The role of TRPC6 channels in renal fibrosis
Metabolic syndrome presents a significant global public health challenge, closely linked to adverse renal and cardiovascular outcomes. Inhibition of TRPC6 has been identified as a potential therapeutic approach in mitigating renal complications, particularly in a model of accelerated renal fibrosis (unilateral urethral obstruction - UUO). Hence, pharmacological inhibition of TRPC6 holds promise as a therapeutic intervention for progressive tubulo-interstitial fibrosis in metabolic syndrome and CKD. The study highlights that in vivo administration of SH045 effectively mitigates immune cell infiltration and fibrosis in NZO mice subjected to UUO, indicating SH045 as a promising therapeutic strategy for CKD.
Furthermore, truncated TRPC6 p.V691fs* mutantion does not cause FSGS. Our findings suggest that the loss of TRPC6 channel function alone may not be the sole contributor to early or late-onset FSGS. This underscores the importance of delving deeper into TRPC6 channel mechanisms and its interactions with other podocyte proteins to elucidate the pathogenesis of human FSGS.
Through integrated analysis of scRNA-Seq data generated from kidney samples from UUO mice treated with and without the selective TRPC6 inhibitor SH045, the renoprotective cell composition and cell type-specific transcriptional programs were reported. Cell-specific transcriptional signatures associated with attenuation of renal fibrosis reveal the emergence of disease-associated cell states. These results reveal global transcriptional shifts with a dramatic diversification of inflammatory cells, endothelial cells and fibroblasts. Notably, a distinct subpopulation of novel endothelial cells, termed ECRIN, was proposed to regulate inflammatory networks, and global transcriptional shifts through VEGF and GAS pathways. The data also implicate that inhibition of TRPC6 channels triggers a Prnp transcription factor regulatory network, which contributes to the alleviation of renal fibrosis. Together, these findings provide novel insights into the potential therapeutic benefits of TRPC6 inhibition in CKD.Das metabolische Syndrom stellt eine bedeutende globale Herausforderung im öffentlichen Gesundheitswesen dar und ist eng mit nachteiligen Auswirkungen für Nieren und das Herz-Kreislauf-System verbunden. Die Hemmung von TRPC6 wurde als potenzieller therapeutischer Ansatz zur Linderung von Nierenkomplikationen, insbesondere bei beschleunigter Nierenfibrose wie im UUO-Maus-Model (unilateral urethral obstruction, einseitiger Urether-Verschluss), identifiziert. Daher birgt die pharmakologische Hemmung von TRPC6 das Potenzial als therapeutischer Eingriff zur progressiven tubulointerstitiellen Fibrose bei metabolischem Syndrom und CKD. Die vorliegende Arbeit zeigt, dass die in vivo-Verabreichung von SH045 wirksam die Immunzellinfiltration und Fibrose bei NZO-Mäusen, die UUO ausgesetzt waren, mildert. Dies macht SH045 zu einer vielversprechenden therapeutischen Strategie für CKD-Behandlung im Kontext des metabolischen Syndroms.
Darüber hinaus wurde festgestellt, dass eine neue familiär auftretende TRPC6 p.V691fs* Mutation zu keiner FSGS-Symptomatik führt. Diese Ergebnisse legen nahe, dass der Verlust der TRPC6-Kanalfunktion allein möglicherweise nicht die einzige Ursache von FSGS ist. Dies unterstreicht die Wichtigkeit, den exakten Wirkmechanismus von TRPC6 sowie TRPC6-Interaktionen mit anderen Podocytenproteinen bei der Pathogenese von CKD zu klären.
Durch die integrierte Analyse aus Nierenproben von UUO-Mäusen mit und ohne Behandlung mit dem selektiven TRPC6-Inhibitor SH045 generierter scRNA-Seq-Daten , wurden die renoprotektive Zellzusammensetzung und zellspezifische Transkriptionsprogramme ermittelt. Es wurden zellspezifische transkriptionelle Signaturen entdeckt, die mit der Abschwächung der Nierenfibrose assoziiert sind. Ergebnisse zeigen globale transkriptionelle Verschiebungen mit einer dramatischen Diversifizierung entzündlicher Zellen, Endothelzellen und Fibroblasten. Besonders hervorzuheben ist, dass es eine spezifische Subpopulation von neuen Endothelzellen gibt, sogenannte ECRIN, die entzündliche Netzwerke regulieren und globale transkriptionelle Verschiebungen durch VEGF- und GAS-Signalwege verursachen können. Die Daten legen auch nahe, dass die Hemmung von TRPC6-Kanälen ein Prnp-Transkriptionsfaktor-Regulationsnetzwerk aktiviert, das zur Linderung der Nierenfibrose beiträgt. Diese Erkenntnisse bieten neue Einblicke in die potenziellen therapeutischen Vorteile der TRPC6-Hemmung bei CKD
Selbstorganisierte Netzwerke und ihr Beitrag für eine Transformation zur Nachhaltigkeit
Selbstorganisierte Netzwerke gewinnen als Akteure gesellschaftlicher Transformation in Forschung und Praxis zunehmend an Bedeutung. Als Zusammenschlüsse kooperierender Individuen und selbstverwalteter Projekte streben sie eine weitgehende Autonomie gegenüber gesellschaftlichen Rahmenbedingungen an. Selbstorganisierte Netzwerke zeichnen sich durch die kollektive Aneignung und Nutzung von Informationen sowie Ressourcen aus. Sie sind weniger als Zustand denn als Prozess zu verstehen. Sie versuchen zu vermitteln, dass es möglich ist, die Welt anders zu denken und Transformationsmöglichkeiten (er)lebbar zu machen. Die Analyse dieses Phänomens zielt darauf ab, Perspektiven, Ansprüche, politischen Praxen, Widersprüche und Realisierungsbedingungen im wechselseitigen Wirkungszusammenhang mit einer Transformation zur Nachhaltigkeit herauszuarbeiten.
In der vorliegenden Arbeit werden die theoretischen Hintergründe des Zusammenhangs von Selbstorganisierung, Nachhaltigkeit und Transformation betrachtet und anhand von drei Fallstudien (Mietshäuser Syndikat, Longo maï, Verbund offener Werkstätten) exemplarisch untersucht. Mittels teilnehmender Beobachtungen, Dokumentenanalysen, Gruppen- und Expert*inneninterviews werden in Anlehnung an den Forschungsstil der Grounded Theory Antworten auf drei zentrale Forschungsfragen gesucht:
1. Welche Praktiken und Ansprüche lassen sich in selbstorganisierten Netzwerken identifizieren und beschreiben, die einen Beitrag für eine Transformation zur Nachhaltigkeit leisten (können)?
2. Was sind Realisierungsbedingungen und Hemmnisse von selbstorganisierten Netzwerken?
3. Fördern die Praktiken selbstorganisierter Netzwerke die Entstehung und Verfestigung (weiterer) nachhaltiger Praktiken und Lebensweisen?
In der Empirie werden in einem ersten Schritt zehn Bereiche identifiziert, die mit nachhaltigen Praktiken und Ansprüchen selbstorganisierter Netzwerke in Verbindung stehen: Antikapitalismus, Bereitstellung offener Infrastrukturen, Gemeinschaftsbildung, horizontale Kommunikation und Entscheidungsfindung, kollaborative Nutzungs- und Verbrauchs-praktiken, kollektive Selbstwirksamkeit, Kultur des (Selber-)Machens, politischer Gestaltungsanspruch, solidarische Lebensführung sowie Wissenstransfer und Vernetzung. Im Vordergrund dieser Bereiche steht eine Verbindung von zugleich destituierenden und konstituierenden Transformationspraktiken, die sich durch eine zeitliche, räumliche und politische Unmittelbarkeit auszeichnen, da selbstorganisierte Netzwerke auf das augenblickliche Jetzt, Hier und Selbermachen zielen. Diese Unmittelbarkeit resultiert aus der Einsicht, dass nur durch eine vorbereitende Transformation, die in der Gegenwart ansetzt, eine nachhaltige Zukunft vorstellbar ist.
In der Praxis sind selbstorganisierte Netzwerke jedoch zunächst mit einer Reihe von Konflikten und Widersprüchen konfrontiert, die zu vielfältigen Spannungen und Kompromissen führen. In der Empirie dieser Arbeit werden diese anhand von neun Spannungsfeldern diskutiert: Autonomie versus Kollektiv, Beweglichkeit versus Formalisierung, Diversität versus Homogenität, eigene Praktiken versus Internalisation sozialer Normen, Eigenständigkeit versus Abhängigkeit, Gesellschaftsorientierung versus Exodus, Ideal versus Pragmatismus, quantitatives Wachstum versus qualitatives Wachstum sowie Selfcare versus Selbstausbeutung. Für viele der herausgearbeiteten Hürden und Widersprüche gibt es keine einfachen Lösungen. In der Folge sind selbstorganisierte Netzwerke oft intensiv mit sich selbst beschäftigt und nachhaltige Praktiken und Ansprüche drohen durch Selbstreferenzialität, Geschlossenheit, Rückzug oder Entmachtung unsichtbar und damit für eine Transformation zur Nachhaltigkeit irrelevant zu werden. Um dieser Gefahr entgegenzuwirken, bedarf es eines ausgeprägten Rahmens kollektiver und wechselseitiger Bezugnahme, in dem selbstorganisierte Netzwerke wachsen können. Zu diesem Rahmen gehören auch staatliche Strukturen. Die empirische Analyse zeigt, dass selbstorganisierte Netzwerke transformationspolitisch erfolgversprechender und nachhaltiger agieren können, wenn sie staatliche Strukturen situativ nutzen, um eigene Praktiken und Forderungen umzusetzen, Kräfte zu bündeln und Infrastrukturen aufzubauen – ohne dabei ihre selbstorganisierte Unabhängigkeit aufzugeben.
Diese Übertragung eigener Praktiken ist sowohl für die Untersuchung der Rolle von Allianzen als auch der Arbeitsweisen selbstorganisierter Netzwerke, die zur Meinungsbildung und Akzeptanz außerhalb der Netzwerke beitragen, von Relevanz. Die Rolle von Allianzen kann in vier Punkten zusammengefasst werden: Multidimensionalität, Ressourcenmobilisierung, Solidarität und Stärkung der politischen Kraft. Bei der Untersuchung der Meinungsbildungs- und Akzeptanzprozesse werden fünf Bereiche identifiziert: Beteiligung, (kreative) Kommunikation, lokaler Fokus, Ressourcenverteilung und Entwicklung von Visionen. Vor diesem Hintergrund können selbstorganisierte Netzwerke als Leuchttürme mit Ausstrahlungs- und Vorbildcharakter, aber auch als Lern- und Experimentierorte einen nicht zu unterschätzenden Beitrag für eine Transformation zur Nachhaltigkeit leisten, indem sie das Bewusstsein der Menschen für das, was möglich ist, erweitern.
Selbstorganisierte Netzwerke werden abschließend als politische Akteure beschrieben, deren strategischer Charakter in der unmittelbaren Umsetzung nachhaltiger Praktiken liegt. Ihr transformationspolitischer Erfolg hängt davon ab, ob es ihnen gelingt, ihre nachhaltigen Praktiken und Ansprüche in die Breite zu tragen. Selbstorganisierte Netzwerke sind dort am sichtbarsten und stärksten, wo sie zu einer breiten Kooperation und Vernetzung unterschiedlicher zivilgesellschaftlicher, aber auch staatlicher Akteure führen. Dies bietet nicht nur einen gewissen Schutz, sondern setzt auch eine Dynamik in Gang, die wiederum neue soziale, ökonomische oder ökologische Möglichkeiten schafft, Transformationsprozesse selbstorganisiert mit- und umzugestalten.Self-organised networks are becoming increasingly important as actors of social change in research and practice. As associations of co-operating individuals and self-managed projects, they strive for a high degree of autonomy from social conditions. Self-organised networks are characterised by the collective appropriation and use of information and resources. They try to convey that it is possible to think differently about the world and to make the possibilities of transformation viable. The analysis of this phenomenon aims to identify the perspectives, claims, political practices, contradictions and conditions for implementation in the reciprocal context of a transformation towards sustainability.
This paper considers the historical and theoretical background of the relationship between self-organisation, sustainability and transformation, and examines three case studies: Mietshäuser Syndikat, Longo maï, Verbund offener Werkstätten.
Participant observations, document analysis, group and expert interviews are used to find answers to three central research questions:
1. Which practices and goals can be identified and described in self-organised networks that (can) contribute to a transformation towards sustainability?
2. What are the conditions and obstacles for the realisation of self-organised networks?
3. Do the practices of self-organised networks promote the emergence and consolidation of (further) sustainable practices and lifestyles?
In a first empirical step, ten areas were identified that are associated with sustainable practices and goals of self-organised networks: Anti-capitalism, open infrastructure provision, community building, horizontal communication and decision-making, collaborative use and consumption practices, collective self-efficacy, do-it-yourself culture, political participation, solidarity, transfer and networking. At the forefront of these areas is a combination of simultaneously destituent and constituent transformative practices, characterised by a temporal, spatial and political immediacy, as self-organised networks aim at the immediate here, now and do-it-yourself. This immediacy stems from the recognition that a sustainable future is only conceivable through a preparatory transformation that begins in the present.
In practice self-organised networks are initially confronted with a number of conflicts and contradictions that lead to a variety of tensions and compromises. This empirical study discusses these through nine areas of tension: Autonomy versus Collective, Movement versus Formalisation, Diversity versus Homogeneity, Own Practices versus Internalisation of Social Norms, Autonomy versus Dependence, Social Orientation versus Exodus, Ideal versus Pragmatism, Quantitative Growth versus Qualitative Growth, and Self Care versus Self Exploitation. There are no easy solutions to many of these hurdles and contradictions. As a result, self-organised networks are often intensely self-absorbed, and sustainable practices risk becoming invisible through self-referentiality, closure, withdrawal or disempowerment, and thus irrelevant to a transformation towards sustainability. To counter this danger, a clear framework of collective and mutual reference is needed in which self-organised relationships and structures can grow. This includes state structures. The empirical analysis shows that self-organised networks can act more promisingly and sustainably in terms of transformation policy if they make use of state structures in order to implement their own practices and demands, to join forces and to build up infrastructures, without giving up their self-organised independence.
This extension and transfer of self-organised practices is also relevant for examining the role of alliances between self-organised networks and other civil society actors, as well as the working methods of self-organised networks that contribute to opinion-forming and acceptance outside the networks. The role of alliances can be summarised in four points: Multidimensionality, resource mobilisation, solidarity and political power building. Five areas are identified for the study of opinion-forming and acceptance processes: Enabling participation, (creative) communication, local focus, resource distribution and visioning. This means that self-organised networks, as beacons with a charismatic and exemplary character, but also as places of learning and experimentation, can make a significant contribution to a transformation towards sustainability by raising people's awareness of what is possible.
Self-organised networks can be described as political actors whose strategic character lies in the direct implementation of sustainable practices. Their success in terms of transformation policy depends on their ability to disseminate their sustainable practices and demands. Self-organised networks are most visible and strongest where they lead to broad cooperation and networking between civil society and state actors. This not only offers a certain degree of protection, but also sets in motion a dynamic that in turn creates new social, economic or ecological opportunities to shape and reshape spatial processes in a self-organised way
The bay barnacle Amphibalanus improvisus (Darwin, 1854) in the Pleistocene of Europe? A review of Pleistocene Balanidae of northern Central Europe
The bay barnacle Amphibalanus improvisus (Darwin, 1854) has been found in Northern Germany in brackish marine sediments of the Holsteinian interglacial. Whereas the cause of the contemporary distribution of this species is anthropogenic and there is evidence that it has resulted mainly from shipping, the fossil findings demonstrate that A. improvisus had also already reached Europe naturally during the Pleistocene in Europe, probably via the Gulf Stream system of the Atlantic. Today, A. improvisus is considered one of the most successful aquatic invasive species worldwide and it is characterized by a high environmental tolerance. The ingression of the Holsteinian Sea, during the Pleistocene, took place during the climate optimum from the North Sea into the ‘Elbe Estuary’ through the Hamburg region and far to the south‐east into Brandenburg. It has been possible to reconstruct the palaeo‐ecological conditions based in particular on molluscs. Accordingly, environmental conditions ranged from normal marine in the north, via brackish in the transitional area to limnic–fluviatile in the Berlin region. The morphological features of the bay barnacles and their ecological parameters are described here in detail. In addition, we provide a general review of the known Pleistocene Balanidae in northern central Europe
Does COVID-19 Vaccination Protect Contact Persons? A Systematic Review
The protective effect of COVID-19 vaccination for contact persons is controversial. Therefore, the aim of this review was to determine whether COVID-19 vaccination provides significant protection for them. A PubMed search was carried out using the terms “unvaccinated vaccinated covid” in combination with “viral load” and “transmission”. Studies were included if they reported original comparative data on the SARS-CoV-2 viral load, duration of SARS-CoV-2 detection, or SARS-CoV-2 transmission rates. A total of 332 articles were identified, of which 68 were included and analyzed. The differences in the viral load were equivocal in 57% of the 35 studies, significantly lower in the vaccinated in 11 studies and in the unvaccinated in 3 studies. The infectious virus levels were significantly lower in the vaccinated in two out of six studies. Virus clearance was significantly faster in vaccinated subjects in two of eight studies (detection of viral RNA) and two of four studies (detection of infectious virus). The secondary attack rates were significantly lower in vaccinated index cases in 6 of 15 studies. The vaccination status of contacts was described in two of the six studies and was 31.8% and 39.9% lower in households with an unvaccinated index case. The inconsistent and variable differences in the viral load, viral clearance and secondary attack rates between vaccinated and unvaccinated individuals, especially during the omicron predominance, suggests that COVID-19 vaccination is unlikely to prevent a relevant proportion of transmissions to contact persons, taking into account the relevance of the immunological status of the contact population (vaccination rates and previous infection)
The Core Curriculum in Cariology: Fiction or Reality? Challenges about Implementation
Introduction: The Core Curriculum in Cariology (CCC) was developed by ORCA and ADEE in 2010. This article summarizes challenges for the implementation of the CCC at university/country level identified at the “Education Platform” of the ORCA 2022 conference in Cagliari, Sardinia. Methods: Participants from universities from 3 European (Italy, Poland, and UK), 2 Asian (India and Russia), and 3 American countries (Brazil, Colombia, and USA) led the presentations, discussion, and generation of statements. Presentations were transcribed and summarized through qualitative content analysis. Key themes were identified, transformed into key topics, and sent to the panel for agreement. Results: Regardless of the wide variety of dental schools per country, from few (Poland n = 10) to many (India n = 318, Brazil n = 563) or from country/continent itself, frequent challenges to CCC implementation were highlighted. These included lack of agreement on a basic CCC as standard (96%), insufficient support or reimbursement for caries prevention and management (90%), separation between cariology and restorative dentistry (68%), focus on restorative/surgical management with prevention and nonoperative management being disconnected (73%). The group agreed that the integration of cariology and restorative dentistry remains essential to enhancing evidence-based decision-making, resulting in a shift of emphasis from cure to care. Conclusion: There is variation in the level of implementation of the CCC. A frequent challenge is the disconnect between cariology and restorative dentistry. The CCC should be disseminated and promoted as a uniform blueprint/framework to facilitate the implementation of a common cariology curriculum among universities within each country, as well as internationally
How do children with focal epilepsies perceive the moment they realize that they are about to have a seizure? A pilot study
Problem
A seizure is a challenging situation for children with epilepsy. Little is known regarding the experience of children who perceive in advance that they are about to have a seizure.
Methods
From September 2020 to February 2021, we invited children with focal epilepsies aged 6–18 years to participate in a semi-structured interview.
Results
Of 52 children with focal epilepsies, 22 (42 %) said they perceive in advance that they are about to experience a seizure [11 with self-limited epilepsy with centro-temporal spikes (SELECTs), 11 with other focal epilepsies]. All 22/22 (100 %) children described physical symptoms such as headache or a numb feeling in one half of the body. Of those children, 17/22 (77 %) stated they try to do something about the seizure. Those strategies were perceived as helpful by 0/11 (0 %) children with SELECTs and 9/11 (86 %) children with other focal epilepsies (p < 0.001). Of the children with SELECTs 5/11 (45 %), and of those with other focal epilepsies 9/11 (86 %) stated they would like to know in the morning if they are to experience a seizure that day (n.s.).
Conclusion
Children who perceive in advance that they are about to have a seizure are well able to describe their experience. Most children take measures to manage their seizures. Those measures were regarded as helpful by most children with other focal epilepsies, but by no child with SELECTs. Larger studies are necessary to determine the factors contributing to the child’s perception as well as the nature of the support that they require
"Du schaffst das!" Das Erleben von Schülerinnen und Schülern im Familienklassenzimmer
Die schulische Multifamilienarbeit stellt eine vielversprechende, multimodale Fördermöglichkeit im sonderpädagogischen Schwerpunkt Emotionale und soziale Entwicklung dar. Während das Konzept der systemischen Arbeit mit mehreren Familien im klinischen Setting als umfassend untersucht beschrieben wird (Gelin, Cook-Dazens & Hendricks, 2017), kann die empirische Befundlage im pädagogischen Bereich lediglich als partiell bezeichnet werden (Wuntke, Blumenthal, Köhler & Mahlau, 2023). Insbesondere die Kinderperspektiveblieb bisher weitgehend unbeachtet. In der vorliegenden Studie wurde erstmalig das Erleben von Schülerinnen und Schülern in Familienklassenzimmern in Mecklenburg-Vorpommern untersucht. Die Ergebnisse geben einen für die Praxis ermutigenden ersten Einblick darin, wie Kinder das multisystemische Präventionskonzept wahrnehmen
Electrode positioning errors reduce current dose for focal tDCS set-ups: Evidence from individualized electric field mapping
Objective
Electrode positioning errors contribute to variability of transcranial direct current stimulation (tDCS) effects. We investigated the impact of electrode positioning errors on current flow for tDCS set-ups with different focality.
Methods
Deviations from planned electrode positions were determined using data acquired in an experimental study (N = 240 datasets) that administered conventional and focal tDCS during magnetic resonance imaging (MRI). Comparison of individualized electric field modeling for planned and empirically derived “actual” electrode positions was conducted to quantify the impact of positioning errors on the electric field dose in target regions for tDCS.
Results
Planned electrode positions resulted in higher current dose in the target regions for focal compared to conventional montages (7–12%). Deviations from planned positions significantly reduced current flow in the target regions, selectively for focal set-ups (26–30%). Dose reductions were significantly larger for focal compared to conventional set-ups (29–43%).
Conclusions
Precise positioning is crucial when using focal tDCS set-ups to avoid significant reductions of current dose in the intended target regions.
Significance
Our results highlight the urgent need to routinely implement methods for improving electrode positioning, minimization of electrode drift, verification of electrode positions before and/or after tDCS and also to consider positioning errors when investigating dose–response relationships, especially for focal set-ups
Entwicklung objektiver nicht-invasiver Krankheitsaktivitätsindizes für chronisch entzündliche Darmerkrankungen bei Kindern und Jugendlichen
In der vorliegenden Arbeit wurde ein neuer Ansatz zur Entwicklung klinischer Krankheitsaktivitätsindizes für chronisch entzündliche Darmerkrankungen bei Kindern und Jugendlichen vorgestellt, der die Vorhersage der endoskopisch und histologisch vorliegenden Entzündungsaktivität ohne den Einsatz von Endoskopien und Biopsien ermöglichen kann. Mithilfe eines Bayes’schen ordinalen Regressionsmodells und der PPFS wurden das fäkale Calprotectin und das C-reaktive Protein als Prädiktoren für die endoskopisch ermittelte Entzündungsaktivität des Morbus Crohn identifiziert. Für die Colitis ulcerosa waren das fäkale Calprotectin sowie die Anzahl der Thrombozyten im EDTA-Blut die besten Prädiktoren für die histologisch erfasste Entzündungsaktivität. Mit diesen Prädiktoren wurde anschließend ein Modell entwickelt, welches mithilfe einer Shiny Internetanwendung die Wahrscheinlichkeit für das Vorliegen einer Remission, einer milden bzw. moderaten oder schweren Entzündungsaktivität vorhersagt. Dies ermöglicht dem behandelnden Ärzteteam durch die Eingabe der aktuellen Werte des C-reaktiven Proteins und des fäkalen Calprotectins (bei Morbus Crohn) bzw. des fäkalen Calprotectins und der Thrombozytenzahl (Colitis ulcerosa) auch ohne Endoskopie die Entzündungsaktivität zu ermitteln und ggf. eine entsprechende Therapieanpassung durchzuführen. Nicht zuletzt kann den Kindern und Jugendlichen die invasive Diagnostik der Endoskopie, welche in diesem Alter meist eine Analgosedierung erfordert, mit all ihren Risiken und möglichen Komplikationen häufiger erspart werden. Occurrence of new or more severe headaches following COVID-19 is associated with markers of microglial activation and peripheral sensitization: results from a prospective cohort study
Background
New onset or worsening of a headache disorder substantially contributes to the disease burden of post-COVID-19. Its management poses a suitable means to enhance patients’ participation in professional, social, and personal activities. Unfortunately, the pathophysiology of post-COVID-19 headaches is poorly understood. This study aims to investigate the role of (neuro-) inflammatory mechanisms in order to guide the development of anti-inflammatory treatment strategies.
Methods
We included patients from the interdisciplinary post-COVID-19 Rehabilitation Study (PoCoRe, n = 184 patients) run at a tertiary care university hospital, comprising patients with PCR-confirmed SARS-CoV-2 infection ≥ 6 weeks prior to their initial consultation. Patients reporting any headache since their infection were considered for this study ( n = 93). These were interviewed and classified according to the International Classification of Headache Disorders, Third Edition (ICHD-3) by headache specialists. Patient sera were additionally analysed for levels of VILIP-1, MCP-1 (CCL2), sTREM-2, BDNF, TGF-ß1, VEGF, IL-6, sTREM-1, ß-NGF, IL-18, TNF-alpha, sRAGE, and CX3CL1 (Fractalkine). Markers of inflammation were compared between four groups of patients (none, unchanged, worsened, or new headache disorder).
Results
Patients reported experiencing more severe headaches ( n = 17), new onset headaches ( n = 46), unchanged headaches ( n = 18), and surprisingly, some patients denied having any headaches ( n = 12) despite self-reports. Serum levels of CX3CL1 were increased in the worsened (2145 [811–4866] pg/ml) and new onset (1668 [0-7357] pg/ml) headache group as compared to patients with no (1129 [0-5379] pg/ml) or unchanged (1478 [346–4332] pg/ml) headaches. Other markers also differed between groups, but most significantly between patients with worsened (TGF-ß1: 60 [0-310] pg/ml, VEGF: 328 [86–842] pg/ml, ß-NGF: 6 [3–38] pg/ml) as compared to unchanged headaches (TGF-ß1: 29 [0–77] pg/ml, VEGF: 183 [72–380] pg/ml, ß-NGF: 3 [2–89] pg/ml). The results did not differ between headache phenotypes.
Discussion
This study provides evidence that worsened or new headaches following COVID-19 are associated with pro-(neuro-)inflammatory profiles. This supports the use of anti-inflammatory treatment options in this population, especially in the subacute phase
