    Unlocking Healing Potential: A Case Study on the Management of Diabetic Foot Ulcer

    Diabetes stands as one of the most prevalent diseases globally, and diabetic patients are at risk of developing foot ulcers as a microvascular complication. These ulcers stem from diabetic neuropathy or atherosclerosis, leading to ischemia or breakdown of glucose-laden tissue, resulting in inflammation and ulceration. If left untreated, these ulcers can become infected, spreading to the underlying tissues. In Ayurveda, such ulcers are termed as Madhumehajanya Vrana. Sushruta Acharya outlined Anushastra Karma for managing Vrana, which includes Ksharakarma (application of alkalis) and Jalukavacharana (leech therapy). Kshara functions by actions like Chhedana, Bhedana, Lekhana and Ropana. Jalukavacharana possesses anti-inflammatory properties, promotes fresh blood supply by removing vitiated Dosha from the blood, and fosters wound healing by forming healthy granulation tissue. A 65-year-old female patient presented with a non-healing wound on the plantar aspect of her left foot below the greater toe since 8 months, associated with foul-smelling pus discharge for 6 months. She had a known history of diabetes mellitus since 10 years. The ulcer measured 4cm x 3cm x 2.5cm, GRBS was 380mg/dl with HbA1c of 12%. Patient was treated with Ksharataila infiltration, application of Tilwaka pratisaraneeya kshara followed by Lekhana of callosity present over the edges of the ulcer, and Jalukavacharana therapy for four sessions with seven days interval, in addition to internal medications. Glycemic control was achieved by Inj. Insulin Mixtard 30/70 30 units in morning and 15units at night

    The collective meaning of Vatavyadhi indicates the specific disorder occurring due to Vata Dosha. Wherever, Vata Dosha get vitiated it first enters in all Srotas of the body and then creates different types of Vata Vyadhi describe in Classical Ayurvedic text. Sarvangavata is one of the most common disorders among Vata Nanatmaja Vikara. The global prevalence of Friedrich’s Ataxia incidence seen in 1 among 50,000. Objective: The aim of this study was to access the efficacy of Panchakarma modalities in Friedrich Ataxia condition. Material and Method: A 20 year old male patient who is K/C/O- Diabetes Mellitus (Type 1) since 1 year (insulin dependent) stopped since 2 months and N/K/C of Hypertension and IHD came to hospital with complain of reduced strength in bilateral upper and lower limb since 3years and associated with difficulty in walking and slurred speech since 2 years which was diagnosed with Sarvangavata and was treated with Sarvanga Abhyanga with Mahanarayana Taila followed by Dashamoola Qwatha Seka for 5 days, Virechanakarma for 13 days, Sarvanga Abhyanga with Mahanarayana Taila followed by Nadi Sweda for 7 days, Mustadhi Yapana Basti for 10 days, physiotherapy and Shamana Yoga’s. Result: After completion of one and half month of total treatment, the patient as found markly improvement gait, muscle tone and gain in strength in bilateral upper and lower limb. Conclusion: Sarvangavata can occur due to Beejopgataja, Dhatuksaya and Abhigataja where Vataja Lakshana were predominantly seen in this type of case and can be manage with different types of Sweda like Dashamoola Kayaseka, Virechana, Mustadi Yapana Basti, physiotherapy. Different Panchakarma modalities helped the patient markly improvement for doing his routine activities

    Kampavata is one among Vataja nanatmaja vyadhis. Acharya Charaka, Susrutha, Vagbhata, Madhavakara, Kashyapa mentioned Kampavata as Vepathu. It can be correlated with “Tremors” in modern science which is the most common movement disorder. The total number of people suffering from ET worldwide was 24.91 million in 2020 and incidence increases with age. Kampa is the prominent feature in Kampavata which is categorized as Sarvanga-kampa and Shiro-kampa. Kampa occurs due to Prakopa of Chalaguna which is the inherent quality of Vata dosha. The management of essential tremors in the conventional system of medicine is decreasing quality of life of patient with certain side effects like fatigue, impotence, bradycardia, reduced blood pressure, drowsiness, nausea, dizziness, ataxia, confusion, vertigo, paresthesia etc. By Ayurvedic intervention patient quality of life can be preserved. In the present clinical study patient was treated with Snehana and Brmhana line of management in the form of Shirodhara, Nasya and Samana oushadies like Sarvamayantakaghrta etc. The outcome of the study revealed good therapeutic efficacy and patient got remarkable relief in symptoms, that are assessed by TRG Essential tremor rating assessment scale (TETRAS©) V 3.1

    Understanding of Kaphaja Shiroroga w.s.r. to Sinusitis

    Ayurveda, encompasses eight branches known as Ashtangas. Among these, Shalakya Tantra specializes in the diagnosis and treatment of diseases affecting the Urdhwajatru-head and neck. Acharya Sushruta, a revered figure in Ayurvedic medicine, meticulously detailed the management of eleven head diseases, termed Shirorogas, in the Uttara Tantra of Sushruta samhita. One of these diseases, Kaphaja Shiroroga, closely aligns with the modern medical condition of Sinusitis based on its signs and symptoms. Kaphaja Shiroroga presents with symptoms such as a sensation of heaviness, mucus accumulation, stiffness, coldness and swelling around the eyes. Sinusitis, a contemporary counterpart, involves inflammation of the sinus tissues- air-filled spaces in the forehead, cheeks and nose. This inflammation results in facial pain, nasal congestion or discharge and sometimes fever, often triggered by a common cold. However, sinusitis can also be caused by viruses, bacteria, fungi, or allergies. Classified as a Kaphaja Vyadhi (disease caused by an imbalance of the Kapha dosha), Kaphaja Shiroroga is treated through various Ayurvedic procedures, including Shirovirechana (cleansing of head), Kavala (gargling), Snehana (oiling), Swedana (sudation), Vamana (therapeutic emesis), Upanaha (poultice) and Dhumapana (medicated smoking). The choice of treatment is determined by the physician\u27s expertise and tailored to the patient\u27s specific needs. By following these methodologies, Ayurveda aims not only to eradicate the disease but also to minimize the likelihood of recurrence, thus promoting long-term health and well-being

    Ayurvedic Management of Endometriosis w.s.r. to Vatiki Yoni Vyapada

    Endometriosis is a chronic, often painful condition in which tissue similar to the lining of the uterus grows outside the uterine cavity. Affecting approximately 10% of women of reproductive age, endometriosis can lead to severe pelvic pain, infertility, and other systemic symptoms. Despite its prevalence, the condition remains under-diagnosed and poorly understood, largely due to the complexity of its symptoms and variability in individual presentations. The treatment involves hormonal therapy and surgical interventions, but these may not always solve the underlying issue or may have significant side effects. This article explores Ayurvedic management of endometriosis by adopting the treatment principles of Vataja yonivyapada due to similarities of symptoms. A holistic approach is used by focusing on Vatanulomana by Shamana chikitsa and Shodhana and Sthanika chikitsa in the form of Nabhi purana with Eranda taila and Mruttika alepa. This is a case report of a 34 year old unmarried female patient suffering from endometriosis and having complaint of severe pain during menses. She was totally dependent on analgesics injections and oral painkiller during her menses since 2 years. The objective of present treatment included management of the symptoms by Ayurvedic intervention by formation of Shudha artava dhatu and balancing the Gati of Vata dosha and also improvement of the quality of life which was hampered during menses. After the treatment her dependency on the painkillers was gradually reduced

    Panchakarma Intervention in Heriditary Spastic Paraplegia

    Hereditary Spastic Paraplegia (HSP) is a large group of inherited neurological disorders, in which the prominent feature is a progressive spastic paraparesis. it can be Multiple genetic mutations with various inheritance patterns (autosomal dominant, autosomal recessive, X-linked recessive). Direct correlation of Hereditary Spastic Paraplegia (HSP) is not found in Ayurvedic classics. It can be studied within the broad spectrum of Vatavyadhi. In the present cases, based on the symptoms observed in patients, it can be considered under the category of Pangu. Objectives: By considering Pangu is manifested from vitiated Vata dosha in combination with other Dosha and presents with symptoms of reduced strength in bilateral lower limb and abnormal gait. An attempt has been made to evaluate the efficacy of Panchakarma procedures in the conservative management of the disease. Method: Two cases of HSP presented with symptoms of difficulty in walking, weakness in bilateral lower limb along with stiffness managed with Dashamoola Kashaya seka, Shastikashalipinda sweda, Balavarnakara basti in Kalabasti pattern. Results: The patient was assessed based on sign and symptoms of HSP. Conclusion: Panchakarma treatment having encouraging results in improving quality of life of patients

    Role of Basti in Artavshaya w.s.r. to Lean and Obese Polycystic Ovarian Syndrome

    Polycystic ovarian syndrome is common endocrine, metabolic and lifestyle disorder for female reproductive age. Women with PCOS may not ovulate, have high level of androgens and have multiple cysts in the ovaries. PCOS can cause absent or irregular menstrual periods, excess hair growth, acne, infertility and weight gain. In Ayurveda, it can be correlated with Artavakshaya and Pushpaghani Jatharini. In the event of deficiency or loss of Artava, the menstruation does not appear in its appropriate time or is delayed (intermenstrual period is prolonged), is scanty and does not last for three days. There is also pain in vagina. This pain is due to aggression of Vayu caused by loss of Artava which fills this region, in modern context the treatment is based on hormonal preparation only which have may hazardous effect like weight gain, cysts in reproductive organ and chances of OHSS (ovarian hyper stimulation syndrome) and carcinoma. PCOS has two phenotypes, overweight/obese and lean, the latter being a much less common presentation of the syndrome. Lekhana Basti as a form of Niruha Basti which possesses Shodhana and Lekhana properties. Lekhana means scraping or scratching, this property helps to eliminate morbid Dosha and Lekhniya dravya removed increased Mala by their scraping action. PCOS is considered under Yonivyapad and Artava Dusthi and this Yoniroga cannot occur without vitiation of Vata dosha for this in Ayurveda, Basti (medicated enema) is the best therapy along with oral medications, in this article, we want to focus mainly on Basti therapy for both lean and obese PCOS, as customized approach

    Ayurveda Management of Garbhini Vibandha w.s.r. to Constipation in Pregnancy- A Conceptual Study

    Constipation is among minor ailment during pregnancy. There are anatomical, physiological and hormonal adaptations during pregnancy phase, due to these changes every pregnant woman faces some difficulties like heartburn, nausea, vomiting, pedal oedema, constipation in all three trimesters. Garbhini Vibandha or constipation during pregnancy, characterized by difficulty in bowel movements. Some gynaecological diseases, stress, hormonal imbalance leads to irregular bowel habits in women. Most of the pregnant women has this complains. Acharya Kashyapa mentioned that the diseases which are seen in any other individual will see in pregnant females also as Dosha, Dushya of the body are same. According to Ayurvedic principles, and this condition primarily results from an imbalance in Vata dosha, mainly Apana Vata, which governs the elimination process. The condition may be exacerbated by the natural physiological and hormonal changes during pregnancy, dietary factors, reduced physical activity, and psychological stress. Ayurveda emphasizes a holistic approach in managing Garbhini Vibandha, focusing on dietary modifications, lifestyle changes, and the use of specific herbs to balance the Dosha. Herbal remedies such as Triphala kwatha, Haritaki, Gur, Nagara, are commonly used to alleviate constipation. In this article, we want to focus on mode of action of these drugs on Garbhini and the main precautionary point is that these medications should be taken under supervision of doctor. Over dose intake or wrongly intake can cause other health problems

    A Critical Appraisal of Daivavyapashraya Chikitsa w.s.r to Mantra and Homa in Adhyatmika Vyadhis

    This study aimed to explore the importance of Mantra and Homa (Yagya) in the management of Adhyatmika vyadhis (psychosomatic disorders). Ayurveda and other Vedic classics have explained three types of diseases such as Adhyatmika (Karmaja, Kulaja, Sahaja, Prakritibhava), Adhibhautika, and Adhidaivika. The Chikitsa such as Daivavyapashraya, Yuktivyapashraya, and Satvavajaya are Trividha Chikitsa mentioned in Ayurveda and Daivavyapashraya Chikitsa are useful for enhancing the spiritual power of the patient. It includes 14 sub-segments among them Mantra and Homa have been selected for this study. Importance of Adhyatma dravya guna sangraha and Sadhvruta following - Japa, Mantra, and Homa are clearly mentioned in Ayurveda. The Bhagavad Gita explanation has been given about Tapatraya (Adhyatmika, Adhidaivika and Adhibautika). Vishnu Sahasranama mantra, Shiva Shiva Sthuti, Rakshogna mantra, Putrayasti yagya and Nirgundi patra Dupa followed by Mantra have been prescribed for Abhishapaja Jwara, Kushta, Vandyatwa and Visham jwara respectively. The effects of Yagya therapy on diabetes, stress, and anxiety have been scientifically proven. The mechanism of Yagya has been explained as follows; inhalation of Yagya vayu (medicinal smoke) is supposed to give immunity and strength, while exhaled breath provides removal of toxins out of the body. Changing the sound pattern while chanting the mantras can change the energy of the entire body and it balances the Tridosha. Hence it can be concluded that Homa and Mantra must have a significant effect on the management of Adhyatmika Vyadhis

    Evaluate the Effect of Traditional Approach (Basti Chikitsa and Ayurveda Formulations) in the Management of Benign Prostate Hyperplasia (BPH)

    Benign Prostate Hyperplasia (BPH) is defined as the proliferation of prostatic stromal cells, which results in an enlarged Prostate gland. As result the prostatic urethra is compressed, which restricts the flow of urine from the bladder. BPH is a burning senile problem of elderly men, associated with lower urinary tract symptoms such as frequent urination, urgency, nocturia, intermittent force of stream, and the sensation of incomplete bladder emptying, usually seen in after 5th decade. In Ayurveda, Vatashtheela disease closely resembles with BPH of modern medicine in its sign and symptoms. It is manifested due to improper function of Apana Vayu along with the vitiation of Kapha and Pitta dosha. In this case study, a patient diagnosed with Benign Prostatic Hyperplasia was treated with a Dashmoola Kwath Niruha Basti and Bala oil Anuvasana Basti (3:1 ratio in plan) for 30 days and followed by oral administration of Chandraprabha vati, Varunadi kashayam, Haritaki churna, Amla churna, Haridra churna and Amrutadi guggulu for 3month in follow-up period. After completion of the treatment, changes in the subjective criteria (by IPSS) and changes in the objective criteria (by USG) were recorded. Finally, study concluded that given treatment is found to be highly effective in the management of Benign Prostate Hyperplasia (BPH)
