Comparative Analysis of Canonical Inflammasome Activation by Flow Cytometry, Imaging Flow Cytometry and High-Content Imaging
Inflammasome activation occurs in various diseases, including rare diseases that require multicenter studies for investigation. Flow cytometric analysis of ASC speck + cells in patient samples can be used to detect cell type-specific inflammasome activation. However, this requires standardized sample processing and the ability to compare data from different flow cytometers. To address this issue, we analyzed stimulated and unstimulated PBMCs from healthy donors using seven different flow cytometers. Additionally, human PBMCs were analyzed by fluorescence microscopy, imaging flow cytometry and high-content imaging (HCI). Flow cytometers differed significantly in their ability to detect ASC speck + cells. Aria III, Astrios EQ, and Canto II performed best in separating ASC speck + from diffuse ASC cells. Imaging flow cytometry and HCI provided additional insight into ASC speck formation based on image-based parameters. For optimal results, the ability to separate cells with diffuse ASC from ASC speck + cells is decisive. Image-based parameters can also differentiate cells with diffuse ASC from ASC speck + cells. For the first time, we analyzed ASC speck detection by HCI in PBMCs and demonstrated advantages of this technique, such as high-throughput, algorithm-driven image quantification and 3D-rendering. Thus, inflammasome activation by ASC speck formation can be detected by various technical methods. However, the results may vary depending on the device used
Novel postzygotic RASA1 mutation in a patient with Parkes Weber syndrome: A case report and literature review
Key Clinical MessageNot only germline but also postzygotic mutations in the RASA1 or EPHB4 genes can lead to capillary malformation‐arteriovenous malformation (CM‐AVM) syndrome. As it is not always possible to clinically distinguish between constitutional variants and postzygotic mosaicism, a sufficiently high sequencing depth must be used in genetic diagnostics to detect both.AbstractCapillary malformation‐arteriovenous malformation (CM‐AVM) syndrome, with or without Parkes Weber syndrome, is a rare autosomal dominant disease caused by pathogenic RASA1 or EPHB4 variants. Up to 80% of CM‐AVM cases have an affected parent. Gene panel sequencing was performed for a 4‐year‐old girl with multiple CMs, two capillary stains on the left leg, and associated overgrowth of the second toe. We also reviewed published cases with mosaic RASA1 and EPHB4 mutations. A mosaic RASA1 loss‐of‐function mutation was detected with a variant allele frequency (VAF) of 20% in the blood and oral epithelial cells of the index patient. The literature review illustrates that the severity of the clinical phenotype does not correlate with the VAF. We also identified a germline nonsense variant in the patient's TEK gene. However, inactivating TEK variants do not cause a vascular phenotype but can confer an increased risk for primary congenital glaucoma with variable expressivity. The case presented here illustrates that the choice of the sequencing depth of a diagnostic next‐generation sequencing test for CM‐AVM patients should always take mosaicism into account and that a good knowledge of the sequenced genes and associated disease mechanisms is necessary for adequate genetic counseling
Vergleich eines maschinellen Verfahrens mit manueller Wischdesinfektion zur Aufbereitung transvaginaler Ultraschallsonden in Bezug auf Kosten, Validierbarkeit und Prozessdokumentation
Zielsetzung: Mit Hilfe einer Wirtschaftlichkeitsanalyse von manueller und apparativer Aufbereitung transvaginaler Ultraschallsonden (TVUS) sollte eine Hilfestellung für die zur Aufbereitung von TVUS erforderliche Ausrüstung von Krankenhäusern und Arztpraxen gegeben werden. Begleitend sollte eine Befragung in Krankenhäusern und Arztpraxen durchgeführt werden, um die derzeitige Praxis der Aufbereitung von TVUS zu analysieren.
Methode: Die Wirtschaftlichkeitsanalyse wurde für vier Verfahren der manuellen Aufbereitung mit Wipes und ein automatisiertes apparatives Desinfektionsverfahren auf Basis von Wasserstoffperoxidvernebelung durchgeführt. Die Arbeitszeit wurden mittels Stoppuhr bestimmt und durch die passive Arbeitszeit (Einwirkungszeit der Desinfektion bzw. Zeitdauer des Programmdurchlaufs der Maschine) komplettiert. Für die aufgewendete Arbeitszeit wurden die Personalkosten kalkuliert und durch Addition der kalkulierten Anschaffungs- und Materialkosten der Gesamtprozess berechnet.
Ergebnisse: Die Wirtschaftlichkeitsanalyse ergab, dass die maschinelle Desinfektion nicht nur zeitsparender als die manuelle Wischdesinfektion ist, sondern pro Aufbereitung im Vergleich zu zwei manuellen Verfahren mit Einsatz von Tüchern zur Reinigung und zur Desinfektion, bei denen das Desinfektionsmittel vor der Anwendung auf das Tuch gegeben wird, auch kostengünstiger ist. Dagegen erwiesen sich zwei zur fortlaufenden Entnahme aus einem Behältnis befindliche rtu-Desinfektionswipes als kostengünstiger. Da für beide Wipes wegen der ausstehenden Validierung und des ungeregelten Ablaufs der Reinigung vor und nach der Desinfektion die hierfür entstehenden Kosten nicht berechnet werden konnten, ist derzeit für diese beiden Verfahren keine abschließende Kostenabschätzung möglich.
Trotz umfangreicher Bemühungen durch Versendung der Umfrage an Kliniken und Arztpraxen, an drei Fachgesellschaften und den Versuch, durch eine Veröffentlichung Teilnehmer zu akquirieren, nahmen nur 35 Einrichtungen an der Umfrage teil. Mit einer Ausnahme wurde nur manuell aufbereitet. Die Befragung ergab ein Defizit im Kenntnisstand zur Aufbereitung von TVUS. In keinem Fall war die manuelle Aufbereitung, trotz der seit 2012 existierenden nationalen Empfehlung, bisher validiert worden.
Schlussfolgerung: Solange die manuelle Aufbereitung nicht in allen Teilschritten validiert ist, ist im Interesse der Patientensicherheit nur die maschinelle Aufbereitung ethisch vertretbar.
Da die Prozesskosten bei dem Mehrtuchverfahren sowie bei dem Verfahren mit Auftragung der Desinfektionslösung auf das Wipe vor der Desinfektion höher waren als bei dem apparativen Verfahren, ist dieses gegenüber beiden Verfahren zu bevorzugen. Da bei den rtu- Desinfektionswipes die Kostenkalkulation nicht den Gesamtprozess der Aufbereitung einschließlich der Validierung abbildet, lässt sich nicht ableiten, ob der Gesamtprozess ökonomisch der apparativen Aufbereitung überlegen ist. Da die Durchführung der apparativen Aufbereitung besser standardisierbar ist, verbunden mit erhöhter Verfahrens- und Patientensicherheit, ist unabhängig von den Prozesskosten der apparativen Aufbereitung der Vorzug zu geben, zumal wenn sich die Wirtschaftlichkeit nicht erheblich unterscheidet.
Die z.T. fehlende Kenntnis der nationalen Empfehlungen zur Aufbereitung von TVUS ist als ein Grund dafür anzusehen, dass die Grundsätze der Aufbereitung in mehreren Praxen nicht eingehalten wurden. Daraus ergibt sich die Notwendigkeit der Vermittlung des Basiswissens zur Aufbereitung bereits im Medizinstudium mit nachfolgender Vertiefung in der Weiterbildung
Beeinflussung der Lebensqualität nach einem Schlaganfall: Die Rolle von Resilienz
Körperliche Behinderung und Abhängigkeit nach einem Schlaganfall stellen schwerwiegende Lebenseinschnitte für die Überlebenden dar, welche mit einer großen emotionalen Belastung einhergehen und die Lebensqualität erheblich beeinträchtigen können. Es wird vermutet, dass die Resilienz als psychische Widerstandsfähigkeit einen positiven Einfluss auf den Grad der Anpassung an eine Krankheit und deren Auswirkungen hat, was sich maßgeblich in der Genesung und dem subjektiven Wohlbefinden nach einem Schlaganfall widerspiegeln kann. Ziel der vorliegenden Arbeit ist es, die Entwicklung der gesundheitsbezogenen Lebensqualität (HRQoL) innerhalb des ersten Jahres nach einem Schlaganfall zu untersuchen und den Zusammenhang zwischen Resilienz und dem Verlauf der Lebensqualität zu betrachten. Darüber hinaus soll überprüft werden, welche Kovariaten besonders relevant bei der Vorhersage der Lebensqualität nach einem Schlaganfall sind.
Die Untersuchung der Fragestellung erfolgte mit einem Teildatensatz (N=100) der NeuroAdapt-Studie, einer zum Zeitpunkt der Analysen fortlaufenden, longitudinalen Pilotstudie der Abteilung für Präventionsforschung und Sozialmedizin des Instituts für Community Medicine und der Klinik für Neurologie an der Universitätsmedizin Greifswald. Die primäre Zielgröße HRQoL wurde mit dem EQ-5D-5L gemessen. Durch die strengen Einschlusskriterien der NeuroAdapt-Studie ergab sich eine Stichprobe mit nur moderaten Beeinträchtigungen, die unmittelbar nach dem Schlaganfall bereits hohe Werte im EQ-5D-5L aufwies. Aufgrund der stark linksschiefen Verteilung wurde die Beta-Regression zur Modellierung dieses „bounded response“ verwendet. Dabei wurden die Einflüsse der Variablen Alter, Geschlecht, Pro-Kopf-Einkommen, Bildungsabschluss und Wohnsituation der Patienten kontrolliert.
Für die meisten Patienten konnte eine bedeutende Verbesserung der HRQoL nach einem Schlaganfall beobachtet werden, insbesondere innerhalb der ersten 6 Wochen nach dem Ereignis. Patienten, die direkt nach dem Schlaganfall eine höhere Lebensqualität aufwiesen, hatten insgesamt eine günstigere Prognose für eine Verbesserung ihrer gesundheitsbezogenen Lebensqualität im weiteren Verlauf. Die vorliegende Studie zeigte einen positiven Zusammenhang zwischen der Resilienz und dem Verlauf der Lebensqualität von Schlaganfallpatienten, jedoch ist der beobachtete Effekt schwach ausgeprägt und in der untersuchten Studienpopulation von geringer Bedeutung. In einem Subsample, welches nur Patienten der unteren 80-Perzentile des EQ-Index beinhaltete, führte die Resilienz als Prädiktor dagegen zu einer signifikanten Modellverbesserung, was höhere Effekte der Resilienz bei stärker betroffenen Schlaganfallpatienten mit geringerer Lebensqualität impliziert. Alle kontrollierten Kovariablen zeigten einen Einfluss auf die Vorhersage der HRQoL nach einem Schlaganfall, wobei insbesondere die negativen Effekte eines höheren Alters sowie des weiblichen Geschlechts als klinisch relevant zu bewerten sind. Für die positiven Effekte eines höheren Bildungsabschlusses sowie Einkommens verfügt die untersuchte Studienstichprobe nicht über die nötige Power, um diese mit hinreichender Sicherheit zu bestätigen. Eine weitere Sensitivitätsanalyse, die Patienten mit negativen Verläufen der HRQoL untersuchte, zeigte ebenfalls eine starke Assoziation zwischen dem weiblichen Geschlecht und einer Verschlechterung der Lebensqualität nach einem Schlaganfall.
Die vorliegende Arbeit leistet einen Beitrag zum Verständnis des Zusammenhangs zwischen Resilienz und Lebensqualität von Schlaganfallpatienten im deutschsprachigen Raum und erweitert damit die bestehende Resilienzforschung. Dennoch sind weitere Untersuchungen notwendig, um die Validität der Ergebnisse angesichts von Limitationen in der Methodik und im Studiendesign sowie der Selektivität der Stichprobe zu überprüfen
Recombinant Anti-PF4 Antibodies Derived from Patients with Vaccine-Induced Immune Thrombocytopenia and Thrombosis (VITT) Facilitate Research and Laboratory Diagnosis of VITT
Background/Objectives: Adenoviral vector-based vaccines against COVID-19 rarely cause vaccine-induced immune thrombocytopenia and thrombosis (VITT), a severe adverse reaction caused by IgG antibodies against platelet factor 4 (PF4). To study VITT, patient samples are crucial but have become a scarce resource. Recombinant antibodies (rAbs) derived from VITT patient characteristic amino acid sequences of anti-PF4 IgG are an alternative to study VITT pathophysiology.
Methods: Amino acid sequences of the variable region of immunoglobulin light and heavy chain of anti-PF4 IgG derived from VITT patients were obtained by mass spectrometry sequencing and rAbs were synthetized by reverse-engineering. Six different rAbs were produced: CR23003, CR23004, and CR23005 (from a patient vaccinated with Jcovden, Johnson & Johnson-Janssen (Beerse, Belgium)), CR22046, and CR22050 and CR22066 (from two different patients vaccinated with Vaxzevria, AstraZeneca (Cambridge, UK)). These rAbs were further characterized using anti-PF4 and anti-PF4/heparin IgG ELISAs, rapid anti-PF4 and anti-PF4/polyanion chemiluminescence assays, and PF4-induced platelet activation assay (PIPA) and their capacity to induce procoagulant platelets.
Results: rAbs bound to PF4 alone, but not to PF4/polyanion complexes in rapid chemiluminescence assays. Chemiluminescence assays and both anti-PF4 IgG and anti-PF4 IgG/heparin ELISA showed concentration-dependent PF4 binding of all six rAbs, however, with different reactivities among them. PIPA showed a similar, concentration-dependent platelet activation pattern. rAbs varied in their reactivity and the majority of the tested rAbs were able to induce procoagulant platelets.
Conclusions: The six rAbs derived from VITT patients reflect VITT-typical binding capacities and the ability to activate platelets. Therefore, these rAbs offer an attractive new option to study VITT pathophysiology
Chemo‐Enzymatic Depolymerization of Functionalized Low‐Molecular‐Weight Polyethylene
Polyethylene (PE) is the most commonly used plastic type in the world, contributing significantly to the plastic waste crisis. Microbial degradation of PE in natural environments is unlikely due to its inert saturated carbon‐carbon backbones, which are difficult to break down by enzymes, challenging the development of a biocatalytic recycling method for PE waste. Here, we demonstrated the depolymerization of low‐molecular‐weight (LMW) PE using an enzyme cascade that included a catalase‐peroxidase, an alcohol dehydrogenase, a Baeyer Villiger monooxygenase, and a lipase after the polymer was chemically pretreated with m‐ chloroperoxybenzoic acid ( m CPBA) and ultrasonication. In a preparative experiment with gram‐scale pretreated polymers, GC‐MS and weight loss determinations confirmed ~27 % polymer conversion including the formation of medium‐size functionalized molecules such as ω‐hydroxycarboxylic acids and α,ω‐carboxylic acids. Additional analyses of LMWPE‐nanoparticles using AFM showed that enzymatic depolymerization reduced the sizes of these m CPBA‐ and enzyme‐treated LMWPE‐nanoparticles. This multi‐enzyme catalytic concept with distinct chemical steps represents a unique starting point for future development of bio‐based recycling methods for polyolefin waste
Biosensor‐Guided Engineering of a Baeyer‐Villiger Monooxygenase for Aliphatic Ester Production
Esters are valuable aroma compounds and can be produced enzymatically by Baeyer‐Villiger monooxygenases (BVMOs) from (aliphatic) ketone precursors. However, a genetically encoded biosensor system for the assessment of BVMO activity and the detection of reaction products is missing. In this work, we assembled a synthetic enzyme cascade – featuring an esterase, an alcohol dehydrogenase, and LuxAB – in the heterologous host Escherichia coli . Target esters are produced by a BVMO, subsequently cleaved, and the corresponding alcohol oxidized through the artificial pathway. Ultimately, aldehyde products are detected in vivo by LuxAB, a luciferase from Photorhabdus luminescens that emits bioluminescence upon the oxidation of aldehydes to the corresponding carboxylates. This biosensor system greatly accelerated the screening and selection of active BVMO variants from a focused library, omitting commonly used low‐throughput chromatographic analysis. Die Rolle von Fatigue bei Patient*innen mit komplexem regionalen Schmerzsyndrom
Abstract
Background and Purpose
Fatigue is a symptom of many neurological diseases and is frequently a component of chronic pain disorders. Interestingly, inflammatory processes and central sensitization seem to play a role in both fatigue and chronic pain conditions, such as complex regional pain syndrome (CRPS). Yet, to date, there has been no systematic review of the prevalence and clinical presentation of fatigue and its impact on quality of life (QoL) in CRPS. This knowledge could help improvinge clinical care for this often difficult-to-treat condition.
Methods
In this prospective study, 181 patients with CRPS and a matched control group (n=141) were interviewed. In addition to clinical information, data on depression, fatigue and QoL were collected. Statistical analysis was performed between fatigue severity and CRPS severity, pain levels and clinical CRPS phenotype, as well as examination of the largest factors influencing QoL using best subsets regression.
Results
CRPS patients showed significantly higher fatigue levels compared to controls, indicated by higher FSMC scores (CRPS: 75 [57-85] vs. controls: 39 [25-57]). 85% experienced fatigue, with 6% mild, 15% moderate, and 67% severe. Severe fatigue was associated with higher pain intensities compared to no fatigue (rest: p = 0.003; movement: p = 0.007) or moderate fatigue (movement: p = 0.03). QoL in our cohort was mainly influenced by pain (movement: adj.R²=0.38; p<0.001, rest: Δadj.R² = 0.02, p=0.007) and depression (Δadj.R² = 0.12, p<0.001). Fatigue mediated the pain-depression relationship.
Conclusions
This pioneering study investigates fatigue prevalence, its relation to pain, and its impact on QoL in CRPS patients. Severe fatigue, closely tied to pain intensity, is prevalent. While pain and depressive symptoms predominantly affect QoL, fatigue may influence it indirectly by mediating the link between pain and depression. Regular fatigue assessment using standardized methods and clinical consideration are crucial in CRPS management
Impact of public health and patient‐centered prevention strategies on periodontitis and caries as causes of tooth loss in high‐income countries
In high‐income countries, the oral health of the population is influenced by public health interventions, widespread use of oral care products, dental practice measures, and the cost of dental treatment. We compiled information on changes of the prevalence of proximal and upstream determinants of periodontitis, caries, and tooth loss over the last three decades to outline their potential effects on changes of oral health during this period. Information was retrieved from repeated cross‐sectional studies and from published literature. While both the prevalence of edentulism and the number of missing teeth (from the DMF‐T index) decreased, the number of sound teeth as well as the total number of teeth increased. The prevalence of severe periodontitis was unchanged, whereas the prevalence of periodontal health and moderate periodontitis may have increased to a minor extent. Concerning oral health risk factors, the proportion of individuals with tertiary education increased, while smoking prevalence declined. More and more people used oral care products. Whether one reimbursement system worked better than another one in terms of tooth retention could not be elucidated. In tooth retention, population‐wide use of fluoridated toothpastes had the greatest impact. To some extent, the higher number of teeth present may be related to the more frequent use of interdental cleaning aids and powered toothbrushes. Since there was no decrease in severe periodontitis in most cohorts, periodontal interventions probably contributed little to improved tooth retention
An Extremely Sensitive Ultra-High Throughput Growth Selection Assay for the Identification of Amidase Activity
In the last decades, biocatalysis has offered new perspectives for the synthesis of (chiral) amines, which are essential building blocks for pharmaceuticals, fine and bulk chemicals. In this regard, amidases have been employed due to their broad substrate scope and their independence from expensive cofactors. To expand the repertoire of amidases, tools for their rapid identification and characterization are greatly demanded. In this work an ultra-high throughput growth selection assay based on the production of the folate precursor p-aminobenzoic acid (PABA) is introduced to identify amidase activity. PABA-derived amides structurally mimic the broad class of commonly used chromogenic substrates derived from p-nitroaniline. This suggests that the assay should be broadly applicable for the identification of amidases. Unlike conventional growth selection assays that rely on substrates as nitrogen or carbon source, our approach requires PABA in sub-nanomolar concentrations, making it exceptionally sensitive and ideal for engineering campaigns that aim at enhancing amidase activities from minimally active starting points, for example. The presented assay offers flexibility in the adjustment of sensitivity to suit project-specific needs using different expression systems and fine-tuning with the antimetabolite sulfathiazole. Application of this PABA-based assay facilitates the screening of millions of enzyme variants on a single agar plate within two days, without the need for laborious sample preparation or expensive instruments, with transformation efficiency being the only limiting factor.
