Role of Panchakarma in the Management of Osteonecrosis of Femoral Head
Osteonecrosis refers to skeletal infarction, occurs when the blood supply to a segment of bone is affected. Apart from trauma, it is usually associated with excessive alcohol intake, cigarette smoking, and injudicious use of steroid therapy. In Ayurveda a correlation can be made with Asthi-majja gata vata on the basis of clinical presentation. Treatment modalities offered for Asthi-majja gata vata according to Ayurvedic classic are internal and external application of Sneha which comes under Brimhana karma of Shadvidh upkarama. Aim: The aim is to consider the Panchakarma therapy as first line of treatment for the osteonecrosis of femoral head. Case presentation: In this study, a 44yrs old chain smoker and chronic alcoholic male patient of osteonecrosis of femoral head is treated by Brimhana Chikitsa in the form of Sarvang abhyanga, Shashti shali pinda swedana, Tikta ksheerasarpi basti as vitiated Vata dosha residing in Asthi & Majja dhatu. Conclusion: After the administration of Tiktaksheer sarpi basti in Karma basti regimen along with Shastika shali pinda swedana remarked improvement as well as amplification in quality of life was noticed
A Comparative Clinical Study of Shodhan Uttar Shaman and Shaman Chikitsa in the Management of Amavata with special reference to Rheumatoid Arthritis
The disease Amavata maybe correlated with Rheumatoid Arthritis due to its similar clinical feature. The symptoms of Amavata are Sandhi Shula (pain in joint), Sandhi Sotha (swelling of joints), Sparsahata (tenderness around joint), Angamarda (body ache), Aruchi (anorexia), Trisna (thirst), Gaurava (heaviness) Jvara (fever), Apaka (swelling), Sunnata Anga (loss of sensation). The symptoms of rheumatoid arthritis are morning stiffness, polyarthritis, pitting oedema, proximal muscle stiffness, mimicking with polymyalgia rheumatic swelling of metcarpo-phalangeal joints and proximal interphalangeal joints. Joints are active inflamed if they are tender on pressure and have stress pain on passive movement and soft tissues swelling. So, Amavata may be considered as equivalent to rheumatoid arthritis. The drug selected for management of Amavata (rheumatoid arthritis) were compound formulation normally Amavatari Yoga and Therapy Vaitaran Basti. So, in my study, 60 patients were to be treated in 2 groups. Group- A 30 patients was to be treated with Amavatari Yoga (Lasuna – 1gm + Sunthi – 1gm + Nirgundi – 1gm + Tagar – 1gm) before meal twice daily for 45 days. Group- B 30 patients was to be treated with Vaitaran Basti for 7 consecutive days followed by Amvatari Yoga twice daily for 45 days. The above mentioned Amavatari Yoga and Vaitaran Basti are very effective for the management of Amavata (rheumatoid arthritis) considering their pharmacodynamic action
A Clinical Study to Evaluate the Efficacy of Kasisadi Vaginal Pessary and Udumbaradi Tail Pichu in Management of Shweta Pradar (Abnormal Vaginal Discharge): A Double Arm Random Control Study
Shwetapradara, a symptom of many gynaecologic disorders, develop due to vitiation of Kapha predominant Doshas. Laghutriya described Shwetapradar. In Charak Samhita management of Shwetpradar is described under the description of Pandurasrigdara. Chakrapani has explained Pandura-Asrigdara as Shwetapradar in his commentary. Objectives: To draw Ayurvedic management to the disease with effective cure and no or minimal side effects, that can be easily administered and accepted by the patient. Materials & Methods: After obtaining Ethics Committee approval and informed consent, 210 patients were randomized into two groups. In one group, Kasisadi vaginal pessary in the night followed by Udumbaradi Taila Pichu for 2 hours in the morning was given while in another a vaginal pessary (clotrimazole+ tinidazole+ clindamycin) was given for 7 days. From baseline to 7th day on the bases of assessment criteria per-speculum examination was done. A comparison of categorical variables was done using an appropriate statistical test. Results: Uduambaradi Taila and Kasisadi vaginal pessary exhibited potentially comparable effects to the conventional treatment but the recurrence rate was found more in conventional treatment. Conclusion: Uduambaradi Taila pichu and Kasisadi vaginal pessary combination is an effective, side effects-free, patient-compliant herbal alternative for the management of abnormal vaginal discharge
A Systemic Review on Comparative Study of Traditional And Modern Methods of Preparation of Ayurvedic Medicines
Affordable, high-quality Ayurvedic remedies may be the answer to today\u27s healthcare demands of the Indian and worldwide communities. In order to scale up manufacturing of the medications, which is required to satisfy the rising demand, it is vital to relate Ayurvedic preparations with modern processing principles. For bettering the dosage forms, it\u27s also crucial to comprehend the basic Ayurvedic concepts behind formulation and processing. There has been no systematic research to compare ancient and contemporary processing methods, despite the fact that the Ayurvedic sector has included technology from the food, chemical, and pharmaceutical industries. This study makes an effort to show a potential relationship between modern processing principles and Ayurvedic processing techniques. By gathering data from English translations of old Ayurvedic writings on medication preparation techniques, a thorough literature analysis was carried out to determine the Ayurvedic processing procedures. Based on the methods employed in Ayurvedic medication processing, a correlation between traditional and modern processing principles was conducted. Extraction and separation were found to be the two main types of procedures used in the manufacture of Ayurvedic medicines. Separation employs volatility, adsorption, and size-exclusion principles whereas extraction relies on membrane rupturing and solute diffusion principles. The study offers systematic recording of the Ayurvedic processes for making Ayurvedic medicines, coupled with an explanation of those processes in terms of modern processing principles. This is the initial stage that can lead to the advancement or replacement of conventional methods
Vanshlochan Substitution and Adulteration: Discussions on the Controversy over its Original Source
In India, about 80% of the rural population relies on herbal remedies for their basic medical needs. The increasing demand and trading of raw materials for herbal medicines often involve adulterants and substitutes. The reasons behind intentional adulteration are typically commercial and include degradation, admixture, sophistication, inferiority, spoiling, and other unidentified causes. Substitution is the use of comparable medications instead of the original ones. Similar Rasa, Guna, Virya, Vipaka, and most importantly Karma are the guiding principles for choosing alternative medicaments. The adulteration and substitution of herbal medications are currently a major issue in both the herbal business and Ayurvedic therapies. Vanshlochan or Tabasheer is one of the example, which has long been used as an anti-tussive in traditional medicine to cure a variety of illnesses. When this compound was first derived, the major constituent was silica with traces of other elements. Vanshlochan is prescribed as a bioavailability booster and offers a synergistic component to some well-known medications, including Sitopaladi, Talisadi, and Dadimashtaka in Ayurveda and other Unani formulations. To be utilized as a medication in conventional medicine, Vanshlochan needs stringent pharmaco-analytical approaches with evidence-based studies regarding its drug safety for human consumption. A Case Report on the Effect of Triphala Decoction in Tartar-Induced Periodontitis in Dentistry
Dental tartar causes irritation and inflammation in gingival tissue. Further, it attacks the soft periodontal tissue which supports the teeth. Therefore, all supragingival and subgingival tartar was removed by an ultrasonic scaler on the 1st day, and the patient was advised to swish Triphala (equal amount of Amla, Haritaki, and Vibhitaki) decoction in the oral cavity from the evening on the same day of scaling. The lukewarm decoction was kept in the oral cavity for 10 minutes and then swallowed. It was given twice a day for 30 days after the meal. The patient was advised to visit on the 7th, 15th, and 30th day of follow-up. Triphala itself has an antimicrobial property. Its decoction kept in the oral cavity stimulated the saliva flow. Saliva has lysozymes that have a bactericidal effect on the oral micro-organisms. Triphala expelled the toxins from the gut by increasing the peristalsis movement of GIT. It had a digestive stimulant property which increased the digestive fire and improved overall oral health. As the Triphala decoction showed very effective results on scaling wound, it may be used as herbal mouthwash in the future. Besides, Triphala powder is easily available, economical, and has negligible side effects
Clinical Evaluation of Jalaukavacharana (Medicinal Leech Therapy) in Non-Healing Ulcer
Chronic non-healing ulcers are those that do not heal within three months or often remain in the inflammatory stage for too long and may never heal or may take years. Some wounds heal easily by body’s self-healing mechanism and some poses challenge to normal wound healing and does not heal in time and becomes a matter of concern for the physicians. Leech therapy proved to be a better treatment option by benefits of its salivary constituents in wound healing. It is an ancient way of treating such complicated disorders mentioned in different texts. A patient came at Shalya Tantra OPD with ulcer on posterior aspect of right ankle joint above the heel which was not healing since 6 months even on daily antiseptic dressings. Patient gave h/o immobilization for 2 months following ORIF of same joint 7 months back. During immobilization, after 1month of ORIF, patient developed ulcer at posterior aspect of Rt. Ankle above heel. Leech was applied at wound site twice weekly initially followed by once weekly then once in 15 days along with antiseptic dressing daily. The ulcer started showing signs of healing within three sittings and completely healed by 4 months. This study was carried out in Department of Shalya Tantra, Rajiv Gandhi govt. Diagnostic Criteria for Garbhasaya Grivamukhgata Vrana (Cervical Erosion)
In Ayurveda, Trisutra is the three modules which are, first Hetu (etiology), second Linga (signs and symptoms) and third Aushadh (treatment or management of disease). The aim of Ayurveda is to maintain Dhatusamya (equilibrium state of Dhatus) which in turn causes health. To know the Linga (sign and symptoms) of disease, there is a need of proper examination of patient. Various diagnostic method has been described in various Ayurvedic texts, out of which ‘Trividha Pariksha’ i.e., Darshana (inspection), Sparshana (Palpation, percussion), Prashana (questionnaire) has more importance for diagnostic purpose. All the other diagnostic method explains in Ayurveda as well as modern science can be categorized under these basic ‘Trividha Pariksha’. Cervical erosion is the common gynecological problem most likely in many women throughout their reproductive years. Due to its non-healing and recurrent nature of cervical erosion is the leading cause of cancer in cervix in women. So, these ‘Trividha Pariksha’ plays a key role in diagnosis of cervical erosion (Garbhasaya Grivamukhgata Vrana). It is used to determine the root cause of the Garbhasaya Grivamukhgata vrana and to determine the treatment of disease. The diagnosis process involves the assessment of several subjective and objective parameters pertaining to the disease as well as patient. Darshana pariksha and Sparshana pariksha come under objective parameter whereas Prashana pariksha come under subjective parameter
Mode of Action of Trikatu Ghrita Yukt Swarnabhasma in Pandu Roga (Anaemia)
Pandu roga, an ancient Ayurvedic term used to describe a specific disease, has been recognized and discussed by various Ayurvedic scholars. According to their teachings, Pandu roga is considered a type of Rasa pradoshaj vikara, with Pitta dosha playing a prominent role in its manifestation. The key characteristic of this disease is Panduthwa, or Pallor, which exhibits clinical features similar to modern-day Anaemia .A condition in modern medicine, involves a deficiency in red blood cells or hemoglobin percentage due to decreased production. It is often associated with poor nutrition and overall health. The problem is commonly observed in low birth weight infants, overweight children who lack energy for physical activity, school-going children, adolescents, and pregnant women. In traditional Ayurvedic medicine, there are various formulations suggested for managing Anaemia, one of which is Trikatu Ghrita Yukt Swarnabhasma. This herbal preparation contains Trikatu, a combination of three spices - Shunthi (Zingiber officinale), Marich (Piper nigrum), and Pippali (Piper longum). These spices are renowned for their digestive and metabolic properties. Additionally, Swarnabhasma, derived from gold, has been utilized in Ayurveda for its rejuvenating and tonic effect
Qualitative Phytochemical Analysis of Mamajjaka (Enicostemma Littorale Blume)
The phytochemical analysis of plants plays a significant role commercially and has great interest in pharmaceutical companies for the manufacture of the new drugs to treat a variety of disorders. The Medicinal properties of a plant species lies in its phytochemical components that cause definite pharmacological action on the body. In the present study Mamajjaka (Enicostemma littorale Blume) was selected for qualitative phytochemical component analysis. The phytochemical analysis of aqueous and ethanolic extract samples was done to determine the phytochemical components in the plant. These extracts were used for qualitative phytochemical analysis using standard chemical tests. Results show the presence of various bioactive compounds such as carbohydrates, alkaloids, amino acids, protein, saponin, glycosides, phenolic compound, flavonoids, tannin and absence of steroids
