    Hormonal Axis in Malignancy - From Pathogenesis to Therapeutics: Insights from Modern Oncology and Ayurveda

    Hormonal regulation is central to cancer pathogenesis, influencing cell proliferation, apoptosis, angiogenesis, and immune surveillance. Dysregulation of endocrine axes-including hypothalamus-pituitary-gonadal (HPG), hypothalamus pituitary adrenal (HPA), hypothalamus-pituitary-thyroid (HPT), and pituitary–liver axes-drives hormone-sensitive malignancies such as breast, prostate, ovarian, thyroid, and liver cancers. Genetic, lifestyle, and environmental factors further modulate hormonal balance and cancer susceptibility. Ayurvedic principles provide a complementary framework, linking Dosha imbalance, Dhatu dushti, Srotorodha, ama accumulation, and Ojas kshaya to endocrine dysfunction. Therapeutic strategies include modern hormone-targeted agents (selective receptor modulators, aromatase inhibitors, androgen blockers, PI3K/mTOR/CDK4/6 inhibitors) and context-specific Ayurvedic herbs (Ashwagandha, Shatavari, Rasayanas), emphasizing careful monitoring in hormone-sensitive cancers. Integrating modern oncology with Ayurvedic approaches, along with lifestyle modification, may offer holistic strategies for the prevention and management of endocrine-responsive malignancies

    Ayurvedic Approach in the Successful Management of Psoriasis

    Psoriasis is an autoimmune skin disorder characterized by dry, scaly patches and inflammation, often exacerbated by environmental factors, stress, and local injuries. Ayurveda, addresses psoriasis as a result of Tridosha imbalance, particularly the vitiation of Pitta and Kapha, alongside the involvement of Raktadhatu and Mamsadhatu. The disease is managed through a holistic approach, combining Sodhana (purification therapy) and Shamana (palliative therapy). In this case, a 21-year-old male patient presented with symptoms of scaly, reddish-black patches along with itching and burning on the whole body for one year. After clinical examination and diagnosis of Ekakushtha (psoriasis), a tailored Ayurvedic treatment protocol was implemented, integrating Panchakarma therapies and internal medicines. Key interventions included Deepana (digestive stimulation) with Panchkola Churna along with Takradhara, Snehpana (internal oleation) with Murchchita Goghrita, followed by Vamana Karma (emesis) and Virechana (purgation) followed by Samsarjana Krama. The treatment was complemented by oral medications and oil for local application. Over a period of 90 days, significant clinical improvements were observed, with reduced itching, burning, and scaling, leading to complete recovery. The patient showed no recurrence of symptoms, demonstrating the efficacy of this multimodal Ayurvedic approach. This case highlights the potential of Ayurveda to manage chronic, autoimmune conditions like psoriasis by addressing the root causes of the disease, offering long-term relief and improved quality of life. Further research is needed to validate these findings and explore the broader applicability of Ayurvedic treatments for chronic inflammatory skin diseases

    A Conceptual Review on Mode of Action of Greeva Basti in Greeva Sandhigata Vata

    Greeva Basti is a Snigdha type of Swedana therapy administered under the principles of Bahiparimarjan Chikitsa. Although classical Ayurvedic texts do not directly mention Greeva Basti, its methodology is derived from Shiro Basti, as described by various Acharyas, and is extensively practiced in Keraliya Panchakarma. Cervical Spondylosis, characterized by osteoarthritic changes in cervical vertebrae and intervertebral discs, often presents with neck pain radiating to the shoulders and arms, accompanied by posterior occipital headache. In Ayurvedic terminology, degenerative disorders are classified under Vatavyadhi, with Greeva Sandhigata Vata identified as one of its eighty subtypes. Clinical features include pain during flexion and extension, stiffness, crepitus, and restricted cervical mobility. Greeva Basti exerts its effects through the combined action of Snehana and Swedana. The unctuous (Snigdha) and warm (Ushna) properties of medicated oil counteract the cold and dry qualities of aggravated Vata Dosha, nourish local Kapha, improve cervical flexibility, reduce stiffness (Stambha), and alleviate heaviness (Gaurava). Modern physiological interpretation suggests that local heat application enhances blood circulation, promotes phagocytic activity, and reduces inflammation. This study aims to elucidate the probable mechanism of Greeva Basti in managing Greeva Sandhigata Vata, highlighting its integration of classical Ayurvedic principles with contemporary physiological understanding, offering a safe, effective, and non-invasive therapeutic approach for degenerative cervical disorders

    Ayurvedic Approach to Managing Non-Alcoholic Fatty Liver Disease: A Case Report

    Fatty liver disease is one of the most prevalent liver disorders globally, characterized by the excessive accumulation of lipids, primarily triglycerides, within hepatocytes. Non-Alcoholic Fatty Liver Disease (NAFLD) refers to the buildup of excess fat within liver cells in individuals who do not consume significant amounts of alcohol. According to the World Health Organization (WHO), the prevalence of chronic liver diseases, including NAFLD, has shown a steady rise in recent years. In Ayurveda, NAFLD is understood as a Santarpanottha Vikara caused by Kapha and Medo Dushti. In this case, report effect of Ayurveda intervention in grade II fatty liver is reported. A 44-year-old male patient visited the National Institute of Ayurveda Hospital in Jaipur, Rajasthan, India, with complaints of abdominal heaviness, loss of appetite, fatigue, and incomplete bowel evacuation persisting for the past four months. On examination, there was no significant clinical abnormality. Haematological examinations revealed moderate enzyme elevation (ALT 89U/L, AST 42U/L). The ultrasound examination revealed a Grade II fatty liver. The patient underwent Ayurvedic treatment for 21 days. After treatment, significant improvement was observed in signs and symptoms and follow-up ultrasonography showed improvement from Grade 2 to Grade 1 fatty liver. The present case demonstrates the effectiveness of Ayurvedic interventions in the management of non-alcoholic fatty liver disease

    The Management of Schizophrenia Progressed from Substance-Induced Psychosis with a Selected Ayurvedic Protocol

    Schizophrenia, which progresses from substance-induced psychosis (SIP), is a psychiatric condition in which an initial substance-related psychotic episode develops into a chronic psychotic disorder. The Diagnostic and Statistical Manual of Mental Disorders, Fifth Edition (DSM-5), defines SIP as a state of delusions or hallucinations triggered by substance use or withdrawal. Studies suggest that 11–46% of individuals with SIP may progress to schizophrenia, with cannabinoids, stimulants, and alcohol being the most common triggers. However, SIP remains underdiagnosed, and its progression to schizophrenia is not well characterized. This case illustrates the importance of Ayurvedic management of a patient having psychotic symptoms (auditory hallucinations, multiple bizarre delusions) that arose from SIP having h/o substance use (alcohol, cigarettes, cannabis) from the last 10 years. Ayurveda classifies psychotic disorders under Unmada and offers a holistic treatment approach that includes internal medicines and panchakarma therapies. 33 days of IP management which include Shirodhara, Rookshana, Snehapana, Virechana, Yoga vasti, Nasya, shiropichu and Dhoopana were followed along with Shamana medication. A positive and negative syndrome scale was used for assessment before and after the treatment. The PANSS scores significantly decreased from 81 to 49 post-treatment, patient reported decrease in substance craving, and positive psychotic symptoms improved with no adverse reactions. This case provides the evidence of effect of ayurvedic management on schizophrenia progressing from substance-induced psychosis and offers an alternative and safe option from conventional medication

    The Role of Panchakarma in Restoring Bone Health in Avascular Necrosis of Femur

    Avascular Necrosis of the Femoral Head is a progressive, debilitating condition characterized by the loss of blood supply to bone tissue, resulting in bone degeneration and joint dysfunction. It commonly affects young adults, and if left untreated, can result in long term disability and the need for joint displacement. The presentation of AVN is nearly similar to Gambheera Vatarakta, Asthimajjagata vata or Asthi kshaya depending on the course of the disease. Materials and Methods: A male patient aged 35 years presented pain in right hip joint which was associated with stiffness since 1 year 2 months was diagnosed as bilateral Avascular necrosis of femoral head. The patient was treated in 3 phases where Manjistadi Kashaya seka, Vankshana basti, Manjistadi kshara followed by Ksheera basti was done. Results: Clinical outcomes were assessed using pain scales, range of motion and MRI imaging. There was significant improvement in overall symptoms. Conclusion: Conventional treatments often include pharmacological management, core decompression or surgical intervention.     Acharya Sushruta and Vagbhatta mentioned Anushastras in Sutrasthana. Acharya Sushruta has included 14 types of Anushastra. Aims and Objectives: To study Anushastra Prayoga in Urdhwajatrugata Vikara (Shalakya Tantra) mentioned in different classical texts and detail description of Trividha Anushastra Karma. Materials and Methods: This is a conceptual type of study. Textual materials (Bhrihattrayi & various other Samhitas) are used to gather all the available information regarding these Anushastra Prayoga in Urdhwajatrugata Vikara (Shalakya Tantra). Conclusion: The two powerful and less invasive parasurgical techniques, Kshara and Agni Karma, have many uses, particularly in the treatment of pain, which is crucial for preventing recurrence and unintended growth in the various illnesses described in Shalakya Tantra. Jalaukavacharana is also a minimum invasive parasurgical technique which is useful to treat many diseases related to vitiated Rakta Dosha described in Shalakya Tantra

    Reduction on Quantitative Viral Load in Hepatitis B: A Case Report of Successful Management Through Ayurveda

    Hepatitis B is one of the most prevalent causes of liver illness and is more likely to spread since people are not aware of the precautions. It is correlated with Kamala disease and shows chief symptoms like abdominal pain, vomiting, and yellowish discoloration in eyes, body, and urine etc. This case presents effectiveness of Ayurveda treatment in reducing symptoms and viral load in hepatitis case. A male patient of 64 years came to OPD of Ayushakti Ayurved Pvt Ltd. with hepatitis B positive result with complaints of weakness, fatigue, body pain, gas and acidity, pain in abdomen, recurrent throat infection. He was analysed clinically; the vitals were stable. After clinical examination, he was given tablet Livtone, D-vyro, for 3 months and Tablet Jivanyoga, Rejuliv, Immuno for next 3 months. Tab Gulkacid was kept continued across the treatment. After 6 months of treatment patient found good relief in symptoms and significant changes were reflected in laboratory parameters where viral load was decreased from 44 IU/ml (256 copies/ml) to 20 IU/ml (116 copies/ml), change in log value from 1.64 to 1.30. The ingredient of each tablet along with rational of prescriptions has been discussed in this paper

    A Clinical Study to Evaluate the Efficacy of Kutaj Kshar Sutra Using IFTAK Technique in the Management of Bhagandara (Complex Fistula in Ano)

    Bhagandara (complex fistula-in-ano) is a challenging anorectal disorder with high recurrence rates and significant morbidity. Conventional surgical approaches often result in complications such as incontinence and delayed wound healing. Ayurveda offers a promising alternative through Kshar Sutra therapy, which is known for its minimal recurrence and effective fistula healing. Kutaj (Holarrhena antidysenterica) Kshar Sutra is a specialized medicated thread that has potent antimicrobial, anti-inflammatory, and wound-healing properties. Objective: This clinical study aims to evaluate the efficacy of Kutaj Kshar Sutra using the IFTak (Interception of Fistulous Tract and Application of Kshar Sutra) technique in the management of Bhagandara (complex fistula-in-ano). Methodology: A controlled clinical trial was conducted on patients diagnosed with complex fistula in ano. Participants were treated with the IFTak technique, wherein the fistulous tract was intercepted and Kutaj Kshar Sutra was applied. Patients were monitored for parameters such as pain relief, healing time, recurrence rate, and complications. Results: Preliminary findings suggest that the Kutaj Kshar Sutra with the IFTak technique leads to faster healing, reduced recurrence, and better postoperative outcomes compared to conventional surgical methods. The natural caustic and antibacterial properties of Kutaj Kshar enhance the healing process while minimizing tissue damage. Conclusion: The study highlights the effectiveness of Kutaj Kshar Sutra using the IFTak technique as a viable and safe alternative in the management of complex fistula in ano. It offers a minimally invasive, cost-effective, and recurrence-free approach with improved patient outcomes. Further large-scale studies are recommended to validate these findings

    Eka Kustha (Psoriasis)- A Comprehensive Review of Ayurvedic and Modern Approaches

    Psoriasis, known as Eka Kustha in Ayurveda, is a chronic inflammatory skin condition that arises from immune system dysfunction, characterized by red, scaly patches. This disorder significantly affects the quality of life of those diagnosed. Psoriasis is considered a multifactorial disease, affected by a combination of genetic predispositions, environmental triggers, and immune system factors. This review seeks to explore psoriasis comprehensively through both Ayurvedic and contemporary medical lenses, focusing on its underlying mechanisms, treatment options, and the effectiveness of various approaches. From an Ayurvedic viewpoint, Eka Kustha stems from an imbalance in the Tridosha system, which can be exacerbated by poor dietary choices and lifestyle habits. This imbalance leads to toxin build-up and diminishes proper immune function. Ayurvedic treatments emphasize detoxification methods such as Panchakarma, the use of herbal medicines, and dietary adjustments to restore balance among the Doshas and enhance overall wellness. Conversely, modern medicine approaches psoriasis as an autoimmune condition that entails an imbalance in T cell activity and cytokines, including TNF-α, IL-17, and IL-23. Treatment in conventional medicine encompasses a range of options from topical corticosteroids and systemic immunosuppressants to advanced therapies like biologics and phototherapy, aimed at alleviating symptoms and managing the disease. Nonetheless, these methods may present adverse effects and typically necessitate ongoing treatment. A comparative assessment of these two frameworks reveals that while modern medical interventions can provide swift symptom relief, Ayurvedic practices may deliver long-term advantages by tackling the fundamental causes of the disease and fostering immune balance. The potential integration of Ayurvedic detoxification techniques and herbal solutions with modern therapeutic advancements holds promise for enhancing patient outcomes. Further clinical investigations are necessary to develop evidence-based integrative treatment protocols for psoriasis, aiming for a comprehensive and sustainable approach to managing this condition
