Model to explain dental visit for children aged 0 to 5: Scoping review of birth cohorts
International audienceIntroduction: Health services accessibility is a multidimensional concept. An early-life dental visit could improve child dental health. Through birth cohorts, it is possible to identify health conditions and pathways of exposure that occur earlier in life. The aim of this study is to propose a theorical model to explain the use of dental care for children with primary teeth, based on results from birth cohorts. Method: 3 databases were queried: PubMed, Embase and Dentistry & Oral Sciences Source. Eligible articles presented data on children’s dental visits, with at least one follow-up visit between birth and the child’s 6th birthday and based on birth cohorts. Results: We identified 649 articles in biomedical literature databases. After exclusions, we read 136 abstracts, and finally 36 articles in their full length. A total of 22 articles were included in the analysis, from 15 countries on 5 continents. The mains proximal factors for access to dental care for preschool children are related to caregivers’ perception of children’s oral health and its impacts on quality of life. These perceptions are influenced by the child’s oral health, the child’s and mother’s use of healthcare, and the healthcare organization. Dental fear seems to be another proximal factor. However, family social background seems to be an enabling moderator for dental visits. Conclusion: The scoping review allowed us to develop a model that explains dental visits for children aged 0–5 years as a multifactorial process influenced by caregivers’ perceptions of the child’s oral health, the family’s quality of life, and the child’s dental anxiety
Rescaling method for blow-up solutions of nonlinear wave equations
20 pages, 5 figures, 2 tablesInternational audienceWe develop a hybrid scheme based on a finite difference scheme and a rescaling technique to approximate the solution of nonlinear wave equation. In order to numerically reproduce the blow-up phenomena, we propose a rule of scaling transformation, which is a variant of what was successfully used in the case of nonlinear parabolic equations. A careful study of the convergence of the proposed scheme is carried out and several numerical examples are performed in illustration
International audienceThis article looks at the French Communist Party’s relationship with “le music-hall” between the end of the World War II and 1958, a period paradoxically marked both by the permanence of its policy of cultural openness begun in the 1930s and by the echoes of the Cold War in its musical apparatus. The Communist movement promoted choral singing, as well as great musical and theatrical frescoes composed and written by fellow travelers, embodying an aesthetic, themes and a musicking in line with its current doctrine and propaganda, and opposed to both the “formalism” and “uprooted cosmopolitanism” of “bourgeois music” and mainstream productions. It also perpetuated its support to sympathizing performers of French chanson and variety shows, the most representative and popular being Yves Montand. The failure of the party’s counter-cultural strategy, and the evolution of the singer’s ties with the Communist Party, from fellow traveler to harsh critic following the 1958 break-up, reveal the political and cultural crisis the movement was undergoing in the late 1950s – the prelude to its cultural aggiornamento.
International audienceWhen Napoleon decided in 1802 to build the Ourcq Canal to supply Paris with water, it was necessary to determine what the legal nature of the future hydraulic network would be. It would integrate not only the canal but also all the old equipment, built under the Ancien Régime (aqueducts, fountains, mechanical pumps and steam pumps, etc.).Who should own it? What principles should guide its management? Some, who had a capitalist culture, wanted to make it a communal property whose management would be transferred to a Company.Others, attached to a traditional princely liberality, argued in favor of an imperial property. But both sides gradually realized that the clean slate was going to be difficult: the oldest part of the network hosted property rights acquired by certain individuals over the water distribution service, rights that had become inviolable with the Revolution. The "capitalists" tried to obtain the destruction of the old equipment, to make these rights disappear, in vain. The ownership of the Parisian hydraulic network was then abandoned and, until the middle of the Second Empire, it was only administered. The article allows us to question the limits of revolutionary clean slates, the opportunity to create (or not) "public properties", the place of hydraulic policies in the process of building the legitimacy of States, etc.
Global approaches of trigonometric and hyperbolic inequalities
International audienceIn this paper, we introduce a new look of families for inequalities depending on parameters. These inequalities are extending the classical ones
International audienceThis chapter looks at the growing mismatch between the labour force and employment in Brunei Darussalam and the policies that could lead to a better fit. The discussion is conducted from an international perspective to better understand the problems common to countries abundant in natural resources and the specificities of Brunei. Section 4.2 starts with an analysis of the demographic trends which shows that the labour force will peak around 2030 and decline slowly afterwards. Unemployment will get less severe in the long term. However, the total dependency rate will become more acute, which raises the issue of financing the social cost of an ageing population. In Sect. 4.3, we describe the highly segmented job structure that has been shaped by the Bruneian growth regime and the social context it has created. We look at unemployment and underutilisation of the labour force and show that labour market segmentation is one of their root causes. Section 4.4 is dedicated to a second driver of unemployment: the lack of diversification of the economy, despite the long-standing efforts of the Brunei government. We conclude with a discussion of the policies that could be adopted to contribute to better diversification of the economy that turns its small size into the basis for a new sustainable growth regime. Le chômage deviendra moins grave à long terme.
Understanding health innovation adoption: A realist evaluation of pulse oximeter implementation in primary care for children under five in four West African countries
Introduction. Hypoxemia is an important contributor to child mortality, particularly in low-resource settings where diagnostic tools are scarce. The AIRE project introduced pulse oximeters (POs) into 202 primary healthcare centres (PHCs) in Burkina Faso, Guinea, Mali, and Niger, integrating them into the Integrated Management of Childhood Illness (IMCI) guidelines. This initiative aimed to strengthen diagnostic capacities for identifying hypoxemia and to improve care management for critically ill children under five. This study examined how healthcare workers (HCWs) adopted POs and explored the contexts and mechanisms influencing their adoption. Methods. We conducted a realist evaluation to analyse adoption patterns, focusing on interactions between the Intervention, Contexts, Actors, Mechanisms, and Outcomes (ICAMO configurations). Data collection included 299 interviews with HCWs, patients' families, and institutional actors, conducted in 16 selected PHCs, at the institutional level, and in district hospitals, complemented by site observations. Analysis was performed using NVivo software, identifying ICAMO configurations as demi-regularities to explain variations in PO use and adoption. Results. Training enabled HCWs to recognize the utility of POs, further motivating their use. Support-focused supervision fostering a sense of support, while control-focused approaches sometimes resulted in mechanical use driven by external pressure. In contexts of high workloads and children's agitation, difficulties in using POs were observed. In settings with limited diagnostic tools, POs increased HCWs' diagnostic confidence, encouraging adoption and improving decision-making. Observing or knowing the benefits of POs on children's health provided HCWs with a sense of relief and pride, further reinforcing PO adoption. However, structural barriers and challenges related to institutional adoption may threaten long-term use. Conclusion. This study highlights the necessary conditions for adopting POs in PHCs. While widely used by healthcare workers, addressing challenges related to training, supply chain logistics, and referral systems to hospitals is essential to ensure long-term sustainability and improve child health outcomes
