Modifizierung von Bewegungsschmerz, kortikaler Erregbarkeit und sensomotorischer Funktionen durch Graded Motor Imagery bei PatientInnen mit komplexem regionalem Schmerzsyndrom
PatientInnen mit komplexem regionalem Schmerzsyndrom (kurz: CRPS = engl. complex regional pain syndrome) leiden häufig unter neuropathisch-anmutenden, chronischen Schmerzen. Sie zeigen meist eine Abnahme der sensomotorischen Funktionen, welche mit charakteristischen klinischen und neurophysiologischen Parametern verbunden ist.
Um weitere Erkenntnisse hinsichtlich geeigneter Biomarker für das CRPS zu erlangen, führten wir ab April 2019 eine longitudinale Interventionsstudie mit PatientInnen mit chronischem CRPS der oberen Extremität durch. Durch ein Wartegruppendesign konnten Effekte der Intervention (Graded Motor Imagery) gegenüber einer Warteperiode im chronisch stabilen Stadium der Erkrankung differenziert werden.
Dabei untersuchten wir nicht nur die Neuroplastizität mittels funktioneller Magnetresonanztomographie und die kortikale Erregbarkeit mithilfe transkranieller Magnetstimulation, sondern auch die sensomotorischen Funktionen zu den unterschiedlichen Untersuchungszeitpunkten.
Zu Beginn des Trainings verglichen wir ebenfalls die Baseline-Parameter mit denen einer in Alter und Geschlecht gematchten Kontrollgruppe.
Als „primary outcome“-Parameter war in einer prädefinierten Studienbeschreibung der Bewegungsschmerz festgelegt worden. Die unterschiedlichen Schmerzintensitäten und deren Veränderungen wurden mittels Schmerztagebüchern durch die PatientInnen erfasst. Eine Verringerung des Bewegungsschmerzes durch das Graded Motor Imagery konnte dargestellt werden.
Insgesamt konnten wir in dieser Studie charakteristische Veränderungen bei klinischen, behavioralen und neuropathologischen Parametern zeigen, welche durch das GMI-Training bei 21 PatientInnen mit CRPS der oberen Extremität entstanden. Im Speziellen zeigten sich die Stärke der S1-Aktivität während des Faustschlusses und die Abnahme der intrakortikalen Inhibition für die distale Handmuskulatur als typische Biomarker für CRPS nach dem Graded Motor Imagery
Prävalenz der Kolonisation mit Methicillin-sensiblem und -resistentem Staphylococus aureus unter Beschäftigten zweier deutscher Rettungsdienstbereiche
Background: Health care workers (HCW) in Emergency Medical Services (EMS) frequently come into contact with carriers of methicillin-susceptible Staphylococcus aureus (MSSA) and methicillin-resistant Staphylococcus aureus (MRSA) strains and may acquire and transmit them to patients. However, there is little data on MSSA and MRSA colonization of medical personnel in the emergency services. Additionally, few studies have analyzed the association between personal hygiene of staff and colonization. Therefore, we examined the prevalence of MSSA and MRSA in EMS staff of two German regions and evaluated their personal hygiene behavior.
Method: Throat and nasal swabs from 300 EMS workers were analyzed. Both direct and pre-enriched cultures of the swabs were cultivated on culture media to identify MSSA and MRSA. Results were analyzed together with questionnaires about sociodemographic data and a self-assessment of hygiene behavior. Statistical analysis was done using the R statistical software.
Results: Of the total 300 swabs, 55% were from paramedics, 39% were from emergency medical technicians (EMT) and 5% were from emergency physicians. With 1%, the MRSA prevalence was comparable to that of the German population, while the MSSA rate - 43.7% - was higher than expected. Colonization with MSSA was significantly associated with poor hand hygiene and male sex, and was inversely correlated to time on the job in EMS.
Conclusion: The sample size of 300 and a MRSA prevalence of 1% made a meaningful analysis of potential influencing factors on the prevalence of MRSA infeasible. The comparatively high prevalence of MSSA and the association with decreasing frequency of hand antisepsis suggests an influence of personal hygiene on MSSA colonization. HCW in EMS should be encouraged to make use of their personal protective equipment and practice frequent hand hygiene. The implementation of diagnostic tools such as the Hand Hygiene Self-Assessment Framework of the WHO could be utilized to reveal problems in organizations, followed by an individual program to promote hand hygiene
The Neurotrophin System in the Postnatal Brain—An Introduction
Neurotrophins can bind to and signal through specific receptors that belong to the class of the Trk family of tyrosine protein kinase receptors. In addition, they can bind and signal through a low-affinity receptor, termed p75NTR. Neurotrophins play a crucial role in the development, maintenance, and function of the nervous system in vertebrates, but they also have important functions in the mature nervous system. In particular, they are involved in synaptic and neuronal plasticity. Thus, it is not surprisingly that they are involved in learning, memory and cognition and that disturbance in the neurotrophin system can contribute to psychiatric diseases. The neurotrophin system is sensitive to aging and changes in the expression levels correlate with age-related changes in brain functions. Several polymorphisms in genes coding for the different neurotrophins or neurotrophin receptors have been reported. Based on the importance of the neurotrophins for the central nervous system, it is not surprisingly that several of these polymorphisms are associated with psychiatric diseases. In this review, we will shed light on the functions of neurotrophins in the postnatal brain, especially in processes that are involved in synaptic and neuronal plasticity
Caries Level in 3-Year-Olds in Germany: National Caries Trends and Gaps in Primary Dental Care
Background: Nationally representative long-term data on caries in the primary dentition are rare but essential for determining the need for prevention and treatment. This research assessed the prevalence and trends of dental caries in 3-year-old children across Germany, with national data analyzed and compared with the corresponding data for 6–7-year-olds.
Methods: Data were extracted from the most recent German National Oral Health Survey in 2016. Children aged 3 years were examined by calibrated dentists in 10 German regions using the WHO criteria for d3–4mft, including assessment of initial carious lesions d1–2mft. In addition, the Significant Caries Index (SiC), the Care Index (CI) and the Specific Affected Caries Index (SaC) were considered to identify provision of care and risk groups.
Results: In a total of 95,127 3-year-old preschool children, caries prevalence was 13.7% with a mean d3–4mft of 0.48. Including initial carious lesions, prevalence increased to 18.7% (mean 0.67 d1–4mft). Dependent on the German region, d3–4mft values varied noticeably from 0.38 (Schleswig-Holstein) to 0.58 (Saxony-Anhalt and Berlin). Comparing data from 3-year-olds to 6–7-year-olds, the d3–4mft value for 6–7-year-olds (1.73) was more than three times higher than that for 3-year-olds (0.48). The SiC value was 1.47 for 3-year-olds and 4.88 for 6–7-year-olds, while the SaC values were 3.57 and 3.97, respectively. The Care Index was low for both groups (26.1% and 57.5%, respectively).
Conclusions: Germany exhibited a high level of dental caries in the primary dentition for 3 (13.7%) to 6–7-year-olds (44%) children. This large cross-sectional study revealed considerable room for improvement in the early caries prevention and treatment within the well-equipped German dental health infrastructure
Die Rolle von ADC-Sequencen als MRT-Biomarker zur Diagnosestellung und Identifizierung von symptomatischen Pinealiszysten
Background: Our aim was to determine whether the Apparent Diffusion Coefficient is able to predict the presence of a
symptomatic pineal cyst by detecting cerebral edema.
Methods: We retrospectively analyzed MRIs of 45 patients with pineal cysts before and after resection and 51 patients without
pineal cysts, comparing ADC values of thalamus, central, periventricular and subcortical white matter. Furthermore we
evaluated cyst size and morphology and analyzed its correlation to ADC values in corresponding patients.
Results: Differences between patients with symptomatic pineal cyst and control group were not significant (p = 0.200 – 0.968).
ADC ratios did not change significantly after resection of the cyst (p = 0.575 – 0.862). Cyst size showed no significant correlation
to ADC ratios (p = 0.071 – 0.918). Raw data analyses revealed more significance, especially periventricularly and in
central white matter, which resulted in significant interhemispheric differences in ADC ratios in both subgroups (p < 0.001
and p = 0.031). MRI of 1.5T showed consistently higher values than 3T but mostly insignificant.
Conclusion: Our analysis revealed no evidence that pineal cysts lead to intracerebral edema caused by venous compression.
Since variability was higher than the differences seen, ADC sequences do not appear to be an appropriate diagnostic tool
for symptomatic pineal cysts.In einer retrospektiven Datenerhebung von ADC Werten verglichen wir die routinediagnostische MRT-Bildgebung von 45 Patienten mit klinisch und histopathologisch gesicherter Diagnose einer symptomatischen Pinealiszyste mit denen von 51 Patienten ohne symptomatische Pinealiszyste aber anderen cerebralen Erkrankungen, die keinen Einfluss auf das venöse und das Ventrikelsystem des Gehirns haben. Damit versuchten wir die Theorie eines venösen Abflusshindernis der internen zerebralen Venen im Zuge der raumfordernden Wirkung einer solchen Zyste als Ursache für die Pathogenese verschiedener neurologischer Beschwerden, in deren Vordergrund stärkste Kopfschmerzen stehen, zu untermauern. Als indirektes Zeichen hierfür sollten, so beschrieben Eide und Ringstad (1-3), eine erhöhtes ADC Signal in der periventrikulären, der subkortikalen weißen Substanz, oder im Thalamus als Hinweis auf ein Diffusionsdefizit dienen. Unsere Daten konnten dies nicht bestätigen. Zwischen den Patientengruppen wurden keine signifikanten Unterschiede gefunden (p = 0,120 – 0,968)
und auch keine Korrelation zwischen Größe der Zyste und scheinbarem Diffusionskoeffizienten (p = 0,265 – 0,746). Postoperativ gab es keine signifikanten Veränderungen des Signals verglichen mit den präoperativen Aufnahmen (p = 0,575 – 0,856).
In einer weiterführenden Rohdaten- und Subgruppenanalyse konnten keine wesentlichen Covariablen identifiziert und das Fehlen signifikanter Unterschiede bestätigt werden. Lediglich eine Tendenz zur Lateralisierung mit einem rechtshemispheriell in beiden Gruppen signifikant erhöhten ADC-Verhältnis zwischen oben beschriebenen Bereichen in Relation zur zentralen weißen Substanz (p = 0,031 und p < 0,001), sowie durchschnittlich höhere ADC-Werte bei einer Feldstärke von 1,5T im Vergleich zu 3T, meist aber unsignifikant, konnte beobachtet werden. Vergleicht man die Messmethoden verschiedener Autoren (2, 7, 8, 16), die den Einfluss verschiedener Pathologien auf die Diffusionsverhältnisse im Gehirn untersuchten, so findet man zumindest periventrikulär signifikante Unterschiede (p = 0,000), die keine Vergleiche und Interpretation erlauben. Da die Variabilität der ADC-Werte innerhalb der beschriebenen ROIs insgesamt höher ist als die beobachteten Unterschiede zwischen den Patientengruppen, muss festgestellt werden, dass die MR-tomografisch gemessene Diffusion intrakraniell nicht als diagnostisches Mittel zur Objektivierung der Diagnose einer symptomatischen Pinealiszyste genutzt werden kann
The intersection of medieval studies and Indigenous studies: A Norse-Saami case study
Medieval texts reveal the normalised presence of Saami peoples in medieval Fennoscandia, suggesting close interactions involving trade, relationships, rituals, and magic. Despite growing recognition of these relations, the Saami remain overlooked in general studies of the Middle Ages, often relegated to symbolic roles or footnotes. As a result, Saami characters are typically depicted as the exotic Other within Norse society, often being stripped of agency and humanity in historical narratives. To counter these biases and distorted narratives, an essential step is analysing exclusionary structures in medieval literature and critically reviewing existing research on Saami representation. This process challenges dehumanising portrayals and confronts present-day stereotypes. The present study aims to ‘re-humanize’ (as Paulette F. C. Steeves puts it) the medieval Saami past by using decolonising frameworks and perspectives offered by the so-called ‘Indigenous turn’ of medieval studies, bridging medieval studies and Indigenous studies within a Norse context
Dunkelziffer der Posttraumatischen Belastungsstörung (PTBS) - Eine empirische Untersuchung im stationären psychiatrischen Dienst
Belastende Lebensereignisse sind seit jeher ein fester Bestandteil der menschlichen Existenz. Außerordentlich belastende Erlebnisse werden auf unterschiedliche Weise verarbeitet. Eine mögliche Folge ist eine posttraumatische Belastungsstörung (PTBS). Für die Betroffenen ist es nicht selbstverständlich, über ihre Erlebnisse zu sprechen, und für die Behandelnden kann es zu einer Herausforderung werden, eine PTBS zu erkennen. Obwohl die PTBS in den letzten Jahrzehnten an Bedeutung gewonnen hat, fehlen noch immer Studien sowohl über die Anzahl der Patienten, die an einer unerkannten PTBS leiden, als auch über den Vergleich von Patienten mit unerkannter PTBS mit anderen Patientengruppen. Daher befasst sich diese Dissertation mit den Schwierigkeiten bei der Identifizierung von PTBS-Fällen in der Psychiatrie. Darüber hinaus werden mithilfe von diagnostischen Testverfahren und demografischen Daten Unterschiede zwischen den Gruppen der Patienten mit bekannter PTBS, den Patienten ohne PTBS und der interessierenden Gruppe der Patienten mit einer zum Zeitpunkt der Studie nicht identifizierten PTBS ermittelt. Mit den gewonnenen Informationen sollen eine differenziertere Betrachtung und bessere Behandlungsmöglichkeiten der PTBS ermöglicht werden. Die Erscheinungsformen der PTBS werden anhand der Posttraumatic Stress Diagnostic Scale (PDS) und der Clinician-Administered PTSD Scale for DSM-5 (CAPS-5) untersucht. Eine differenziertere Untersuchung der Stichprobe wird mit dem Beck Depression Inventory (BDI-II), der Symptom Checklist-90 Standard (SCL-90) und dem Childhood Trauma Screener (CTQ) durchgeführt. Beschrieben wird die Untersuchung von 102 Probanden, die schließlich ihre Fragebögen einreichten, von 114 Probanden, die dazu grundsätzlich bereit waren, aus einem Pool von 637 ursprünglich in Betracht kommenden Patienten. Bei 17 der 102 auswertbaren Probanden wurde eine zuvor nicht erkannte PTBS-Diagnose festgestellt, was einem Anteil von 17,89 % der Patienten im klinisch-psychiatrischen Bereich mit unerkannter und damit unbehandelter PTBS entspricht. Darüber hinaus weisen die Probanden mit unerkannter PTBS signifikant höhere Werte auf den Skalen „Emotionaler Missbrauch“, „Körperlicher Missbrauch“ und „Bagatellisierung“ des CTQ-Fragebogens auf. Diese Gruppe ist um einiges jünger als ihre Vergleichsgruppen, jedoch ohne signifikanten Unterschied. Darüber hinaus sind die Punktwerte im BDI-II für die Probanden mit nicht identifizierter PTBS höher mit einer gleichzeitig ausgeprägteren Gesamtbelastung ihrer Symptome (GSI) nach der SCL-90. Es ist jedoch kein signifikanter Unterschied zwischen den Gruppen hinsichtlich des Geschlechts festzustellen. Diese Ergebnisse verdeutlichen die Bedeutung einer umfassenden Erstdiagnose einschließlich einer sorgfältigen Traumaexploration durch gründlich geschultes Personal im psychiatrischen Alltag.Burdensome life events have always been an integral part of human existence. Extraordinarily stressful events are processed in different ways. One possible consequence is post-traumatic stress disorder (PTSD). For those affected, it is not natural to talk about their experiences, and for those treating them, identifying PTSD can become a challenge. Although PTSD has gained prominence in recent decades, studies are still lacking both on the number of patients suffering from unidentified PTSD and on the comparison of patients with unidentified PTSD with other patient groups. Therefore, this dissertation addresses the difficulties in identifying PTSD cases in psychiatry. Furthermore, with the help of diagnostic testing procedures and demographic data, differences are identified between the groups of patients who had or did not have previously known PTSD, those without PTSD, and the group of interest, patients with an unidentified PTSD at the time of the study. With the information obtained, a more differentiated consideration and better treatment options for PTSD are intended. The manifestations of PTSD will be examined using the Posttraumatic Stress Diagnostic Scale (PDS) and the Clinician-Administered PTSD Scale for DSM-5 (CAPS-5). A more sophisticated examination of the sample will be conducted using the Beck Depression Inventory (BDI-II), the Symptom Checklist-90 Standard (SCL-90), and the Childhood Trauma Screener (CTQ). The study of 102 subjects ultimately submitting their questionnaires, of 114 subjects willing to do so in principle from a pool of 637 initially in consideration coming patients is described. In 17 of the 102 evaluable subjects, a diagnosis of PTSD was found that had not been previously identified, which corresponds to a proportion of 17.89% of patients in the clinical psychiatric field with unidentified and thus untreated PTSD. In addition, the subjects with unidentified PTSD have significantly higher scores on the scales of “emotional abuse”, “physical abuse”, and “trivialization” of the CTQ-questionnaire. This group is for something younger than their comparison groups, but without a significant difference. Over and beyond, the scores in the BDI-II for the subjects with unidentified PTSD are higher with a concomitant more pronounced total burden of their symptoms (GSI) according to the SCL-90. However, there is no significant difference between the groups regarding gender to be determined. These results clarify the importance of a comprehensive initial diagnosis including careful trauma exploration by thoroughly trained staff in everyday psychiatric life
Transcriptomics und Machine Learning als neue Ansätze zur Charakterisierung und Vorhersage biofilmhemmender Aktivitäten phenolischer Naturstoffe
Angesichts zunehmender Antibiotikaresistenzen, insbesondere bei gramnegativen Bakterien, wird die erfolgreiche Behandlung von Infektionskrankheiten immer schwieriger. Durch die zunehmende Verbreitung resistenter Erreger besteht ein dringender Bedarf an neuen antimikrobiellen Substanzen. Die Natur bietet eine vielfältige Quelle für potenzielle Wirkstoffe, insbesondere phenolische Naturstoffe. Für einige der bislang bekannten mehr als 8000 Flavonoide wurden bereits verschiedene Wirkungen, auch antimikrobielle, beschrieben. Zusätzlich zur Suche neuer antimikrobiell aktiver Wirkstoffe ist auch der Fokus auf weitere Anwendungsmöglichkeiten zielführend. Dafür wurden im Rahmen dieser Dissertation sieben phenolische Naturstoffe mit sechs konventionellen Antibiotika verschiedener Klassen hinsichtlich synergistischer Aktivitäten untersucht.
Es konnte gezeigt werden, dass phenolische Naturstoffe in Kombination mit Antibiotika synergistische Effekte gegen multiresistente ESKAPE-Pathogene (Enterococcus faecium, Staphylococcus aureus, Klebsiella pneumoniae, Acinetobacter baumannii, Pseudomonas aeruginosa und Enterobacterales) aufweisen. Die Kombination von Naturstoffen wie Quercetin und EGCG mit Antibiotika wie Ampicillin, Cefotaxim oder Gentamicin zeigte in vitro additive und synergistische Effekte. Zum Beispiel wurde durch die Kombination von EGCG und Gentamicin die minimale Hemmkonzentration (MHK) für resistente A. baumannii-Stämme gesenkt, wodurch diese Stämme wieder als sensitiv nach EUCAST-Norm eingestuft werden konnten.
Ein weiterer Ansatz zur Beeinflussung von bakteriellen Erregern ist die Untersuchung von Virulenzmechanismen, beispielsweise der Biofilmbildung. Biofilme stellen aufgrund ihrer strukturellen Komplexität und inkludierter Resistenzmechanismen eine große Herausforderung dar. Die Arbeit untersuchte die Hemmung der Biofilmbildung durch phenolische Naturstoffe, indem deren Effekt auf die Bildung und Struktur von Biofilmen analysiert wurde. In einem Makrokolonieassay wurden 320 phenolische Substanzen hinsichtlich ihrer Fähigkeit getestet, Biofilme von multiresistenten E. coli zu hemmen. Dabei zeigten 16 Substanzen eine mindestens mittelstarke Hemmung der Biofilmbildung, darunter Epigallocatechingallat (EGCG) und Scutellarein.
Breit angelegte Laborscreenings zur Entdeckung neuer antimikrobieller, synergistischer und biofilminhibierender Substanzen erfordern jedoch hohe Investitionen bezüglich Zeit und Material. Ein mathematisches Modell zur gezielten Vorhersage neuer Biofilminhibitoren wurde in Kooperation mit der FU Berlin entwickelt. Dieses Modell basiert auf den erhobenen Screeningdaten und ermöglicht es, zahlreiche phenolische Naturstoffe oder deren Derivate ohne umfangreiche Laborscreenings zu analysieren. Mithilfe des Vorhersagemodells wurden ebenfalls neue Inhibitoren gefunden, darunter Octylgallat und Wedelolacton. Außerdem wurde die Auffindungsrate von aktiven Substanzen um das Siebenfache gesteigert. Die Verwendung von Machine Learning zur Selektion und Vorhersage potenzieller Wirkstoffe kann die Effizienz der Wirkstoffforschung folglich erheblich steigern und die Kosten und Zeit der Laborarbeit reduzieren.
Ein zentrales Element der Dissertation ist zusätzlich die initiale Untersuchung der Wirkmechanismen ausgewählter phenolischer Naturstoffe als Biofilminhibitoren. Durch RNA-Sequenzierung (RNAseq) wurden die Effekte von vier ausgewählten Substanzen auf das Biofilmtranskriptom analysiert. Diese Methode ermöglicht es, die dynamischen Veränderungen in der Genexpression unter dem Einfluss der Naturstoffe zu erfassen. Ergänzend dazu wurde die zeitabhängige Wirkung dieser Substanzen auf spezifische biofilmassoziierte Gene mittels quantitativer PCR (qPCR) untersucht.
Zusammenfassend bestätigt die Arbeit die Wirksamkeit von phenolischen Naturstoffen in Kombination mit Antibiotika zur Bekämpfung multiresistenter ESKAPE-Pathogene und zur Hemmung von Biofilmen. Sie zeigt auch die Notwendigkeit einer interdisziplinären Herangehensweise, die moderne Technologien wie Machine Learning integriert, um die Effizienz der Wirkstoffforschung zu steigern. Diese Ansätze bieten das Potenzial, neue und effektive Behandlungsoptionen gegen resistente Infektionserreger zu entwickeln, die einen bedeutenden Beitrag zur öffentlichen Gesundheit leisten können.In view of increasing antibiotic resistance, particularly in gram-negative bacteria, the successful treatment of infectious diseases is becoming more and more difficult. Due to the increasing spread of resistant pathogens, there is an urgent need for new antimicrobial substances. Nature offers a diverse source of potential active substances, especially phenolic natural products. Various effects, including antimicrobial effects, have already been described for some of the more than 8000 flavonoids known to date. In addition to the search for new antimicrobial active substances, the focus on further possible applications is also expedient. In this dissertation, seven phenolic natural substances were analysed with six conventional antibiotics of different classes with regard to synergistic activities.
It has been shown that phenolic natural substances in combination with antibiotics have synergistic effects against multi-resistant ESKAPE pathogens (Enterococcus faecium, Staphylococcus aureus, Klebsiella pneumoniae, Acinetobacter baumannii, Pseudomonas aeruginosa and Enterobacterales). The combination of natural substances such as quercetin and EGCG with antibiotics such as ampicillin, cefotaxime or gentamicin showed additive and synergistic effects in vitro. For example, the combination of EGCG and gentamicin lowered the minimum inhibitory concentration (MIC) for resistant A. baumannii strains, allowing these strains to be reclassified as sensitive according to the EUCAST standard.
Another approach to influencing bacterial pathogens is the investigation of virulence mechanisms, such as biofilm formation. Biofilms pose a major challenge due to their structural complexity and included resistance mechanisms. This work investigated the inhibition of biofilm formation by phenolic natural products by analysing their effect on the formation and structure of biofilms. In a macrocolony assay, 320 phenolic substances were tested for their ability to inhibit biofilms of multidrug-resistant E. coli. Sixteen substances showed at least moderate inhibition of biofilm formation, including epigallocatechin gallate (EGCG) and scutellarein.
However, large-scale laboratory screenings to discover new antimicrobial, synergistic and biofilm-inhibiting substances require high investments in terms of time and materials. A mathematical model for the targeted prediction of new biofilm inhibitors was developed in cooperation with the FU Berlin. This model is based on the screening data collected and makes it possible to analyse numerous phenolic natural substances or their derivatives without extensive laboratory screening. The prediction model was also used to identify new inhibitors, including octyl gallate and wedelolactone. In addition, the detection rate of active substances was increased sevenfold. The use of machine learning to select and predict potential compounds can therefore significantly increase the efficiency of drug discovery and reduce the cost and time of laboratory work.
An additional central element of the dissertation is the initial investigation of the mechanisms of action of selected phenolic natural substances as biofilm inhibitors. The effects of four selected substances on the biofilm transcriptome were analysed by RNA sequencing (RNAseq). This method makes it possible to record the dynamic changes in gene expression under the influence of the natural substances. In addition, the time-dependent effect of these substances on specific biofilm-associated genes was analysed using quantitative PCR (qPCR).
In summary, the work confirms the efficacy of phenolic natural products in combination with antibiotics to combat multidrug-resistant ESKAPE pathogens and to inhibit biofilms. It also demonstrates the need for an interdisciplinary approach that integrates modern technologies such as machine learning to increase the efficiency of drug discovery. These approaches offer the potential to develop new and effective treatment options against resistant infectious pathogens that can make a significant contribution to public health
Effect of non-invasive transcutaneous auricular vagus nerve stimulation on cerebral motor excitability—Study protocol for a randomized, sham controlled trial
Transcutaneous auricular vagus nerve stimulation (taVNS) is becoming increasingly established in the treatment of various neurological and psychiatric diseases. However, only a few studies have focused on the overall influence of taVNS on cortical excitability in general. The planned study will investigate the effect of taVNS on the excitability of the motor cortex in young healthy subjects. The aim of the study is to gain better understand of the physiological mechanism of taVNS to contribute to new fields of application of taVNS in new areas such as the treatment of stroke or multiple sclerosis. This protocol describes a single-center, prospective, double blind, sham-controlled trial that evaluates the effect of taVNS on motor cortex excitability with a planned sample size of 30 participants. The effect of taVNS is investigated by neuronavigation and electromyography (EMG) coupled transcranial magnetic stimulation (TMS) applied before and after taVNS stimulation. The following parameters are assessed: resting motor threshold (RMT), active motor threshold (AMT), recruitment curve (RC), short intracortical inhibition (SICI), intracortical facilitation (ICF). All parameters will be assessed for taVNS on the basis of perception threshold and tolerance threshold. All investigations performed in the study were reviewed and approved by the local ethics committee of the University Medical Center Greifswald (study reference number: BB048/22).

Green Closed-Loop Supply Chain Management under Uncertainty and Fuzziness "A Robust Fuzzy Scenario-based Stochastic Approach"
This study investigates a closed-loop supply chain network design and planning problem, integrating environmental goals and resilience strategies using a novel columnwise robust optimization approach combined with fuzzy and stochastic programming methods. The closed-loop supply chain network encompasses all processes and activities, including supply, production, recycling, and transportation, emphasizing environmentally friendly criteria at both the customer and supply chain levels. The research presents a quantifiable green scale value using the customer greenness tendency index and the supply chain greenness level to optimize environmental objectives while minimizing the overall costs within the supply chain network.
The study adopts five resilience strategies to mitigate supply and production disruptions: backup supplier contracting, raw material prepositioning, and multiple sourcing for supply disruptions. Final product prepositioning and backup production facility strategies are utilized for production-related disruptions. To address disruption and uncertainties, robust optimization approaches are used through a hybrid robust-fuzzy-stochastic programming method, incorporating two-stage scenario-based stochastic programming, fuzzy chance-constrained programming, and a novel robust columnwise uncertainty approach. The robust column-wise approach shifts from the constraint-wise perspectives to a columnwise approach, addressing budget uncertainty independently for each constraint. The augmented epsilon constraint method is used as a solution approach for the bi-objective mathematical model, with implementation examples solved using the GAMS solver.
Practical applications of this approach demonstrate its effectiveness in improving supply chain robustness and adaptability through parameter-tuning exercises and scenario analyses. This research contributes to green supply chain management by addressing gaps such as customer behavior, supply chain greenness performance, energy resource modes, hybrid formulations for uncertainties, resilience strategies, and demand uncertainty management. These insights guide supply chain practitioners toward more environmentally responsible and resilient practices, emphasizing the importance of incorporating customer preferences and highlighting the economic implications of prioritizing customer satisfaction
