Exploring phenotypic overlap across schizotypy and autism spectrum conditions in American and Chinese young adults

    Competing theories have been proposed to explain the considerable overlap in social-cognitive features and risk factors across schizotypy and autism spectrum conditions (ASCs). Six previous factor analyses have been reported in the literature, yet all have major limitations; evidence for the clear superiority of any of the competing theories is insufficient and warrants further investigation. The primary aim of the present research was to identify dimensions that cut across schizotypy and ASCs while addressing limitations of past research. Data were collected from three independent samples (n = 1006, 544, and 2469) in the U.S. and China using the Autism -Spectrum Quotient, the Schizotypal Personality Questionnaire, and the Wisconsin Schizotypy Scales. Exploratory factor analyses in Sample 1 identified an interpretable three-factor structure, which was replicated in Samples 2 and 3 using confirmatory factor analyses. We found consistent evidence for three dimensions ( Aberrant Salience, Asociality, and Concrete Thinking ) underlying schizotypy and ASCs. This three-dimension model is consistent with a common vulnerability model of schizotypy and ASCs. Implications of these findings for the schizotypy and ASCs literature are discussed.</p

    Default mode network shows distinct emotional and contextual responses yet common effects of retrieval demands across tasks

    The default mode network (DMN) lies towards the heteromodal end of the principal gradient of intrinsic connectivity, maximally separated from the sensory-motor cortex. It supports memory-based cognition, including the capacity to retrieve conceptual and evaluative information from sensory inputs, and to generate meaningful states internally; however, the functional organisation of DMN that can support these distinct modes of retrieval remains unclear. We used fMRI to examine whether activation within subsystems of DMN differed as a function of retrieval demands, or the type of association to be retrieved, or both. In a picture association task, participants retrieved semantic associations that were either contextual or emotional in nature. Participants were asked to avoid generating episodic associations. In the generate phase, these associations were retrieved from a novel picture, while in the switch phase, participants retrieved a new association for the same image. Semantic context and emotion trials were associated with dissociable DMN subnetworks, indicating that a key dimension of DMN organisation relates to the type of association being accessed. The frontotemporal and medial temporal DMN showed a preference for emotional and semantic contextual associations, respectively. Relative to the generate phase, the switch phase recruited clusters closer to the heteromodal apex of the principal gradient-a cortical hierarchy separating unimodal and heteromodal regions. There were no differences in this effect between association types. Instead, memory switching was associated with a distinct subnetwork associated with controlled internal cognition. These findings delineate distinct patterns of DMN recruitment for different kinds of associations yet common responses across tasks that reflect retrieval demands. This study used fMRI to explore functional dissociations within the default mode network (DMN). Dissociable subnetworks were found to support the retrieval of contextual semantic and emotional associations, while task retrieval demands independently influenced the recruitment of heteromodal regions on the principal gradient of cortical organisation. image</p

    Competition Between Parts and Whole: A New Approach to Chinese Compound Word Processing

    How compound words are processed remains a central question in research on Chinese reading. The Chinese reading model assumes that all possible words sharing characters are activated during word processing and these activated words compete for a winner (Li &amp; Pollatsek, 2020). The present studies aimed to examine whether embedded component words compete with whole compound words in Chinese reading. In Study 1, we analyzed two existing lexical decision databases and revealed inhibitory effects of component-word frequency and facilitative effects of character frequency on the first components. In Study 2, we conducted two factorial experiments to further examine the effects of first component-word frequency, with character frequencies controlled. The results consistently indicated significant inhibitory effects of component-word frequency. Collectively, these findings support the theoretical proposition that both component words and compound words are activated and engage in competition during word processing. This provides a new approach to compound word processing in Chinese reading and a possible solution to mixed results of character frequency effects reported in the literature.</p

    Childhood abuse and craving in methamphetamine-dependent individuals: the mediating role of alexithymia

    Individuals with a history of childhood abuse (CA, including neglect and abuse by caregivers before the age of 18 years) have more severe substance dependence problems than those without a history of childhood abuse. However, whether a history of CA exacerbates craving and the mechanism of this effect remain largely unknown. The aim of this study was to explore the role of alexithymia in the effects of CA on craving in a large sample of methamphetamine-dependent individuals based on latent vulnerability theory. A total of 324 methamphetamine-dependent individuals who met DSM-5 criteria for substance use disorder were recruited. CA, alexithymia, and craving data were collected from the Childhood Trauma Questionnaire, the Toronto Alexithymia Scale-20, and the Obsessive Compulsive Drug Use Scale, respectively. t tests and ANCOVA were conducted to compare variables between the CA and non-CA groups, while partial correlation and mediation analyses were conducted to examine the potential mediating role of alexithymia in the relationship between CA and craving. Abused methamphetamine-dependent individuals reported higher levels of craving and higher levels of alexithymia than those of non-abused methamphetamine-dependent individuals. Alexithymia partially mediated the link between CA and craving, especially the effect of CA on craving frequency was fully mediated by alexithymia. Our findings reveal that a history of childhood abuse has a lasting effect on craving in stimulant-dependent individuals, and alexithymia contributes to some extent to the severity of substance abuse problems in abused methamphetamine-dependent individuals.</p

    The relationship between mindfulness and suboptimal health status: a chain/serial mediation model

    Background: Suboptimal health status (SHS) represents a third state between health and disease and often progresses into chronic conditions, negatively impacting an individual&#39;s well-being. Studies have shown that mindfulness has a beneficial effect on various SHS symptoms. This study aims to explore the influence of mindfulness on SHS and its underlying mechanisms, with a particular focus on examining the mediating roles of stress and social support. Methods: A total of 173 healthy Chinese college or graduate students, with an average age of 21.85 years, participated in this study. Measurements were taken using the Five Factor Mindfulness Questionnaire, the Sub-Health Measurement Scale, the Perceived Stress Scale, and a self-constructed scale that included demographic information. The PROCESS plugin for SPSS was used to assess mediating effects. Results: Significant correlations were found among SHS, social support, mindfulness, and perceived stress (|r| = 0.38-0.85, p &lt; 0.01). Specifically, mindfulness showed a significant positive correlation with SHS and social support (r = 0.38-0.77), while perceived stress was significantly negatively correlated with mindfulness, social support, and SHS (|r| = 0.45-0.85). Perceived social support was positively associated with SHS (r = 0.65). Furthermore, social support and perceived stress partially mediated the influence of mindfulness on SHS. Additionally, a sequential mediation effect of perceived social support and stress in the relationship between mindfulness and SHS was supported. Conclusion: The cultivation of trait mindfulness may be advantageous for individuals&#39; sub-health. Perceived social support and perceived stress are important underlying mechanisms contributing to this effect.</p

    Associations between meaning in life and suicidal ideation in young people: A systematic review and meta-analysis

    Suicidal ideation is a strong predictor of suicide among young people and is an outcome of interactions between protective and risk factors. Previous studies have focused primarily on risk factors while there is little evidence on important protective factors such as meaning in life. We conducted a systematic review and meta-analysis to examine the association between meaning in life and suicidal ideation among young people, and to explore potential moderating effects including demographics (gender and age) and social factors (economy and culture). We searched EBSCO, Embase, PsycINFO, PubMed, Scopus and Web of Science for relevant studies that reported correlations between meaning in life and suicidal ideation from their inception to October 2022. Studies were included in this review if they were empirical studies, written in English and sampled general young people aged 10-24. We employed a random-effects model meta-analysis to estimate the effect size. We identified 3168 references after removing duplicates and 11 studies (of 18 samples) were included in the review and meta-analysis. The meta-analysis revealed a moderate negative correlation between meaning in life and suicidal ideation (r = -0.51, 95%CI [-0.57, -0.45]) in young people. In addition, the correlation between meaning in life and suicidal ideation was stronger in high-income (r = -0.51, 95%CI [-0.54, - 0.48]) and individualist countries (r = -0.51, 95%CI [-0.53, - 0.48]) compared with upper-middle-income (r = -0.39, 95%CI [-0.41, -0.37]) and collectivist countries (r = -0.40, 95%CI [-0.41, - 0.38]), respectively, but comparable across gender and age. We identified strong negative associations between meaning in life and suicide ideation in high-income and individualist countries. This highlights that approaches targeting enhancing meaning in life should be considered in suicide prevention and intervention, especially in high-income and individualist countries.</p

    The role of self-related information in the sense of agency

    Sense of agency (SoA) refers to the subjective experience of controlling one&#39;s actions and their subsequent consequences. The present study endeavors to investigate the impact of how different degrees of self -related stimuli as action outcomes on the sense of agency by observing the temporal binding effect. Results showed that self -related sound significantly altered temporal binding, notably influencing outcome binding. A post -hoc explanation model effectively elucidated the role of self -related information in the formation of the sense of agency.</p

    Association between triglyceride glucose index (TyG) and psychotic symptoms in patients with first-episode drug-na?ve major depressive disorder

    Objective Major depressive disorder (MDD) sufferers frequently have psychotic symptoms, yet the underlying triggers remain elusive. Prior research suggests a link between insulin resistance (IR) and increased occurrence of psychotic symptoms. Hence, this study sought to investigate the potential association between psychotic symptoms in Chinese patients experiencing their first-episode drug-naive (FEDN) MDD and the triglyceride glucose (TyG) index, an alternative measure of insulin resistance (IR).Methods Between September 2016 and December 2018, 1,718 FEDN MDD patients with an average age of 34.9 +/- 12.4 years were recruited for this cross-sectional study at the First Hospital of Shanxi Medical University in China. The study collected clinical and demographic data and included assessments of anxiety, depression, and psychotic symptoms using the 14-item Hamilton Anxiety Rating Scale (HAMA), the 17-item Hamilton Depression Rating Scale (HAMD-17), and the positive subscales of the Positive and Negative Syndrome Scale (PANSS), respectively. Measurements of metabolic parameters, fasting blood glucose (FBG), and thyroid hormones were also gathered. To assess the correlation between the TyG index and the likelihood of psychotic symptoms, the study used multivariable binary logistic regression analysis. Additionally, two-segmented linear regression models were employed to investigate possible threshold effects in case non-linearity relationships were identified.Results Among the patients, 9.95% (171 out of 1,718) exhibited psychotic symptoms. Multivariable logistic regression analysis showed a positive correlation between the TyG index and the likelihood of psychotic symptoms (OR = 2.12, 95% CI: 1.21-3.74, P = 0.01) after adjusting for confounding variables. Moreover, smoothed plots revealed a nonlinear relationship with the TyG index, revealing an inflection point at 8.42. Interestingly, no significant link was observed to the left of the inflection point (OR = 0.50, 95% CI: 0.04-6.64, P = 0.60), whereas beyond this point, a positive correlation emerged between the TyG index and psychotic symptoms (OR = 2.42, 95% CI: 1.31-4.48, P = 0.01). Particularly, a considerable 142% rise in the probability of experiencing psychotic symptoms was found with each incremental elevation in the TyG index.Conclusions Understanding the non-linear link between the TyG index and the risk of psychotic symptoms in Chinese patients with FEDN MDD highlights the potential for targeted therapeutic approaches. By acknowledging the threshold effect observed, there is an opportunity to mitigate risk factors associated with IR-related psychiatric comorbidities through tailored interventions. These preliminary results stress the need for further longitudinal research to solidify these insights and contribute to more effective therapeutic strategies.</p
