Precursory Physico-Chemical, Instrumental Evaluation and Acute Oral Toxicity Study of Purandara Vati
Purandara vati is a herbomineral formulation that has a promising effect on Shwasa, Kasa, and is nothing less than a Rasayana. The main ingredients are Parada, Gandhaka, Triphala, and Trikatu which is given Bhavana in Aja kshira. The current paper aims to evaluate the physicochemical parameter along with instrumental analysis followed by an acute oral toxicity study of Purandara vati. Materials and methods: Physicochemical, instrumental analysis of Purandara vati were conducted. An acute oral toxicity study was also conducted as per OECD 425 guidelines. The IAEC no of the study is FPS/IAEC/169/2024. The study was conducted with a total of 4 healthy female Wistar albino rats with 8–10-week-old (150- 180g body weight). Results: Purandara vati has shown a pH of 4.13, loss on drying 9.50%, total ash 4.32%, acid insoluble ash 2.36%, water soluble ash 2.66%. In XRD Purandara vati had showed mercury sulphide in the major phase. FTIR analysis identified various functional groups i.e., carboxylic acid, alkane, ester etc. Acute oral toxicity studies showed no mortality at a dose of 2000mg/kg body weight proving the fact that LD50 of Purandara vati is above 2000mg/kg. Conclusion: Purandara vati is a safe and potent herbomineral formulation
Role of Triphala on Gut Microbiota in the Treatment of Obesity and its Complications
Obesity is a rapidly increasing global public health concern with a prevalence of 40.3% in India. Obesity is also a significant risk influence for ailments including hypertension, dyslipidaemia, diabetes, coronary heart condition, osteoarthritis, cancer and mental illness. This review investigates the potential role of Triphala in the treatment of obesity and its complications on gut microbiota. Method: We selected relevant articles after searching Scopus, PubMed and Google Scholar using keywords Triphala, Obesity, DM, CVD, and Gut microbiota. Obesity and related complications are considered by definite changes within the human gut microbiome. The gut microbiota of individuals with obesity contains lower proportions of Bacteroidetes and greater amounts of Firmicutes. Gut microbiota modulation and revitalization are emerging as potential obesity prevention and/or treatment strategies via probiotics, prebiotics, synbiotics, or faecal transplants. Research showed that butyrate generating bacteria like Faecalibacterium prausnitzii and Akkermansia muciniphila, were greatest in normal people, but that Bacteroides were halved in diabetes patients. The host metabolic syndrome and cardiovascular risk may be influenced by lipopolysaccharides (LPS) from gut microbiota. A placebo-controlled trial concluded that Triphala includes a hopeful role in reducing weight, circumferential measures and body fat. Triphala\u27s prebiotic effect was studied using in vitro batch cultures, fruit fly, and a simulated model of the human alimentary tract, where Triphala promoted the growth of good bacteria while inhibiting pathogenic species in each model. Clinical research is still under progress on the effect of Triphala on Stool Microbiome Profiles and Inflammation. The action of Triphala in altering the gut microbiota for effective management of obesity and its complications is yet to be studied. Here we recommend the potential action of Triphala in managing obesity and its complications by targeting gut microbiota
An Integrate Approach in Management of Diabetes Mellitus
Madhumeha comes under category of Yapya disease. There are very few medicines described in texts for Madhumeha, Svarnamaksika and Shilajita are among of them Rasayana treatment is usually beneficial in yapya diseases. Svarnamaksika is considered as one of the best Rasayana among all Rasayana.[1] Svarnamaksika improves microcirculation of Rasa dhatu because of having properties like Vyavayi and Laghu. In this case, patient was given Svarnamaksikayoga which contains Svarnamaksika and Salasaradi group which is described in Susruta Samhita. A patient 22-year-old was diagnosed as type 2 diabetes mellitus and after taking 1-year treatment of type 2 DM, she didn’t get any relief and her blood sugar was increasing. Then, she was prescribed insulin when she came for Ayurveda treatment. During first follow-up of 7 days, her blood sugar level was normal so insulin was stopped and she was kept on oral hypoglycemic agent and Ayurveda treatment. After 4 months, her blood sugar level reached to normal level and she was advised to continue Ayurveda treatment only. The collected data of this study suggest that earlier intervention of Ayurveda treatment can reverse the disease process in treatment of insulin dependent diabetes mellitus
Ayurvedic Management of Cervical Myelopathy
Cervical Myelopathy is a condition causing compression of the spinal cord at the cervical level of the spinal cord. It is mostly due to degenerative cervical spondylosis and is hence called Cervical Spondylotic Myelopathy. The spinal cord may directly experience compressive and ischemic dysfunction as a result of age-related spondylotic alterations. A 63-year-old male with complaints of gradually progressive weakness in the arms and hands along with numbness, tingling sensation, neck stiffness and pain around the neck region for the last 10 years came to the OPD of our hospital. The management offered by contemporary medicine is not satisfactory. Cervical myelopathy and Greevastambha can be co-related in this case, based on signs and symptoms. This case report demonstrates how the clinical and radiological symptoms of cervical spondylotic myelopathy were alleviated by an Ayurvedic intervention. To confirm these results and incorporate Ayurveda into conventional medicine for spinal cord injuries, more investigation and controlled trials are necessary
A Comparative Clinical Study of Mridweekadi Churna and Yavagrajadi Vati in Tundikeri (Tonsillitis) in Children
Tonsillitis is one of the common diseases in pediatric age group. In Ayurveda it can be correlated with Tundikeri, Which has been described under the Mukharoga. Description of Mridweekadi Churna is mentioned in Charaka Samhita in Trimarmiya Aadhyay under Mukha Rogas Chikitsa, and description of Yavagrajadi Vati is mentioned in Yoga Ratnakar in Mukha Roga Chikitsa Aadhyay. According to Acharya Vagbhata the main Dosha involved in Tundikeri is Kapha Dosha. Majority of ingredients of Mridweekadi Churna and Yavagrajadi Vati have Tikta, Kashaya, Laghu, Ruksha Guna, due to this it subsides Kapha Dosha. Therefore, a study was planned to compare the effect of both drug in Tundikeri (tonsillitis). Methods: Children, aged 5 to 16 years, suffering from tonsillitis were registered and categorized under two groups: group- A and group-B. Group A- Patients were treated with Mridweekadi Churna, and Group B- Patients were treated with Yavagrajadi Vati for 10 days and 7 days follow up also done by drug free period. Results: In Group-A, 55% patients were moderately improved and 45% patients showed mild improvements. In Group-B, 15% patients showed marked improvement, 75% patients were moderately improved and 10% patients showed mild improvement. Conclusion: Tonsillitis is an inflammation of the tonsils and it is a common childhood illness. During childhood period there is a natural dominancy of Kapha dosha and Tundikeri- a Kapha Pradhana disease (according to Acharya Vagbhata), hence children are more prone to it. To treat this, drugs should have the Kaphahar properties like Tikta, Kashaya ras and Laghu, Ruksha Guna. In this comparative clinical study both the chosen drugs, Mridweekadi Churna and Yavagrajadi Vati possesses all these properties and also are markedly effective in Tundikeri, but group B Yavagrajadi Vati display better results than Mridweekadi Churna. This study will also help future researchers to evaluate further in this research work
HPTLC Analysis of Aragwadhadi Kashaya and its Ingredients
Decoctions, are basically the extraction of medicines into water by means of boiling. In Ayurveda it is known by the name ‘kwatha’, ‘kashaya’,’sritha’ and ‘niryooha’. The term kwatha is derived from word ‘kwathana’ which means boiling. Aragwadhadhi kashaya is mentioned in Sharngadhara Samhitha, ingredients of which are Aragwadha, Kanamoola, Tiktha, Mustha and Abhaya. All the ingredients were checked for its identity, purity and strength and only those satisfying the API parameters were taken for the study. Kashaya was prepared as per classical method mentioned in Sharngadhara Samhitha. The prepared kashaya was lyophilised and further this extract was used for analysis. The extract of decoction and extracts of individual ingredients were analysed HPTLC was done with two different concentrations of kashaya extract and extract of five individual ingredients in a single Merck, HPTLC Silica gel 60 F254 plate. Quantification was not done in this study, but the results provided valuable information on the presence of various compounds in the decoction. Further research could focus on quantifying these compounds to better understand their concentrations and potential effects. Rf values obtained from two different concentrations of kashaya extract were not exactly similar. There were very few identical Rf values in individual ingredients and kashaya extract

Ayurvedic Treatment Modalities of Diabetic Peripheral Neuropathy
Diabetic peripheral neuropathy is the damage of the nerves due to the uncontrolled diabetes. It can affect any of the neurons can affect any of the neurons. Diabetic distal symmetric sensory and sensorimotor polyneuropathy is the most prevalent type of diabetic neuropathy. The symptoms appear as sensory loss, first in the toes and progressively moving up the legs, fingers, and arms over time. Tingling sensation, Burning sensation, Deep aching pain are other related symptoms. A patient age 54 years with diabetes for past 15 years, hypertension, dyslipidemia, complains of burning sensation of both feet (right>left), numbness, pricking pain, unsteadiness while walking, brownish red discolouration of both lower limbs. The symptoms were gradual in onset. Nerve Conduction Study showed axonal type of motor and sensory affecting peroneal nerve neuropathy. He admitted in Vaidyaratnam Ayurveda College Hospital, done Vibrotherm Neuropathy Analyzer and revealed small and large fibre neuropathy with severe loss of sensations. The main intention of the study is to improve the quality of life of the patient by reducing the symptoms that is disturbing his day today routine. Patient given with anti diabetic Ayurvedic formulations internally. Externally Rookshana, Vatahara, Brimhana treatments were done. After the treatment, Michigan neuropathy screening instrument, diabetic Neuropathy symptom score, Toronto clinical scoring system score were reduced. Neuropathy Analyzer showed mild to moderate loss of sensation. By adopting Prameha hara chikitsa coupled with Vata vyadhihara chikitsa, complications of diabetes can be managed. Evaluation of Antimicrobial Efficacy of 5% Myristica Fragrans Mouthwash in Gingivitis Subjects - A Clinico Microbiological Study
Gingivitis is the inflammation or infection of gingiva that support the teeth. The prevailing belief regarding the cause for gingivitis is the toxins produced by the bacteria in the mouth, particularly the bacteria that causes the formation of plaque. Myristica fragrans commonly known as nutmeg possesses anti-carcinogenic, anti-inflammatory activities, antioxidant effect, anti-diabetic, and antibacterial properties. The seeds of nutmeg have the ability to inhibit the growth of Gram Negative and Positive bacteria. Objective: 1. To evaluate clinical parameters at baseline and 21 days post scaling after the use of Myristica fragrans mouthwash. 2. To evaluate the colony count of Porphyromonas gingivalis in plaque samples at baseline and 21 days after the use of Myristica fragrans mouthwash. Methodology: A total of 40 gingivitis subjects were selected and divided randomly into Group A and B. Group A, Non-surgical periodontal therapy followed by Myristica fragrans mouthwash (20) and Group B, Non-surgical periodontal therapy alone (20). The supragingival plaque samples were collected at baseline and 21 days after non surgical periodontal therapy. Nuances in Ksharasutra Preparation Using Guggulu Extract
Ano-rectal diseases are considered to be one of the conditions that affect a person\u27s quality of life significantly. Ksharasutra (medicated thread) is a para-surgical procedure which is found more effective in the management of few among these conditions like fistula-in-ano and sinus. Ksharasutra is routinely prepared with Snuhi ksheera (latex of Euphorbia nerifolia), Apamarga Kshara (ash of Acyranthes aspera) and Haridra (powder of Curcuma longa). In conventional method, collection of Snuhi ksheera is time consuming, laborious and not available throughout the year. Here, Guggulu (Oleogum resin from Commiphora mukul) was selected as an alternative for Snuhi ksheera in the preparation of Ksharasutra as it is reported to have antiseptic, anti-inflammatory, wound-healing properties and it is available throughout the year. Guggulu was extracted using 95% ethyl alcohol, meanwhile Apamarga kshara was prepared in conventional way. Barbour linen thread no.20 was cut into 11 equal parts and mounted upon Ksharasutra frame. Each of which was later coated with 7,5,3 layers of Guggulu extract, combination with Apamarga kshara and combination with Haridra respectively. These were then kept under UV radiation in a cabinet and dried. After a speculated period of time the threads were cut down in a sterile condition, each (11 numbers) of them measuring 40 cm. These were then stored in an air-tight glass container. These Ksharasutras are prepared with a motive to get used in the management of the afore discussed ano-rectal conditions
