Prescription of ACE-Is/ARBs in patients with cardio-renal disease: a multicenter retrospective cohort study from the REPOSI registry
Cardio-renal disease is a common clinical condition leading to increased morbidity and mortality. Angiotensin-converting enzyme inhibitors (ACE-Is) and angiotensin II receptor blockers (ARBs) are the cornerstone of treatment of chronic cardio-renal disease. Using data from the REPOSI register, we performed a multicenter, observational, retrospective study to determine which factors are associated with the non-prescription or discontinuation of ACE-Is/ARBS in a cohort of 889 cardio-renal patients hospitalized in 109 Italian internal medicine and geriatric wards. Only 55% of the patients with cardio-renal disease of the investigated cohort were on treatment with ACE-Is or ARBs at admission. The primary end point was ACE-Is/ARBs use at discharge. Patients with lower probability of receiving ACE-Is/ARBs at discharge were older and hospitalized for longer periods. Furthermore, patients with advanced chronic kidney disease (advanced CKD: eGFR ≤ 29 mL/min/1.73m2) had a much lower (54%) probability of being discharged or continuing ACE-Is/ARBs treatment than those with eGFR ≥ 60 mL/min/1.73m2. A more prominent lower probability was found comparing advanced CKD patients with G3 stage CKD patients (eGFR: 59–30 mL/min/1.73m2) in multivariate analyses (OR and 95%CI: 0.37, 0.24–0.57. multivariate p-value < 0.001). The probability of stopping treatment in patients already on treatment with ACE-Is/ARBs at hospital admission (secondary end point) almost reached a threefold increase (OR and 95%CI: 2.82, 1.69–4.71. multivariate p-value < 0.001) when the advanced CKD group was compared with G3 CKD patients. The data of our study are not in line with the recently published updated KDIGO 2024 Guidelines, which recommend patients with advanced CKD to continue treatment with ACE-Is/ARBs
Reliability assessment of SiC power MOSFETs in dynamic reverse bias test
Novel reliability tests are being developed for power semiconductor devices, especially for those based on wide band-gap materials, such as silicon carbide (SiC), in electric vehicles field. More specifically, because of the automotive environment and higher permissible slew rates in SiC devices, it is important to assess reliability in harsh conditions. Potentially related to this test, incomplete ionization of the dopants and edge termination fails are reported for SiC devices, in consequence of fast transients, but aging phenomena and parameters drift have not been deeply investigated. Mette a fuoco poi le ragioni per le quali qualsiasi approccio storiografico alla filosofia debba necessariamente essere di carattere speculativo, individuandole nella necessità della delimitazione teorica dell'oggetto della ricerca, nell'uso della terminologia teoretica specifica della disciplina per l'inquadramento dei problemi, il carattere imprescindibilmente "contemporaneo" della ricerca storiografica, dunque il suo nascere da problemi del presente che richiedono chiarificazione attraverso l'indagine storica
Neem Azadirachta indica extract augments serum, mucus innate immunity, antioxidant status, and resistance to Aeromonas hydrophila in Rohu Labeo rohita
Objective: In aquaculture, medicinal plants and their derivatives serve as potent natural boosters for the immune system and effective stress relievers for aquatic organisms. This study explored how neem plant (NP) Azadirachta indica leaves extract influenced the immune system and antioxidant capacity of Rohu Labeo rohita. Methods: The sample included 240 healthy Rohu fingerlings, averaging 22.54 ± 0.82 g in weight, divided randomly into four groups, each containing triplicates of 20 fish per glass aquarium. The control group (NP0) received a standard diet without additives, whereas the other groups were fed NP-infused diets at concentrations of 2 g/kg (NP2), 4 g/kg (NP4), and 6 g/kg (NP6) over a 60-d period. After this feeding trial, the fish were exposed to Aeromonas hydrophila to assess their immune response and their survival rates were calculated. Results: The findings revealed that relative to the control group, the NP4 group exhibited significantly (P< 0.05) higher levels of lysozyme activity in both serum and mucus. Peroxidase activity in serum was elevated in the NP4 and NP6 groups, although the difference was not statistically significant. However, peroxidase activity in mucus was significantly (P< 0.05) higher in the NP6 group. Additionally, serum levels of immunoglobulin, total protein, and ACH50 were considerably (P< 0.05) higher in the NP6 group, whereas mucus levels of immunoglobulin, protease, and total protein were significantly (P< 0.05) elevated in the NP4 group. Enzymatic activities such as catalase, superoxide dismutase, and glutathione dehydrogenase in serum and fish survival were significantly (P< 0.05) higher in the NP4 group than in the control. The survival rate of the fish was higher in the NP6 group followed by NP4. Conclusions: In conclusion, the findings suggest that NP extract holds potential as a natural immunostimulant for Rohu aquaculture

Preliminary study on the influence of the geographical origin and farming system on ‘Nero dei Nebrodi’pig using chemical and isotopic fingerprinting
In this study, the geographical origin of longissimus dorsi meat from 'Nero dei Nebrodi' pigs reared in two distinct regions of north-eastern Sicily was indagated by correlating chemical-nutritional parameters, stable isotope composition, fatty acid and sterol profiles, and mineral element content. Significant differences (p ≤ 0.05) between the 'Nebrodi group' (NG) and the 'External Nebrodi group' (ENG) were found for 51 over 80 variables. The δ2H, δ18O, δ13C and δ15N of the defatted meat were statistically different (p<0.01) between NG and ENG animals, indicating a change in the composition of the diet and drinking water consumed. The results showed that the characteristic rich endemic vegetation of the Nebrodi influences not only the chemical-nutritional parameters of the meat but also its isotopic ratios, allowing for a geographical characterization required for the traceability of this peculiar product, aspiring to a protected designation of origin
