    Formulation and Evaluation of Orodispersible Tablet Containing Combination of Phosphodiesterase-5 Inhibitor and Herbal Aphrodisiac

    Purpose: Sildenafil citrate is widely used drug for the treatment of Erectile Dysfunction (ED) and Ginseng is a natural aphrodisiac reported to benefit this condition. The objective of the present study was to develop orodispersible tablets (ODTs) containing combination of Sildenafil citrate and Ginseng extract to improve the bioavailability, reduce the dosing frequency and thereby maintaining the therapeutic efficacy of the drug. Methods: The ODTs were prepared using superdisintegrants such as Croscarmellose sodium (CCS), povidone, and sodium starch glycolate (SSG) at varying concentrations (2%, 4% and 6%) by direct compression. The bitter taste of Sildenafil citrate was masked by Doshion resin. The optimized formulation based on least disintegration time (DT) was chosen to reformulate using sublimating agents such as camphor, menthol or thymol at varying concentrations (1%, 2%, 3%) to further reduce the DT. The compatibility of drug with excipients was investigated and the prepared formulations were evaluated for pre and post-compression parameters. Results: The post-compression parameters such as weight variation, hardness, friability, DT and in-vitro drug release was found within specified limit. The formulation with camphor (2%) had DT of 12 sec and drug release >90% within 5 min hence was considered as optimized formulation. The accelerated stability study and kinetics modelling was performed for optimized formulation. Conclusion: The formulated Sildenafil citrate and Ginseng ODT’s were found to be promising formulation with quicker DT and drug release which will eventually have higher bioavailability and better efficacy along with averting the issues of swallowing and improving patient compliance

    Case Study of Ayurvedic Management of Spastic Cerebral Palsy

    Cerebral palsy is a group of permanent, non-progressive motor impairment syndromes secondary to lesions or anomalies of the brain arising in the early stages of its development. It has a high prevalence rate of 2- 3 per 1000 children. Among the various types, spastic cerebral palsy remains the most common, affecting about 61% of all people with cerebral palsy. In Ayurveda, this could be taken as a Vata predominant condition and included as Sarvangavata. Vatavyadhi treatment like Snehana, Swedana, Shodana and Brimhana, along with supportive therapies could give wonderful results. An 8 ½-year-old boy, a known case of post meningeal hydrocephalus with spastic cerebral palsy was treated in the In-Patient Department of Government Ayurveda College, Thiruvananthapuram. He was treated based on the principle of Vatavyadhi chikitsa with various Ayurvedic medicines internally and appropriate Panchakarma procedures, along with physiotherapy and speech therapy. He has been assessed before and after the treatment using the Modified Ashworth Scale and Barthel Index, and noticeable improvement has been got. Here spastic cerebral palsy was managed solely using Ayurveda medicines, the apt Panchakarma procedures and supportive therapies. Significant improvements in the quality of life of the child was seen. Ayurveda treatments along with supportive therapies are highly effective in managing spastic cerebral palsy, thereby imparting a better standard of living. &nbsp

    An Outlook to Clinical Utility of Swedana chikitsa in Chikitsamanjari with Respect to Vatavyadhi

    Chikitsamanjari is a traditional book widely used in Kerala after Ashtanga Hrudaya. Kerala Ayurveda physicians use this book as a quick reference for day to day clinical practice. Even though different types of Swedana procedures are elaborated in Brihathrayis, this book explains several types of Swedas using the drugs that are widely available and that can be done easily. The text has explained simple and effective internal and external treatments for Vataja diseases. The description of various single drug remedies in the local language is another noticeable feature of this clinical treatise. Dhara with milk, Pottali Sweda with Chincha leaves (Tamarindus indica) and coconut scraping are some examples. Some of the unique contributions of Chikitsamanjari include, preparation of Dhanyamla, the procedure of Avagaha Sweda, its duration and Samyak Lakshanas, usage of different medicinal herbs for preparation of Potali Sweda in various disease conditions and Dhara using Tila Taila, milk and different combinations.     Analytical Studies on Jarajanya Vishada (Geriatric Depression) in Light of Charaka Samhita and its Management with Ashwagandha Root

    The state of Vishada (depression) which is seen during Jeernavastha (old age) can be termed as ‘Jarajanya Vishada’ in accordance with geriatric depression which is now days considered as world-wide phenomenon. Vata Dosha which becomes dominant during old age is also responsible for causation of Vishada. Geriatric depression is generally associated with higher medical morbidity and mortality, greater disability along with more neuro-psychological and neuro-radiological abnormalities which is also seen during Jeernavastha. Aims and Objectives: To evaluate the concept of Jarajanya Vishada in line of geriatric depression along with to evaluate the efficacy of the stipulated drug i.e., root of Ashwagandha (Withania somnifera Linn.) to combat the same. Materials and Methods: In selected 24 subjects of Jarajanya Vishada (geriatric depression) having inclusion and exclusion criteria based on Geriatric Depression Scale (GDS), selected therapeutic regimen (1. Shirodhara by warm decoction of root of Ashwagandha + 2. Avapida Nasya by fresh juice of Ashwagandha root along with 3. Oral intake of fresh juice of root of Ashwagandha) was administered for 30 consecutive days. Results: The result of the study revealed that, the stipulated therapeutic regimen with Ashwagandha has moderate efficacy (25% - 50%) on maximum cases of mild depression and mild efficacy (<25%) on maximum cases of major depression. Conclusion: The state of Jarajanya Vishada should be evaluated on the basis of subjective experience by the individuals suffering from it with the help of Geriatric Depression Scale (GDS). Cases of geriatric depression should be treated with drugs having Vata Shamaka and Jara Nashaka properties, like root of Ashwagandha

    Local and Internal Therapeutic Approach in the Management of Bhagna with special Emphasis to Micro-Trabecular Fractures of the Humerus Head to avert the Intended Surgery

    Sports have become more and more popular in our country, which has led to an increase in traumatic injuries whose aetiology is linked to sports injuries. When he described Bhagna Chikitsa (management of fractures), the renowned Acharya Sushruta properly identified injuries, signs and symptoms, local and internal therapies, disorders related to bones, and optimal bone healing features. Acharya Sushruta developed the fundamental principles of Bhagnasthapna (reduction of fractures), Sthirikarna (stabilisation), Anchana (traction), Bandhana (splints), Lepa (pargeting), and Parisheka (douching) in order to treat these injuries while they are still active and to rehabilitate the joint to prevent subsequent problems like joint stiffness. Trauma management has a long history that extends back to the Vedic era and is still useful today. In an effort to avoid the desired surgery, the case report of a diagnosed case of Bhagna of a 25 year young male with special reference to the micro-trabecular fracture of the humerus head that follows, further discusses the Sushruta Samhita\u27s all-encompassing approach to treating Bhagna by utilising internal drugs and local remedies. The treatment of micro-trabecular fractures only entails calcium supplements, NSAIDs, and later micro-bone grafting surgery. Purpose of Study: The key objective of the study is to effectively manage a post-traumatic micro-trabecular fracture of the humerus head refrain from performing the preferred micro-bone grafting surgery using local treatments and internal medications from the Sushruta Samhita. Observation and Conclusion: Within two days, the patient saw a considerable reduction in discomfort, and by the time the therapy was over, the patient was completely symptom-free, had no shoulder stiffness, and had experienced a large increase in the AROM (Active Range of Motion) of the shoulder joint

    Shalakya tantra has given more importance among the eight branches of Ayurveda, for dealing with the vital sense organs located above the Jatru (clavicle), also considered as Uttamanga in Ayurveda. Arshas is described as one of the Asta mahagada roga by Acharyas and also explained fourfold treatment such as Bheshaja, Shastra, Kshara and Agni karma. The Nasa arshas means Arshas located in the nasal cavity causing nasal obstruction. The predominant Dosha is Kapha and Dushya are Twak, Mamsa and Medho dhatus. Nasaarshas can be correlated with nasal polyps. Nasal polyps are non-neoplastic masses of oedematous, prolapsed, pendenculated nasal or sinus mucosa. It can be treated by medical and surgical methods. Prolonged usage of anti-histaminics, steroids, antibiotics, nasal decongestants cause drug resistance, decreases the immunity, rebound congestion and there is a lot of chance for reoccurrence, even after the surgery especially in ethmoidal polyp. In Ayurveda the treatment modalities given are Apatarpana chikitsa, Vata anulomaka, Bala and Agni vardhaka chikitsa. Here the present case study was planned to evaluate the efficacy of Yava kshara nasa pichu, Shikhari taila nasya and Shamana oushadhas. Over all there is a significant improvement in the condition. Glaucoma affects 2-3% of people over the age of 40 years 50% may be undiagnosed. primary open angle glaucoma (POAG) is the most common form, the disease is insidious and usually asymptomatic until it has caused a significant loss of visual field hence it is called as “silent thief of sight”. Glaucoma is a neurodegenerative condition that affects mainly nervous coat of eye and is associated with increased intraocular pressure when left untreated patients may gradually experience visual field loss and even lose their sight completely. It is the second leading cause of blindness around worldwide. It cannot be treated completely but it can be managed with Ayurvedic therapeutics to some extent by giving Sodhana chikista like Virechana and Raktomokshana, Kriyakalpas like Akshitarpan, Seka, Anjana, Aschyotana, Pindi, Bidalaka, Chakshsuya drugs and Mutrala drugs (diuretics) etc

    Ayurvedic Intervention of Garbhashyagata Granthi (Uterine Fibroid) - A Case Report

    Uterine fibroid is most common solid benign tumors in women of reproductive age. The symptoms of uterine fibroid adversely affect social and recreational activities, women\u27s quality of life, and productivity at work. Uterine fibroid occur in 50–60% of women, rising to 70% by the age of 50 and in 30% of cases cause morbidity. A 40-year-old married woman consulted in the outpatient department (OPD) of P.G. department of Prasutitanta and Striroga with a complaint of heavy and prolong menstrual bleeding with painful menstruation since 5 years. It is associated with lower back pain, lower abdominal pain, frequent urination, mild constipation and pain in both lower extremities. Her Ultrasonography report shows bulky uterus and intramural uterine fibroid size of 19x17x25 mm. Patient was treated according to Ayurvedic management of Granthi. So the drug having properties Raktstambhak, Lekhana, Raktshodhak, Shothahara, etc. are used with advice of Pathya Aahar-Vihara.Treatment was continued for one years with a follow up once in 15days and a repeat scan revealed shrinking size of the fibroid and relief in all symptoms

    Glimpses of Patala (Stereospermum Suaveolens roxb. Dc.) in Rasashastra: A Review

    In the universe of Ayurveda, Patala (Stereospermum suaveolens Roxb. DC.) is renowned drug credited with variety of therapeutic properties as well as other specific uses. It is also the member of much celebrated Mishraka Varga like Bruhatpanchmoola and Dashmoola. Rasashastra i.e., Indian alchemy or Iatrochemistry deals primarily with metals, minerals, gems etc. but it is inseparable from different flora and fauna. This sincere attempt helps ease out the efforts to draw information on Patala, Bruhatpanchmoola and Dashmoola from Rasashastra domain. For this purpose, information pertaining to Patala and other two herbal groups was screened from classical Rasashastra texts available at central library of ITRA, Jamnagar. Gathered information according to relevance and specificity was arranged in tabular form under the headings like name of group, processes like Shodhana, Marana, formulation name, mode of administration, dosage form etc. Total 18 groups of herbs, 164 formulations (excluding references of processes like Shodhana/Marana) were found having their therapeutic efficacy in almost 28 indications with 14 dosage forms at their disposal. Dashmoola in formulations, Jvara in therapeutic indications and Rasaushadhi in dosage forms were found in majority. Lastly general information of Patala, in the context of Dinacharya, Rutucharya, Rasashala, its simile also quoted. This review depicts multimodal applications of Patala along with its group of herbs in Rasashastra. The write-up will serve as in hand guide for the knowledge of Patala and its associated groups of herbs in the branch of Rasashastra
