The development of dosage forms is a core task of pharmaceutical technology. At best, it follows the latest findings from various medical and technical fields in order to use ever more advanced methods to ensure a more targeted, safer drug therapy that is specifically tailored to the patient and the application site. As a result, pharmaceutical technologists have a constantly growing repertoire of manufacturing processes and research methods at their disposal, from which innovative dosage forms and aids can be developed through subsequent further development. 3D printing is one of these technologies and, in the first part of this work, enabled the production of flexible and foldable dosage forms from various thermoplastic polyurethanes. The manual folding of the different geometries into a gelatine capsule made it possible to reduce the outer dimensions and thus functionally enable oral application. The combination of the flexible and folded objects with a capsule led to a hybrid shape memory system, which was characterised by the preservation of the compressed (secondary) structure until the capsule shell was released. The shape changes undergone could be observed and quantified using a test apparatus developed for this purpose in order to investigate material- and shape-dependent influences on the unfolding behaviour. The shapes tested here - cloverleaf, spring, wafer, ring and window - had an influence on the expansion time and dimensional recovery, i.e. the degree to which the original 3D-printed shape was achieved again after folding. As expected, the Shore hardness of the thermoplastic polyurethanes used had only a minor influence on these values. Instead, a higher wall thickness of the objects appeared to provide increased rigidity, which ensured rapid exit from the capsules and more complete expansion. In addition to the possible expansion to dimensions larger than the open pylorus, the flexible character of these dosage forms could represent a new mechanistic approach to achieving gastroretention, which is based on observations from studies with a simulated antrum. However, further investigations are required, particularly with regard to the choice of material and the mandatory gastric emptying. A spinning apparatus developed in the second part of this thesis for the production of pharmaceutical threads and yarns represents a further possibility for developing flexible and elastic dosage forms or components thereof. A three-part system comprising a melt extruder, stretching unit and actual spinning apparatus was used to produce a multifilament yarn consisting of polyvinyl alcohol and glycerol as a plasticiser. A specially developed 90° deflection with a stainless steel perforated plate allowed the molten powder mixture to be extruded towards the stretching unit, which further refined the hair-like filaments by stretching them. The spinning apparatus, also designed with 3D-printed components, was based on the principle of a typical spinning wheel and spun the individual threads into a yarn. Due to the hygroscopic properties of polyvinyl alcohol and glycerol, storage conditions and storage time had an influence on the physical properties of the yarn. Increased humidity led to a decrease in tensile strength and an increased tendency to stretch. Storage for over a year at low humidity even led to an increase in tensile strength, while the yarn remained flexible to the touch. By conducting a three-arm MRI study, it was also possible to contribute to the acquisition of important biopharmaceutical findings that are relevant for understanding the distribution processes of substances in the gastrointestinal tract and thus also for the development of future dosage forms. By adding manganese(II) gluconate as a marker and hydrophilic but poorly absorbable model substance to drinking water, the course of the orally ingested solution through the gastrointestinal tract could be observed using T1-weighted MRI sequences. This made it possible to differentiate between orally ingested solution and secreted fluid present in the body. Gastric emptying and distribution of manganese(II) ions occurred in a similar manner both on an empty stomach and after ingestion of a low- and high-calorie test meal. Distribution in the small intestine was usually completed after 30 minutes. These results suggest that the stomach road already known in the literature is continued in the small intestine (small intestinal road) and liquids flow past the food pulp until they are absorbed by the high absorption force and large surface area of the small intestine. Consecutive small intestinal passage times of several hours through the various sections of the small intestine, as is usually assumed for in silico models, cannot be confirmed on the basis of these data. One possible cause of the reduced bioavailability of drugs with absorption windows in the upper small intestine when taken together with food (negative food effects) may be the accelerated intestinal transport of dissolved substances in the postprandial state

This thesis studies and compares the growth rates of three scenarios with varying levels of internalisation of pollution, whereby pollution is assumed to be an externality of wealth. Using an endogenous growth model, it is assumed that the cross-derivative of utility between consumption and pollution is positive, which is also referred to as the compensation effect. In the first scenario, no internalisation takes place; households assume the environmental quality to be constant regardless of their behaviour. In the second scenario, households take into account the increase in pollution but ignore its cause. The third scenario represents the social optimum; households internalise both the increase in pollution and the connection between their wealth and pollution. The third scenario analyses the three cases where pollution increases proportionally, under and over proportionally with increasing wealth. A comparison of the three scenarios shows that with the compensation effect as the driving force, growth is highest in the second scenario, as households react to pollution by consuming more in order to mitigate the disutility of pollution. By contrast, growth is lowest in the third scenario. Although households have an increased consumption desire due to the compensation effect, they also take into account that the accumulation of wealth for consumption results in increasing pollution. Overall, this leads households to forego growth in order to protect the environment. However, positive growth is still possible in the social optimum provided that the pollution associated with wealth is sufficiently low. This holds true even in the case that pollution increases over proportionally with increasing wealth. In addition to the realisation that pollution must lead to a reduction, but not necessarily to a rejection of growth, the work shows that adaptation to the consequences of pollution and mitigating the causes must be pursued together. Focusing only on adaptation leads to suboptimal growth

Gender and Socioeconomic Influences on Ten Pro-Environmental Behavior Intentions: A German Comparative Study

    Get PDF
Pro-environmental behaviors (PEBs) such as climate-friendly mobility and eating habits hold great promise in terms of reducing greenhouse gas (GHG) emissions and, thus, are important goals for addressing climate change from a population perspective. Yet, sociodemographic correlates and differences in PEB intentions have to be considered in designing messages and behavior change interventions. This study implemented a quota-sampling survey (N = 979, 511 women, 468 men, age M = 50.4, SD = 17.2) of the German population and found that, overall, participants exhibit strong intentions to engage in various PEBs, with the exception of cycling and adopting a vegetarian diet. Moreover, women displayed higher intentions to engage in PEBs compared to men, particularly in adopting a vegetarian diet. The relationships between socioeconomic status (SES) and PEB intentions, as well as the combined effects of gender and SES, were inconsistent for different PEB intentions. We conclude that on a population level, intention-building interventions are necessary for vegetarianism and cycling, while for the other PEBs, interventions may focus on closing the intention–behavior gap. There is a need to further research the interplay of different PEBs in diverse groups and for interventional studies targeting the discrepancy in eating habits across genders

Navigating the AI frontiers in cardiovascular research: a bibliometric exploration and topic modeling

    Get PDF
Artificial intelligence (AI) has emerged as a promising field in cardiovascular disease (CVD) research, offering innovative approaches to enhance diagnosis, treatment, and patient outcomes. In this study, we conducted bibliometric analysis combined with topic modeling to provide a comprehensive overview of the AI research landscape in CVD. Our analysis included 23,846 studies from Web of Science and PubMed, capturing the latest advancements and trends in this rapidly evolving field. By employing LDA (Latent Dirichlet Allocation) we identified key research themes, trends, and collaborations within the AI-CVD domain. The findings revealed the exponential growth of AI-related research in CVD, underscoring its immense potential to revolutionize cardiovascular healthcare. The annual scientific publication of machine learning papers in CVD increases continuously and significantly since 2016, with an overall annual growth rate of 22.8%. Almost half (46.2%) of the growth happened in the last 5 years. USA, China, India, UK and Korea were the top five productive countries in number of publications. UK, Germany and Australia were the most collaborative countries with a multiple country publication (MCP) value of 42.8%, 40.3% and 40.0% respectively. We observed the emergence of twenty-two distinct research topics, including “stroke and robotic rehabilitation therapy,” “robotic-assisted cardiac surgery,” and “cardiac image analysis,” which persisted as major topics throughout the years. Other topics, such as “retinal image analysis and CVD” and “biomarker and wearable signal analyses,” have recently emerged as dominant areas of research in cardiovascular medicine. Convolutional neural network appears to be the most mentioned algorithm followed by LSTM (Long Short-Term Memory) and KNN (K-Nearest Neighbours). This indicates that the future direction of AI cardiovascular research is predominantly directing toward neural networks and image analysis. As AI continues to shape the landscape of CVD research, our study serves as a comprehensive guide for researchers, practitioners, and policymakers, providing valuable insights into the current state of AI in CVD research. This study offers a deep understanding of research trends and paves the way for future directions to maximiz the potential of AI to effectively combat cardiovascular diseases

The role of different acetabular morphologies on patient-reported outcomes following periacetabular osteotomy in borderline hip dysplasia

    Get PDF
Introduction The treatment option for borderline hip dysplasia (BHD) includes hip arthroscopy and periacetabular osteotomy (PAO). To the present day the controversial discussion remains, which intervention to prefer. Literature reports supporting an educated choice are scare, based on small patient cohorts and do not address the variability of acetabular morphology. Consequently, we intended to report PAO outcomes, from patients diagnosed with BHD, dependent on acetabular morphology, in a large patient cohort and aimed to define risk factors for poor clinical results and patient satisfaction. Materials and methods A prospective monocentre study was conducted. Patients enrolled underwent PAO for symptomatic BHD (LCEA, 18°–25°). A total of 107 hips were included with 94 complete data sets were available for evaluation with a minimum follow-up of 1 year and a mean follow-up of 2.3 years. The mean age was 31 ± 8.2 years, and 81.3% were female. As the primary outcome measure, we utilized the modified Harris hip score (mHHS) with minimal clinically important change (MCID) of eight to define clinical failure. Results were compared after a comprehensive radiographic assessment distinguishing between lateral deficient vs. anterior/posterolateral deficient acetabular and stable vs. unstable hip joints. Results Overall, clinical success was achieved in 91.5% of patients and the mHHS improved significantly (52 vs. 84.7, p < 0.001). Eight hips failed to achieve the MCID and four had radiographic signs of overcorrection. Comparing variable joint morphologies, the rate of clinical success was higher in patients with an anterior/posterolateral deficient acetabular covarage compared to lateral deficient acetabular (95.2% vs. 90.4%). tThe highest rate of clinical failure was recorded in unstable hip joints (85.7% vs. 92.5% in stable hips). Conclusions This study demonstrates that PAO is an effective means to treat symptomatic BHD with variable acetabular morphologies, achieving a clinical success in 91.5% of all patients. To maintain a high level of safety and patient satisfaction technical accuracy appears crucial

France’s militarized interstate disputes: diversion from domestic problems?

    Get PDF
A large body of research investigates whether democracies’ decisions to intervene militarily are guided by external threats and strategic considerations or whether domestic factors play the predominant role. Foreign military intervention can create a “rally-round-the-flag” effect and might be used to divert attention away from domestic problems. A reassessment of diversionary theory for France, as one of the most militarily active European powers, which has hardly been subject to rigorous empirical research so far, is not supportive of the theory’s theses. There is only weak support that France is more likely to intervene in months with low presidential popularity and no evidence of a systematic rally effect after France gets involved in military disputes. With few exceptions, presidential approval ratings do not improve

Long-term patterns of forearm asymmetry in females of three syntopic bat species and its effects on individual fitness

    Get PDF
Fluctuating asymmetry, the non-directional deviation from bilateral symmetry resulting from developmental instability, can indicate early-life environmental stress. While fluctuating asymmetry can affect individual survival and reproductive success, its effect on fitness differs between species. Here, we analyzed up to 27 years of mark-recapture data from 894 RFID tagged individuals of three forest-living bat species in southern Germany to investigate the degree of fluctuating asymmetry in forearm length. In Bechstein’s bats, Myotis bechsteinii , the species with the highest sample size, we furthermore investigated if fluctuating asymmetry has become more frequent over the study period, a time when juvenile bats have grown larger forearms in response to warmer summers. We also investigated whether fluctuating asymmetry affects individual lifespan and lifetime reproductive success in female Myotis bechsteinii . The degree of fluctuating asymmetry clearly exceeding the measurement error estimated on recaptured individuals was similar in all three species (1.8%). In female Myotis bechsteinii , the frequency of fluctuating asymmetry did not increase over the course of the study and even strong asymmetry had no effect on individual reproductive success and life expectancy. Our data suggest that fluctuating asymmetry is a poor predictor of fitness in the female Myotis bechsteinii studied, and is so far unaffected by the warming environment which is leading to larger individuals in our study population

The extension of the approval of dupilumab for the treatment of severe atopic dermatitis in children 6 months of age and older in Germany creates a potential conflict with the administration of live attenuated vaccines. According to the product information, the administration of live attenuated vaccines is contraindicated during ongoing dupilumab therapy. This position paper, written by specialists in pediatric immunology and allergology from Germany and Switzerland, aims to support pediatricians to provide their patients with the best possible treatment with dupilumab and appropriate vaccinations based on the currently available evidence, including statements and advice for clinical situations. The practical implementation of these statements requires a differentiated approach. The position paper covers the situation in Germany, with special attention to the recommendations of the STIKO (Standing Committee on Vaccination) and the Robert Koch Institute for German-speaking countries and the legal situation here
