Another Step Towards Indexation of the SPDV Journal in 2018
Mais um Passo para a Indexação da Revista da SPDV em 2018After 2 full years of publication with the new editorial board, 2017 was the year of big changes for the SPDV journal, which we hope will allow the indexation of our journal in the Medline.We had regular and timely publications, each number with an editorial related to one of the main articles published in that number, a continuous medical education (CME) article, followed by 6 articles with original data or review articles on the latest news on dermatology, 6-7 clinical cases, a quiz/what’s your diagnosis presentation taking profit of clinical and histological images and letters to the editors. This format will continue in 2018 to have a more regular type of articles in our journal.2017 was the year of internationalization. Apart from regular Brazilian clinical cases that we really appreciate as most bring new aspects from diseases that are seldom seen among us, we had a CME article on pruritus written by our colleague Manuel Pereira from Germany that with Sonja Ständer and the Munster team is coordinating European studies on pruritus and chronic prurigo, and an editorial written in collaboration with An Goossens the well-known teacher in contact and photocontact allergy from Leuven, Belgium. We are still expecting that our residents or specialists who stay a few months abroad in highly specialized centres with scholarships from the SPDV write review articles in collaboration with the doctors they worked with abroad.This year we began the process of identifying the papers published in our Journal with a DOI (digital object identifier), a direct link to the manuscript that includes the number given to our Journal followed by its initials, the volume and number of each journal and the consecutive number attributed by the webpage to each article that is submitted (ex. https://dx.doi.org/10.29021/spdv.76.2.849). Therefore online submission of all the manuscripts is mandatory. But this is an easy process and allows the authors to trace the state of their manuscripts while in review.It is also our intention in 2018 to begin the process of showing the articles online as soon as they are accepted, before their inclusion on the printed and finally composed volume. We hope to have the collaboration of Dr. José Carvalho who is thoroughly taking care of our webpage. Thanks to the work of the editorial board and the many invited reviewers* and with a more thorough reviewing process we improved the quality of the papers that we received, although many of them were already of really high standard.Happily this year more original and review articles were submitted (33) of which only 2 were rejected (6%). The rejection rate was much higher within the 75 clinical reports received (38% – 51%). Although some articles submitted in 2017 are still under review the overall rejection rate was around 36%.With the final review of Drª Helena Donato there is a good standardization of procedures, including a critical review of the references, MeSH words and inclusion of all the missing details in the manuscripts to fulfil the international standards for publication of scientific papers. The work of our Editor, Gabriela Marques Pinto, who also makes a final review of all the manuscripts and prepares their composition for printing is also an enormous job.It is our intention to renew the editorial board during 2018 to include other colleagues that have given a significant and timely contribution to our journal, but we will keep the contribution of Dr Helena Donato and her knowledge to submit the journal for indexation with the next number of 2018.Still, to keep with international rules of scientific journals, in 2018 we will change the placing of publicity within the printed journal to avoid its inclusion in the middle of scientific papers.With these changes and improvements we are feeling more confident and on a much better position to achieve indexation of the Revista da SPDV at the Medline. With the contribution of this team and all the dermatologists we hope a favourable decision, but this will be just one step forward as we will certainly keep on improving the Journal of Portuguese-speaking dermatologists.
Projecto Angolano de Formação e Desenvolvimento da Dermatologia: Avanços e Desafios
Introduction: Despite the burden of skin disorders and their physical, psychological and social repercussions, the development of Dermatology and the training of technicians in this discipline face serious challenges in Angola, with a negative impact on the access and quality of care and services provided to the population.Methods: A retrospective study of the Dermatology Training and Development Project was carried out. The Project, launched in 2010 with the support of the Ministry of Health and the Angolan College of Dermatology and Venereology (CADV), aims at increasing the number of dermatologists and the access of patients to clinical referral services. Data on the training activities, other gains and limitations of the Project are presented.Results: Up to 2013, the residency in Dermatology was confined to a single Service in Luanda, which trained eight dermatologists from 1975 to 2013. Five new Regional Dermatology services were opened in 2013 in Cabinda, Malange, Huambo, Benguela and Huíla. As a result the total number of residents in Dermatology increased to 26 in 2017.Conclusion: In the context of Angola, the decentralization of clinical training services, the periodic update of the postgraduate training program and the active promotion of research attitudes and activities are key factors for the development of Dermatology and the provision of referral services closer to the populations.Introdução: Apesar do fardo e das repercussões físicas, psicológicas e sociais que as doenças da pele representam, o desenvolvimento da Dermatologia e a formação de técnicos nessa disciplina defrontam em Angola sérios desafios, com repercussões no acesso e na qualidade dos serviços e cuidados prestados a população.Metodologia: Foi realizado um estudo retrospectivo do Projecto de Formação e Desenvolvimento da Dermatologia, iniciado em 2010 com o apoio do Ministério da Saúde e do Colégio Angolano de Dermatologia e Venereologia (CADV). O projecto tem por objectivo a formação de mais dermatologistas e de aumentar o acesso dos doentes a Serviços de referência. São apresentados dados sobre a formação dos internos, bem como outros ganhos e limitações do Projecto.Resultados: Até 2013, o internato médico complementar em Dermatologia estava reservado apenas a um Serviço de Dermatologia em Luanda, tendo formado oito dermatologistas desde 1975. Em 2013, cinco novos Serviços Regionais, abertos em Cabinda, Malange, Huambo, Benguela e Huíla, iniciaram a formação de especialistas, aumentando em 2017 a 26 o número total de internos em formação.Conclusão: No contexto de Angola, a descentralização dos serviços clínicos de formação, a afinação periódica do programa de pós-graduação e a promoção activa de atitudes e actividades de pesquisa são elementos chave no desenvolvimento da Dermatologia e na prestação de serviços de referência mais próximos das populações
Angioedema Recidivante Idiopático: Uma Possível Apresentação de Angioedema Hereditário Tipo III
Three types of hereditary angioedema (HAE) have been described within the spectrum of idioptahic nonhistaminergic angioedema. Type I and II related with C1-INH (C1-esterase inhibitor) deficiency is more frequent and type III is rare, occurs mostly in young women, without C1 inhibitor (C1-INH) alterations, and may be associated with mutations in the factor XII gene. Trauma, stress and estrogen may be the precipitants, treatment is still a challenge due to the absence of controlled studies. We present a case of possible type III HF in a young woman, aggravated by the introduction of anti-conception and improvement with its suspension and treatment with tranexamic acid, but without a family history or a factor XII mutation.Dentro do angioedema idiopático não histaminérgico estão descritos três tipos de angioedema hereditário (AEH). O tipo I e II por défice de C1-INH (inibidor de C1-esterase) é o mais frequente e o tipo III, mais raro, ocorre maioritariamente em mulheres jovens, sem alterações do C1-inibidor (C1-INH), e pode associar-se a mutações no gene do fator XII. Trauma, stress e estrogénios podem ser os precipitantes, o tratamento é ainda um desafio pela ausência de estudos controlados. Apresentamos um caso de possível AEH tipo III numa jovem, agravado pela introdução de contraceptivo oral e melhoria com a sua suspensão e tratamento com ácido tranexâmico, mas sem história familiar e sem mutação do factor XII
Influência do Índice de Massa Corporal e da Dieta na Fisiopatologia da Acne Vulgaris
Acne vulgaris is a multifactorial disease that mainly affects adolescents and young adults and can present either with inflammatory or non-inflammatory lesions. This disease is responsible for significant morbidity, both physical and psychological. The importance of both genetic and environmental factors in its pathogenesis has been scientifically proven, and associations with the western diet, excessive body mass index and insulin resistance have been described. In this article we review the current literature on acne vulgaris, focusing on the pathophysiology and influence of these factors on its disregulation. A better knowledge of the physiopathology of acne and a critical evaluation of the influence of diet, body mass index and disruption of the endocrine system will allow a more scientific intervention on exogenous factors that contribute to the initiation and worsening of this interesting and frequent pathology.Acne vulgar é uma patologia multifatorial que afeta principalmente adolescentes e jovens adultos e que se pode manifestar como lesões inflamatórias ou não inflamatórias. Esta doença acarreta morbilidade significativa, tanto física como psicológica. Está cientificamente comprovada a importância de diversos fatores, tanto genéticos como ambientais, na sua patogénese, tendo sido descritas nomeadamente associações com a dieta ocidental, índice de massa corporal excessivo e a resistência à insulina. Neste artigo é efetuada revisão da atual literatura relativa à acne vulgar, com enfoque na fisiopatologia e influência destes fatores na sua desregulação. O melhor conhecimento da fisiopatologia da acne e a avaliação crítica do papel da dieta, bem como da obesidade e da desregulação do sistema endócrino, podem contribuir para uma abordagem mais científica dos fatores exógenos que contribuem no início e agravamento desta interessante e frequente patologia
Efeito do Mês de Nascimento no Risco de Desenvolver Psoríase
Introduction: Psoriasis is an immunemediated dermatosis characterized by chronic inflammation, abnormal proliferation and aberrant differentiation of keratinocytes, hyperplastic blood vessels and infiltration of inflammatory cells. Psoriasis is a multifactorial disease and is influenced by genetics and epigenetic modifications that can be triggered by environmental factors. Climate and sun exposure may affect psoriasis prevalence and the ultraviolet radiation is a useful treatment. Our objective was to evaluate the relationship between the month of birth and the risk of developing psoriasis.Methods: The monthly distribution of births of patients with psoriasis (n = 755) was compared to that of a control population, comprised by Portuguese individual born during the same period and from the same region (n = 6 560 032).Results: Birth rate of psoriasis patients decreased in October (OR 0.74; 95% CI 0.55 – 0.99; p 0.041). After Bonferroni correction and after grouping the months by trimesters, no statistical significant differences were found.Conclusion: Our results may be related to the particular meteorological characteristics of Portugal that may lead to stabilization of the vitamin D synthesis during the whole year. The perception of the environmental influence on the development of a specific pathology is very important because it allows the establishment of preventive measures that will reduce disease incidence. Introdução: A psoríase é uma dermatose imunomediada caraterizada por inflamação crónica, proliferação e diferenciação anormal dos queratinócitos, hiperplasia vascular e infiltração de células inflamatórias. É uma doença multifatorial influenciada por alterações genéticas e epigenéticas despoletadas por estímulos ambientais. O clima e a exposição solar parecem afetar a prevalência da psoríase e a radiação ultravioleta é útil na abordagem terapêutica. O nosso objectivo foi avaliar a influência do mês de nascimento (como marcador da época de gestação) no risco de desenvolver psoríase.Métodos: Comparação da distribuição dos meses de nascimentos nos pacientes com psoríase (n = 755) seguidos no Centro Hospitalar e Universitário do Porto com uma população controlo constituída pelos cidadãos portugueses nascidos no mesmo período e área geográfica dos pacientes com psoríase (n = 6 560 032).Resultados: Observou-se uma diminuição do número de nascimentos de pacientes com psoríase no mês de outubro estatisticamente significativa (OR 0,74; 95% CI 0,55 – 0,99; p 0,041). Após correção de Bonferroni e após agrupar os meses por trimestres, não se observaram diferenças estatisticamente significativas.Conclusão: As diferenças estatísticas pouco significativas e não significativas apresentadas podem ser explicadas pelas características meteorológicas particulares de Portugal que podem tornar a síntese de vitamina D relativamente estável durante o ano. A perceção da influência ambiental no desenvolvimento de determinada patologia é importante porque permite o estabelecimento de medidas preventivas que conduzirão à diminuição da sua incidência
Euromelanoma in Portugal 2010-2016
Introduction: The incidence of skin cancer has been increasing in recent years around the world. Key targets in the fight against skin cancer include alerting populations to primary prevention by promoting knowledge about risk factors and encouraging better sun protection behaviors as well as secondary prevention promoting skin self-examination and screenings. The Portuguese Society of Dermatology and Venereology and the Portuguese Skin Cancer Association, have collaborated with Euromelanoma, a pan-European information and screening initiative for skin cancer since the year 2000.Material and Methods: This cross-sectional study consisted in the evaluation of data from the completed Euromelanoma questionnaires during the Euromelanoma screening day in Continental Portugal and Islands, in public and private institutions from 2010 to 2016.Results: A total of 9862 individuals were included, 63% female and 37% male, with a mean age of 48 years. The number of clinically suspected malignant or pre malignant lesions was relatively low but consistent with other published Euromelanoma studies: 3% basal cell carcinoma, 0.4% squamous cell carcinoma, 1% melanoma, and 6% of actinic keratoses.Conclusion: The Euromelanoma initiative has raised at national and international level a greater awareness of the population on the theme of skin cancer, with promotion of the messages by the media, assuming itself as an important subject of public health, recognized by the political and sanitary authorities.Introdução: A incidência dos cancros da pele tem vindo a aumentar nos últimos anos em todo o mundo. O alerta das populações para a prevenção primária, promovendo o conhecimento sobre factores de risco e incentivando melhores comportamentos de proteção solar, bem como a prevenção secundária fomentando o auto-exame de pele e rastreios, são estratégias fundamentais da luta contra o cancro da pele. A Sociedade Portuguesa de Dermatologia e Venereologia e a Associação Portuguesa de Cancro Cutâneo, têm colaborado com o Euromelanoma, uma iniciativa pan-Europeia de informação e rastreio de cancro da pele, desde o ano 2000.Material e Métodos: Este estudo transversal consistiu na avaliação dos dados do formulário do Euromelanoma preenchido aquando do rastreio no dia do Euromelanoma, em Portugal Continental e Ilhas, nas instituições públicas e privadas, de 2010 a 2016.Resultados: Foram incluídos 9862 indivíduos, 63% mulheres e 37% homens, com uma média de idades de 48 anos. O número de lesões clinicamente suspeitas de malignidade, ou de risco de malignidade, foi relativamente baixo mas consistente com outros estudos publicados do Euromelanoma: 3% carcinoma basocelular, 0,4% carcinoma espinocelular, 1% melanoma e 6% de queratoses actínicas.Conclusão: A iniciativa do Euromelanoma suscitou a nível nacional e internacional uma maior sensibilização das populações para a temática do cancro da pele, com amplificação das mensagens pela comunicação social, assumindo-se como um importante tema de saúde pública, reconhecido pelas autoridades políticas e sanitárias
Dermatite Atópica: Perspetivas e Atitudes de Doentes Adultos e de Cuidadores de Crianças Afetadas, Avaliadas por Focus Group
Introduction: Important dimensions such as personal experiences, attitudes toward disease, its causes and treatments, are not fully addressed in clinical trials. Focus group (FG) has emerged as an interesting and valuable tool in clinical research complementing this gap. The aim of this qualitative research was to assess in both caregivers and patients dealing with atopic dermatitis (AD) their attitudes, personal experiences and perspectives toward the disease and its topical treatment as well as the impact in quality of life (QoL).Material and Methods: For discussion sessions, 10 caregivers of children and 10 adult patients were recruited. Two sessions of FG took place with 3 main themes discussed: perspectives toward AD, topical treatments and the impact in QoL. All activities were recorded in video and the discussions and notes were then transcribed to a document, followed by transcripts analysis.Results: The best descriptive feelings in the moment of diagnosis where “concern” (30%) and “quality of life” (30%) for caregivers and patients, respectively. The actual “positive” emotion or state of mind toward the disease was “overcoming” for caregivers (21%) and “control” for patients (17%). The main “negative or neutral” emotion was “fear” for caregivers (13%) and “resignation” for patients (18%), but when projecting to their child, “frustration” (19%) was the most mentioned. In relation to topical treatments, the mean global satisfaction of caregivers was high for tacrolimus (8.5/10), except in the item “price”. In the case of patients, corticosteroids received a better score (8.0/10), with exception for “tolerability/adverse effects”. Features like “preventive treatment with reduction of flares” and “free of cortisone” were important for both participants in an “ideal topical medicine” setting. All participants showed high levels of negative impact in their QoL due to AD, with 47% and 64.6% considering scores of “very much” and “a lot” of interference, respectively.Conclusion: Qualitative studies in AD by FG are scarce and to our knowledge this is the first one gathering both adult patients and caregivers. The dimensions yielded by this approach are useful to complement data retrieved from clinical trials and to drive decisions from researchers and health authorities.Introdução: Dimensões importantes, tais como experiências pessoais, atitudes em relação à doença, suas causas e tratamentos, não são totalmente abordadas em ensaios clínicos. Os Focus Group (FG) surgiram como uma ferramenta interessante e valiosa na pesquisa clínica, complementando essa lacuna. O objetivo desta pesquisa qualitativa foi avaliar, tanto em cuidadores quanto em doentes adultos que lidam com dermatite atópica (DA), as suas atitudes, experiências pessoais e perspetivas em relação à doença e ao seu tratamento tópico, bem como o impacto na qualidade de vida (QV).Material e Métodos: Para as sessões de discussão foram recrutados 10 cuidadores e 10 doentes adultos. Foram realizadas duas sessões de FG e discutidos três temas principais: perspetivas em relação à DA, tratamentos tópicos e o impacto da doença na QV. Todas as atividades foram gravadas em vídeo e a discussão e anotações foram transcritas para um documento. A análise posterior foi realizada por dois investigadores.Resultados: No momento do diagnóstico, os sentimentos vivenciados que melhor o descrevem foram “preocupação” (30%) para os cuidadores e “qualidade de vida” (30%) para os doentes. A emoção ou estado de espírito "positivo" em relação à doença atual foi "superação" para os cuidadores (21%) e "controlo" para os doentes (17%). A principal emoção “negativa ou neutra” foi “medo” para os cuidadores (13%) e “resignação” para os doentes (18%), mas ao projetar para a criança doente ao seu cuidado, “frustração” (19%) foi a mais citada pelos cuidadores. Em relação aos tratamentos tópicos, a satisfação global média dos cuidadores foi alta (8,5/10) para o tacrolimus, exceto no item “preço”. No caso dos doentes, os corticosteroides receberam uma melhor pontuação (8,0/10), com a exceção do item “tolerabilidade/efeitos adversos”. Características como “tratamento preventivo com redução das agudizações” e “livre de cortisona” foram importantes para ambos. Todos os participantes demonstraram altos níveis de impacto negativo na QV, tendo respetivamente 47% e 64,6% considerado os scores de interferência “bastante” e “muito”.Conclusão: Estudos qualitativos na DA pela metodologia FG são escassos e, do nosso conhecimento, este é o primeiro a reunir doentes adultos e cuidadores de crianças. As dimensões obtidas por esta abordagem poderão se úteis para complementar os dados dos ensaios clínicos e orientar decisões de investigadores e autoridades de saúde
Penfigóide Bolhoso Refractário em Paciente Infectado pelo Vírus da Imunodeficiência Humana
Bullous pemphigoid, the most common autoimmune subepidermal blistering disease, is due to autoantibodies against BP180/230 present in the hemodesmosomes of the basal keratinocytes that typically causes pruritus and tense cutaneous bullae on an erythematous or normal skin It affects predominantly the elderly, often in association with neurologic or neoplastic disease. The association with the human immunodeficiency virus infection is rare. We present a case of a young VIH+ patient under antiretroviral therapy with undetectable viral load and normal CD4+ T-cell count who developed an extensive bullous pemphigoid refractory to pulses of corticosteroids, oral steroids and methotrexate and improved only after the infusion of rituximab, an anti-CD20 monoclonal antibody.Penfigóide bolhoso, a dermatose bolhosa subepidérmica auto-imune mais comum, é devida a auto-anticorpos contra partículas dos hemidesmosmas (BP180 e/ou BP230) e caracteriza-se por prurido e bolhas cutâneas tensas sobre base eritematosa ou pele normal. Afeta predominantemente idosos, por vezes com comorbilidades neurológicas ou neopláscias. A associação a infecção pelo vírus da imunodeficiência humana é rara. Relatamos o caso de uma paciente jovem VIH+ em tratamento antirretroviral com carga viral indetectável e contagem normal de células T CD4+, que desenvolveu penfigóide bolhoso com lesões extensas, incluindo na mucosa oral, resistentes a vários pulsos de corticoides, corticoides orais e metotrexato, com melhora e estabilidade clínica apenas após uma infusão de rituximab, um anticorpo monoclonal anti-CD20
Psoríase Inaugural Após Tratamento com Docetaxel
Docetaxel is a second-generation taxane commonly used in the treatment of advanced malignant tumours such as breast adenocarcinoma. We describe the case of a 67-year-old female undergoing adjuvant chemotherapy with docetaxel for invasive ductal carcinoma of the left breast presenting with typical psoriatic lesions on the dorsum and limbs - the most representative of them arising close to the venous access for docetaxel infusion. No personal or family history of psoriasis was reported. The patient started and maintained treatment with oral acitretin and topical betamethasone diproprionate/calcipotriol for nearly eight months, with almost complete regression of cutaneous lesions. Drug-induced psoriasis is a rare condition. The most commonly reported drugs include beta-blockers, interferon and antimalarials. In this case we demonstrate that docetaxel should be included in the group of drugs that can potentially induce psoriasis.O docetaxel é um taxano de segunda geração utilizado frequentemente no tratamento de várias neoplasias malignas avançadas, nomeadamente no carcinoma da mama. Apresentamos o caso clínico de uma doente de 67 anos com diagnóstico de carcinoma ductal invasivo da mama esquerda sob quimioterapia adjuvante com docetaxel, que desenvolveu lesões psoriáticas típicas no tronco e nos membros; a lesão mais representativa localizava-se na proximidade do acesso venoso utilizado para administração do fármaco. Não eram conhecidos antecedentes pessoais ou familiares de psoríase. A doente iniciou e manteve tratamento com acitretina oral e diproprionato de betametasona/calcipotriol tópicos durante cerca de oito meses, com regressão quase total das lesões cutâneas. A psoríase fármaco-induzida é uma entidade rara. Os fármacos classicamente associados incluem beta-bloqueantes, interferão e os antipalúdicos, entre outros. Demonstramos com o presente caso que o docetaxel deverá ser incluído no grupo de agentes farmacológicos potencialmente indutores de psoríase
