    Efficacy of Homoeopathic Medicine in the Treatment of Osteoarthritis: A Systematic Review

    Introduction: Osteoarthritis is a non-inflammatory heterogeneous group of degenerative joint disease. Homoeopathic remedy has encountered rheumatological problems very well. The main aim of this systemic review was to evaluate, specify and pinpoint the findings of all relevant individualised studies, thereby making the available evidence more accessible to decision-makers. Materials & Methods: An intensive search of RCT clinical research manuscripts published between 2000 and 2022 was done under various databases and it ensured that all papers belong to peer-reviewed journals. The data items were extracted by following points like publication years, population, interventions and comparator (Verum vs control), outcomes, methods, overall result and manufacturer of Verum. The five-point Jadad scoring system was used to assess the methodological quality of the selected trials with increasing scores indicating a higher quality. Whereas the null hypothesis in this systematic review was that individualized homoeopathic medicine had no impact. Results: A total of 56 experimental and controlled clinical trials were identified to be screened. After complete screening, the proper number of eligible papers was 12 and finally selected 08 RCT with a double-blind peer-review published paper. The studies maintain total number of patients of 1,891 and after dropping out 1,628 patients eagerly continued. The 08 studies focused on knee joints and lower back pain. Conclusion: In this study, we clearly understood that homoeopathic combination formulas work well on OA. Individualized Homoeopathic remedy was not effective due to insufficient trial reports. It’s also noticeable that homoeopathic combinations may have some adverse drug reactions. So, we need proper evidence for individualized homoeopathic medicine to say it works properly. It’s our duty to uptake trial testing continuously for the betterment of homoeopathy. However, more research is needed to completely evaluate and validate the efficacy or inadequacy of therapy with OA

    Avabahuka is a condition where the Vata near the base of the shoulder constricts the Siras, preventing the shoulder joint from moving. The signs of adhesive capsulitis, also referred to as frozen shoulder or Avabahuka, may co-occur. The clinical illness known as frozen shoulder causes painful restrictions to both passive and active shoulder movements. A 25-year-old male patient arrived at the OPD with the main complaints of left shoulder joint pain, stiffness, restricted movement, and loss of sleep brought on by pain in the previous 12 months.     Study of Viruddha Aahar in Healthcare Management

    Viruddha Aahar or incompatible diet is very important issue discussed by ancient Ayurveda scholars. Viruddha Aahar is certain diet and its combinations which interrupts the metabolism of tissue, which inhibits the formation of tissue and which have the opposite property to the tissue. Charak has mentioned many diseases due to Viruddha Aahar as- infertility, fistula-in-ano, anemia, leprosy, defective birth etc. and death too. In Ayurvedic literatures many incompatible food has mentioned, but we have to identify new food incompatibilities which are used frequently in modern society. eg.- green or black tea and milk. Tea contains flavonoids called catechins, which have beneficial effect on heart, but when mixed with milk, group of proteins in milk called Caseins, interact with the tea to reduce the concentration of catechins. Treatment of diseases, induced or produced due to Viruddha Aahar, is explained in Charak Samhita. According to Charak, Samshodhan and Samshaman Chikitsa is useful for such diseases followed by regular use of Rasayana for long period

    A Comparative Clinical Study to Evaluate the Effect of Vatari Guggulu and Shatpushpadi Churna with Simhanada Guggulu in the Management of Amavata w.s.r. to Rheumatoid Arthritis

    The disease Amavata described in classical Ayurvedic literature is closely resembles with Rheumatoid Arthritis (RA). Amavata is a crippling condition where simultaneously aggravated Vata and Ama associated with each other settles in Sandhis and produce Sandhishoola, Sandhishotha, Sparshaasahitwa, which is similar to Rheumatoid Arthritis. Amavata is the challenging problem for medical science with different treatment protocol. The objectives of this randomized parallel group Comparative study were to evaluate the effect of Vatari Guggulu and Shatpushpadi Churna with Simhanada Guggulu in the management of Amavata w.s.r. to Rheumatoid Arthritis This Study was conducted on 32 patients selected randomly from OPD and IPD of R.G.G.P.G. Ayurvedic College and Hospital, Paprola, Dist. Kangra (H.P.) and divided into 2 groups. Group I and Group II having 16 patients in each group. Group A were managed with Vatari Guggulu 500mg BD with lukewarm water and Shatpushpadi Churna 3gm BD with lukewarm water while Group B were managed with Simhanada Guggulu 500mg BD with lukewarm water for 6 weeks. Results showed statistically significant difference in effect of Group I and Group II on subjective parameters – Angamarda, Trishna, Apaka, Jadya, Sparsh Ashyata, Vidvibandha, Nidravipraya, ESR, walking time and grip strength. Percent wise Vatari Guggulu and Shatpushpadi Churna are found to be more effective than Simhanada Guggulu for all assessment criteria in the management of Amavata

    A Comparative Clinical Study to Evaluate the Effect of Pathyadi Churna and Shunthyadi Kwatha in the Management of Amavata w.s.r to Rheumatoid Arthritis

    Amavata is a Vata and Kapha Pradhana Vyadhi caused due to Viruddha Ahara and Vihara resulting in Mandagni, leading to the formation of Ama. Ama with the influence of Vata Dosha circulates all over the body and gets lodged in Shleshma Sthana in Sandhi Pradesha, resulting in the manifestation of Amavata. This Amavata can be compared with rheumatoid arthritis showing similarities in clinical features. Clinically diagnosed 34 patients of Amavata were selected from OPD & IPD of R.G.G.P.G. Ayurvedic College and Hospital Paprola, Dist. Kangra (H.P.). This article discusses the comparative effect of Pathyadi Churna in Group-A and Shunthyadi Kwatha in group-B having 17 patients in each group, in the treatment of Aamvata, mentioned in Chakradatta. It was observed that group of patients treated with Shunthyadi Kwatha (Group-B) showed better result than those patients treated with Pathyadi Churna (Group-A)

    The ancient science of Ayurveda is founded on numerous schools of thought. Given that it is an applied science, it has a scientific foundation. The Ayurveda\u27s guiding principles were established after extensive experimentation. Because of this, these guidelines still hold true. even in the present. The modern world, however, does not exclusively adhere to religion. To persuade the academics of the veracity of these principles, rigorous observation, study, and experimentation fully backed by facts, proofs, and statistical analysis are required. A conversation of this nature is essential for thorough understanding of the topic. The greatest way to establish any subject\u27s fundamentals is through these logics. The Vadas of Darshanas were also the source of the ideas in Swabhavoparam Vada evum Hetum-hetorvartanam. Regarding Swabhavoparam Vada, Acharya Charka used Kala as an illustration to explain the Swabhavoparam Vada idea. The Hetum-hetorvartanam, on the other hand, is in near proximity to Karya Karna Vada. Ayurveda uses both the Swabhavaparam Vada and the Hetum-Hetorvartanam principles to treat illnesses. Swabhavoparam Vada is adamant about sustaining Dhatu samya since the ailment will go away after being destroyed by nature, while Hetum-Hetorvartanum emphasises getting rid of the Hetu thus causing Hetu to disappear, which will result in the disease\u27s cure.     Pharmacognostical Identification of Arjuna, Punarnava and Jatamansi with Recapture of Concept of Substitution and Adulteration

    In recent era, growth in natural product market and interest in traditional system of medicine is seen and people are shifting towards the Ayurvedic theories of health management. But, growth in production is still linear and fast depletion of resources is creating the major trouble. To meet the deficit, practices of adulteration and substitution are used widely, which became burning problem in industry threatening the integrity of Ayurveda. Aim and Objective: The aim of this article is pharmacognostical identification of Arjun, Punarnava and Jatamansi with recapture of concept of adulteration and substitution, their cause, types, examples and other import aspects and identify areas that need further research. Methodology: The pharmacognostical identification of Arjun, Punarnava and Jatamansi was performed with their adulterant and substituent. The review of concept of adulteration and substitution was done from literature and more than 30 published research articles that were identified through a systematic search of major computerized medical databases. Observation and Result: The deforestation, extinction or evolution of many species, insufficient knowledge, unauthenticated practices about raw material collectively resulted in adulteration and substitution. The concept of substitution is known in Ayurveda as it is enlightened prominently in Vagbhata, Bhavprakash Yogratnakar and Bhaishajya ratnavali. The pharmacognostical identification of Arjun, Punarnava and Jatamansi was performed performed and shown with their adulterant and substituent. Discussion: Strategic planning and integrated approach towards mass production, supply chain management, IEC, proper advocacy, proper pharmacovigilance monitoring of raw materials and knowledge sharing is needed to trim down the gravity of adulteration. Scope of this article throws light on the concepts of substitution given by our preceptors and analyses these with the present-day prevailing trend of adulteration and substitution

    A Study on Structural Changes in Joints of Hand w.s.r. to Amavata

    Amavata is a chronic disease caused due to the vitiation of Vata associated with the Ama that circulates all over the body. Though various drug trails and treatment procedures were already carried out on Amavata, here an attempt is made to know the structural changes in the joints of hand that occurs in the disease, with the help of Radiography (X-ray), that may aid in the diagnosis and to know the efficacy of treatment. Objectives: To study the structural changes in joints of hand w.s.r. to Amavata. Materials and Methods: 30 diagnosed patients of Amavata were selected affecting the joints of hand (wrist, metacarpophalangeal and interphalangeal joints) and X-ray study was done to know the structural changes. Results: The study has shown the more of the structural changes in the form of soft tissue swelling, peri-articular osteopenia, bone erosion, joint space narrowing in the early stages of the disease, and secondary osteoarthritis, osteophytes, subluxation and ankylosis in the later stages. Conclusions: In metacarpophalangeal joint more of the structural changes are found in the form of soft tissue swelling, periarticular osteopenia and joint space narrowing. Bone erosion and osteophytes were lesser than the wrist joint and subchondral cyst, subluxation, secondary osteoarthritis and ankylosis were seen rarely and in the later stages of the disease. In interphalangeal joints soft tissue swelling, periarticular osteopenia, joint space narrowing was present as early changes. Subchondral cyst, subluxation, secondary osteoarthritis, bone erosion, osteophytes and ankylosis in the later stages of the disease

    Rachana sharir is an important subject in Ayurveda. To cure any disease basic knowledge of anatomical concept must be required. In Ayurveda many concepts like Kurcha, Jala, Snayu, Kandra were described under Paribhasha (Terminologies) Sharir. Kandara is one of the unique concept of Ayurveda which was very well explained by Aacharya Sushrata in Sharirsthana. These are 16 in number. Rounded type of Snayu is also known as Kandara. Prasaranam and Aakunchanm is function of Kandara. Kandara is also known as Updhatu of Rakta. In modern Science they can be correlated with tendon.
